We have a fleet of 2023-2025 Nissan Frontiers. Starting in December of 2024 we began having problems with the vehicles electrical system going wild and shutting the trucks down. It causes all of the lights to come on and the truck to stall and not restart, We did not realize that we had a problem until it happened to multiple vehicles over the last several months. A few of them were brought to
... the dealership where we were told they could not find any problems and just replaced the batteries. So, when some of the others did it, we just replaced the batteries ourselves. This week asked again if there was a known issue and were told Nissan only knows of a battery drain issue. These vehicles are shutting down sometimes while our people are driving them. They also try to restart themselves sometimes. They are automatically applying the breaks when this happens wile the ABS light comes on. This is an extremely dangerous situation for our fleet drivers as well as the motoring public. It has happened to 10 of the 15 Frontiers that we own.Read more

On Sunday, September 21st, 2025, my 2013 Kia Optima EX suffered sudden catastrophic engine failure while driving on the highway. The check engine light was flashing, the car misfired badly, lost complete power, and became unsafe to operate. The vehicle was then towed to Wagner Kia of Shrewsbury, MA. On Monday, September 22nd, Kia diagnosed the engine with holes in the intake valves (Cylinder 3)
... and multiple misfire codes (P2191, P0272, P0301, P0304, P0172, P0303). The dealership documented that major engine repairs would be required, including two intake valves, two exhaust valves, valve cover gaskets, hraf bolts, two ignition coils, and a valve job. This sudden engine failure created an immediate safety risk of stalling or losing power while in motion, which could have led to an accident. Despite Kia’s history of Theta II engine recalls and settlements, my VIN is not included, leaving this major engine defect unaddressed. I am requesting that NHTSA investigate and consider expanding Kia’s existing engine recalls to cover this engine failure, as it represents a serious safety issue consistent with Kia’s documented engine problems.Read more

Summary: Sudden loss of engine power while driving due to lifter failure. Details: While driving my 2022 Chevrolet Suburban equipped with the 5.3L V8 (RPO L84), the engine suddenly lost power without warning. The vehicle began misfiring and lost propulsion in traffic, creating a hazardous situation. I was unable to maintain normal speed and had to maneuver to safety while the truck rapidly lost p
... ower. The dealer later diagnosed the problem as a collapsed lifter/valvetrain failure — an issue that GM has already documented in service bulletins (PIP5776G, 23-NA-043) and a service update (N212353840). These bulletins show GM has long been aware of defective lifters in 5.3L engines. This is not just a drivability concern. A sudden engine failure and loss of propulsion while driving poses a clear safety risk, especially when merging, crossing intersections, or towing. I maintained the vehicle properly and have all service records. I request NHTSA investigate these widespread lifter failures in GM’s 5.3L V8 engines as a safety defect, since they can and do cause sudden loss of power in motion.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The low-pressure fuel pump (fuel delivery module) appears to be failing, consistent with Recall 25V-586 / Alfa Romeo Campaign 93C. This causes fuel starvation and loss of motive power. 2. How was your safety or the safety of others put at risk? The vehicle has experienced loss of motive power (L
... OMP) while driving. At times it jerks or hesitates when accelerating from a stop, and while climbing a hill it suddenly lost power, displayed a Service Electronic Throttle warning, and went into limp mode. This created a risk of being unable to accelerate safely in traffic, increasing the chance of being rear-ended or unable to merge. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. I described the symptoms to Alfa Romeo Service dept, and they told me it is related to the recall and that other customers have reported the same symptoms and warning lights. However, they advised that there is currently no remedy available, and I must wait for the fix before scheduling an appointment. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet. Alfa Romeo advised that no remedy is available at this time, so the vehicle has not been formally inspected by the dealer or manufacturer. Advised not to drive long distances until fix determined. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. Check Engine Light and Service Electronic Throttle came on as car lost power and entered limp mode with code P0299. Alfa Romeo Service Dept tech said these warnings match the recall. Rep said this shows how loss of motive power from fuel starvation manifests electronically & ECU interprets fuel starvation as underboost, which then triggers limp mode.Read more

