Passenger (recovering from leg surgery in full leg brace) was forced to exit onto a snow bank and enter a tow truck cab, risking re-injury. In addition, the vehicle experienced transmission failure around the same timeframe, significantly affecting the safety and reliability of the vehicle for family transportation. Initially they told me it was the temperature regulator, however, they put a new regulator on and tried to re-pressurize the system. Repairs: replaced both EV coolant pumps (PN 85605761), replaced Drive Motor Inverter Module 2 (PN 24059125), coolant flush, software update. RO #BUCS138101. GM TAC Case #88689105. Six days after return, on 03/30/2026, the same defect recurred with the same fault codes. Vehicle has now been back at the dealer 38+ additional days with no resolution. GM had pre-sale knowledge of this exact defect on this exact VIN. The vehicle was a GM corporate evaluation unit (factory option code OAR — EVALUATION VEHICLE) operated out of GM Tech Center in Warren, MI. GM took it out of service on 08/07/2025 at 30,169 miles. I took delivery 16 miles later at 30,185 miles. A GM Driver Inspection Form left inside the vehicle at delivery is handwritten with "Limited Speed 12 mph" — the exact limp-mode behavior. GM auctioned the vehicle through CVO Warren Distribution Center without disclosing this condition. I request ODI open a preliminary evaluation of cooling-system thermal-protection failures on 2022–2024 Hummer EV Pickups and SUVs, particularly units with prior corporate/fleet use.Read more

I am reporting a recurring safety issue following the Acura rod bearing recall repair on my 2019 Acura MDX SH-AWD. The original recall resulted in catastrophic engine failure and Acura replaced the long block assembly. Since that repair, the vehicle has repeatedly failed and returned to the dealership multiple times for the same issue pattern. Symptoms include: * “Emissions System Problem”

... espite confirming the seriousness of the defect, Toyota has refused to repurchase the vehicle and has failed to provide any timeline whatsoever regarding when the engine replacement remedy will become available. Toyota’s position has simply been that the engine will be repaired “once a remedy is available,” without any estimated repair date or meaningful resolution for affected consumers. The recall itself states that the defect may result in engine knocking, rough running, engine no-start, and/or sudden engine stall. Toyota further acknowledges in its recall documentation that engine stall while driving can lead to loss of motive power and increase the risk of a crash. This is a major safety issue, not a minor inconvenience. I live and drive in [XXX] , where highway driving and high-speed traffic are unavoidable. Due to the risk of sudden engine failure or loss of power, my family and I no longer feel safe operating the vehicle under normal driving conditions. As a result, we have drastically limited use of the truck despite continuing to make monthly loan payments, insurance payments, and registration expenses. Additionally, the truck has experienced multiple other recall and repair-related issues since purchase. Several recalls have already required service visits, and additional unresolved recalls remain pending. I own a 2019 Mazda CX-5 with approximately 36,000 miles. The vehicle developed a strong burning oil smell while driving, particularly through the HVAC/vent system. I initially reported the odor to a Mazda dealership, but no issue was identified. Approximately two months later, the smell returned and worsened. ... he cylinder head near the exhaust manifold. According to the repair documentation, oil was accumulating on the exhaust due to seepage from the cylinder head. The repair required full cylinder head replacement. This created significant safety concerns because leaking oil was contacting hot engine/exhaust components while the vehicle was being operated. Burning fumes entered the passenger cabin through the vents while driving. I was concerned about the risk of fire, smoke inhalation, sudden oil loss, and possible engine failure while on the road. There were no warning lights prior to the failure. The primary symptom was the persistent burning smell. The dealership replaced the cylinder head. The original repair estimate was approximately $6,000. Mazda later provided partial goodwill assistance, but I was still required to pay approximately $2,500 out of pocket. After researching this issue further, I found numerous similar NHTSA complaints involving 2019 Mazda CX-5 vehicles experiencing cylinder head leaks/cracking near the exhaust manifold, burning smells, smoke or fumes entering the cabin, and major engine repairs at relatively low mileage. Mazda has acknowledged similar cylinder head concerns in CSP11 and TSB 01-002/23 for certain VIN ranges but denied coverage for my vehicle because the leak involved oil rather than coolant and because my VIN was outside the covered range. I am requesting that NHTSA investigate whether this represents a broader cylinder head defect affecting additional 2019 Mazda CX-5 vehicles outside the currently covered VIN range.Read more
