I am writing to formally lodge a complaint regarding a recurring issue I have experienced with the head gasket in my vehicle. I have owned several vehicles of this make and model, and I have observed a troubling pattern of blown head gaskets that has not been adequately addressed. Specifically, my current vehicle has suffered from this problem, resulting in significant inconvenience and additiona
... l repair costs. This issue appears to be prevalent across this year model as well, suggesting a potential design flaw or manufacturing defect that warrants further investigation. I respectfully someone to look into this matter to determine if a broader safety concern exists that needs to be addressed. Owners of vehicles affected by this issue may be at risk of serious engine damage and costly repairs. Thank you for your attention to this important issue. I look forward to your response.Read more

The engine consumes oil which is a known problem at Hyundai with the Tucson for the year 2020. The car has ~51,000 miles on it. When we took the car to the dealer at which we bought the car they diagnosed that the engine was burning oil. We were told that the engine is covered by standard warranty (10 yr. 100,000 miles). there is a process Hyundai has established for engines suspected of havin
... g this issue - first we had to have them check the oil and engine and then we had to drive the car 1000 miles and bring it back to them. when we brought it back they confirmed the the engine was consuming oil. They asked us to provide our maintenance records on the car (oil changes). We were able to find evidence of 5 oil changes but we know the we changed the oil on the car every 3-5k miles as we always do. The next step in the process is a combustion chamber cleaning which costs $1400 which would normally be covered by warranty but since we did not have all records of oil changes they will not cover it so we are being told we have to pay the $1400 out of pocket and also the the cleaning is not assured to fix the issue with the engine. If it fails to fix the issue the car will need a new engine which Hyundai is telling us they will not do under the warranty and it will be our financial obligation ($10-15K additional paid by us). It is clear that Hyundai is trying to shirk their warranty obligation to us. So we are stuck - we have to pay out of pocket for a service that may not resolve the issue and if it does not the car needs a new engine. If we sell the car back to the dealership they will only pay us $6000 when the car with a good engine is worth $15K. We take our obligation of regular maintenance seriously and have been loyal buyers at this dealership for >20 years never having an issue with proper maintenance. We think it unconscionable that Hyundai is leaving us holding the bag for a problem they know exists with theses enginesRead more

I am reporting the failure of Ford Motor Company to perform a mandatory safety recall (Recall 22S43 / NHTSA ID 22V413) on my 2019 Ford Escape. This failure is compounded by evidence of falsified internal service records (OASIS reports) that misrepresent the vehicle's recall status as 'completed.' Furthermore, the failure to perform this specific recall directly prevented a necessary diagnostic
... inspection of the transmission linkage and adjacent engine compartment. Had this recall been performed, the technician would have identified the progressive corrosion and mechanical stress that subsequently destroyed the high-pressure fuel pump and the Transmission Control Module (TCM). The manufacturer meticulously recorded routine maintenance while ignoring a critical federal safety mandate. Their negligence directly caused a cascading mechanical failure, rendering my vehicle an unsafe, non-operational hazard. I am seeking a formal investigation into these falsified service records and the manufacturer's failure to remedy this safety hazard. I attempted to resolve this manner at the lowest levels, before taking this route. Unfortunately, to their detriment; Ford basically told me I was on my own. I have already alerted the Attorney General for the State of Tennessee, as well. The subsequent loss of the vehicle as a result of the abhorrent behavior of Ford Motor company has resulted in time lost with my kids and my mental health appointments with the Veterans Administration. I can not make it to my therapist or get my medication. I have the Carfax and Oasis reports as further evidence to back these claims.Read more

While I was driving home from the grocery store with my 4 kids our 2021 GMC Yukon XL (5.3L V8 engine), the vehicle suffered a sudden, catastrophic mechanical engine failure without warning. The engine began violently shaking, lost major propulsion, and dropped into a severe misfire state when we were coming down the highway. ... he ability to safely accelerate or maintain highway speeds while hauling my children.A certified technician diagnosed the issue as a complete failure of the active fuel management hydraulic valve lifters and bent pushrods, a known manufacturing defect explicitly documented in GM Technical Service Bulletin PIP5776F. The internal failure required a total engine teardown and valvetrain replacement, costing over $10,000 out of pocket.General Motors has failed to issue a safety recall for this specific 5.3L valvetrain defect, despite it causing unexpected engine stalls, severe power loss, and immediate traffic hazards for drivers. The affected component is the fuel injector system covered under Ford’s safety recall. According to the recall notice, a fuel injector may crack and allow fuel and/or fuel vapors to accumulate on top of the engine, creating a risk of fire if exposed to hot engine or exhaust components. The vehicle is available for inspection upon request. The safety risk is significant because the defect involv
... es the potential for an under-hood fire while driving or while the vehicle is parked after operation. I use this vehicle as my primary means of transportation and have concerns about my safety and the safety of passengers due to the possibility of a fire occurring before a repair is available. The issue has been confirmed by Ford through its issuance of the safety recall. I have contacted Ford and dealership service departments regarding the recall. However, I have been informed that repair parts are not yet available, leaving the vehicle unrepaired despite the acknowledged safety defect. The vehicle has been inspected by authorized Ford dealerships in connection with related concerns. I have also communicated directly with Ford regarding the recall and the lack of an available remedy. No specific warning lights or messages related to a cracked fuel injector appeared prior to my awareness of the defect. My concern arises from Ford’s recall notice identifying a condition that may lead to fuel leakage and an engine compartment fire. Because parts are unavailable, I am unable to have the safety defect corrected despite Ford’s acknowledgment of the risk.Read more

