I am the 2nd owner of my 2016 Ford Escape 1.6L ecoboost, my aunt and uncle were the original owners. Between us we have every maintenance record of every on time and professionally performed requirement the Escape has needed and it ran beautifully. Then, in February 2026 the idle started to get a bit rough and I was preparing to put in the OEM sparkplugs when 3/28/26 the engine overheated, I was s
... tranded on the side of a 45 mph street waiting for it to cool so I could move to safety. Between that incident and 4/29/26 I went thru 9 gallons of coolant, replaced the sparkplugs, put the front end up and "burp' the system as there was constantly air bubbles being introduced from a source I could not find. I had the hoses and clamps inspected, replaced the coolant reservoir and the reservoir cap, the vehicle started to shutter upon starting even after it was warmed up and every day needed to be burped and topped off before and after driving in town but often had no coolant loss during freeway driving, finally 4/28/26 I noticed smoke from the exhaust when I started the car in the morning and that smoke was accompanied by a foul, burnt, electric smell. While burping the air and topping the coolant that morning, the engine sputtered and died 2 separate times. I decided that it was time for a cooling system diagnostics as I still could still see no evidence of an external leak. The Ford technician recorded this tech story. "The vehicle ran rough and smoked for a couple seconds after starting.the vehicle ran its self test and had p0316, b0303, p1299, and p0217. These dtcs relate to cylinder misfire on startup and cylinder misfire and cylinder 3 and over temp/protection being active. ... I suspected a possible coolant intrusion concern into cylinder 3, performed a pressure test on the cooling system and observed a drop in the pressure. I remove the spark plug and found coolant on the cylinder walls and the top of the piston in cylinder 3." Advise to replace long block assembly.Read more

Component: Engine My 2020 Hyundai Palisade (approximately 155,856 miles) experienced a serious and dangerous engine-related failure that resulted in a vehicle fire. My 16 year old daughter was driving alone at the time of the catastrophic failure and fire. For approximately six months prior to the incident, the vehicle was consuming oil at an abnormal rate with no visible external leaks. This
... condition persisted without clear cause. While driving, the vehicle suddenly developed a new oil leak, and the engine failed without warning, leaving me stranded on the roadway. Immediately following the failure, the engine compartment caught fire. There were no prior indications of an active oil leak before this sudden failure. The progression from oil consumption to catastrophic failure and fire presents a serious safety concern. This incident created a dangerous situation with risk of injury or worse, and it appears to be more than normal wear and tear. I am reporting this as a potential safety defect involving engine failure and fire risk. Mileage at time of incident: approximately 155,856 miles No prior accident or external cause contributed to the fire.Read more

The failed component is the engine assembly in my 2017 Hyundai Sonata SE with the 2.4L Theta II GDI engine. The failure involves excessive oil consumption and internal engine failure, specifically suspected piston ring deterioration allowing oil to pass into the combustion chambers. The vehicle is available for inspection upon request. Safety was put at risk because the engine was losing oil rapi
... dly. If the oil level drops too low, the engine can seize, stall, or lose power while driving. Sudden engine failure at road or highway speeds could increase the risk of a crash. The issue required constant oil monitoring and made the vehicle unsafe to rely on. The problem was confirmed by both an independent repair shop and a Hyundai dealership. At about 99,813 miles, an independent shop diagnosed oil burning from the tailpipe. I then paid for major repairs, including cylinder head and valve work, but the oil consumption continued. A later independent inspection found internal oil consumption consistent with failed piston rings and oil bypassing into the combustion chambers. The Hyundai dealership performed Hyundai’s oil consumption testing process, including combustion chamber cleaning and rechecking oil level. The vehicle failed and engine replacement was approved. The dealer quoted $11,400-$12,000 for replacement. The vehicle has been inspected by an independent repair facility, a Hyundai dealership, and reviewed by Hyundai Motor America Customer Care under case number 42489659. Hyundai denied assistance, stating the vehicle is outside warranty due to time and mileage. Symptoms first appeared around 99,813 miles and included excessive oil consumption, burning oil from the tailpipe, reduced power/dragging, abnormal rattle/noise during low-speed acceleration, and poor fuel economy. After repairs, the oil level still dropped below minimum quickly. I am filing this complaint because this condition may cause sudden engine damage, stalling, or loss of power while driving.Read more

