On 7/16/2026 exactly 1 week after i purchase the vehicle after fueling i drove about 1 mile when the car started to misfire, when attempting to accelerate. Check engine light flashes a few times and then went off. I then attempted to reverse into my driveway and when attempting to do so the vehicle shut off completely. But started right back up. I then called the dealership and was asked to bring
... it in. The dealership has the vehicle about 2 days and said it has been repaired it had bad spark plugs. When to get the car it drove fine with no issues until 7/28/2026 when i made an attempt to drive to work, i had driven about a mile from my home and car to a red light when the light turned green i attempted to accelerate and the vehicle was hesitate. Then a few warning lights came on that showed issues with electronic throttle control and electronic stability system service. The car would not accelerate over 20 mph and would shake when i attempted to press on the accelerator, the car was then towed to the dealership and much it has been at since 7/28. I received a online request for service approval from the tech on 7/29 in which they stated cylinders 1 &4 were showing misfired and they would look into replacing the spark plugs but that they would continue to work on the car. I then received a message from my service advisor on 7/31 that stated the spark plugs were replaced but the vehicle was still having issues, i should wait until 8/3 for an update. I have not even had the vehicle for 30 days and it’s been in the shop twice for the same issue with about 1,000 miles on it.Read more

This vehicle is subject to a known Theta II connecting rod bearing defect (Recall 162 / KSDS) and a related class action settlement. On October 2, 2025, the vehicle stalled while entering the highway. It was taken to Ken Garff Hyundai Surprise, and the following day, October 3, 2025, it entered engine protection mode (limp mode) during the dealership's own test drive, the exact behavior the Knock
... Sensor Detection System is designed to trigger for connecting rod bearing wear. The dealership's own service advisor documented in writing, as early as December 27, 2025, that he did not like how the vehicle ran and drove and wanted to resubmit a warranty claim. Despite this documented concern, on May 5, 2026, the dealership's service manager told Hyundai corporate that nothing was wrong with the vehicle and that it could be picked up. Hyundai corporate maintained that the engine had passed bearing clearance testing on two occasions, October 3, 2025 and May 23, 2026. I was told the vehicle was safe and retrieved it on June 29, 2026, out of necessity after my family's other vehicle was totaled in an accident. Approximately ten days later, the engine failed completely while I was driving with my family in the vehicle. Hyundai's own repair order documented the crankshaft as seized at over 95 lb-ft of torque. Hyundai subsequently approved and completed a full engine replacement. I am filing this report because a vehicle with a documented, known defect and a documented internal safety concern was represented to me as safe to drive, and it failed catastrophically with my family inside it within days of that representationRead more

