My 2025 Chevrolet Tahoe RST Performance Edition equipped with the 6.2-liter V8 experienced a catastrophic engine failure at approximately 28,400 miles. The Tahoe was towed to an authorized Chevrolet dealership. The dealer confirmed an internal engine failure requiring complete engine replacement. The vehicle and failed engine are currently at the dealership in Lawrence, Kansas, and should be avai
... lable for inspection upon request. The precise internal cause has not yet been provided to me in writing. I was advised that obtaining and installing the replacement engine is expected to take approximately 60 to 90 days or longer. The scheduled 30,000-mile maintenance was performed by the Chevrolet dealership on June 29, 2026, when the odometer read 27,607 miles. The engine failed less than two weeks later, after approximately 800 additional miles. Two days before the failure, I noticed a knocking or ticking sound from the engine. Immediately before the failure, I heard loud squeaking or screeching followed by a severe metallic clanking sound, as though loose metal components were being thrown around inside the engine. The engine then shut off and the vehicle became immobilized in Park at an intersection. I was the first vehicle in a left-turn lane and could not shift the Tahoe into Neutral to push or coast it out of the roadway. Heavy traffic turning toward the highway was forced to maneuver around the disabled vehicle, creating a significant risk of a collision. The outside temperature was approximately 95 degrees, with a heat index of approximately 109 degrees. My dog was with me, and it took more than an hour to arrange transportation for him to a safe location before I could return to the disabled vehicle and continue waiting for the tow truck.Read more

This is a recurring defect documented since early 2023: Hyundai's own Blue Link telematics auto-flagged an Engine Control System DTC on XXX; a jump-start was required XXX (dealer RO #XXX); check engine light and stored CAN-bus codes U100387, U100100, U004600, U010100 on 08/10/2023 (RO #XXX); and a September 2023 incident in which the center display shut off during reverse, disabling the backup camera. Hyundai performed Service Campaign 9B4 (BMS software update, DTC P1BB20, TSB 25-01-010H-1) on this VIN on XXX; the same failure category recurred 84 days later. The vehicle was towed to Safford Hyundai of [XXX] onXXX and has remained there 65+ consecutive days. I am submitting this report to notify the National Highway Traffic Safety Administration of a safety‑related defect involving the BMW N20/N26 engine timing chain assembly. My vehicle experienced a sudden and complete engine failure at highway speed due to a timing chain malfunction. This failure occurred without warning and created an immediate safety hazard for my [XXX] son, who was driving at
... the time. Loss of engine power at speed eliminated the ability to accelerate, maintain speed, or safely maneuver, exposing him and surrounding motorists to significant risk. This issue is not an isolated incident. BMW has issued internal service bulletins acknowledging premature timing chain wear and failure in N20/N26 engines. Independent technicians, consumer forums, and numerous owners have documented identical failures, often occurring well before the expected service life of the timing chain. The nature of the defect—catastrophic engine failure without warning—presents a clear safety concern rather than normal mechanical wear. BMW has declined to provide assistance, stating that “there is nothing we can do,” despite the well‑documented history of this defect and its safety implications. The failure of a known defective component at highway speed is foreseeable, preventable, and materially compromises vehicle safety. I am requesting that NHTSA investigate this recurring timing chain defect in BMW N20/N26 engines, including: The frequency of sudden engine failures Whether BMW’s internal documentation reflects awareness of a systemic safety issue Whether the defect warrants a recall or other corrective action to prevent further incidents This failure placed a young driver in a dangerous situation and could easily result in collisions, injuries, or fatalities. On July 3, 2026 the check engine light, check VSA warning and check emissions warning indicators all illuminated simultaneously and the vehicle began to misfire and sputter which triggered an immediate slow down of traffic behind me. Purchased new, Feb, 2020. Received piston ring defect recall notice (SC209). Serviced by dealer July, 2021. In 2025, noticed engine knocking noise. Car stalling out. No warning lights. At the 3000 mile mark, I pulled