All spark plugs fouled again; all spark plugs replaced. Valve cover gasket was leaking, so that was replaced as well. Yup, I paid for the repairs out-of-pocket. Episode Four: May 02, 2025. Vehicle goes into limp mode again, with CHECK ENGINE LIGHT turning on. Another spark plug fouled up, so it was replaced. Ignition coil replaced as well. Episode Five: June 19, 2025. Limp mode and Check Engine Lamp illuminates yet again, so Hyundai of North Charleston (SC) initiates an oil consumption test. Without any warning, the V8 L87 6.2 liter engine suddenly seized up, began shaking with significant reduced power. I was fortunate that I could drive slowly to safety and was not on the expressway. In my research, I understand this is related to a design/manufacturing issue with the connecting rods and crankshafts in this type of engine. As I investigated the issue, I found that 600,000 vehicl
... es were recalled with the recall either completely replacing the engine or upgrading the oil type from standard to a thicker, higher-viscosity 0W-40 full synthetic to provide better protection under high stress, along with a new oil cap and filter. In our case, we were directed to change the oil and even with that change, we experienced this dangerous engine failure which cost us $12K ! For that cost of repair, we elected to have the engine replaced for about $1K more to get a 100K mile warranty on the engine. However, our additional concern with the replacement engine is they gave us a remanufactured version of the L87 and we have not found confirmation that they have adequately addressed the design/manufacturing flaws so may be vulnerable to the same seizing issue. We appreciate your attention to this widespread GM issue.Read more

Incident Details: While traveling on MD-152 (Mountain Road) in Fallston, MD, the vehicle suffered a sudden and complete loss of motive power without any prior warning or dashboard indicators. There were no warning lights, no temperature spikes, and no mechanical noises before the engine seized. This sudden failure occurred in active traffic on a busy road, creating an immediate risk of a rear-end
... collision. Reason for Denial & Safety Risk: The vehicle is currently at a certified Kia dealership. I am being denied a remedy for this catastrophic engine failure due to "insufficient maintenance records." However, the Kia service department has confirmed in writing that "sludge is not a factor" in this case. Mechanically, the absence of sludge proves the engine was not neglected. Manufacturer Failure: After being denied and exploring other avenues, I have learned that the 2017 Sorento 3.3L V6 is covered under Kia Warranty Extension WTY035 (15 years/180,000 miles) for catastrophic engine failure related to head bolt and gasket defects. By using a lack of paperwork to deny a claim for a "sludge-free" engine, Kia is bypassing its safety obligations for a documented manufacturing flaw. I am reporting this as a dangerous "loss of motive power" defect that mirrors existing Kia engine safety recalls across their product line, and warrants federal oversight.Read more

I was also told there is evidence of oil leakage from the turbocharger area. The shop indicated that the engine likely requires replacement due to internal failure, with an estimated repair cost between $10,000 and $15,000. No prior warning of coolant intrusion was given during a recent service visit approximately one week earlier. This issue creates a serious safety risk because the engine may fail unexpectedly while driving. The problem appears to worsen after the vehicle sits, increasing the risk of failure during normal daily use such as commuting or transporting my child.Based on my research, similar issues involving coolant intrusion have been reported in Ford vehicles equipped with the 1.5L EcoBoost engine. This appears to be a known defect that can lead to engine failure without adequate warning.I am requesting that this issue be investigated as a potential safety defect.Read more

Two days later, I was informed that the vehicle was no longer able to shift into gear and was now completely inoperable. I was told I would be responsible for towing the vehicle off their premises. According to the service department, the vehicle now requires replacement of the EME module, at an estimated cost of $12,000. This assessment was particularly troubling because the vehicle was fully drivable when it arrived at the dealership. At no point prior to service did the car have difficulty shifting into gear. It is deeply concerning that the vehicle became a “brick” while under the care of an authorized BMW service center, with no accountability taken for what occurred during their diagnostic process.Read more

