The car at high revs at hard acceleration suddenly loses power, causing it to upshift and stop accelerating. Manufacturer said on heavy acceleration, that the intake pipe to the engine collapse upon heavy acceleration, as they are plastic and can compress and expand. They then re-attach when under less load. This is only common if accelerating hard.

The torque converter on my 2019 Nissan Titan PRO-4X failed at approximately 60,000 miles. Symptoms began as cold-start shudder lasting several minutes, progressing to pronounced shudder under light throttle acceleration between 25–50 MPH, erratic shifting at the 3-4 and 4-5 gear transitions, and TCC (torque converter clutch) slip. These symptoms created unpredictable drivetrain behavior at highw
... ay speeds, presenting a safety risk to myself and other drivers. The problem was diagnosed and confirmed by an independent service center using live PCM data. TCC slip readings of -27/748 RPM were recorded during a test drive, matching the failure criteria specified in Nissan Technical Service Bulletin NTB22-099A, which Nissan issued to address this exact torque converter defect in this model. The repair — torque converter remove and replace using OE parts — is currently underway. The failed component is available for inspection upon request. Warning symptoms (cold-start shudder and light-throttle shudder) were present for approximately one month before the failure became undriveable. Nissan was contacted regarding the known defect and declined to provide assistance, citing warranty expiration. The TSB coverage window for this defect expired approximately eight months before symptoms became acute.Read more

My 2013 Hyundai Santa Fe Sport is subject to Hyundai Recall 162 / NHTSA Recall 17V-226, concerning premature connecting-rod-bearing wear that can cause engine knocking, engine failure, and loss of motive power. My vehicle developed engine knocking and a flashing check-engine light while I was driving. The vehicle is now at an authorized Hyundai dealership. Hyundai initially denied an engine repl
... acement based on mileage. I escalated the matter to Hyundai Motor America Corporate Consumer Affairs. Hyundai has now confirmed in writing that, because my vehicle is more than eight years old and has more than 150,000 miles, Hyundai is applying the high-mileage provision of the Theta II settlement and will not replace the engine. Instead, Hyundai is offering either a repurchase or $2,000 in lieu of engine replacement. I understand that Hyundai is relying on the court-approved settlement. My concern is specifically whether Hyundai’s application of that settlement to my vehicle is consistent with the administration of Recall 162 and the federal safety-recall remedy, given that my vehicle is experiencing the engine failure that Recall 162 was designed to address. I am requesting NHTSA’s review of Hyundai’s handling of this safety-related recall matter.Read more

I have needed to replace my spark plugs multiple times because of an issue where, apparently when in eco mode, the pistons in the third cylinder flood with oil that causes the spark plugs to misfire. I was informed this is an issue common to Honda Odysseys on or about May 19th, when I was experiencing a misfiring engine and brought it to AAA Reynoldsburg on E Broad Street in Reynoldsburg, OH. Hond
... a is aware of the issue, and when I brought my car to Honda in July, after the new spark plugs started misfiring, one of the people at Honda told me the issue is because of government regulations that require lowering emissions. I don't know if that's objectively true or not, but if it's an issue Honda is aware of, as they are, they should correct the issue without charging me for the 'engine overhaul' or the new spark plugs. I have an email attachment showing the spark plug issue and other issues with the car. I don't hold Honda at fault for some of the issues, but if they know about the underlying cause that contributes to a misfire, I believe they should correct it. The video will not upload, but I am including a link to the video Honda sent me that shows the issues with the car, including the spark plugs misfiring because of one of the pistons flooding with oil. My 2023 Tahoe HC 6.2L VIN Passed the Catastrophic Engine Safety Recall stating my Engine was SAFE. The dealership performed a change in oil velocity per GM's directives. Then the engine seized upon accelerating. No warnings, just dead, yet able to coast through a large intersection on a -20F day where NOTHING worked on the vehicle. Even though the engine would not turn over when hitting the S
... tart button, it would only allow 15 minutes worth of Emergency Flashers before the sensors shut the vehicle completely down. We had to abandon the vehicle on the side of the road, with key fob in this NEW vehicle, hike to safety, and for warmth. Total mess up as how to tow the vehicle, even when I advised NOTHING (but SiriusXM) worked and could not put vehicle into Neutral. Had to wait almost a month for a Replacement Engine. Once Replacement Engine installed, I expressed how my vehicle did not feel, nor sound the same as the original. The dealership ensured that it was a 6.2L Engine Replacement, without being able to tell me where it came from, how many miles on it, or the serial number to this new engine. I was told to give it 500-1000 miles to allow Replacement Engine to "settle". Exactly 300 miles in, on the expressway, just coming out of a bottle neck slowdown, I got up to around 45-50 mph and it was like I hit an invisible wall! Vehicle lost propulsion. Seat belts locked. No brake lights since I was accelerating. Extremely thankful nobody was behind me. Obviously let go of the gas. Pause. Then I was able to drive the vehicle to a safe location off of the expressway, and shut it down. Immediately called dealer. Restarted, drove about 15 miles to safety, went to shut off and it would NOT turn off. Sounded as if trying to start it while already running. 20 min to turn off. 5 months at dealer, they could not mimic/see any issues, even though I could "feel" the loss of propulsion at lower speeds. The check-engine light came on my 2nd day of ownership (less than 100 miles driven by me, but 2032 miles on it when purchased new - RV conversion from Entegra Coach through LaMesa RV), on October 5, 2025. After diagnosis at a Ram dealership (Larry Miller Dodge Ram in Tucson AZ), they identified the code was misfire on cylinder 6, which caused low compression, with a root cause of the intake valve
... s being improperly seated during manufacturing. The dealership replaced some cylinder and engine parts, but the issue recurred on Cylinder 4 after driving it about 5000+ miles, on June 13, 2026. Again, misfire check-engine code and loss of compression on cylinder 4. The engine ran very rough and had loss of power. The engine is still under investigation by another Ram dealership service department (Crest Auto World, Center Conway, NH). I have learned that loss of compression can cause the engine to stall, lose acceleration power (which it did), and potentially throw a rod and blow a hole in the engine. Since this was caused by a manufacturing defect, I am also concerned that there are other vans with this issue on the road.Read more

