While driving, on approach to a stop signal, at an intersection the car stopped moving. The display said engine malfunction, have inspected. The electronics and engine continued to operate, but the car would not move when gas pedal was depressed. I shut off and restarted my vehicle numerous times, with no effect on the problem, the car still would not move. The vehicle behind me had to brake unexp

I am reporting a serious safety defect involving my 2013 Hyundai Santa Fe Sport equipped with a Theta II engine While driving and exiting the freeway with my wife as a passenger, the engine suddenly failed after knocking symptoms and completely seized/locked while the vehicle was still in motion. The vehicle immediately lost motive power and came to a stop, creating an immediate and dangerous tra
... ffic safety hazard. Had this engine seizure occurred moments earlier while traveling at freeway speed, it could have caused a catastrophic accident involving serious injury or death. The Hyundai dealership diagnosed the vehicle with rod bearing failure, which is the known defect associated with Hyundai Theta II engine failures. My VIN falls under the Theta II extended engine warranty program, and the dealership confirmed eligibility and accepted the vehicle for inspection under that coverage. Despite this, Hyundai denied the repair claim, first alleging maintenance-related issues and later stating the vehicle was out of warranty. I maintain that the vehicle was properly maintained with oil changes performed approximately every 6,000/8,000 miles. Some calendar-time gaps occurred because the vehicle was stored and driven very little during my recovery from a personal injury surgery and recovery, but the vehicle was maintained on mileage intervals. This engine defect resulted in sudden engine seizure and total loss of motive power while driving, creating a serious safety risk to vehicle occupants and surrounding motorists. I am reporting this defect so it can be documented and investigated.Read more

Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

The engine has been burning oil since last May and Hyundai has been putting me through the ringer of hoops to get it fixed. The dealership has told me this is a known issue and I know of others who have the same car and are having the same issue. Someone needs to hold them responsible.

My vehicle recently idled roughly and shut off at low speeds and while stopped at intersections on my way to work. I brought it to an authorized Kia dealership the very next day. After researching my VIN, I discovered Kia Safety Recall #SC336 (NHTSA Campaign 25V099), which identifies a piston oil ring defect known to cause excessive oil consumption and internal engine damage. I promptly brought my
... vehicle to the dealership specifically to address this recall. However, the dealership did not complete recall-related repairs. Instead, they determined—without sufficient justification—that my vehicle’s issues were not related to the recall, but to a "crush washer". My vehicle exhibits the exact symptoms outlined in the recall, including excessive oil consumption, rough engine operation, and stalling. The dealership then attempted to attribute the oil consumption to a crush washer on the oil drain plug, despite there being no visible signs of oil leakage under my vehicle at any time. This explanation does not reasonably account for the level of oil loss I have experienced or the internal engine condition. According to official recall documentation filed with the NHTSA, the defect involves improperly manufactured piston oil rings that can damage cylinder walls, leading to excessive oil consumption, abnormal engine noise, rough operation, and ultimately engine failure or stalling. The recall further warns that continued operation may result in engine seizure and loss of drive power. Independent automotive reports confirms that this defect has affected over 137,000 vehicles, including 2021 Kia Seltos models, and in many cases requires full engine replacement due to internal damage caused by faulty piston rings. Additionally, documented consumer complaints describe the same pattern I have experienced—excessive oil consumption with no external leaks, worsening engine performance, and dealerships attributing the issue elsewhere despite clear alignment with the recall defect.Read more