The primary failure is the engine assembly and turbocharger, specifically caused by the failure of the oil pump drive belt and tensioner system (subject of Safety Recall 23S64 / NHTSA 23V-905). Despite an attempted recall "remedy" on August 25, 2025, the engine suffered internal oil starvation due to belt debris (rubber fibers) remaining in the oil passages. The dealership has confirmed in writing
... that the turbocharger failed and the engine requires a "long block replacement" due to "debris in the oil passages" and a "clogged turbo oil feed filter." The vehicle is currently held at dealership, and the failed components are available for inspection. The vehicle experienced a sudden loss of motive power while in operation, a condition specifically cited in the safety risk section of Recall 23S64. This loss of power occurred during acceleration, creating an immediate risk of a rear-end collision or being stranded in active traffic. Additionally, per the recall notice, this failure path can lead to a loss of the mechanical vacuum pump, which provides power braking assist. A sudden increase in braking effort combined with a stalled engine significantly increases the risk of a crash. The problem has been confirmed by the dealership. On January 5, 2026, the service department diagnosed a failed turbocharger then on April 17th 2026, the service department diagnosed the problem to be a internal engine debris clogging oil feed lines. They verified the engine has oil pressure at the pump but is failing to deliver oil to critical components (turbo/upper block) due to contamination from the defective belt system. The vehicle has been inspected by the authorized Ford manufacturer representative at the dealership. I have also escalated the case to Ford Customer Relationship Center, though they have yet to provide a $0-cost safety remedy as required by federal law. I own a 2019 Audi Q8. The vehicle was in the dealership’s possession for approximately three (3) weeks for service. During that time, I was not informed of any significant mechanical concerns or issues. After retrieving the vehicle, it displayed an oil pressure warning while in operation. I immediately returned the vehicle to the dealership for evaluation. At that time, the dealership added oil
... (“topped off” the vehicle) and released it back to me without identifying or addressing the underlying cause of the warning. Following this, I made multiple attempts to have the vehicle properly diagnosed. I experienced difficulty obtaining assistance, including calls being disconnected, and I was not provided meaningful support or a thorough evaluation of the issue. Shortly after leaving the dealership, the vehicle began producing abnormal knocking noises and rapidly became unsafe to operate, requiring it to be towed. This situation presents a significant safety concern, as a sudden loss of power or drivability while driving can increase the risk of an accident, particularly in traffic conditions where maintaining speed is necessary. The condition has since been confirmed by a service provider as a major mechanical failure. Additionally, I have received documentation from the manufacturer indicating a known issue related to piston skirt defects associated with this vehicle. Despite the vehicle being in the dealership’s possession for an extended period, this issue was not identified, diagnosed, or addressed. I was also not provided clear assistance or guidance regarding potential warranty consideration related to this known issue. The vehicle is currently available for inspection upon request.Read more

The 2023 Toyota Tundra Hybrid’s powertrain design may be insufficient to maintain basic safety maneuvers (specifically "limp mode" or emergency clearing) during an engine failure while operating under high-load conditions in mountainous terrain. This creates a life-threatening hazard for the occupants and other motorists. Statement of Fact: As an owner of a 2023 Toyota Tundra Hybrid, I primar
... ily utilize this vehicle to tow a 9,000 lbs travel trailer through the high-altitude, steep-grade regions of Utah. These routes frequently consist of narrow, two-lane mountain passes with no shoulders and steep drop-offs. Failure Under Stress: Engineering logic suggests that engine stress is highest during heavy towing on steep grades. If a primary engine failure occurs in these conditions, the hybrid system’s battery and electric motor capacity are, in my experience, inadequate to move a combined Gross Combined Vehicle Weight (GCVW) of approximately 15,000+ lbs to a safe location. While the hybrid system may allow a light, unladen vehicle to limp to a shoulder under "ideal conditions," it lacks the torque and sustained power to move a heavy trailer on a grade once the internal combustion engine is compromised. In the event of a failure, the vehicle would likely become "dead in the water" in the middle of a live, narrow lane on a blind mountain curve. Without the ability to reach a turnout or even a marginal shoulder due to the weight of the load, the vehicle becomes a stationary target for other high-speed mountain traffic, posing a severe risk of rear-end collisions and multi-car accidents. I am requesting that the NHTSA investigate the performance of the Tundra Hybrid’s "fail-safe" modes specifically under maximum towing capacity on grades.Read more

