While driving on the freeway at approximately 65–70 mph, the engine suddenly stopped responding to all accelerator pedal inputs. The vehicle experienced an immediate, dangerous loss of motive power in active traffic. The dashboard displayed an amber check engine light alongside the central master warning light (an amber triangle with an exclamation point inside). I was forced to coast across act
... ive lanes of highway traffic to reach the right-hand shoulder. A State Trooper was on the shoulder and approached me, but there was no formal report. After turning the vehicle completely off and waiting, the system temporarily reset, and throttle response returned. The following day, the exact same sudden loss of accelerator response occurred while driving in local traffic, again creating a severe risk of a rear-end collision. The vehicle has been taken to an authorized Audi dealership for a diagnostic evaluation. No diagnostic trouble codes (DTCs) have been provided by the dealer yet. There was no crash or injury, but the sudden, repetitive loss of propulsion on a high-speed roadway constitutes an extreme safety hazard. I am the original owner and have maintained all scheduled maintenance intervals.Read more

I purchased a 2022 Ford F150 in September 2024. I began to notice occasional sluggishness when accelerating as the weather warmed up around March 2025. I contacted my local Ford dealership sharing the issue. The service manager suggested I consistently use a high octane fuel, which I consistently do, explaining I may just have "bad gas". The problem continued primarily when I was in heavy traf
... fic with high temperatures. The truck acted as if it had a clogged fuel injector, sluggishness from start, low idle reducing below 1,000 RPM. I contacted a privately owned mechanic shop advising of my idle issues. The mechanic stated he doesn't believe a fuel injector would clog at 50,000 miles. In June 2025 received notification of a recall for my vehicle which I confirmed by VIN. The recall (#25V455) summarized the vehicle could sustain a loss of fuel pressure and flow from the fuel tank due to fuel pump failure. The letter continued, I may experience poor engine performance, rough idle or reduction in power, the same issues I had been complaining about. I contacted the Ford dealership and they explained Ford had not determined how to remedy the problem. Fast forwarding to present, 3 August 2026, over a year since the recall letter and Ford has yet to remedy the problem. What's the hold up? Will Ford finally determine the remedy when my family is killed on the interstate because the vehicles fuel pump fails and it stalls? It appears to be apparent Ford does not want to take the financial hit of such a recall. This is unacceptable and needs to be looked into by the NTSB.Read more

In April 2024 on a road trip to [XXX], the engine suffered a catastrophic failure without warning near [XXX]. The connecting rod or rods broke, and one of them exited the engine. State Police and Local Fire Department responded because a passer-by saw smoke from the expired engine and reported a fire - there was no fire, but the engine oil was leaking down onto the exhaust pipes, causing smoke.
... The local Honda dealership in Bloomington, IL and Honda itself were not very cooperative, claiming the engine was not maintained properly. I submitted our mechanic's records as proof twice to Honda, yet they still resisted. Finally I contacted the original Honda of Naperville dealership where we bought the vehicle, and that got Honda to partially refund the money (under extended warranty) that we spent to repair the motor. In the end, the motor was replaced with a used 25,000 mile unit, which was the only option under the warranty. That did not sit well with us, so we capitulated, accepted the used engine, then sold the Passport back to the Bloomington Honda. Overall, we had to pay for towing 35 miles, the expense of renting a vehicle for about 1 week, and other extra costs plus the time and hassle associated with multiple phone calls to Honda. The total expenses incurred was $1,738 for towing and a rental vehicle to complete the trip. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a safety-related transmission problem I experienced with my 2021 Ford F-150 equipped with the 10R80 automatic transmission. While driving normally, the transmission suddenly shifts hard/lags on gears 3-5 and sometime higher gears 7-9 at higher speeds. I’ve also noticed a buzzing/metallic rattling coming near the engine from inside. There is also times i can tell i have no power wh
... en trying to speed up and the other day i almost thought i was going to get T-Boned. This occurred while on my daily traffic commute to work, creating a potentially dangerous situation because the truck did not respond as expected when I needed predictable acceleration or propulsion. I believe this incident may be related to the known 10R80 CDF clutch-cylinder/sleeve problem. Ford technical information identifies axial movement of the CDF sleeve that can cause hydraulic-circuit leaks and harsh or delayed engagement and shifting. This condition can result in slipping, RPM flares, delayed engagement, unexpected gear changes, harsh downshifts, lurching, or loss of propulsion. This is a safety concern, not merely a transmission quality or reliability issue. Unexpected loss of propulsion or unpredictable shifting while merging, accelerating into traffic, crossing an intersection, turning, or traveling at highway speed can create a collision hazard. Because I personally experienced this unexpected transmission event, I request that NHTSA investigate the 10R80 CDF clutch-cylinder/sleeve condition in Ford F-150s. Please review Ford warranty claims, dealer repair records, transmission replacements, consumer complaints, and field-failure data to determine whether this is a safety defect requiring a recall. I believe owners should not have to wait for a potentially dangerous transmission failure before this known condition is addressed.Read more

