[Recurring No-Start Issue with Multiple Warning Alerts – Safety Concern] I am reporting a recurring vehicle issue involving failure to start, which I believe may pose a safety risk. The issue has occurred five times: August 11, 2023 April 12, 2025 June 13, 2025 December 12, 2025 April 1, 2026 During these incidents, the vehicle failed to start and displayed multiple warning messages on the dash
... board. On some occasions, the vehicle only started after several hours. The vehicle becomes completely inoperable when this issue occurs. After the third occurrence, I brought the vehicle to an authorized service center. However, the issue was not resolved and has continued to occur. This repeated failure raises serious concerns about vehicle reliability and safety, as the vehicle may become inoperable without warning. I have attached documentation, including repair records and photos of the dashboard warnings. I am submitting this report so that this issue can be properly investigated.Read more

Incident date: May 17, 2026
My 2019 Kia Sorento equipped with a Theta II engine entered limp mode and displayed a flashing check engine light along with an active P1326 diagnostic trouble code. The P1326 code is a proprietary Kia/Hyundai specific code that can only be generated when the Knock Sensor Detection System (KSDS) software update is present on the vehicle. This software was developed and distributed by Kia specifica
... lly to detect Theta II engine bearing failure and rod bearing wear before catastrophic engine damage occurs. The presence of the P1326 code is confirmation by Kia's own proprietary detection system that this engine is experiencing the exact failure pattern associated with the Theta II engine defect. The vehicle was towed to an authorized Kia dealership. The dealership refused to perform a BTC test or enter the vehicle into Techline, citing that the VIN does not appear in the Theta II settlement database. Kia America customer care has also denied coverage. The vehicle remains inoperable. Safety risk is significant. The Theta II engine defect has been directly linked to engine fires in Kia and Hyundai vehicles. Limp mode and sudden loss of power while driving creates serious risk of accidents, injury, or death to the driver and other motorists. The 2012-2019 Kia Sorento is a model year range specifically identified in Theta II multidistrict litigation. Kia's own KSDS software flagging a P1326 code on this vehicle while Kia simultaneously denies any defect-related coverage is contradictory and raises serious safety concerns for owners whose VINs were excluded from settlement but whose vehicles are exhibiting identical failure symptoms.Read more

Subject: 2024 Jeep Grand Cherokee 4xe Safety Defects, Recall Failures, and Loss of Safe Vehicle Use Complaint Narrative: I am the owner of a 2024 Jeep Grand Cherokee 4xe plug-in hybrid vehicle manufactured by FCA US LLC / Stellantis. I am filing this complaint due to ongoing safety-related defects and recall-related concerns affecting the vehicle, including repeated recalls, reduced confidence in
... vehicle safety, loss of vehicle use, and concerns regarding battery system reliability and fire risk. My vehicle has been subject to multiple recalls and/or service campaigns involving the high-voltage battery system and related charging/safety systems. These recalls and warnings have created serious concern regarding the safe operation, charging, parking, and ownership of the vehicle. Specific concerns include: Repeated recall notices affecting vehicle safety Concerns regarding high-voltage battery failure and/or fire risk Instructions or warnings affecting normal use of the vehicle, including charging limitations and parking restrictions Significant inconvenience and loss of use Time spent at dealership service departments for inspections, software updates, or attempted remedies Recent, additional recall 78C/NHTSA 25V-766 Reduced reliability and diminished confidence in the safety of the vehicle Potential diminished resale value due to ongoing recall history The repeated nature of these issues raises concerns that the remedies may not fully resolve the underlying defects. As an owner, I no longer feel confident in the long-term safety and reliability of this vehicle. I request that NHTSA continue investigating the safety and effectiveness of the recall remedies affecting Jeep 4xe plug-in hybrid vehicles and evaluate whether consumers are receiving adequate permanent repairs and protections. My 2022 Chevrolet Silverado 2500HD High Country 6.6L Duramax engine has an exhaust leak that causes diesel exhaust to travel into the pickup's cab through the outside air vent and air conditioning system during the exhaust system's regeneration. This occurs when the regeneration occurs in city driving especially at low speeds, stop lights, stop signs, parking lots, etc. The pickup has been to the
... dealer to find and make necessary repairs twice and is currently at the dealer's shop for the third time. This issue with the 6.6L Duramax pickups appears to be a common theme amongst owners and GM should be providing dealers with the appropriate fix with a recall of the exhaust system. This is a safety hazard as carbon monoxide enters the cab with the diesel exhaust and must be rectified. On my pickup, this started at approximately 55,000 miles. Thank you for your prompt attention to this serious air quality issue inside the pickups' cabs.Read more