At approximately 43,044 miles, my 2021 Audi A7 55 TFSI e quattro (PHEV) was diagnosed with a failed N632 coolant changeover valve — a component of the PHEV thermal management heating circuit. The vehicle has a complete, documented service history with no maintenance gaps. The N632 is an electronically controlled valve that routes coolant between the combustion and hybrid/electric circuits and is
... essential for cabin heating in EV mode, battery pre-conditioning, and engine warm-up. Failure of this valve can result in loss of cabin heat, coolant leaks into the engine bay, and potential cascade of electrical faults if coolant migrates into the wiring harness. The repair estimate is $1,700. The factory warranty, Audi Care plan, and third-party extended service contract (United Car Care Protection Plus) have all denied coverage. This failure on a low-mileage PHEV with full service history suggests a premature component defect in Audi's PHEV thermal management system. I am requesting NHTSA investigate whether this constitutes a pattern of failure across 2021 Audi PHEV vehicles.Read more

Check engine light illuminated on my 2021 Chevrolet Tahoe. Upon diagnostic scan, the following emissions-related fault codes were present: P11D5 (NOx sensor), P14D9 (NOx sensor), P2383 (EGR pressure sensor), and P2C7A (NOx catalytic converter efficiency). All components are available for inspection upon request. The vehicle was taken to an authorized Chevrolet dealership for diagnosis. The dealer
... acknowledged the emissions codes but stated that unrelated components would need to be repaired at owner expense before the emissions system would be addressed, despite these codes falling within the federally mandated 8-year/80,000-mile emissions warranty. The fault codes were confirmed by the dealership. No safety incident has occurred to date, however degraded NOx and EGR function can result in increased harmful emissions output and potential engine performance issues. Warning lamps appeared without prior incident or symptom. The manufacturer has not offered to cover the emissions components under the federal emissions warranty without the owner first paying for additional repairs that may or may not be causally related to the emissions system failure. When the dealer was directly asked, "Are you saying the emissions system itself is functioning correctly, and something else caused these codes to appear?" — the dealer did not provide an answer. This question was asked in order to clarify whether the emissions components themselves are degraded or failed, which would confirm warranty coverage eligibility. The dealer's failure to answer this question directly is noted as part of this complaint.Read more

My CX-5 has 86,120 miles. I noticed a burnt oil smell coming into the car and later found oil droplets on my garage floor. I took it in to my neighborhood garage and they found an oil leak coming from the cylinder head. They recommended I take to my Mazda dealer. I had it towed to Pearson Mazda, Richmond,VA. They inspected it and stated I had a cracked cylinder head. I have a 100K extended warrant
... y that will cover most of the cost to repair it. However, I found the Mazda CX-5 2018-2021 models have a cylinder head defect. I found a Mazda Service Alert, #SA-031/21 and on Reddit a Mazda CX-5 owners page that has many stories of the same problem, cracked cylinder head. Some have had Mazda cover the repair cost or given a major discount and provided an extended warranty after repair. I reviewed this with Pearson Mazda and they stated that my car does not fall in the vehicle numbers that they will cover. This is a known issue and they will not acknowledge it for my car. I have contacted Mazda Corporate and waiting for a callback. Litigation has been filed for the CX-5 Turbo customers and they have received restitution. This is also a major issue on non-Turbo customer’s SUVs. I have read that the 2018-2021 manufacturing (casting) of the cylinder head was defective. However, Mazda is not admitting it because a RECALL would cost them millions to fix the cylinder head issue on all the affected vehicles. But, they are at fault and should work with all customers that have been affected by this issue. Therefore, I am requesting NHTSA to make this a public safety matter and have Mazda be accountable for the issue. Thank you!Read more