Not long after I purchased my car, i noticed it was lacking power during takeoff and when driving uphill which forced my car to hit very high RPMs during normal use. There was also a P0299 code that appeared then disappeared frequently. I discovered a turbo special coverage N232395330 and took it to Williams GMC in Charlotte. After i told them very clearly what happened MULTIPLE times, confirming
... the underboost and the code, I even brought it in with the code, they told me the code wasnt "active" and that there no problems...even after i told them I didnt feel safe and couldnt pull out at intersections with fear of being rear-ended. I had videos, they refused to look at and told me to come back when the code was permanent. I drove the vehicle for the next year, 20k miles in order to get the code permanent, after a trip home, through the mountains, really tough without the turbo, my car was consistently pinging 4-5000 RPM under the speed limit on inclines...this happened every time I drove through, but this time the code was active the entire time. 2-3000 miles with the code, finally it was permanent. I brought the car back to Williams GMC, to get the turbo changed. I updated them on recent occurrences, flashing check engine light, the code active for 2-3k miles, and they performed a road test, and declared it wasn't a problem. I told them i recently spent 5k on repairing my cooling and AC system because the turbo, they said that wouldn't affect those components, meanwhile i know they have plastic parts that can melt and the engine being forced to push 4-5k RPM the extra heat could easily melt those parts. On July 31, 2026, while traveling approximately 55 mph on a Maryland highway, my 2025 Toyota RAV4 unexpectedly lost power and shut off while in motion. There was no check-engine warning before the failure. The vehicle became disabled in a travel lane. Police responded, and the lane had to be closed because the vehicle could not be safely moved. There was no crash, fire, or injury. The vehicle was
... taken to an authorized Toyota dealership. The dealer documented that it had “shut off mid drive,” reported finding no codes in the engine ECU, and performed service campaign 26TC03 concerning an incorrect mass airflow sensor reading. The dealer replaced a radiator air guide and stated that the vehicle should return for further diagnosis if it stalled again. On August 2, only approximately 36 miles after that service, the vehicle failed to restart at approximately 40,529 miles and required a jump. LTA and pre-collision system malfunction warnings appeared. The pre-collision warning returned while the vehicle was moving, and the Toyota connected app also recorded the malfunction. The vehicle was then towed back to the dealership and is available for inspection. The cause remains unknown. The concern involves an intermittent engine shutdown/no-start condition, a possible 12-volt electrical or charging-system failure, and the loss of forward-collision and lane-assistance functions. The dealer’s prior multipoint inspection marked the battery as acceptable, but no actual CCA battery-test result was documented.Read more