My 2016 Kia Soul is now consuming oil excessively and emitting smoke from the exhaust. These symptoms only began after a dealership performed recall-related work (knock sensor replacement and KSDS software update). The vehicle is covered under Kia’s Technical Service Bulletin ENG222 (Rev. 7, Feb. 2025), which lists the Kia Soul (2012–2024) as eligible for evaluation and repair of oil consumpt
... ion due to piston oil ring failure. This bulletin requires that a sealed oil consumption test be performed when customer complaints of oil consumption or smoke are verified. Despite this, Kia America has denied warranty coverage, stating the vehicle passed the rod bearing clearance test (BCT) and therefore does not qualify for an engine replacement under the class action settlement. This is misleading and incomplete because: The oil consumption issue is a separate covered defect under TSB ENG222. My vehicle is within the mileage (94k) and time limit (15 years/150k miles). Kia has refused to authorize the required oil consumption test. The smoking and oil consumption symptoms only began after the dealer performed recall service. This denial leaves me with a vehicle that is unsafe to drive due to risk of sudden engine failure, increased emissions, and potential fire hazard from oil burning. Kia is not following its own published service procedures, leaving consumers without protection despite official warranty extensions. I request that NHTSA investigate Kia’s handling of oil consumption claims under ENG222, as owners are being denied the mandated oil consumption test and warranty repairs even when they clearly qualify.Read more

Problems began on 11/27/2024, less than 3,000 miles after I purchased the vehicle, and have been ongoing ever since, including prior emergency towing events due to similar safety issues. Kia’s repeated failures to provide a lasting fix or safe remedy have left me without reliable transportation and have put both myself and other drivers at risk. [XXX] [XXX] Phone: [XXX] Email: [XXX] [Date] National Highway Traffic Safety Administration Office of Defects Investigation (ODI) 1200 New Jersey Avenue, SE Washington, D.C. 20590 Subject: Safety Complaint Regarding 2020 GMC Yukon XL Denali – Repeated Engine Failures (VIN: [XXX] ) To Whom It May Concern: I am writing to formally file a safety complaint regarding my 2020 GMC Yukon XL Dena
... li (VIN: [XXX] ) which I purchased in 2021. This vehicle has experienced repeated and dangerous mechanical failures that raise significant safety concerns. •March 2024: The oil pump failed, resulting in immediate engine issues. Repairs were completed under warranty, but the check engine light remained on afterward. •April 2024: The vehicle broke down again. At that time, the cams and lifters were replaced in addition to other service, but the underlying issues were not resolved. •August 2025: The vehicle failed once again. After taking it in, the service department could not determine the cause. When they believed they had identified the issue and took it for a test drive, the vehicle broke down again and had to be towed back to the shop. At that point, I was informed that the vehicle now requires a complete motor replacement and a new fuel pump. Each of these failures has left me stranded and placed me and my family in unsafe situations on the road. Despite contacting the dealership (Joseph Cadillac of Dublin), General Motors, and the warranty company, none have taken responsibility to properly resolve the issues. I believe this represents a potential defect in the engine or related systems that could impact many other consumers. I respectfully request that the NHTSA investigate this matter to determine whether a broader safety recall or manufacturer action is warranted. Thank you for your attention to this matter and for helping ensure consumer safety. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2022 Chevrolet Suburban (VIN: [XXX] ) that is included in NHTSA Recall No. 25V274 / GM Recall No. N252494000 for engine defects in the 6.2L L87 V8 engine. My vehicle was inspected at an authorized GM dealer today, on 9/23/2025 (Don K in Whitefish Montana). Instead of replacing the engine, the dealer performed the recall “remedy” of switching to higher-viscosity oil, changing the oil c
... ap and filter, and providing an owner’s manual insert. However, the dealer service manager specifically told me that my engine will eventually fail due to the known defect and advised me not to drive the vehicle on long trips. He also told me to be careful driving if I have children, and it is only a matter of time before the engine fails, which will look like sudden shuttering, loss of acceleration and it will no longer run. He told me wait time to get a new engine after it fails is currently 8 months. This is alarming because the recall notice states that engine failure can cause sudden loss of propulsion without warning, which could be catastrophic going across multiple lanes of highway (from my home street), and entering and exiting my street via the 4 lane highway "suicide lane". I am very worried about this in the coming winter season with snow and ice. The current “oil upgrade” remedy does not resolve the underlying manufacturing defect in the connecting rod and/or crankshaft. Even after the recall service, my vehicle remains unsafe to operate, and the dealer’s own warning confirms this. I request that NHTSA review GM’s recall remedy to ensure that it adequately addresses the safety defect. A true remedy should involve replacement of the defective engine components, not just an oil change, or an immediate buy back of my automobile by the company so I can purchase another safer vehicle. My family and I do not feel safe driving this vehicle, and the manufacturer’s current approach does not eliminate the risk of catastrophic engine failure. Hi. I’d like to point out I have not had an incident YET thankfully. However, I want to highlight that the early M340i production releases (2019 and 2020) are HIGHLY likely to experience catastrophic engine failure due to an oil pump being made of plastic on some models. Of the 20 or so complaints listed under this vehicle on NHTSA, I’ve already seen PLENTY of complaints regarding this issue a
... nd people actually experiencing this issue. I believe if a car has a fair chance of causing catastrophic damage to its engine on its own, it poses as a safety risk for anyone in or around the car. The fix is to spend $6000-$7000 to have the oil pump replaced with the new 2021 oil pump, but most normal people cannot afford that easily and are driving around with plastic pumps still hoping for the best. I don’t understand how so many complaints of the same safety issue on this website for this vehicle (and other vehicles, which have the same B58 engine) have not resulted in a recall. Hopefully this can change because a car destroying itself shouldn’t be taken lightly and I’d like to second what everyone else on here is saying about the oil pump issue.Read more