The vehicle experienced a catastrophic internal engine failure caused by connecting rod bearing failure. Prior to the failure, the engine developed abnormal internal knocking/noise, indicating bearing deterioration. The engine is currently available for inspection. While traveling approximately 55 mph on [XXX], the check engine warning light illuminated. Within about one mile, the engine rapidly
... lost power and developed a severe knocking noise. The sudden loss of power at highway speed created a serious safety hazard by reducing my ability to maintain traffic speed and safely maneuver the vehicle. I had my child in the vehicle, and both we and surrounding motorists were placed at risk of a collision while I attempted to move to exit traffic. The failure has been confirmed by an independent repair facility, Absolute Automotive LLC in Parker, Colorado. Their inspection determined the engine suffered internal damage due to connecting rod bearing failure. I have the engine replacement invoice, technician's diagnostic notes, and photographs documenting the engine damage. The vehicle has not been inspected by Honda. I contacted Honda to report the failure and request assistance, but I was informed there was no recall for the 2021 Honda Passport equipped with the 3.5L V6 engine and that Honda would not provide assistance unless a recall existed. Before the engine failed completely, the engine developed abnormal internal knocking/noise. While driving on [XXX], the check engine warning light illuminated, and within approximately one mile the engine lost power and produced a heavy knocking noise, rendering the vehicle unsafe to operate. I am submitting this complaint because a sudden internal engine failure resulting in loss of power at highway speeds presents a significant safety hazard and should be investigated. I was practicing backing up while turning my new approximately 3,000 lb equipment trailer for safety & maneuvering skill purposes. There was no payload in my trailer that I was towing. While slowly maneuvering & turning my equipment trailer in reverse at 1–3 mph, our 2023 Ford F-150 XLT suffered a commanded, abrupt, unexpected sudden acceleration while in reverse, which was not via the gas pe
... dal or my input. The trailer violently jackknifed, made a loud noise on impact to the truck resulting in damage to the truck bed’s right quarter panel at the rear, and a broken right tail light. It happened so fast I had to slam on the break hard, since the truck felt like it would keep backing into the jackknife position with powertrain seeming to struggle to overcome the break for a short instant. I believed at that instant that a mechanical or software or both malfunction caused this since this happened so fast and abruptly. Also to note, the parking break has often turned on or off automatically both during this incident and prior to, both while the vehicle was moving motionless. After looking at the damage, it first appeared to me to be less than a $1,000.00 repair bill, since only a small portion of the right rear corner of the fender was dented and the tail light broke. On Monday and Tuesday however I was surprised to learn after getting estimates that the damage is around $4,000 or more, since the entire quarter panel needs to be replaced - aluminum Quarter panels cannot be welded, and it’s is an 8 foot bed with a huge quarter panel and lots of painting required. I have learned Ford Motor Technical Service Bulletin’s TSB 24-2070 and TSB 24-2176, or possibly other TSB(s) could be the cause. No other vehicles were involved. I am submitting this complaint because I believe my Volvo electric vehicle has a serious and recurring safety-related defect that has not been permanently repaired despite multiple repair attempts. Since purchasing the vehicle new, it has repeatedly failed to operate as intended, left me stranded multiple times, and continues to display warning messages while driving. I purchased the vehicle new
... on November 20, 2025, for approximately $85,000 with zero miles. The first failure occurred on December 4, 2025, at approximately 578 miles when the vehicle stopped charging. Volvo attempted repairs, but the same charging defect returned. The vehicle required additional repairs on January 21, 2026 (1,304 miles), early March 2026 (2,846 miles), and April 27, 2026 (3,810 miles). Despite multiple repairs and component replacements, the same issue continued. By the fourth repair, the vehicle had been out of service approximately 80 days. Volvo later approved a manufacturer repurchase; however, while waiting for the repurchase process, the vehicle developed additional failures where it would completely lose power and become inoperable. The vehicle required towing on multiple occasions, leaving me stranded and without transportation. During a later repair visit, Volvo kept the vehicle for approximately 39 days and stated they could not duplicate the problem and that the vehicle was operating as designed. The day after receiving the vehicle back, the same failure occurred again. The vehicle has now required seven repair attempts. Even after the latest repair, I continue to receive dashboard warnings while driving instructing me to stop the vehicle. Due to the repeated failures, loss of vehicle operation, and unresolved warnings, I have lost confidence in the safety and reliability of this vehicle and have changed my driving habits to avoid being stranded. I believe these recurring failures represent a potential safety defect that warrants investigation by NHTSA.Read more