the dipstick. Bone dry. Drove to the dealer and told them it was very low. They sent me to the lounge and said they would pull the car in and come get me. When they didn't come, I went to check, to find a service rep putting oil in my car out in the parking lot. He claimed it was only about one quart low. Then, another consumption test at 1000 miles. Low again. They wanted to do another 1000 mile check after this. They changed the oil and told me to come back at 1000 miles. When I came back, I asked to be present to witness the oil level. They took me to the lounge, said they were pulling the car into the shop, and would come get me. Again, they pulled the stick without my presence, and told me it was full. Now, they want me to do another 4000 mile test. I took the vehicle to two independent mechanics. Their borescope inspection proved engine damage. The dealer is refusing to replace the engine under the previously completed Safety Recall Campaign SC147 (NHTSA Campaign 17V224000). The engine is currently exhibiting clear signs of connecting rod bearing failure—the exact mechanical hazard SC147 was issued to address. The dealership inspected the vehicle, confirmed internal engine failure, and verified that SC147 was completed on this VIN. How
... ever, they denied coverage citing an incomplete software update (Product Improvement Campaign PI1803) associated with a separate class-action lawsuit. The dealer is incorrectly conflating the requirements of the class-action warranty with the vehicle's independent eligibility under the safety recall. Independent of the class-action parameters, I am requesting a repair under the standalone 15-year/unlimited-mileage extended warranty provided specifically for vehicles that completed SC147 (detailed in the SC147 Safety Recall Campaign Q&A dated May 22, 2017, and Kia Warranty Bulletin 2017-09, both uploaded with this submission). The vehicle is well within this 15-year extended warranty period. Additionally, the PI1803 Knock Sensor Detection System update is a diagnostic detection tool, not a physical remedy for the underlying crankshaft machining defects identified in 17V224000. After receiving the car back, there is still a rough idle, transmission is out of sync, no diagnosis or adjustment for the auto/on off system at lights. Took the car back and has been at the dealer while they discuss/diagnosis with Audi technical. Several discussion with the service manager Chris Calhoun regarding these problems and resolution. Received an email 7/28/29 that the dealership wants to return the car after adjusting the transsmission, no determination of the root cause or adjustments to the auto on/off system, but i have escalated with Audi Customer Care Case#XXX.The vehichle has been in their shop for 3 weeks- 7-7 to 7/29. In December 2025, my check engine light came on. Diagnostic codes read P0128 (coolant thermostat) and U1467 (module communication). I took the vehicle to a mechanic, who diagnosed and repaired the issue. Shortly after, the check engine light returned, so the same mechanic replaced the ECM (plug and play). This repair did not resolve the check engine light. The mechanic recommended I take the vehic
... le to a dealership and identified a related service bulletin tied to Safety Recall N25 / NHTSA Campaign 13V-177 (three-way coolant valve). To confirm this diagnosis, I sought a second opinion from another independent mechanic. That shop purchased the coolant valve part directly from a dealership, installed it on my truck, and the same issue persisted. They also were unable to resolve it and advised me to take the vehicle to a dealership. I researched NHTSA Campaign 13V-177 (Chrysler Manufacturer Campaign N25) and confirmed that Chrysler recalled certain 2013 RAM 1500 trucks built between June 25, 2012 and December 12, 2012, because the coolant bypass valve may stick in a position that prevents coolant flow to the heater core. The official remedy for this recall is replacement of the coolant valve along with an updated module calibration, performed by a dealer. I was told by dealership staff that there is no active recall on my vehicle. However, my truck matches the affected description, and I have not had the calibration portion of the recall remedy performed, only the physical part replacement, which two independent mechanics have already completed without resolving the underlying issue. My vehicle is currently at Landmark Dodge in Morrow, GA. Despite this being an open federal safety recall (13V-177/N25) with a remedy that includes a required software calibration update, the dealership is attempting to charge me for this repair, even though I have already paid to have the physical part replaced twice by independent mechanics.Read more