1. Component failed and availability for inspection: The engine assembly block failed due to a manufacturing defect known as "coolant intrusion into the cylinders." The vehicle and the defective engine are currently located at my residence and remain fully available for federal or manufacturer inspection upon request. 2. Safety risk: The safety of myself and others was severely compromised. Coolan
... t entering the combustion chambers causes sudden, severe engine misfires across all cylinders, erratic power loss, and instantaneous stalling. Operating a vehicle where the engine can suddenly lose all power on high-speed roads creates an immediate risk of rear-end collisions and catastrophic traffic accidents. Furthermore, high-pressure internal coolant leaks pose an inherent risk of engine fires. 3. Problem confirmed by a dealer: Yes. The problem has been officially diagnosed and confirmed by an authorized Ford dealership service center, which documented internal coolant intrusion. 4. Inspected by manufacturer, police, or insurance: The component was inspected and verified by the vehicle manufacturer's authorized dealership technicians. It has not been inspected by the police or insurance representatives, as no crash has occurred yet. 5. Warning lamps, messages, or symptoms prior to failure: Prior to the diagnosis, the vehicle exhibited a "Check Engine Light." Currently, the vehicle is suffering from critical failure symptoms: the Check Engine Light is flashing constantly, the engine misfire codes are actively showing on all 4 cylinders, and the vehicle shakes violently at low RPMs (rough idling). These severe symptoms first appeared approximately 2 moths ago and have progressively worsened due to the coolant intrusion.Read more

Driving down a highway at 70 mph and a notification popped up saying to pull over and turn off the vehicle because the engine would shut down soon. I lost the ability to accelerate and impeded traffic. Had to pull over on the side of the road with minimal shoulder. This has happened dozens of times; all at high rates of speed and often in heavy traffic.
Incident date: May 18, 2026
While driving home during rush hour traffic on the highway with my children in the vehicle, without warning it stopped accelerating when I passed the gas pedal and started slowing down. When I let go of the gas pedal it would speed back up. I couldn't safely merge over to the right with heavy traffic and ended up on the left shoulder. I turned the vehicle off and tried to restart it, it strugg
... led to start, but when I let go of the key it would turn over. The brake system lights and check engine light were illuminated after the incident, but not during. The code was for p0087 and honda service confirmed it's a failed high pressure fuel pump, but it's not under the recall so the repair isn't covered. I'm extremely lucky to have gotten to the side of the road without causing an accident. I was not driving my normal route that day, but if i had been this would have happened on a stretch of the highway that's under construction where there was no shoulder to pull onto. So if I'd have gone a different way home that day, we'd have most certainly been involved in a collision. They've anyway Texans the low pressure fuel pump but these high pressure ones are just as dangerous when they fail, even more so!Read more

Greensboro Transit Agency (GTA), operated by RATP Dev USA, is formally notifying you of an engine compartment fire involving one of our transit vehicles. Vehicle Information: •Fleet Number: 764 •Manufacturer: New Flyer •Vehicle Type: Heavy-Duty Transit Bus •Incident Date: [XXX] •Location: Greensboro, North Carolina Summary of Incident: While in revenue service, the operator reported hear
... ing an abnormal noise and observed white smoke coming from the rear engine compartment area. The operator safely evacuated passengers and emergency responders arrived quickly to suppress the fire. No injuries or fatalities occurred. A third-party fire investigation was conducted by Halsey King & Associates. Based on the preliminary findings, the fire is believed to have originated from a catastrophic failure of the Thermo King A/C compressor clutch bearing assembly. The bearing reportedly overheated, seized, and caused the compressor belt to ignite while still being driven by the engine. The investigation identified evidence consistent with: •A seized A/C compressor clutch bearing •Extreme heat generation within the clutch assembly •Belt ignition and progression of fire through engine compartment components At this time, our maintenance records do not indicate any previously documented defects associated with this specific Thermo King assembly prior to the incident. Our fleet currently has inspection and preventive maintenance procedures in place related to belt alignment, pulley alignment, belt tension, clutch air gap measurement, and related A/C system inspections. We are providing this notification for awareness, documentation, and any further review deemed appropriate. On Saturday 16th Oct 2026 around 10 AM EST, while driving down the ramp from Parking Floor 2 to Parking Floor 1 in the JFK Terminal 4 Short Term Parking garage (John F. Kennedy International Airport, New York), the Tesla vehicle suddenly accelerated forward very fast on its own without any input on the accelerator pedal. Either I was not pressing the gas pedal at the time, or I was slowing pressin