... as happened over a dozen times since August 2025. Mercedes says there is no fix. It’s been in the shop more than 10 times for over 60 days. They have even sent out field engineers who cannot duplicate the issue, yet there are codes stored in the cars computer. The car also does not always shift into reverse or drive from park. The transmission often has a small 1-2 second delay upon pressing the accelerator from a stop, which sometimes puts me into the path of an oncoming car, say when turning left on a flashing yellow. Since the car has a delayed response, it puts me I harms way when I may have had time to go, but then it didn’t move. It also drives very jerky at times, shifts late or not at all. I have video of this but cannot load it through this form, so we need to send it a different way. I’ve opened multiple buy back requests as a lemon but Mercedes says no. A 1/2in piece of the magnetic strip was missing and over 10 cracks within the strip itself observed. This magnetic strip is used by the Engine Control Module to measure engine RPMs. The strip's failure caused the EMC to shutdown the engine without warning which is a significant hazard for controlled vehicle movement at highway speeds. The vehicle’s radiator developed a coolant leak that was discovered and confirmed by the dealership during a routine oil change, tire rotation and state inspection. There were no warning lights, warning messages prior to the dealership discovering the leak. The radiator required replacement. The dealership also told me that they had the replacement radiator on hand because this type of radiator
... failure had been occurring fairly often with this type of vehicle. The radiator failure presented a potential safety concern because a coolant leak can worsen without warning and result in a sudden loss of coolant, engine overheating, or vehicle breakdown while driving. Because there were no warning lamps or other symptoms before the leak was discovered, we may not have had any indication that the cooling system was failing. The problem was confirmed by the dealership during inspection, and the dealership determined that the radiator was leaking and needed to be replaced. The vehicle/component was inspected by the dealership. No manufacturer, police, or insurance inspection has occurred, to my knowledge. There were no warning lamps, messages, overheating, or other noticeable symptoms prior to the dealership finding the leak. The vehicle is only three years old, with 39000 miles on it at the time of the failure. I am reporting this because a radiator failure at such a young vehicle age, particularly one that develops without any warning to the driver, could potentially create a safety hazard if the coolant loss becomes severe while the vehicle is being operated. I am reporting a safety-related defect involving the 10R80 automatic transmission in my 2022 Ford F-150 STX. While slowing to a stop during normal driving, the transmission unexpectedly performed an extremely harsh downshift. The truck jerked forward violently as it shifted into a lower gear, making it difficult to slow the vehicle in a smooth and predictable manner. This occurred without warning
... and created a dangerous situation while approaching an intersection. A vehicle should decelerate in a controlled and predictable way. Instead, the sudden downshift upset the vehicle’s behavior and reduced my confidence in its safe operation. If this happens in heavy traffic, on wet roads, or near pedestrians, it could easily contribute to a crash. This is not simply an issue of comfort or drivability. It is a safety concern because it affects the driver’s ability to safely control the vehicle. My truck is equipped with Ford’s 10R80 transmission, which has been the subject of thousands of owner complaints describing harsh downshifts, delayed engagement, slipping, gear hunting, and other dangerous shifting behavior. Publicly available technical information also discusses failures involving the CDF clutch cylinder and sleeve, which can cause hydraulic pressure loss and unpredictable transmission operation. Based on my experience and the large number of similar reports from other owners, I believe this issue deserves a thorough safety investigation. NHTSA should review warranty claims, dealer repair records, transmission replacements, technical service information, and consumer complaints to determine whether this transmission contains a safety-related defect that warrants a recall or other corrective action. Owners should not have to wait until a harsh downshift causes a serious collision before this issue is addressed. I respectfully request that NHTSA investigate this defect and require an effective repair to protect drivers, passengers, and everyone sharing the roadRead more

Two issues- 1. When pulling out into traffic the engine/gas did not catch and we slowly creeped out across lanes before the gas took. This almost caused us to become t-boned by a passing car. When the gas took the car lurched forward fast which could have been another accident. 2. When driving with adaptive cruise control the forward collision warning fault light and service adaptive cruise cont
... rol light kept turning on and off with a service needed message on the heads up display. This flashed on and off repeatedly causing driver distraction and also limiting vehicle safety. This vehicle has been taken to the dealership multiple times since purchase with various resets done (Transmission control module, powertrain control module) and still numerous panel error lights turn on (engine malfunction light, traffic signal recognition fault light) and issues occur (ac stops blowing cold air, inside screens freeze) but can never be replicated or fault code founds. The biggest safety issue is the lagging engine/gas that randomly occurs from a stop as you can’t predict when it’s going to happen and it will cause an accident at some point.Read more

