The engine began making a metallic knocking noise while at idle and very soon after began to make the same noise under acceleration. This noise sounded very much like the onset of rod bearing failure. The vehicle was driven straight home and parked. We called the dealership the next day and made a service appointment. As I drove the vehicle to the dealership, the engine noise became worse and was
... clattering very loudly as I pulled into the dealership parking lot. I pulled into a parking spot and let the engine idle with the transmission in Park while I was collecting some personal items when the engine stalled. I tried several times to start the engine but it would not crank. The dealership engine tech later confirmed that the engine was seizedRead more

Vehicle experienced catastrophic engine failure while traveling approximately 70 mph on a highway. I heard a loud popping noise and immediately lost power. The vehicle would no longer accelerate normally and I was forced to pull over. White smoke was visible from the engine compartment, and a large amount of engine oil leaked from the vehicle. This failure created a safety risk because it occurre
... d at highway speed, resulting in sudden loss of power while traveling with surrounding traffic. The vehicle required towing and could not be driven from the scene. Prior to the failure, the vehicle had exhibited symptoms beginning around May 11, 2026. A check engine light illuminated and an independent mechanic diagnosed a cylinder 3 misfire and replaced the ignition coil. Following that repair, the check engine light returned. Additional symptoms included stalling at idle, knocking noises from the engine, and intermittent oil pressure warning lights. The vehicle was subsequently inspected by an independent mechanic who diagnosed a thrown connecting rod with damage to the engine block, including a hole in the engine near the transmission area. The mechanic advised that the engine requires replacement. The damaged engine is currently available for inspection. The vehicle has not been inspected by the manufacturer. No injuries or crash occurred, but the sudden engine failure and loss of power at highway speed created a significant safety concern.Read more

On June 3, 2026, my 2021 Chevrolet Silverado 1500 LT Trail Boss, which has a 6.2L L87 V8 engine, stalled and shut off unexpectedly while I was stopped at a traffic light on a public highway. The engine died without warning, leaving me stranded in traffic and creating a serious safety risk for me and other drivers because of the chance of a collision. I had to call the police to come and sit behind
... me to keep myself safe from a collision while I waited for a tow truck. A dealer diagnosed the vehicle with DTC P0016, indicating a crankshaft/camshaft position correlation fault. This points to either an engine timing problem or an internal engine failure. The dealer confirmed the issue and gave me a repair estimate of about $11,000. Both the vehicle and repair records can be inspected if needed. Before the failure, the vehicle showed signs of an engine timing or internal issue and eventually triggered the check engine light with code P0016. After researching, I discovered many reports from other owners with the same 6.2L L87 engine who faced similar problems, including P0016 faults, engine stalling, internal engine damage, and complete engine replacement. While some vehicles with this engine have been part of recalls or special coverage programs, my vehicle is not included, even though it shows the same failure pattern. At the time of the failure, the vehicle has about 140,336 miles. I believe my vehicle has the same defect affecting other 6.2L L87 engines. I ask that NHTSA look into whether more vehicles should be added to any current or future recalls or defect investigations. Thank you for your attention to this matter.Read more

2016 Hyundai Veloster with 1.6L Gamma engine experienced repeat catastrophic engine failure consistent with NHTSA Recall 20V746000 / Hyundai Recall 198 for connecting rod bearing failure. Failure #1: At 124,036 miles, engine developed loud knocking noise increasing with RPM, check engine light, and DTCs P0300 (random misfire) + P0303 (cylinder 3 misfire). Dealer repair order documented "broken int
... ake valve on cylinder 3." Engine was replaced at my expense because Hyundai stated VIN was not covered under Recall 198, despite failure mode matching the recall. Failure #2: Hyundai replacement engine failed at 164,410 miles (40,374 miles on replacement engine). Identical symptoms returned: loud metallic knock that increases with RPM, same DTCs P0300/P0303, misfire cylinder 3. Vehicle is currently inoperable. Safety Concern: NHTSA Recall 20V746 states connecting rod bearings "may wear prematurely, which over time can result in engine damage" and "can increase the risk of a fire." Both of my engines suffered catastrophic internal damage at ∼40k-50k mile intervals with identical codes and cylinder 3 failure. A broken intake valve is engine damage directly caused by bearing wear allowing piston-to-valve contact. This is a pattern defect. My VIN was excluded from the recall, yet suffered the exact failure twice. The replacement engine supplied by Hyundai had the same defect. This presents a safety risk of sudden engine stall, loss of power, or fire while driving. Requested Action: Investigation into why VINs experiencing the exact failure mode are excluded from Recall 20V746, and assistance obtaining remedy from Hyundai.Read more