No other vehicles were involved. The impact caused: My Tesla Model Y to sustain severe front-end damage and appear to be a total loss. Injuries to both passengers. Minor scratches on my hands. Sequence of Events: I was driving the vehicle at low speed down the ramp from Parking Level 2 to Level 1 in JFK Terminal 4 Short Term Parking. Without pressing the accelerator, the car unexpectedly accelerated. Braking did not prevent the impact. The vehicle came to a stop only after hitting the fence. Today, I have reported this as a sudden/unintended acceleration event to Tesla and requested a full vehicle data log review. I would like Tesla to investigate following. 1. Why the car suddenly accelerated on very speed? 2. Why the steering got locked when it happened? 3. Since it is the closed parking lot, why Tesla did not stop automatically before hitting fence? Hyundai denied assistance solely because the vehicle was outside the warranty period by time and mileage, despite the seriousness of the failure. I am concerned this issue may be related to broader engine or oil-related defects reported in Hyundai vehicles, including possible oil starvation, excessive oil consumption, or internal engine contamination that could contribute to turbocharger damage or failure. The vehicle experienced engine failure due to coolant intrusion into the cylinders on a 1.5L EcoBoost engine. The problem has been confirmed by Ford and is consistent with the known coolant intrusion defect affecting many Ford 1.5L EcoBoost engines. Ford is
... sued Customer Satisfaction Program 21N12 for certain affected vehicles due to this exact issue. The vehicle has approximately 76,000 miles and is under the mileage limit referenced in Ford’s program, but Ford denied full coverage and only offered partial financial assistance toward the repair. This issue creates a serious safety concern because the engine can lose power, misfire, overheat, or fail unexpectedly while driving. The failure appears to be caused by a defect in the engine design allowing coolant to enter the combustion chamber. I believe this is a widespread defect and should be covered for all affected vehicles experiencing coolant intrusion and engine failure.Read more

The cylinder head was rebuilt last year, associated with a similar instance. I am now fully aware that the 2019 Tiguans are known to experience all of these problems, but when I brought my car in on previous occasions, that never got mentioned. I am told XYZ is an issue, then billed for repairs. The car was originally leased, and when I purchased it at the end of the lease, I also purchased a full premium warranty. Engine seized up while driving 60 mph on a known GM recall, after passing recall pico test
Incident date: May 12, 2026
In the second year of ownership I noticed that occasionally I would get no throttle response from the gas pedal when trying to accelerate. After several occurrences I began to pay close attention as to when this was occurring. I found that this occurs mostly, not exclusively, when I am slowing the vehicle to a near stop, not a full stop and then accelerating to continue, like slowing to round a
... corner on a green light and getting into traffic. For example, I was exiting a Home Depot parking lot, the driveway exits to the roadway. As I approached the driveway to enter the roadway I slowed, preparing to stop. Seeing no traffic on the roadway I then pushed the accelerator to enter the roadway. The pedal was dead, I got absolutely no acceleration. I pushed the pedal down slowly, nothing. I pushed the pedal to the floor, nothing. The truck rolled out onto the road, I coasted to the side of the road. Had there been traffic I'd have been hit. This has happened 20 times in two years and 80K miles. The most recent event was this week with my wife present, very scary, dead pedal, not throttle response. If you research "Dead Pedal" on GOOGLE, you will get the following response: The "dead pedal" in the Ford F-150 PowerBoost—a dangerous lag or total loss of throttle response—is a widely reported issue, particularly when accelerating from low speeds, turning, or right after starting. [1, 2, 3] The cause can range from a software glitch to transmission issues or failing sensors. Ford said there was a TSB, however the work performed had no effect on the dead pedal on my truck. Invoice attached from March 2026, it just occurred again this week. I am going to trade the vehicle in but I doubt that Ford will disclose this flaw to the next buyer, it will be their's to discover, hopefully not by "accident". I am going to trade this vehicle in and I'm sure the new buyer will have no idea of this potentially deadly flaw.Read more