While operating my vehicle under normal driving conditions, I experienced a sudden and complete loss of engine power without warning. The engine shut off abruptly in active traffic, creating an immediate safety hazard and increasing the risk of a collision for myself and surrounding motorists. Following the stall, the vehicle would not restart. The engine cranks but does not start (a “dry crank
... ” condition). There were no prior warnings, indicators, or drivability issues suggesting an impending failure. This vehicle is equipped with the L87 6.2L V8 engine, which is subject to a known recall involving internal engine defects that may result in sudden failure. The symptoms and failure mode observed are consistent with reported cases of internal engine damage, including potential bearing or rotating assembly failure. At the direction of GM and the dealership, the vehicle was previously brought in for recall inspection. The recall was subsequently marked as completed. However, I am now being advised that the recall cannot be reopened and that I am responsible for the repair costs. A completed recall inspection does not eliminate manufacturer responsibility when the underlying defect later results in failure. I understand there is an ongoing and recently reopened NHTSA investigation into GM’s “Pico test” inspection process, as multiple engines have reportedly failed after passing this procedure. Given that this failure occurred after the recall inspection was completed, this incident raises concerns regarding the adequacy and effectiveness of the prescribed remedy. This event represents a serious safety issue due to the sudden loss of power in traffic without warning. I am submitting this complaint to document the failure and to request further review of the recall remedy and associated inspection process.Read more

I am reporting a safety-related engine defect involving sudden internal engine failure, excessive oil consumption, persistent bearing knock, and overheating of the catalytic converter. A certified mechanic documented the engine is failing internally and that the cat. converter has been damaged a
... s a direct result of engine defect. The failure pattern is consistent with known Hyundai engine defect investigations involving bearing wear, oil consumption, stalling, and fire risk. The overheating cat. converter presents a significant safety hazard, including potential engine seizure. I purchased the vehicle used in March 2025. I am the third owner. Despite maintaining the vehicle responsibly (Regular oil changes), the engine failure progressed rapidly. Hyundai Motor America has denied a goodwill repair solely on the basis of mileage, even though the defect clearly originated long before my ownership and aligns with previously identified safety-related engine issues. Additionally, Hyundai's records show that this vehicle had an engine-related recall active during prior ownership period; the recall was never performed by the Hyundai dealership that sold the vehicle to the previous owner. This raises concerns about improper recall handling and whether the defect should have been corrected before the vehicle was resold. Given the safety implications, the documented defect pattern, and the unresolved recall history, I am requesting NHTSA's review of this matter. Went to park car in a park to drop daughter off and it sputtered out and stalled out of nowhere with no warning. I had received a lamp warning very sporadically prior to this and on occasion that stated 'reduced steering assist drive with care' this resulted in heavy steering for that drive until car was shut off and then it would go away and be fine again, fixed the car through Knudtsen Chevy due
... to a recall about a month before on the issue with the engine being turned off and it thinking it was still hot and still running the fan (even though the car was not running). Knudtsen fixed that issue. Low battery voltage light did not come on however battery was older so had battery installed at park to ensure the issue was not due to the battery. The oil was fresh and showed no issues, changed 2 weeks before fresh oil no issues found during service. When tried to restart the car it would think it would try to start and then sputter out and not stay in idle. Les Schwab left and called a mobile mechanic to bring the computer. Computer was hooked up and it would try to start, however, it would not know it was already running and try to start again on it's own...the codes that came up were the PO101 code came up for Mass Air Flow issue...and then the codes would go in a wild sequence of fail.....mechanic checked for loose connections around air filter, took air filter out and it was clean no issues, checked pipe no visible issues. We walked away and walked around the park to give it some time to sit and then we tried it again and it tried and sputtered out and died again would not stay idling. Then all of a sudden we heard it try to start itself without us trying to start it and it clanked. We hauled it to Knudtsen for a diagnosis. They stated "the motor will not turn this way or that way and it is seized or locked up". This is a 2021 with only 89,000 miles it should have a longer life than this. The vehicle will randomly shut off while driving. A message will appear stating "Check the Starting System" Attempts to restart the engine are unsuccessful. ... s before the engine would remain running. When the engine would stall the dash would go dark and the engine would not remain running this happened about a month ago in hot weather. It occurred again on April 25th, 2026 way worse this time. The temperature in the area reached 91 Degrees Fahrenheit. My vehicle was sitting outside in the heat for at least 5 hours, same issue, The vehicle would then proceed to stall in the middle of the freeway, I was able to coast to the breakdown lane where the vehicle would remain unable to start and the message "Check Starter System" would display. Looking on NHTSA webpage before filing a complaint, it seems that this is a rather common issue. VW had a recall for other models (Campaign 28H7) for models such as the Jetta, Golf and other like models. This is a really dangerous issue and could end up getting someone seriously injured or worse killed in a collision if my vehicle stalls like this. The vehicle is scheduled for a service appointment in the near future (Local VW dealer is at least 2 weeks out before they can even look at my vehicle) After viewing this information, I can't help but believe it is possible the 28H7 recall is affecting more models than anticipated.Read more