Our 2019 Ford Escape began shaking upon starting the vehicle. We took it in for diagnostics to the Ford Dealership. This car has less than 40k miles on it after 7 years of ownership. We were told we would need to completely replace the engine due to a coolant leak. Upon further research, a recall for our exact make and model was identified that described the exact same issue that was identifi

The dealership has not been helpful at all in putting me in contact with a Ford Representative that can tell me WHY my particular vehicle did not qualify for the assistance program. After contacting Ford corporate (or tried several times to get someone on the phone instead of a automatic prompt system) I was told that they would send my complaint to the Auto Safety Office BUT they could NOT guarantee I would receive a call back. NOW, the Ford dealership is saying that we have been DENIED assistance and that our case is with the Auto Safety Office and they will not be able to provide an update about our complaint. During this time, the dealership provided us with a loaner vehicle while trying to address the issue. They are now demanding that we return the loaner vehicle. This is also the dealership that services the 2019 Ford Escape in question. It was recalled previously for improper bushings which we complied with the recall and had it repaired accordingly. For an American made vehicle with less than 40k miles this is incredibly disappointing that Ford is unwilling to assist in the complete and total engine replacement that is being offered to others with the EXACT SAME issues that FORD identified. On or around XXX a my 2021 Silverado started making a rattling sound from the engine. The truck began to stall and the engine shut off three times while driving. I attached scanner the next day and the track showed P00011 and P00016 codes. Those codes are for the Camshaft Sensor and the Crankshaft Sensor. I went online and did research about the two codes and discovered there had been a couple re
... calls for these codes concerning my year model truck: (Recall Number 25V-274; GM: Campain N252494000). NHTSA Engineering Analysis EA25007 or RQ26001 I spoke with the Dealer I purchased vehicle from (Lynn Layton Chevrolet - Decatur, Al)and they said they will not begin repairs until they receive payment from GM or myself. I then spoke with Tyler at GM Corporate Customer Service. He said my VIN was not included in the GM Campaign|Recall. I asked why not because it is a 2021 Silverado and I’m having the exact same problems as vehicles included in the recall. He said because they checked their system and the parts used on my truck were from a different manufacturer than those included on the recall. I said that shouldn’t matter because my truck is having the same exact problems. I asked him to check and see if GM would be able to provide financial assistance to have motor repaired or replaced. Tyler told me that GM will not do repairs - they will only replace the engine. He said GM would not provide any financial assistance because my truck has 170k plus miles on it. I informed Him that I had previously taken my truck to Lynn Layton Chevrolet about these symptoms and it was diagnosed as transmission/torque converter problem. They changed transmission fluid and charged me ~$700. I then asked if GM would authorize a “Goodwill Replacement”. He said they would not. I called Lynn Layton Decatur back and asked how much would engine and reps if I have to pay for it. They said 10k -12k A local mechanic said he will fix for $6,000. Please help me get this resolved. I was driving to work on a normal day under normal circumstances when a check engine light won my dashboard. Seconds later my car started showing car is over heating. I immediately swayed from the speed lane to the side of the road almost getting into a crash as other vehicles were at full speeds behind me. I immediately called dealership and made an appointment. My vehicle experienced a prem
... ature failure of the cooling coolant valve, a critical component of the engine cooling system. This failure resulted in diagnostic trouble codes, warning messages, and concerns regarding the reliability of the vehicle. A component of this importance should not fail on a vehicle that is only three years old and has been properly maintained. When I brought the vehicle to an authorized Hyundai dealership, I was required to pay a $248 diagnostic fee simply to identify the failed component. After the diagnosis, I was presented with an estimated repair cost of approximately $1,600 to replace the defective coolant valve and associated parts. I believe these charges are unreasonable because the failure appears to involve a defective component rather than normal wear and tear. Consumers should not be expected to pay significant diagnostic and repair costs for a major cooling system component that fails prematurely. I have also learned that Hyundai has issued recalls and/or service campaigns affecting certain other vehicles for cooling system-related issues. However, my vehicle is apparently not included in those actions, despite experiencing a similar premature failure. While I understand that recalls apply only to specific vehicles and conditions, I respectfully request that investigators determine whether the defect affecting my vehicle is related to those existing actions or whether additional vehicles, including my 2023 Hyundai Kona, should be evaluated for expanded coverage. My research also indicates that other Hyundai Kona owners have reported similar coolant valve failRead more