The 3rd cylinder connecting rod failed under 120k miles. The rod knocked the starter loose while the vehicle was going 70 MPH, and caused a fire. It was inspected by the dealership it was purchased from. There were no symptoms or warnings at all, it was sudden and unexpected. Myself and my passenger were both shaken and thankfully had time to pull over when it happened, and we managed to put the f

Vehicle lost power leaving me stranded. All lights on dash came on, lost power, and barely able to steer to get off the road. We had to have it towed. The vehicle was towed to our mechanic, who states the motor seized up. Mechanic states there is no repair for this issue. Car has just been paid off. Now we have nothing!

While traveling at night on the toll road between Washington, DC and Pittsburgh, PA, the vehicle suddenly shut off without warning. Power was lost completely, including power steering, while in motion on a major freeway, creating an immediate and severe risk of collision for us and surrounding motorists. Although the vehicle did restart, it stalled again shortly afterward while still on the toll r
... oad. The failure repeated when decelerating on the highway or coming to a stop after exiting. At no point were there warning lamps, check-engine lights, messages, or reduced-power mode indicators prior to the stalls; the shutdowns occurred without any advance notice. The vehicle was towed to an independent BMW-only repair shop in Pittsburgh. Their initial assessment indicated a high likelihood that the failure involved the VANOS assembly bolts, though the engine was not disassembled at that time, so the diagnosis could not be confirmed with certainty. Because the shop did not have immediate availability, we were asked to transport the car home until they could perform a full repair. After sitting for several weeks, the car was towed back for further inspection, at which point the shop discovered that a VANOS bolt had likely sheared off and caused irreparable damage to the engine. Our only options now are to replace the engine or scrap the vehicle. The vehicle remains in Pittsburgh, PA in our possession and is available for inspection upon request. At this time, the failure has not been reproduced or confirmed by a BMW dealer, nor has it been inspected by the manufacturer, police, or insurance representatives. Importantly, there is an existing recall on VANOS bolts for the 328xi station wagon, but our vehicle’s production date ([XXX]) falls just outside the recall window of October 1, 2009–August 4, 2011. Notably, for the sedan body model, the recall window was extended until December 6, 2011. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Over the past nine months, I’ve been experiencing ongoing issues with my 2018 Hyundai Tucson engine and transmission. After four consumption tests and a flush confirming that the engine is burning between 0.8 and 0.3 liters of oil for every 1,000, I was informed Hyundai refuses to either repair the issue or conduct testing to determine the cause. The timeline is summarized below: •February 2
... 025 – I was told by [XXX] my vehicle simply did not have enough oil after the last oil change; •March 2025 – I was informed an initial consumption test needed to be completed; •May 2025 – a second consumption test revealed the engine was burning more than 1 liter per 1000 and a flush was completed; •July 2025 – a third consumption test was completed, revealing that while the engine was excessively burning oil, it was not currently at 1 liter per 1000 based on that test; August 2025 – the oil light came on & I was instructed to put a liter of oil into my vehicle to make it to the certified dealer for next steps. The next steps were another consumption test. Unfortunately, because I was told to add oil to make it to the dealership, I was then informed that this added oil would alter the testing, and we had to start over. •September 2025 – I am now awaiting the results of the fourth consumption test. I have receipts for every 6 months or 7,500 of service since purchasing the vehicle., Despite the engine and transmission being under warranty, the dealership has refused to conduct testing to determine the cause of the issue or remedy the excessive consumption. Recently bought this car a month ago. I started having problems with acceleration, as if the car was “bucking” every time i would press the gas normally at a red light or stop sign, accelerating from a stop. The vehicle wouldn’t make it up a hill while accelerating it was very hard bucking. I tried pressing the gas a little harder and the check engine light flashed and the car went into limp
... mode so i had to pull over. Made it back home and i was driving normally and every light you can imagine came up on the dash. I’ve heard from many others with the accord they’ve had the same problems but once they get it fixed the problems keep happening or the vehicle blows up. I just paid full in cash for this car and my total is fix it is already at 5k to fix.Read more

I own a 2018 Honda Accord Sport. About 2–3 months ago, the car began having repeated engine misfires (P0301), hesitation, and stalling while driving. At the same time, multiple dashboard warnings appear, including Steering Assist Reduced, Brake System Problem, Collision Mitigation Braking System Problem, Adaptive Cruise Control System Problem, Emission System Problem, and Hill Start Assist Probl
... em, these failures occur suddenly while the vehicle is in motion, creating a serious risk of loss of engine power, reduced braking performance, loss of steering assist, and disabled crash-avoidance systems. I have already performed extensive maintenance: new ignition coils, a full tune-up, compression test, spark test, and battery/alternator check, yet the issues remain. Diagnostic scans also showed U0416-68 (invalid data from ABS/VSA module) in addition to the misfire. This suggests not only a fuel/propulsion defect but also a possible computer/control module failure that links engine, brakes, steering, and ADAS systems. These conditions match known recalls for the Accord, including 23V858000 (fuel pump failure), 20V314000 (low-pressure fuel pump), and 20V771000 (BCM software issue), yet my VIN does not currently show as included. The fact that multiple critical safety systems fail simultaneously makes this a major safety hazard that could easily lead to an accident if the car stalls, loses braking, or loses steering assist in traffic. I opened a case with Honda Customer Service (Case #15868389), but they refused to cover the repair since my VIN is not listed, despite the failures being identical to those described in the recalls. I strongly believe my vehicle should be included, and I request that NHTSA investigate whether the recall VIN range needs to be expanded.Read more