On XXX, I was driving northbound on US-101 from [XXX] to [XXX] to pick up a family member for an appointment. The vehicle had been operating normally at freeway speed. As traffic slowed for a construction zone near Summerland, I intentionally reduced speed to approximately 20 mph. At that point, I heard a sudden abnormal rotational mechanical noise from the engine compartment, best described as a
... flapping or rubbing sound. The noise had not been present earlier during the drive. Within moments, the engine lost power and shut down. The vehicle lost propulsion while in traffic, and power steering assist was lost after the engine stopped. I was able to safely maneuver the vehicle off the roadway. The engine would not restart. Before this incident, I did not observe any meaningful warning of an impending catastrophic engine failure. I did not observe an engine overheating warning, an oil-pressure warning, or any other dashboard warning indicating an imminent engine seizure. Before purchasing the vehicle, and during my ownership, I used Ross-Tech VCDS (Audi/VW-specific diagnostic software) as part of proactive maintenance and monitoring. Nothing identified an impending catastrophic engine failure. The vehicle was transported to an independent European repair facility for inspection. The shop determined that the engine is mechanically seized and advised that the precise initiating cause cannot be determined without engine disassembly. An Audi dealership likewise indicated that additional manufacturer-level diagnostic procedures would be required. A case was created, and Audi declined assistance. I am submitting this complaint because the engine failed suddenly during normal operation without a meaningful advance warning, resulting in an immediate loss of propulsion in traffic. I believe this type of unexpected catastrophic engine failure presents a potential safety concern if it occurs while the vehicle is being operated on a public roadway. I am writing to formally report a serious and recurring safety defect with my 2025 Acura MDX A-Spec (VIN [XXX]) involving the Auto Idle Stop system. The engine repeatedly fails to restart after the Auto Idle Stop feature shuts it off. When my wife releases the brake pedal (for example, at a traffic light), the vehicle either stalls completely or experiences a major delay before restarting sometim
... es lasting more than 20 seconds, leaving the my wife unable to accelerate. This creates an immediate and unacceptable safety hazard, particularly in intersections or heavy traffic. I previously brought the vehicle to Acura of Langley regarding this exact issue. Despite that visit, the problem has continued. Most recently, on today, while my wife was driving with my kids, the vehicle again entered Auto Idle Stop at a traffic light. When the light turned green, the engine completely shut off and would not restart for more than 20 seconds. This left her stranded in traffic and unable to move the vehicle, creating a clear risk of collision. This is not an isolated complaint. The National Highway Traffic Safety Administration (NHTSA) has opened Engineering Analysis EA25004 into precisely this condition “No Restart After Auto Start/Stop Engages” affecting, among other models, 2016–2025 Acura MDX vehicles. American Honda has also issued dealer communications specifically seeking 2023–2025 MDX vehicles with client complaints of no auto re-start after Auto Idle Stop for inspection prior to any repair attempts. Class-action investigations and prior litigation involving related Idle Stop failures further underscore the known nature and seriousness of this defect. Car purchased July 4, 2024. Idle stop/start was working but after several months, it became defective. After driving for some time, idle stop/start wouldn't work properly and cut the engine off when idling. I would have to turn the car, open my door, and try to restart the car. Component Involved:Engine Mechanical / Variable-Displacement Engine Oil Pump (Part #11418635032 / Updated #11419895359) Description of Safety Hazard / Defect:The early-generation B58TU engine features a variable-displacement oil pump equipped with an internal thermoplastic (plastic) adjusting ring. Due to continuous thermal cycling and mechanical stress, this plastic ring is prone to cracking and
... shattering without warning. When this internal component disintegrates, it causes an immediate loss of oil pressure to critical engine components. This creates a severe safety hazard, as the vehicle can experience a sudden loss of motive power or complete engine seizure while travelling at highway speeds, placing the occupants and surrounding traffic at immediate risk of a collision.Furthermore, a known software bug or sensor interference tied to this specific mechanical failure prevents the iDrive system from successfully completing an electronic oil level measurement, thereby masking the impending failure from the driver until catastrophic engine failure occurs.Relevance of International Actions:In May 2026, BMW Korea officially acknowledged this identical component design flaw by launching a proactive, free preventative repair campaign for B58TU engines manufactured between January 2019 and June 2021. BMW is actively replacing the defective plastic-ring oil pumps with an updated, all-metal version in that market.North American consumers are driving vehicles equipped with the exact same defective part, manufactured during the same window, and facing the same catastrophic safety risks. I am requesting that the agency open a formal defect investigation into BMW North America to mandate a safety recall or an extended component warranty program.Read more

My 2017 Audi Q7 with the 3.0T engine has experienced repeated engine/ignition system failures. The vehicle displays a "System Malfunction" warning light accompanied by shaking and stalling. This same malfunction has occurred at least 3 separate times over the past few years. Each time, a repair shop diagnosed and replaced faulty ignition coils, and the vehicle ran normally for a period of time bef
... ore the identical malfunction returned. The vehicle is currently inoperable due to the most recent occurrence. This creates a safety risk because the stalling and shaking can occur while driving, with no clear warning beyond the malfunction light appearing at the same time as the symptoms begin. A stall or loss of power while in motion, especially in traffic or on a highway, could lead to a collision. The problem has been confirmed and repaired multiple times by an independent European auto repair specialist, who stated this is a known, recurring issue with this engine/model. The vehicle has not yet been inspected by the manufacturer directly. Separately, the vehicle has also developed excessive oil consumption, requiring approximately 2 gallons of oil added roughly every 6 weeks, with the frequency increasing over the last 2 months. I am uncertain whether the oil consumption and the recurring coil/ignition failures share a common root cause, but both issues are ongoing and unresolved. The vehicle and its components are available for inspection upon request.Read more