I am reporting a catastrophic engine failure and engine-compartment fire involving my 2017 Lincoln MKZ Select, equipped with a 2.0L EcoBoost engine, VIN [XXX]. The vehicle had approximately 99,864 miles when the final failure occurred. Shortly after I purchased the vehicle, the original engine developed severe overheating, loss of cabin heat, white exhaust smoke, misfiring, and coolant intrusion.
... On December 16, 2024, at approximately 95,354 miles, the vehicle was taken to Murgado Ford Lincoln of Chicago. The dealership documented diagnostic codes P0303 and P0316 and determined that coolant was entering cylinder No. 3. Pursuant to Ford Technical Service Bulletin 22-2229, the dealership replaced the long-block engine and related one-time-use components. The replacement service-engine assembly was identified on the repair invoice as part No. J2GZ-6006-E. The vehicle was returned to me on approximately January 30, 2025. After the engine replacement, the vehicle continued to experience serious problems. Within days, it stalled in an intersection, produced white smoke, lost cabin heat, and had to be returned to the dealership. The vehicle later continued to exhibit hesitation, harsh shifting, loss of power, and other abnormal drivability symptoms. On September 4, 2025, while I was operating the vehicle under normal driving conditions in Chicago, I heard a sudden loud metallic grinding or mechanical noise coming from the engine area. Gray smoke then began coming from the vehicle and quickly progressed into heavy dark smoke. A fire developed in the engine compartment, forcing me to stop and exit the vehicle. The fire rapidly intensified and was followed by an explosion or explosive sound. There was no collision that initiated this incident. The vehicle sustained catastrophic fire damage and was rendered a total loss. The failure occurred only approximately seven months after the replacement long-block engine was installed and after I had repeatedly experien INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

Subject: Vehicle Safety Incident Report – Fuel Pump Disconnection and Oil Leak To: National Highway Traffic Safety Administration Date: July 27, 2026 I am submitting this report to detail a recent vehicle malfunction that posed significant risks to my safety and that of others on the road. While driving to work, I experienced a situation where my vehicle was unable to shift gears properly or
... accelerate as expected. This loss of acceleration and responsiveness created substantial control problems, making it difficult to maneuver safely in traffic. Notably, no dashboard warning lights were illuminated at the time, which gave me the false impression that the vehicle was operating correctly. Initially, I perceived a dragging sensation in the car, which I believed was manageable. However, the situation quickly escalated when I observed smoke coming from the vehicle. The presence of smoke not only impaired my visibility but also posed a serious risk of fire or engine failure. The danger was heightened as I attempted to drive uphill; the car struggled even more, increasing the potential for an accident or collision due to impaired maneuverability. Following this incident, I had my vehicle towed to an auto repair shop for a diagnostic check. The diagnostic report revealed that the fuel pump was not attached to anything and that oil was leaking into the pan. These findings confirm that the issues were real and not just perceived. The disconnected fuel pump and oil leak represent systemic failures, which could have had catastrophic consequences if not addressed promptly. In conclusion, this experience highlighted grave risks to both personal and public safety. I urge vehicle manufacturers and regulatory bodies to prioritize detecting and preventing such malfunctions.Read more