The vehicle is a 2019 Volkswagen Jetta (VIN [XXX]). The primary malfunction appears to involve the vehicle's electrical and starting system. The vehicle intermittently displays a "Starter System Faulty" warning message and has shut off unexpectedly while driving, particularly during periods of hot weather. The exact failed component has not been definitively identified, but the symptoms are consis
... tent with an ignition switch or electrical system defect. The vehicle is available for inspection upon request. This defect creates a serious safety risk because the vehicle can lose power and stall while in motion. The unexpected shutdown places the driver, passengers, and surrounding motorists at risk of a crash, especially when driving at higher speeds or in traffic. The problem has occurred multiple times and has persisted despite repair attempts. More than $1,000 has been spent on repairs related to this issue, but the problem has not been resolved. The vehicle has been inspected and repaired by service technicians; however, the underlying cause has not been successfully corrected. The vehicle has not been involved in a crash as a result of the defect, but the recurring stalling and loss of power present an ongoing safety concern. Volkswagen has been contacted regarding the issue, but assistance has not resolved the problem. Prior to the vehicle shutting off, warning messages including "Starter System Faulty" appeared on the dashboard. The issue is more likely to occur during hot weather conditions. These warning messages began appearing before the stalling events and continue to occur despite repair efforts. At just 30,000 miles — less than half the mileage at which these failures are typically reported — my GLC 43 was found to have active oil leaks in multiple locations: 1. Front engine covers — oil is seeping from both the front cover gasket area and migrating downward along the engine block. The dealer recommends a full reseal of the front covers at a quoted cost of $1,944. 2. Radial shaft
... sealing ring of the front axle gear — is actively seeping gear oil, with a quoted repair cost of $1,718. My concern is not limited to cost. Oil migrating from the front cover area toward the lower engine, accessory belt, and turbocharger assembly creates a documented fire risk. The turbocharged M276 engine operates at extremely high surface temperatures in the areas where this oil is accumulating. I will be candid: I have genuine concern about operating this vehicle until these repairs are completed, and I am troubled that a safety-relevant failure of this nature is occurring at such low mileage on a vehicle in this price class.Read more

On June 7, 2026, I was driving my 2020 GMC Terrain. I was going at about 40MPH along a very busy main road. While slowing for a red light the Check Engine Light turned on and the car displayed a notice about reduced engine power. Seconds later, the brake pedal became stuck and would not press down all the way and I lost braking ability. The car rattled/shook violently, made a grinding noise, and t
... hen the engine shut off and the car stopped abruptly, narrowly avoiding hitting the car in front of us. I turned on the hazard lights. I was not able to get the car to start again, nor was I able to get the car to shift into any other gear including neutral. We were nearly hit by other vehicles multiple times while attempting to get the car to move again, and to get myself and my child safely out of the road. Myself and multiple bystanders attempted to engage the transmission interlock override multiple time. We were eventually able to engage it and push the car to the side of the road. This failure created an extremely unsafe situation for all involved as this road is very busy and there were numerous near-collisions both with my disabled vehicle, and with other drivers serving out of the way and into other lanes at the last second. As time went on and the battery drained, most of the car's exterior lights turned off and other drivers had a hard time seeing my car. I used a strobe flashlight to alert other drivers but this required that I had to stand in the road near the vehicle. Police were called immediately when I found that we could not move the car out of the road, but they did not arrive. Ultimately the vehicle was stuck and inoperable in a travel lane for over two hours. The car was then towed to a nearby repair shop. The mechanic evaluation stated that the brake vacuum pump had failed resulting in loss of braking ability and expensive damage to the engine and other components. The manufacturer is aware of this issue. See: Thieme, et al. v. General Motors LLC.Read more