The vehicle's auto idle stop system is malfunctioning, resulting in the engine failing to restart from a complete stop. This total failure has occurred on five separate occasions, including while stopped in active traffic intersections. Prior to these total restart failures, the vehicle showed the following symptoms: 1) Initially, the vehicle experienced rough engine idling, abnormal vibrations,
... and a raw gasoline odor after driving. 2) This progressed to a noticeable delay between releasing the brake pedal and the engine attempting to ignite, during which the starter sometimes sounded strained. 3) This has culminated in the current defect where the engine shuts off normally when stopped at an intersection, but completely fails to immediately restart when the brake is released. This defect completely immobilizes the vehicle in active traffic lanes, exposing us to an immediate and severe risk of a rear-end collision. When the engine fails to restart, the vehicle locks the transmission out of drive and triggers a warning on the dashboard. It is a highly hazardous situation, especially with a lot of distracted drivers on the roadways.Read more

Hello. We purchased this 2018 Honda CR-V new in 2018 from the local Honda dealer, Honda of Lake Jackson, TX. In the Spring of 2025 the A/C stopped working. We took it to a local shop and they had to replaced the following: Blower motor replaced, blower motor replaced, A/C evaporator replaced, A/C expansion/orifice valve replaced, Heater core replaced. This was around a $3,000 repair. A month or
... so later in May of 2025 we took the car to the Honda dealer because of steering instability issues. We were advised that the entire Steering Column would need to replaced due to known safety issues (drive shaft can fail completely at highway speeds). The following was repaired at this time: --Rack and pinion replaced, steering gear replaced, valve cover gasket replaced Vehicle was at 61,984 miles at this time. This was a $5,000+ repair. It cost so much that we actually had to take out financing to pay it, which in essence essentially renewed our car payment (car was already paid off at this time). Now for the past 8 months or so we have had a "juddering" issue where you can feel the transmission CTV system jump and jerk as you accelerate to highway speeds. This issue also puts the entire transmission at risk of failure. The Honda dealer here checked the vehicle out and suggested we do a transmission fluid swap. This cost $189 for the "diagnostic fee" and $179 for the swap. So another nearly $400. They did the swap and said that it did not fix the issue and now we need to pay $2800 to have the "Torque convertor" replaced. This is $10,000 plus dollars in repairs, nearly half to cost of the vehicle. This is a ridiculous amount of issues for a vehicle that had only 61,000 miles. We have always had regular maintenance performed also. From what I have read, Honda knows about these issues but has not issued a recall. In addition I've read that "juddering" CTV issue is a known issue in 2015-2018 Honda CR-V's. Needless to say we are not happy with Honda about any of this.Read more

The dealership informed me that they are unable to perform the recall due to being unable to scan the diagnostic codes therefore cannot inspect the engine/oil pan as per the recall instructions - informed me that the engine needs to be replaced first and that I should contact Nissan Consumer Affairs for a good-will engine replacement approval. I contact Nissan Consumer Affairs and was informed that they will not be replacing the engine and cannot perform the recall until the engine is replaced; instead offered me 50% off the engine replacement cost.Read more

While driving my 2005 Nissan Altima in active traffic with my [XXX] child, the vehicle suddenly stalled without any warning. The engine completely shut off while in motion, creating an immediately safety hazard in live traffic conditions. Immediately after the stall, I observed oil leakage and smoke entering the cabin. The vehicle became inoperable and we were stranded roadside. This failure is