2023 Cadillac Escalade, 6.2L V8 (RPO L87). Subject of Recall 25V-274 and NHTSA investigation RQ26-001. Over the past year, this vehicle has been out of the shop only about 90 days total. The rest of the year it has been in dealer service at GM/Cadillac dealerships for a recurring, undiagnosed loss-of-power defect. It is currently in dealer service. FAILURE: The vehicle has suddenly lost all moti
... ve power on three separate occasions. Engine runs, vehicle loses all drive power, will not restart. Two events occurred at approximately 80 mph on an interstate highway. One occurred with children in the vehicle. The only warning is a “service rear axle” message that appears just before power loss. No diagnostic trouble code has isolated the fault. Root cause has not been diagnosed. REPAIR PATTERN: Six dealer visits across two Cadillac dealerships in one year. Interventions have included two battery replacements, a full engine replacement under Recall 25V-274, an approximately $1,500 charge categorized as diagnostic with no repair labor documented, and most recently a “wiring harness manipulation / re-wrap” performed over approximately 90 days. Dealer personnel described the fix as “manipulation fixed it” — which is not a diagnosed repair. On at least one return of the vehicle, dealer personnel stated in the presence of a witness that they were not certain the defect had been fixed. The “service rear axle” warning returned within three days of the most recent repair. POST-RECALL: The engine was replaced under Recall 25V-274 and the loss-of-power failures continued afterward. This is directly relevant to RQ26-001, examining post-recall-remedy failures.Read more

