I'm reporting a safety issue with BMW vehicles equipped with the B58 engine, primarily 2019-2021 models. These vehicles have an oil pump with an internal plastic component that regulates oil pressure, and this part is fracturing in the field, causing sudden loss of oil pressure. The failure typically occurs during cold starts in low temperatures. When it fails, oil pressure becomes erratic or is l
... ost entirely, often with little to no warning before engine damage or vehicle disablement. Warning signs, when present, include inability to measure oil level electronically, abnormal oil pressure, or sudden drive train malfunction warnings. These can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a dangerous situation because the vehicle can become disabled without warning while driving or after startup. This is especially hazardous when cars become immobilized in traffic or during winter conditions when failures occur most frequently, leaving occupants exposed to freezing temperatures while awaiting assistance. Pump replacement requires extensive disassembly and commonly costs $6,000-$8,000, assuming no engine damage. A critical concern: owners cannot determine if their vehicle has the original plastic pump or the later metal revision. BMW's VIN-based parts lookup now shows updated components even when original plastic parts remain installed, and BMW has removed the original plastic part number from dealer networks entirely. Failures are reported across multiple production years, and the scope of affected vehicles remains unclear. Because this condition causes sudden vehicle disablement without adequate warning, especially in cold weather, I believe this warrants investigation as a potential safety-related defect.Read more

2018 Cadillac Escalade ESV Platinum, 6.2L V8 L86 engine, under 100,000 miles. Engine seized and was towed to an authorized GM dealership. Component is available for inspection. Prior to seizure, the vehicle experienced a
... highway stall preceded by a harmonic balancer failure, belt system failure, and charging system collapse. The vehicle lost propulsion at highway speed, creating a crash risk to myself and other drivers. The failure mode, spun rod bearing with crankshaft damage, is physically identical to the defect documented in GM Safety Recall 25V-274, covering connecting rod and crankshaft manufacturing defects in 6.2L V8 L87 engines in 2021-2024 vehicles. The L86 and L87 are variants of the same Gen V small block architecture with the same bottom end design. I contacted Cadillac Customer Care and requested a Field Technical Specialist review. GM instructed the dealership to delay filing a technical assistance case until my third party warranty determination was made. GM subsequently refused to file any technical case, stating the L86 is not included in the recall and they have no plans to include it. The failure has been confirmed by an authorized GM dealership. I am requesting NHTSA evaluate whether the recall scope should be expanded to include 2018 L86 engines, and whether GM's refusal to conduct technical investigation while directing a third party warranty to absorb a potentially recall-related repair constitutes inadequate recall handling.Read more

Two nights ago I was at Domino’s Pizza. ... dash and it would say charging malfunction once I got home we took a look at the engine and noticed that none of the belts were working at all and then I started to google what could possibly be the problem and a couple of different recalls came up recall number 12V550000 and that is engine and engine cooling engine diesel what the name of it is and the recall date is 1127 2012 and then the next one is 2012 BMW X5 diesel SUVs from the model year 2012 923 BMW X fives have been recalled because they have defective engine belt, tensioners and cracks that develop along the welds if the belt tensioner fails there could be a sudden loss of power steering assist that is sounds like exactly what happened to my car and then there’s another one recall number B121918 which is from September 2019 and it is called idler pulley bolt and come to find out that actually was a recall on my vehicle, but the previous owner is the one that had it repaired so I’m not sure if BMW is willing to repair it again but there’s not a lot of miles on this cards a 2012 I bought it with 95,000 miles on it and it currently only has 131,000 miles on it. I am reporting a serious safety defect involving repeated engine stalling and failure to restart associated with the auto idle stop/start system on a 2025 Acura MDX. The vehicle was leased in December 2024. Beginning in late summer and early fall of 2025, the vehicle intermittently shut off at stops and failed to restart when releasing the brake. This resulted in the vehicle becoming unresponsive
... in active traffic conditions, creating a significant safety hazard. The vehicle was presented to an authorized dealer for this issue on multiple occasions: August 15, 2025 September 9, 2025 October 31, 2025 After repeated unsuccessful attempts to diagnose and repair the condition, the dealer ultimately replaced the starter on October 31, 2025. This repair did not permanently resolve the issue. In April 2026, the vehicle again began exhibiting hesitation and delayed engine restart following auto idle stop events. On April 30, 2026, the vehicle stalled again, demonstrating that the defect persists despite prior repair efforts. This condition is intermittent and unpredictable, which increases the risk, as the vehicle may fail to restart when entering or crossing intersections or when moving with traffic. The inability of the vehicle to reliably restart creates a clear safety concern. Given the repeated repair attempts and recurrence of the defect, this appears to be a systemic issue rather than an isolated component failure.Read more

I replaced all 4 coils, and the misfire issue persisted. It is intermittent for now, but indicative of a head gasket failure according to the dealership. The issue is most frequent when accelerating to highway speeds- ie a highway on ramp. One time the misfire put my car into "limp mode" on a tight on ramp, forcing me to restart the car in a dangerous situation. The other time it happened, I was thankfully able to coast downhill to an exit to restart my car. In either case, it created a dangerous loss of power, and given the head gaskets are not a wear item- it should not be the drivers responsibility to replace the faulty part.Read more

Police responded to the scene, Honda Roadside Assistance was contacted, and the vehicle was towed to an authorized Honda dealer. The suspected affected systems include the brake system, electrical system, battery/charging system, ABS/VSA system, Auto Idle Stop system, and related control modules. The vehicle is currently available for inspection at an authorized Honda dealer. The issue has not yet been fully diagnosed or confirmed by the dealer. The dealer advised that the battery may need to be replaced before diagnosis can continue. However, the battery may have discharged because the vehicle remained in a fault state with warning lights/system alerts active after the original brake/electrical failure. Therefore, the dead battery may be a consequence of the original failure, not necessarily the root cause. There were no known prior warning signs before this incident. The warning messages and lamps appeared at the time of the failure. A full diagnosis is pending.