I am reporting a critical Exhaust Gas Recirculation (EGR) valve failure on my 2023 Hyundai Tucson Hybrid. The failure resulted in an immediate loss of engine power, reduced vehicle performance, reduced fuel efficiency, and the vehicle entering limp mode while driving, creating a significant safety hazard in traffic. Diagnostic & Technical Details: Error Codes Captured: Diagnostic trouble codes i
... n the P0400 series (EGR system flow/circuit malfunction) were confirmed during diagnostic inspection. Root Cause: The component failure is directly linked to an internal short circuit in the EGR valve assembly, caused by solder flux contamination during manufacturing. Manufacturer Response & Escalation Denial: When contacting Hyundai regarding this failure, I was informed that my vehicle’s VIN is "not included in Safety Recall 260" (NHTSA Campaign 24V-308). I formally escalated this claim to Hyundai Corporate, but my request for reimbursement and coverage was explicitly denied on the grounds that my VIN falls outside the designated recall parameters. However, this exact failure mode, part assembly line, and manufacturing defect (solder flux contamination) are identical to the safety defect described in Recall 260 as well as Hyundai Service Campaign 9B3. Hyundai is restricting coverage based on narrow VIN manufacturing windows despite identical, defective parts being installed across non-recalled vehicles. Because a sudden EGR failure causes unexpected loss of power while traveling in traffic, and because Hyundai Corporate has refused to cover this out-of-pocket repair, I am requesting that NHTSA investigate this issue and compel Hyundai to expand the scope of Recall 260 to protect affected owners and mandate reimbursement for out-of-pocket repair costs.Read more

I am filing this complaint regarding a sudden and unexpected engine failure in my 2012 Kia Sorento LX equipped with the 2.4L Theta II engine. Vehicle Information: Year: 2012 Make/Model: Kia Sorento LX Engine: 2.4L Theta II VIN: [XXX] While driving, my vehicle experienced a sudden engine failure and lost power, leaving the vehicle inoperable. This occurred without warning. The vehicle received ro
... utine maintenance, including regular oil changes, and the failure was not due to intentional neglect or misuse. I am concerned because this vehicle is equipped with the Kia Theta II 2.4L engine family, which has been associated with previous recalls, investigations, settlements, and numerous consumer complaints involving engine seizure, premature failure, stalling, and safety concerns. I understand my specific VIN may not currently fall within an active recall; however, my vehicle has experienced the same type of catastrophic engine failure reported by other Kia owners with similar engines. I am concerned that vehicles outside current recall VIN ranges may still be experiencing the same defect. An unexpected engine failure while driving creates a serious safety concern due to the potential for sudden loss of power and increased risk to occupants. I am requesting that NHTSA review whether additional 2012 Kia Sorento vehicles equipped with the 2.4L Theta II engine may be affected and whether Kia's current recall population adequately addresses the known engine failure concerns. I have also contacted Kia requesting that they review my vehicle, authorize a no-cost diagnostic evaluation, and determine whether the engine failure is related to known Theta II engine defects. On June 23, 2025, the vehicle experienced its first sudden loss of propulsion while traveling at highway speed and was towed to Ryan Auto Mall in Buffalo, Minnesota. On January 5, 2026, the identical defect occurred again. The vehicle once again lost propulsion while traveling at highway speed and was towed to the dealership. From January 5, 2026, through May 13, 2026, my vehicle remained at the dealership for approximately four and one-half months while Stellantis and its authorized dealer attempted to diagnose and repair the recurring defect. Following that extended repair process, I was advised that the vehicle had been repaired and that the issue had been resolved. Settlement Agreement: On May 13, 2026, I executed a Release Agreement with FCA US LLC. As part of the negotiated settlement, I was required to enter into a lease for a 2026 Jeep Grand Cherokee. The only reason I agreed to sign the Release Agreement and enter into the new lease was because I relied on Stellantis' representations that the 2025 Jeep had been successfully repaired and that the dangerous defect had been permanently corrected. Had I been informed that the vehicle remained susceptible to the same failure, I would never have agreed to retain the vehicle or execute the related lease agreement.Read more