I was traveling at highway speeds in active, high-velocity traffic when my vehicle suffered a sudden, catastrophic mechanical failure that nearly resulted in a severe multi-car accident. Without any warning, the vehicle completely lost its ability to accelerate, flatly refusing to pick up speed or maintain momentum despite pressing the gas pedal. This unexpected, immediate drop in propulsion ma
... rooned me in high-speed lanes, leaving the vehicle dangerously vulnerable as oncoming traffic rapidly closed the distance behind me. Drivers behind me were forced to aggressively swerve and brake to avoid a high-speed rear-end collision. Moments after this life-threatening loss of power occurred, the Check Engine Light illuminated on the dashboard. Subsequent technical inspection revealed a severe engineering defect: the internal seals of the factory coolant control components failed, forcing pressurized engine coolant backward into the engine’s primary vacuum system. This pressurized fluid completely contaminated the vacuum lines, the vacuum reservoir tank, and the electronic solenoids—including the N75 boost control solenoid. By filling the vacuum infrastructure with liquid coolant, the engine was instantly starved of its ability to regulate turbocharger boost, triggering a critical "limp mode" and a P0299 underboost fault. The catastrophic failure of an internal cooling seal directly compromising an active drivability safety system represents an unreasonable and severe defect. It transforms a routine drive into an immediate, uncontrollable highway hazard. I am filing this report because this known coolant migration pattern presents a clear, present, and life-threatening danger to owners operating this engine platform.Read more

On June 1, 2026, software update 68C was performed at the dealership on my 2025 Jeep Wrangler 4xe. On June 6, 2026, at approximately 9:00 a.m., after driving the vehicle for about 25 minutes, I experienced a serious malfunction. While making a turn at a speed of less than 10 mph, the vehicle suddenly and without warning lost throttle and propulsion. It appeared to hesitate as though shifting betw
... een electric and gasoline power, and then abruptly and erratically lurched forward, accelerating to what felt like maximum speed. The Jeep struck a chain-link fence, which slowed its movement, but it continued to surge forward. I applied the brakes and attempted to maintain control of the vehicle, but it kept lurching ahead. At no point did the vehicle emit a safety warning, sound an alert, or attempt to stop automatically, even though it normally responds to obstacles when operating properly. I repeatedly attempted to place the vehicle in reverse, but it did not respond. I turned the engine off and back on several times, and only after more than three attempts did the vehicle finally recognize reverse, allowing me to back away from the fence. A man in a truck assisted me and contacted the police. Both the responding police officer and the assisting individual expressed concern about the potential danger, particularly because the vehicle is a 4xe. I am now extremely hesitant and fearful to operate or move the vehicle. The vehicle is available for inspection and has not been inspected yet by dealership or insurance. When i first purchased the vehicle off the lot a few years back (its a 21, so probably in 22?) Brand new off the lot minimal miles. Within the first few months we started having issues with the power wanting to shift down. We took it in, the dealership found nothing wrong with it and cleared the codes. This happened a few more times. The last 2 times I took it in. They mentioned that there were re
... calls on it and could possibly be causing the issue. Took care of that (last august- have docs. to prove it as along with what they did and what we stated). Fast forward now 2 months ago (April of 25) and it starts acting up again and wants to downshift, shifting hard, stuttering and not wanting to drive, the same things we have been complaining about since beginning with lights on etc. However, this time we tried to shut it off and give some down time to get back home. After about 10 mins, we went to start the truck to get it home, and it would not start. We had it towed to the dealership. Once they got it, after 2 weeks of waiting we were finally presented with, the starter went out and the wiring box needed to be replaced (this has to be done before they can look into the original problem again) (because without those 2 things fixed, they wont be able to do) it is now June. I took the vehicle home, due to not having funds to fix right away. After doing some research and some digging, it would appear that the issues that we are having with the truck are issues that are already being recalled on this specific truck and model. However, for mine it is not, because I have the 2.7 l turbo and not the 6.8. THere have been numerous complaints about the known issue.(which i found out is the other wiring by the engine that is being chafed which is causing the wires to break and causing the other box and alternator to go and be replaced). I am writing to report a severe safety defect regarding my 2018 Jeep Renegade. The vehicle suffers from an extreme internal oil consumption defect, burning 1 quart of oil every 1,000 miles. According to certified automotive diagnostics, this is five times the normal operating rate. This rapid oil depletion causes the vehicle to abruptly lose power, stall, and completely shut down while driving in
... active traffic without warning. Sudden engine failure at highway speeds creates an immediate, critical risk of collisions. The vehicle’s history directly matches documented manufacturer issues. The original purchasing dealership notified me that the vehicle was impacted by Customer Satisfaction Notification W80 for abnormal oil consumption. My research also confirmed related under-hood wiring recalls for this model year that cause fuel pressure drops and engine stalling. However, when I presented the vehicle to an authorized Jeep dealership in Newnan, Georgia, due to repeated stalling, the service department refused recall coverage because my specific VIN was not flagged in their database. They replaced a valve cover gasket instead, but the severe oil depletion and dangerous stalling persisted. A subsequent inspection by an independent licensed automotive shop confirmed the excessive oil burning and determined the engine has suffered fatal internal wear, requiring a complete engine replacement. Because the manufacturer excludes my VIN from the official recall population, they refuse to provide any administrative or financial remedies. However, my vehicle is exhibiting the exact mechanical failures, dangerous stalling, and severe oil consumption targeted by NHTSA’s investigations into the engine. Excluding a vehicle experiencing identical safety failures based solely on an arbitrary VIN window leaves consumers unprotected. This unaddressed defect ultimately rendered the vehicle un-driveable, forcing me to surrender it, causing financial hardship and negative credit impact.Read more