The vehicle is available for inspection upon request. How was
... your safety or the safety of others put at risk? This malfunction occurs intermittently and without warning when the vehicle is at a complete stop in traffic. The vehicle cannot be driven under its own power in this condition, often leaving it disabled in a traffic lane with other vehicles stuck behind and/or passing by at normal travel speeds. As such, it endangers both the occupants of my vehicle and the other vehicles in the vicinity. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. My local Honda dealership acknowledged the problem and completed a PGM-FI software update. However, this update did not correct the issue, and Honda refuses to take financial responsibility for the repairs necessary to correct the problem. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The following timeline details my interactions with my local Honda dealership regarding this issue: 4/13/23 - The dealership inspected the vehicle and claimed the Auto Start/Stop system was "operating as designed"; 9/6/24 - The dealership completed a PGM-FI update as directed by a Honda product update campaign; 2/12/26 - The dealership inspected the vehicle, suggested battery replacement (which tested good on 3/3/26), and if still not corrected, advised replacement of the starter at my expense. I have fighting this issue since January and have not gotten any help or relief from Chevrolet. This issue caused a significant safety risk and has cost me $12,000. I am writing to formally report a serious safety issue involving my 2021 Chevrolet Silverado that resulted in a sudden loss of power while merging onto an interstate highway. On XXX, I brought my vehicle in for service to replace the
... upper and lower ball joints, as well as the control arms. I retrieved the vehicle from the repair shop on XXX, and departed the same day for travel to [XXX]. Later that evening, while traveling southbound on [XXX] near [XXX], I stopped at Exit XXX. Upon re-entering the highway and accelerating to merge into traffic, the vehicle began shifting through gears abnormally. While in approximately 6th gear at around 2,900 RPM, a loud banging noise occurred, followed immediately by a significant loss of engine power. At the same time, the check engine light illuminated. Due to the sudden and unexpected loss of power while actively merging into highway traffic, I was nearly rear-ended by another vehicle. I was forced to take immediate evasive action and pull onto the shoulder to avoid a collision. This incident created a highly dangerous situation not only for myself but for surrounding motorists. This failure occurred without warning and directly impacted my ability to safely operate the vehicle in a high-speed traffic environment. I believe this presents a significant safety concern that warrants investigation. Please let me know if any additional information is required. I am a [XXX] resident. While commuting to work in [XXX], my GMC Terrain experienced a sudden, catastrophic failure of the brake vacuum pump while traveling at highway speeds. When I attempted to slow down for the exit, the brake pedal instantly became extremely stiff and nearly impossible to depress. I realized I had suffered an absolute loss of power braking assist. To avoid a high-speed collisio
... n with heavy highway traffic, I was forced to deliberately steer onto the roadside rumble strips, relying on tire friction to slow the vehicle down. This severe near-miss left me terrified and distraught. Because the shoulder was narrow, my disabled vehicle was left partially protruding into the active roadway. Passing cars sped by so close that my vehicle was violently rocking, creating an incredibly unsafe situation. I was forced to call the State Police for emergency assistance. They dispatched a trooper and a tow truck to remove me and the vehicle safely from the highway. The vehicle was towed to a mechanic in Massachusetts. The mechanic discovered that the vacuum pump had internally exploded. This explosion sent catastrophic metal shards and debris directly into the engines oil galleys and oiling system. While the vehicle has 121,776 miles, this is not a normal wear-and-tear component failure. The brake vacuum pump suffered a catastrophic internal mechanical explosion at highway speeds, completely eliminating my ability to stop the vehicle. A mechanical component failure should never cause an immediate, life-threatening loss of brake control, regardless of mileage. This is a known, systemic engineering defect that is currently the subject of active federal class-action litigation (XXX v. General Motors LLC). This defect did not just cause an immediate life-threatening braking failure, it completely contaminated the oiling system, destroyed the entire engine, and rendered the vehicle a total loss requiring a full engine replacement. My 2019 Ford Escape overheated, had the vehicle towed. The repair shop said it was coolant intrusion into cylinder 3, first was to replace the head gasket, upon disassembly the mechanic notice gouges in cylinder head. Endurance would not cover the replacement of head gasket or cylinder head. car displayed overheated pull over safely. Not sure if this will be a reoccurring problem.the mechanic sa