I am reporting what I believe is a design defect involving the Secondary Air Injection (SAI) System on my 2014 Toyota 4Runner. My vehicle illuminated the Check Engine Light and stored Diagnostic Trouble Code P2440 (Secondary Air Injection System Switching Valve Stuck Open). During my inspection, I found that the factory-installed foam air pump filter had completely deteriorated and was missing. T
... he filter appears to have disintegrated over time rather than being removed during maintenance. The filter is intended to prevent contaminants from entering the Secondary Air Injection pump and switching valves. Once it deteriorates, debris can enter the system, potentially contaminating the switching valve and causing the P2440 fault. This failure caused my vehicle to enter reduced-power ("limp") mode, resulting in a significant loss of engine power and acceleration. This created a safety hazard because the vehicle could not accelerate normally while driving, increasing the risk when merging into traffic, crossing intersections, or traveling at highway speeds. Toyota has discontinued the original replacement foam filter, preventing owners from restoring the system with the factory-designed component. Numerous other owners have reported similar failures involving deteriorated or missing foam filters, P2440/P2442 diagnostic codes, and costly Secondary Air Injection System repairs. I respectfully request that NHTSA investigate whether deterioration of this factory foam filter is a design or material defect that allows contamination of the Secondary Air Injection System and can lead to limp mode and reduced engine performance. If confirmed, Toyota should provide an appropriate remedy, such as redesigned components or an extended warranty for affected vehicles. This failure appears to result from deterioration of an original factory component rather than improper maintenance or normal wear and presents a potential safety concern for other Toyota 4Runner owners.Read more

I first experienced an engine misfire in February 2025. The vehicle had a P0301 misfire code, and at the time I was unaware of Honda’s extended warranty related to this issue. We had a trusted mechanic friend inspect the vehicle and replace the spark plugs and ignition coils. He noted that there was significant carbon buildup. After those repairs, the vehicle appeared to run normally, and we bel
... ieved the issue had been resolved. Later, I received Honda’s extended warranty notice and realized that the issue I experienced may have been related to the condition covered by the warranty. I contacted my local Honda dealership in August 2025 and explained the prior misfire, repairs performed, and the warranty notice I had received. However, because the check engine light was not currently illuminated, I was told there was nothing that could be done unless the vehicle was actively displaying the issue. I also contacted Honda Corporate regarding this matter. I was advised that reimbursement for some of the repairs I had already completed might be possible, but that it would need to be reviewed. I chose not to submit those receipts at the time because I was concerned that doing so might impact my ability to use the extended warranty coverage if the problem returned. Unfortunately, the issue appears to be progressing. On June 2, 2026, the vehicle set a P3400 code, which is related to the Variable Cylinder Management (VCM) system. At that time, the oil level was low. After adding oil and completing several driving cycles, the check engine light turned off. However, the vehicle continues to consume oil at an alarming rate. I have had to add additional oil again in July. The problem is progressing - it is the issue that should be covered by warranty but holds refuses to do anything because the light isn’t currently on,Read more

Vehicle suddenly slipped into limp mode. Vehicle was then immediately taken to have code read. Code came back P1326. Was advised due to previous recall campaign(s) to contact Hyundai of America for guidance. Was told that vehicle needed to be confirmed per the BCT at any Hyundai dealership to which the agent scheduled the appointment myself and that i should get the other 4 recalls fixed as well.
... My spouse drove the vehicle there and that was our last true communication verbally with Hyundai of America. Vehicle barely made it to the dealership and then spouse was offered a ride home an hour and a half later without paperwork or any type of understanding other than it would take time for the service team to get it on the BCT. A week later I was told it would cost $250. We have argued that we were never told it would cost anything as it relates to this major recall nor the others and in fact it would be performed at zero cost. Then, a service manager stated that now the vehicle needed $10k worth of work and began having finance firms contact me that he said were employed by Hyundai but are not. Here we are a month later, and still no vehicle and we have contacted Hyundai of America numerous times and have all communications regarding such contact and have gotten nowhere. We have yet to actually speak to someone over the phone. We were assigned a case manager who is now on vacation. We're without an income now because after not having transportation to work my partner has lost his job, and we are in dire condition. Inside of my vehicle that this dealership is holding hostage is my infant’s car seat and a brand new catalytic converter. They have refused to communicate with me and supposedly with this case manager according to her emails. We are in need of help.Read more