I experienced a sudden coolant leak on my 2017 Ford F‑150 equipped with the 3.5L EcoBoost engine. The Ford dealership diagnosed the issue as leaking turbocharger coolant lines. The coolant was leaking in the turbo area and dripping onto extremely hot exhaust components. This creates a risk of rapid coolant loss, overheating, loss of power while driving, and potential engine compartment fire. Th
... e Ford service advisor told me directly that he has “seen this many times,” which indicates this is a recurring failure on EcoBoost engines and not normal wear. This suggests the coolant line design or fittings may be defective and failing prematurely. The repair estimate was extremely high because the dealer included replacement of both exhaust manifolds. This failure occurred without warning. If the coolant loss had happened at highway speed, it could have caused sudden engine overheating or power loss. I am requesting NHTSA investigate turbo coolant line failures on Ford EcoBoost engines due to the safety risk of coolant leaking onto hot turbo and exhaust components.Read more

Bad starter. When utilizing auto start/stop, the engine would fail to restart at stop. The car would need to be shifted back to park, turned all the way off, and then back on to restart, resulting in a delay - an unsafe practice at an intersection.

When I started the car at 10pm , a loud pop was heard and warning lights on the dashboard indicated an electrical problem. I turned off the radio and anything extra that could use up power; during the next 45 minutes I took surface streets instead of the freeway to drive 22 miles back home and speed was limited to 40 mph. The next day the same warning lights were on the dashboard and speed was l

While driving at highway speeds, the vehicle displays a "Drive Train Malfunction" warning message and the traction control warning light illuminates. Shortly after the warning appears, the vehicle loses power and may enter a reduced-power mode or stall, creating a serious safety hazard in traffic. This has occurred while traveling on the highway, forcing sudden deceleration and creating the risk o
... f being struck by other vehicles. The check engine light is illuminated, and diagnostic code P0A1E is present. The problem is intermittent and unpredictable, making it difficult to determine when the next failure will occur. The vehicle has been taken to Ford for diagnosis multiple times, and significant money has been spent attempting to repair the issue. Despite these efforts, the root cause has not been identified and the problem continues to occur. This defect affects the drivetrain system and impacts the safe operation of the vehicle. The sudden loss of power on the highway places the driver, passengers, and surrounding motorists at risk. Based on discussions with other owners, this appears to be a common issue affecting Ford Explorer Police Interceptor Utility vehicles and warrants investigation by NHTSA.Read more