On XXX I was waiting at a light and the check engine light was blinking. When I started up, the car was very sluggish. It continued to be sluggish on my trip home - 5 miles. I brought it to Meineke [XXX] [XXX]. They ascertained that the engine was shot. They flushed out the oil in the engine and there was lots of metal in the oil. They instructed me to go to Hyundai immediately. There was a e
... ngine recall and their research showed that I was included under the recall warrenty. I've included their assessment. Both the drives to the Meineke from my house and to Hyundai from Meineke were scary. I didn't feel safe in the car. I went directly to [XXX], manager of Larry Miller Hyundai at [XXX], Phone [XXX]. He said that he needed to clear the recall with Hyundai. He called me back in a few days and told me that all the test diagnostics were made and that a censor malfunction was the cause of the problem and the front breaks needed work. I gave him the go-head. The cost was $1,319. I wasn't convince that was the primary problem. I use my car for short trips. I was driving to the gym on XXX - having driven less than 50 mile since the recent repair - once again I stopped at a light and the check engine light lit up permanently - not flashing. The engine started to shake when it was idle and there was an unusual sound coming from it. I finished my drive to the gym although the drive just didn't feel normal or safe. Right now (XXX) I am having my car towed to Hyundai. It is not safe to drive. My belief is that corporate Hyundai refused to fix the engine repair part of a class action lawsuit which includes replacing the engine. They had to see that my recall warrenty ends in September and they figured they'd stall until the warrenty was no longer valid. This is just a guess, but a very educated guess. They had to know that the engine was no longer safe. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I have a 2026 BMW X5 50E. I experienced the same malfunction twice in the same day. The first occurrence was when I was taking my husband to a doctors appointment. While parking the car and when I was almost at a stop the vehicle lurched forward and was stopped by the concrete curb. My husband asked me what happened and I said I don’t know. The second occurrence was about two hours later after
... visiting a second doctors office I headed to a lunch spot. I pulled into a parking spot and had come almost to a stop and once again the vehicle lurched forward with an even more powerful sudden acceleration. The vehicle struck a metal railing, bordering, an outdoor dining area. Once the railing was hit It pushed into a large freestanding metal continuous privacy panel. The privacy panel collapsed on people and caused multiple injuries. This incident endangered the safety of myself and the safety of many others. There were multiple injuries. I have not yet had the vehicle inspected by a third-party. I received no warnings on the car or through the app prior to the failure. These incidents occurred on July 31, 2026 between 10 AM and 12 PM. I have read and saved the error codes available on the car. There may be some that are associated with the incident. Also of note the incident occurred several days after a software update on July 26 at 1:30 AMRead more

The vehicle intermittently fails to start in two different situations. First, when the auto Stop/start system shuts the engine off while the vehicle is stopped anywhere, the engine sometimes refuses to restart, leaving the vehicle immobilized. Second, there are occasions when I park the vehicle, turn it off normally, and upon returning, the engine will refuse to start.

My Tiguan leaks and burns oil like crazy. Now it has 111k miles on it, but this has been an ongoing issue for the last five years since I've had it. I now have to put a quart of oil in every 100 miles. This is not the fault of me getting an oil change, this is a persistent problem caused by the manufacturing of the car. I have had the engine nearly seize up while driving on the freeway due to low

This vehicle is burning oil! I have stayed on top of getting the regular/required oil changes but inbetween we have had to constantly top off the oil. I also get all oil changes at the dealer. Last time when I complained about it the guy told me that it would be a lot of work to try to find out why and left it at that. Driving home on the interstate my truck started making a knocking sound and the vehicle would shake over 45 miles an hour. We took to a dealer that said the lifters were bad, and potentially caused damage to the cam shaft. There was no warranty. I asked how this happens under 60,000 miles and was told it’s just part failure. And with the 6.2 engines, it’s a known occurrence. I then see that Ch

When the vehicle engages auto-off at a stoplight, the engine stalls when trying to reengage.