Diagnosed 07/20/2026 with catastrophic oil ring failure requiring engine replacement. Engine can stall due to oil degradation/absence of causing uncontrolled vehicle handling and fire. This would put myself and others at risk. It has been diagnosed at dealership. Complaint Narrative: The subject vehicle is experiencing its third total engine failure due to an inherent manufacturing and design def
... ect, presenting a severe and repeating highway safety hazard. The original factory engine failed catastrophically in 2015 at 116,440 miles and was replaced under NHTSA Recall 15V-568. The second engine failed in 2021 at 244,710 miles and was replaced under NHTSA Recall 17V-226. The third engine (the second replacement) has now failed prematurely at 334,935 chassis miles, lasting a mere 90,225 miles. An authorized Hyundai dealership diagnosed a total failure of the oil control rings. This mechanical defect allows engine oil to bypass the pistons directly into the combustion chambers. This causes severe, rapid oil consumption, spark plug fouling, engine stalling, and total loss of motive power at highway speeds without prior dashboard warning illumination. The manufacturer is failing to provide safe, durable remedies for its federally mandated safety recalls (see warranty info Magnuson-Moss Warranty Act (15 U.S.C. § 2301 et seq./ Breach of Implied Warranty of Merchantability). Instead, they are installing subsequent replacement powerplants that suffer from the exact same structural vulnerabilities and component lifecycle failures. This creates a continuous risk of sudden engine seizure and traffic collisions for consumers.Read more

While traveling from [XXX] to [XXX] for vacation, I was in [XXX], and when I pulled off the interstate and came to a stop sign the vehicle quit. At first, I was unable to restart the Vehicle, but after sometime I got it started and was able to pull it off the road while the engine was making very loud clattering noise and the vehicle would not stay running. Prior to this there were no warning, lig
... hts or indications that the engine was going to quit. I checked the engine oil and it was full. I had the vehicle transported to a GMC dealer in [XXX], where it was diagnosed as requiring a new engine. The vehicle is a 2026 GMC Sierra 1500 Denali purchased in December 2025 and has 8,865 miles on it. A little over 4000 miles I had the vehicle oil changed at Starling GMC Titusville Fl. At the time of my service visit, I asked the service writer, [XXX] to check the engine idle because at times it seemed to run a little rough and it quit while pulling in my driveway one day. [XXX] advised me that there is no idle adjustment on the vehicle, so I said I’ll just keep an eye on it. There were never any indications of an engine problem prior to this or even as of now there are no warnings displayed on the instrument panel. I also subscribe to onstar and it indicates everything in the vehicle is normal with no issues. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

... ating an immediate risk of a serious crash, injury, or death to me, my passengers, and other motorists. I had to pull over immediately, and the fire department responded to extinguish the fire and ensure everyone's safety. **3. Has the problem been reproduced or confirmed by a dealer or independent service center?** Yes. The Kia dealership inspected the vehicle and confirmed that the engine has failed and requires replacement. However, they attributed the failure to "engine wear and tear" despite the vehicle having only approximately 111,000 miles and having been regularly maintained according to Kia's recommended maintenance schedule. Two months before the failure, the same dealership repaired an engine oil leak, and they have been unable to explain why the engine failed and caught fire. **4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others?** The vehicle has been inspected by the Kia dealership. The local fire department responded to the vehicle fire at the scene. At this time, the vehicle has not been inspected by the manufacturer or any other parties. **5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear?** Approximately two months before the engine failure, the engine warning light illuminated due to an oil leak. I immediately took the vehicle to the Kia dealership, where they repaired the oil leak at a cost of approximately $3,000. After the repair, there were no warning lights, messages, unusual noises, or other symptoms beforeRead more