I am submitting this complaint regarding my 2022 Hyundai Tucson equipped with the 2.5L engine. The vehicle has experienced multiple incidents of severe engine misfiring while driving. During these events, the engine begins to shake violently, the entire vehicle vibrates, and engine power is significantly reduced. The vehicle becomes unable to accelerate normally, creating a serious safety hazard,
... especially when driving on highways or in traffic. The malfunction has occurred several times. Diagnostic trouble codes P0302 and P0303 have been recorded, indicating cylinder misfires. In some cases, turning the engine off and restarting it temporarily restores normal operation. On other occasions, adding fuel system cleaner appeared to temporarily improve the condition. However, the problem eventually returns. Because the condition is intermittent, the Hyundai dealership has been unable to reproduce the failure during inspection and therefore has not performed a repair under warranty. Despite multiple visits, the root cause remains unidentified. I am concerned that the sudden loss of power and severe engine vibration could result in an accident if the vehicle is unable to maintain speed or accelerate while traveling at highway speeds. This issue has significantly affected the safe operation of the vehicle. I request that this matter be investigated, as it appears to be a recurring safety-related defect.Read more

My vehicle was brought to an authorized Hyundai dealership In November 2025 to complete safety recall campaign KSDS (Campaign 966). Soon after picking up the vehicle after the dealer flashed the ECU, the vehicle went into a permanent, dangerous "limp mode" that severely limits the engine's RPMs, making it impossible to accelerate to highway speeds. The vehicle's computer displays zero OBD-II diag

The dealership is refusing to address this issue, falsely claiming a plugged catalytic converter, despite the vehicle having passed a strict California Smog Check with perfect emissions and zero catalyst codes, just a few weeks before the update was performed, and ran perfectly normal. This recall software update has rendered my vehicle extremely unsafe to drive on public roads, and the manufacturer's authorized repair facility is refusing to remedy the electronic damage caused by the recall installation. I am trapped driving a disabled vehicle in a permanent, dangerous software loop.Read more

NHTSA Defect Report (Initial Portal Filing) Manufacturer Target: General Motors (GM) Filer: [XXX] Classification: Mechanical Defect & Administrative Evasion Narrative for Submission I am filing this formal defect report regarding a catastrophic, systemic safety failure present in the 2025 Chevrolet Traverse architecture (VIN: [XXX]), manufactured by General Motors. The vehicle was originally purc
... hased at AutoNation Timonium. THE SAFETY DEFECT AND TELEMETRY PROOF On March 18, 2026, the vehicle suffered a total failure of the transmission and drivetrain systems. It was immediately surrendered to Preston Chevrolet, where it remained completely out of service for 34 consecutive days. GM's own OnStar Diagnostic Telemetry automatically dispatched critical fault alerts while the vehicle was stranded, explicitly logging the following irrefutable failure codes: DTC U1962 (PCM - Engine and Transmission System): Warning stated: "System transferring power to the drive wheels not performing as expected." This indicates a catastrophic decoupling or failure to transfer motive power. If this failure occurs at highway speeds, it results in a total loss of propulsion, creating an extreme rear-end collision hazard. DTC U0452 (VIU - OnStar System): A simultaneous failure in the Wireless Communication and Safety system, meaning that when the drivetrain catastrophically fails, the vehicle's automated emergency SOS and crash-reporting telemetry is actively blocked or compromised. ELEVATION TO ADMINISTRATIVE EVASION General Motors and its authorized dealer network were provided with formal written notice of this defect. The 30-day statutory right to cure window fully expired without any mechanical resolution. Rather than deploying engineering solutions to rectify the severe safety defects they released into the public domain, GM has instead deployed aggressive legal maneuvers designed to evade accountability.