Although my vehicle previously received recall-related service, this incident suggests the prior remedy did not prevent recurrence of the defect. Sudden engine shutdown while driving in traffic Loss of vehicle power without warning Smoke entering passenger cabin Risk of crash involving child passenger Request: I am requesting that NHTSA: Review this incident for potential recall remedy failure Investigate whether this represents recurrence of a known defect Determine whether additional corrective action is needed for this vehicle population INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

We had the car towed to a mechanic shop where we were told that the entire engine
... was blown and needed to be replaced. We had the car towed to the local Hyundai service center so they could inspect the engine. We discovered that the make, model, and year of our Veloster had been subject to an engine recall and these cars were given an extended warranty to 15 years/150,000 miles. Hyundai inspected the engine and refused to provide assistance under the extended warranty/recall. Replacement of the engine is going to cost close to $10,000.Read more

The contact owns a 2019 GMC Sierra 1500. The contact stated that while driving approximately 65-70 MPH, the engine seized and made an abnormal sound, nearly causing a semi-truck to jackknife and crash into the vehicle. There was no warning light illuminated. The contact was left stranded in the west desert of Utah, where a wildfire had started, and the contact did not have phone service. Additiona
... lly, the vehicle stopped on a two-lane road with heavy semi-truck traffic, leaving the contact frightened and terrified for her safety. The contact jumped out of the vehicle. Migrants who were tending the sheep nearby assisted the contact in pushing the vehicle to the side of the road. The vehicle was towed to the dealer, where it was diagnosed with engine failure, and the dealer determined that the engine needed to be replaced. The vehicle was not repaired. The contact related the failure to NHTSA Action Number: EA25007 (ENGINE, ENGINE AND ENGINE COOLING). The manufacturer was notified of the failure, and the contact was misinformed that the model year was not included in the NHTSA investigation. The contact informed the manufacturer that she no longer felt safe driving the vehicle, regardless of the engine replacement, and the contact advised the manufacturer to guarantee that the failure would not recur after the engine replacement. The manufacturer then referred the contact to the NHTSA Hotline for assistance in investigating the safety issue. The vehicle was not repaired. The failure mileage was 122,187.Read more

On 04/28/2026, when diagnosing a reoccurring check engine light on the vehicle at a Chevy Certified Service, the mechanic "Borescoped cylinder 4 and found significant scuffs on the cylinder wall which along with the misfires indicate a cracked piston. The big picture appears to be the piston cracked which caused excessive oil consumption and misfires. The excess oil and unburned fuel overloaded th
... e converter causing it to fail. The engine and converter will need to be replaced." In 2017, a recall was issued by GM to mitigate Stochastic Pre-Ignition (SPI) Causing Cracked Piston (17019). Between the initial purchase of the vehicle (11/2016) and performed service required by this recall (03/10/2017), there were approximately 5000 miles driven. No engine diagnosis was performed or recommended. After the 17019 was addressed, the engine misfires continued along with reoccurring CEL. C.S.P. N182195660 Predictive Monitoring for Stochastic Pre-ignition (SPI) released in 2019 states the 1.5 L LFV engines in certain 2016 and 2017 model year Chevrolet Malibu vehicles may have a preignition condition that requires immediate repair. This condition may cause the engine to perform poorly, misfire, run rough, and/or excessively consume oil. It may also illuminate the vehicle’s service engine light. Over time, if not repaired, the condition can damage the vehicle’s pistons and engine. The VIN for this Malibu was not attached to this notice despite it having SPI recall history. On 06/2023 (after the N182195660 program ended), at 80k miles a diagnosis revealed a potential cracked cylinder with no mention of the pre-ignition condition that could cause the crack, and the only way to fix is to replace the entire engine (as per dealer recomendation). Since vehicle was out of warranty, the repair est. was nearly $10k. I firmly believe my 2016 Malibu has suffered the consequences of poor mismanagement on this recall, and am demanding mitigation from GM to fix this issue.Read more