On or about XXX I brought my vehicle into the dealership as my 2020 Hyundai palisade suffered a severe, sudden engine malfunction while driving at highway speeds. The vehicle began shaking violently, losing power, and making loud mechanical noises forcing me to navigate dangerous highway traffic to get it to the dealership. Prior to the failure, in XXX, the vehicle was brought into an authorized d
... ealership as the vehicle would not drive properly, very slow acceleration and the vehicle feeling like it was stalling causing a danger to the road. . The dealership performed an inspection on the vehicle and diagnosed "carbon buildup due to mileage and age," they performed an authorized emissions software update, changed the oil, and performed a paid fuel injection cleaning. The dealership did not issue a safety warning or advise against driving the vehicle long distance back to [XXX]. A few months after this service, the vehicle suffered total engine failure. Professional teardown of the Hyundai Lambbda ll V6 engine show that unresolved carbon buildup and software defects lead to piston ring failure, severe misfires, oil starvation, and sudden loss not motive power. Operating a vehicle experiencing severe mechanical shaking and power loss at highway speeds poses an immediate risk of an accident. This is what I wrote to Stellantis. Dear Stellantis Customer Care, My vehicle has now stalled five separate times while I was making a left turn through an intersection. When this happens, the engine shuts off unexpectedly, the vehicle shifts into park and I must attempt to restart it while sitting in the roadway. During the most recent incident, the vehicle would not restart on the first a
... ttempt and required a second attempt before it would run again. This creates an extremely dangerous situation. Losing power in the middle of a left turn places me, my passengers and other motorist at risk of a serious accident. The vehicle has approximately 43,000 miles and has been taken to my Chrysler dealership for diagnosis. Unfortunately, I have repeatedly been told that they cannot duplicate the problem and therefore cannot repair it. Despite those visits, the dangerous condition continues to occur. I am requesting that Stellantis immediately: • Open a formal case regarding the safety issue. • Assign a regional field engineer, or star technical specialist to inspect and diagnose my vehicle. • Work directly with my dealership to determine the cause of this intermittent stalling condition. • Advise me whether the vehicle is safe to drive while this issue remains unresolved. I have been a responsible owner and have sought assistance through the dealership, but I cannot continue driving a vehicle that unexpectedly shuts off in the middle of an intersection. I am asking Stellantis to take this matter seriously, and provide a timely resolution before someone is seriously injured. Please contact me as soon as possible regarding the next steps. I would appreciate receiving a case number and the name of the representative assigned to assist with this matter. I intend to file a safety complaint with NHTSA regarding this matter. I am submitting this complaint regarding my **2025 Acura MDX**, which I currently lease. The vehicle has an ongoing starting system malfunction that has not been resolved despite multiple service visits. I believe this issue presents a significant safety risk. The primary issue involves the **Auto Idle Stop/Auto Start-Stop system**. When the vehicle automatically shuts off at a red light, stop si
... gn, or while idling, it will occasionally fail to restart when I release the brake or attempt to accelerate. Instead of restarting normally, the vehicle hesitates, stutters, or remains off. This creates a dangerous situation because the vehicle is unexpectedly disabled while in traffic, increasing the risk of being rear-ended or causing an accident. In addition to the Auto Idle Stop issue, the vehicle also has problems during the initial startup. When I attempt to start the vehicle using the push-button ignition, it frequently fails to start on the first attempt. I often have to press the start button a second time before the engine starts, and even then, the engine stutters or hesitates before finally turning over. This should not occur in a brand-new 2025 vehicle. I have taken the vehicle to the dealership multiple times for this issue: * During the first visit, I was told the battery was the cause. * During the second visit, the starter was replaced. * After the problem continued, I returned again. The dealership tested both the battery and the replacement starter and advised that both components were operating properly. * The dealership then opened a case with Honda/Acura engineering for further investigation. Despite these repeated repair attempts, the issue remains unresolved. I have been told that it is likely a software issue and that engineers are "looking into it." In the meantime, I was instructed to simply disable the Auto Idle Stop feature every time I start the vehicle. This is not an acceptable solution. Problem was diagnosed as a starter pulling excessive voltage causing hard starts and failure of the auto start/stop system. The starter was replaced under warranty. Vehicle is just over a year old with 10k miles. I was lucky there wasn't an accident or any injuries but with cars swerving by me while I blocked the intersection before the police arrived, it could easily have been much worse. Even after the system was repaired, I do not feel safe driving with the automatic start/stop feature engaged. I know I can turn it off every time I get in the car, but it is too easy to forget to do that. I would like for Honda to reprogram the system to allow the driver to permanently disengage the auto stop feature, but I'm told that is not currently an option.Read more

Starting problem. The vehicle sometimes (randomly) refuses to start after shutting off. For example, after filling gas, or turning off (idle off) at a stop light. It just refuses to start. Terrifying. Researched on the Internet and found thousands of complaints about this going back years. This needs a recall.

I live in [XXX], where summer temperatures are extremely high. This week, afternoon temperatures have been reaching nearly 100°F (38°C). My vehicle is a mild-hybrid Volvo XC40, and there is no option to permanently disable the automatic start-stop function (or at least every time I start the car). This week, while driving home from work at around 4:00 PM, I stopped at a traffic light and the eng
... ine shut off automatically. As a result, the air conditioning immediately reduced its cooling performance, making the cabin noticeably less comfortable in the extreme heat. I instinctively pressed the accelerator to turn the engine back on and I nearly collided with the vehicle in front of me. In my opinion, this creates a genuine safety concern. Volvo should provide drivers with the ability to disable the automatic start-stop feature through a software setting. While the system is intended to reduce emissions and improve fuel economy, drivers should be able to decide when those benefits are appropriate and when safety, comfort, or local driving conditions make continuous engine operation the better choice. A permanent on/off setting would allow owners to use the feature when desired while avoiding situations where the automatic engine shutdown negatively impacts driving safety or cabin comfort. I am reporting a severe, unresolved safety hazard involving my 2023 Lexus IS350]. The vehicle is continuously flashing 'Braking System Malfunction,' 'Crash Light,' and 'Engine Light' alerts on the dashboard. This is the second major acceleration and braking system failure since purchasing the vehicle brand new.The authorized dealership, Meade Lexus, has repeatedly failed to properly diagnose or re
... pair this critical braking issue over a 7-month period. On multiple service visits, the dealership documented that they simply 'lubed' the rubber around the gas cap, reset the dashboard warning lights, and returned the vehicle to me without performing actual mechanical repairs to the braking system. The service manager explicitly instructed me that I must drive the vehicle back to the dealership every single week just to have the safety lights turned off manually. Driving a vehicle with an active, un-diagnosed braking and crash-system fault is a major safety liability and makes the vehicle dangerous to operate. The manufacturer and dealer have failed to resolve this safety defect under warranty."Read more