[XXX] [XXX] [XXX]; [XXX] [XXX] [XXX] Date: XXX To: National Highway Traffic Safety Administration Office of Defects Investigation (ODI) Subject: Formal Safety Defect Complaint – 2017 Hyundai Sonata Theta II Engine Failure, Excessive Oil Consumption, Intake/Engine Component Failure and Safety Hazard Dear Sir or Madam: I am submitting this formal complaint and request for immediate investigat
... ion into the dangerous and ongoing mechanical failures involving my 2017 Hyundai Sonata equipped with the Theta II engine. I believe this vehicle contains a serious manufacturing and/or engineering defect that presents a substantial safety hazard to consumers. My vehicle has experienced a pattern of progressive mechanical failure beginning with abnormal and excessive oil consumption, which appears to be directly associated with malfunctioning engine airflow and intake-related components. Specifically, the lower engine airflow system and intake functionality appear defective, placing excessive strain on the intake manifold system and contributing to cascading engine failures. As a result of these failures, the vehicle has experienced: Severe and excessive oil burning/consumption Intake manifold malfunction and abnormal operating stress Suspected overheating and/or premature head gasket failure Ignition coil failure and electrical shorting/misfires Progressive engine deterioration rendering the vehicle unsafe and unreliable Increased risk of sudden engine shutdown, stalling, fire hazard, or catastrophic engine failure while operating The vehicle has become increasingly unreliable and inoperable due to these conditions. These failures raise significant concerns regarding public safety, especially given the known history of complaints and recalls associated with Hyundai Theta II engine defects. I am deeply concerned that this vehicle may reflect a broader pattern of defective manufacturing, insufficient quality control. While driving my 2019 Honda Accord Hybrid at highway speed, the engine suddenly suffered catastrophic failure. The engine block ruptured, leaving a large visible hole with internal engine components exposed. Oil was expelled into the engine bay and ignited/burned briefly. This occurred without prior warning at highway speed and created a sudden loss-of-propulsion hazard. The vehicle has approxima
... tely 48,500 miles and has had regular on-time maintenance via Honda dealership. No warnings or other symptoms were active prior to engine rupture event. Fire dept & NYS police arrived on scene prior to being towed. The vehicle is currently preserved at Sussex Honda dealer for inspection and awaiting diagnosis.Read more

Rough idle started at 11,400 miles. Took to GMC dealer near me and had thrust bearing clearance measured. Measured .0054"(within spec). They released truck back to me. 19,400 mile oil change found large metal(non magnetic) slivers in the filter media. I cut filters to check for issues due to known issues with this engine(metal). I called gmc corporate, i was told to not drive the truck. I paid o
... ut of pocket for the tow 142 miles away. Dealer told me they checked thrust bearing and came in at .0055", no noises, and no check engine light. Idle is very rough on start up. GM corporate stated a service advisor will call. Never got a call. I have sent and displayed the evidence and offered to show oil filter contents to both dealer and corporaten both have not done anything about this safety issue. This is a SEVERE safety issue. Engine is highly likely to lock up and cause serious injury to both me, my family, and others on the road. Dealer has released truck back to me and my family with clean bill stating they didnt find anything wrong. They refused to see my documentation of metal slivers or the 3 rimg binder full of maintenance records. As of now i will be escalating this to higher up in GMC corporate. SEVERE safety risk for my family and everyone on the road. GMC dealers refusing to diagnose.Read more

I heard knocking as I was leaving gas station. I was scared about what was going wrong and if it was safe to drive home. I was alone and it was after dark. I took the next day off and took the car to my mechanic. He said it sounded like rod knock but didn’t want to do the engine replacement so he sent me to another mechanic who also said it sounded like the engine needed to be replaced. I di
... dn’t have the money or the time to deal with it so it has been parked for the last five months. Today I took it to the dealer (that I bought it from new) for a diagnostic inspection. They stated that the noise was coming from main shaft, unable to repair, will need to replace engine. Cost estimate to replace engine is $15,964.36. I do not remember a check engine light coming on, only the lights for the driving sensing items. Which I read were also problems on this vehicle. Photos of the inspection report have been uploaded. A mistake was made on the mileage it is 103,203 not 103,703. Thank you for your time.Read more