As a loyal GM customer who has purchased three Cadillac Escalades and had them exclusively serviced at Cadillac dealerships, , I am writing regarding my 2023 Escalade, equipped with the 6.2L V8 (L87) engine, affected by the recall (N252494000) impacting approximately 597,630 vehicles . I recently took my vehicle to a Cadillac dealership for the recall inspection, where it passed, received a switc
... h to o@-40 Mobil 1 Supercar oil, and was granted an extended warranty of 10 years or 150,000 miles from the purchase date. Despite this , I am deeply troubled by GM's handling of the recall, which has caused significant unease, undermined my trust, failed to honor my loyalty, and imposed unfair burdens. I respectfully request clarification and action on the following issues: Inadequate Recall Remedy and Impact on Gas Mileage - The remedy for vehicles that pass inspection- switching from 0W-20 to higher-viscosity 0W40 oil- raises serious doubts about its effectiveness in addressing a manufacturing defect in the crankshaft and/or connecting rods, which GM acknowledges can lead to engine failure and increase crash risk. Many owners, including myself, question whether thicker oil is a permanent solution or a temporary workaround. Additionally, the switch to 0W-40 is likely to reduce fuel efficiency due to increased engine drag, imposing an unaddressed cost in deduced gas mileage. This is especially concerning for me, as I rely on my Escalade for annual drives from [XXX] to [XXX] for the winter months. Can GM provide technical evidence proving the oil change fully resolves the defect, and will the company compensate owners for the loss in fuel economy? Inconsistent Communications Regarding the Extended Warranty - The dealership assured my vehicle is "fine" after passing the inspection, yet GM is providing an extended warranty of 10 years or 150,000 miles. This contradiction - claiming the engine is defect-free while extending warranty coverage - suggests GM anticipat INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INRead more

The vehicle's electronic variable valve timing actuator (E-CVVT motor) failed, triggering a severe "Limp Home" engine mode. This defect causes dangerous vehicle surging/jumping during acceleration, limits engine output to 1500 RPM while idling, generates abnormal engine noise, and prevents the vehicle from exceeding 60 mph. This makes merging onto fast-moving highways or maintaining the flow of tr
... affic highly hazardous. The component defect was formally diagnosed at an authorized dealership (Herson’s Kia) on March 30, 2026. The manufacturer, Kia America, was formally notified of this powertrain hazard but has delayed providing a remedy under the PI1802/KSDS warranty extension for over 49 days by demanding unrelated fluid maintenance histories for this non-lubricated electronic component. Because of the manufacturer's procedural obstacles, I am forced to operate an inherently unsafe powertrain configuration for essential transportation.Read more

In August 2025, my car went into "limp mode" (go no faster than 40 MPH and it was lurching and speed dropping) on the freeway, which was scary and dangerous. Once home, I called a certified mechanic. When this mechanic tried to start the car, it made a terrible clacking noise. He told me it sounded like the main bearings were going out. He discovered that there was a recall on this particular
... vehicle because the main bearings would fail. The local dealer confirmed that the car was subject to the recall and requested that the car be brought in. Because I am on limited income, I could not afford to have the car towed until November 2025. Once there, the dealership verified that the main bearings went out. A claim was made to the manufacturer. The claim was denied claiming "outside influence." I provided service records to the manufacturer, but before they had all the maintenance records, they closed my case and will no longer take documents or even speak to me or the dealer. I have been without a car for almost 10 months. I have seen documented cases that trying to start the car could lead to dangerous engine fires.Read more

There was a recall on my PVC part in my 2023 atlas. An effect of a clogged pvc is an oil leak. The Positive Crankcase Ventilation (PCV) system relieves built-up pressure inside your engine. When it fails or clogs, this pressure traps air inside the crankcase. The excess pressure often forces oil out through the weakest points, which can mimic a bad seal or gasket. I took my car to the dealership
... in Md and they claim that I need to get new seals on several parts of my car but they claim that my oil leak its not related to the PVC. This is a 2023 car with one owner and no previous problems. There are thousands of complaints online about the PVC part continuing to fail and continuing to cause oil leaks in consumers cars. The oil leak has also caused my driving belt to fail and in my case shred while driving. This is unsafe and makes cars slow down instantly and can cause major car accidents. Why are consumers expected to pay for seal replacements when its related to failed parts on a car. The VW dealership said that my engine light would need to be on for them to say its a result of the failed PVC but my engine light was on when I dropped my car off. The dealership said they will not replaced parts that were effected from failed PVC due to car being out of warranty. My research tells me that Volkswagen is legally responsible. Because auto manufacturers are governed by strict liability laws, they are responsible for injuries and property damage flowing from the defective product they sold.VW wants consumers to pay $842 dollars to replace a part that cost less than $20. The VW company said that I needed to get my recall repair done earlier in order for them to honor the damages however Ive never seen a recall that came with a time limits on car repairs. This is highway robbery and VW needs to be held responsible for failing to maintain accountable and damages to consumers.Read more