COMPONENT/SYSTEM: Air Conditioning (HVAC) / Climate Control System and closed-loop refrigerant lines. The vehicle and all climate control components are available for immediate inspection. SAFETY RISK: Total loss of cooling in peak summer heat (90°F-95°F+) causes cabin temperatures to rapidly exceed 110°F. This sudden heat trapped in the vehicle caused me (the driver) acute heat exhaustion, cu
... lminating in a medical fainting episode. The vehicle regularly transports my newborn baby and toddler, subjecting them to severe heatstroke risks. Additionally, an ongoing 4-year chemical refrigerant leak inside an enclosed EV cabin creates an active toxic inhalation hazard for all passengers. DEALER CONFIRMATION: Yes. The failure has been repeatedly confirmed and documented by authorized Hyundai dealerships, including Paramus Hyundai, Garvey Hyundai, and Liberty Hyundai. Technicians repeatedly measured zero refrigerant charge/pressure, performed UV dye tests, and completed major repairs including AC line and radiator rebuilds without resolving the leak. INSPECTION HISTORY: Yes. Inspected multiple times by authorized Hyundai dealership technicians under official Corporate Case Nos #42549212, #41020884, and #23781133. WARNING LAMPS/SYMPTOMS: No dashboard warning lamps or error messages ever appeared. The only symptom was the AC suddenly blowing boiling hot air as refrigerant leaked out. This failure first appeared in Summer 2022 (shortly after vehicle purchase) and has recurred every summer for 4 consecutive years despite repeated warranty repair attempts.Read more

My car started experiencing a loss of power and began to shake a lot when I was parked. When they put the scanner on it, it said that cylinders number 2 and 3 had faults, so they replaced them. But after a few days the same message returned, so they think it's the injectors. In addition to all that, the problem started after the fuel pump was replaced at the Honda dealership in Lancaster, MA.

I own a 2019 Mazda CX-5 AWD equipped with the 2.5L naturally aspirated SKYACTIV-G engine with cylinder deactivation. At approximately 70,491 miles, the vehicle began leaking and burning engine oil, accompanied by a strong burning-oil odor and visible white smoke from the engine bay. This began on or around 7/15/2026 I took the vehicle to an authorized Mazda dealer, which confirmed that the cylind
... er head is cracked and leaking engine oil. I was quoted approximately $10,000 to repair. The repair is not covered because the vehicle is just outside the factory warranty. I received regular Mazda dealership maintenance through 52,000 miles, then regular 3rd party shops after. The oil is leaking onto/near the exhaust manifold. Oil contacting the hot exhaust creates a serious fire hazard, which I can detect from the burning-oil odor and smoke. Separately, ongoing oil loss risks sudden engine failure or stalling while driving, which could cause a loss of motive power and a crash, especially at highway speed or in traffic. This appears to be a widespread manufacturing defect. Many owners of 2018–2021 Mazda CX-5, and related Mazda3 and Mazda6 vehicles equipped with the 2.5L naturally aspirated engine with cylinder deactivation, have reported the identical failure — cylinder head cracking near the exhaust manifold causing an oil leak — frequently at low mileage. Mazda issued an internal service alert (SA-058/20) addressing cylinder head cracking on these engines, which shows the manufacturer is aware of the defect. Despite this, affected non-turbo owners are being denied repair assistance and are paying thousands of dollars out of pocket to fix a defect that presents a fire and crash risk. No crash, fire, or injury has occurred. I am reporting this because the defect is safety-related, appears to affect a large number of identically built vehicles, and warrants investigation and a potential recall to protect owners — especially those outside the warranty period.Read more
