I am writing to express my concern regarding an ongoing and persistent issue with excessive engine oil consumption in my 2015 Hyundai Santa Fe, Subject: Severe Oil Consumption Issue - 2015 Hyundai Santa Fe - VIN: [XXX] The vehicle consumes an alarming amount of oil, approximately 1 quart every 300 miles. This rate of consumption is far beyond what is considered normal and strongly suggests a ser
... ious mechanical or manufacturing defect. I have already taken the vehicle to a mechanic to diagnose and attempt to resolve this problem. Based on their professional advice, the following parts were recently replaced: 1.Engine Valve Cover Gasket 2.Turbo Charger Oil Line 3.Spark Plugs Unfortunately, despite these repairs, the issue of excessive oil consumption remains unresolved. The rate of oil loss remains consistent and dangerously high. This was first informed by a service center when I took the car for a random check. Then, after traveling 1,000 miles, the engine oil level dropped again. After that, I am keeping an eye on that. This level of oil consumption poses a significant risk to the engine's long-term health and reliability, and it is a major inconvenience and expense for me as the owner. I am deeply concerned that a major engine component may be failing. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Engine misfiring and rough idle with occasional flashing engine light. Dealer said no codes to know what to look for despite replicating issue. Eventually it threw a Cylinder 5 misfire and thus GM dealer was able to look further. They found metal in oil pan and concluded a bearing issue and would need to replace engine.

Blown Head Gasket (Cylinder 3 Misfire) After having 2018 Chrysler Pacifica Touring L serviced at Chrysler Dealership, technicians found small leak coming from Oil/Coolant Filter housing unit. Part was replaced and few days later vehicle had a misfire and had flashing Check Engine Light (CEL). Car was towed to dealership, technicians said code seen was P0303 (Cylinder 3 Misfire). Tech, replaced Ign
... ition coil for cylinder 3 and cleared codes. Two days later on start up, P0303 (cylinder 3 misfire) came back with CEL. Tech cleared codes and could not re-create CEL again. Two days later P0303 came back. Technicians performed more diagnostics and confirmed blown head gasket. This seems to be a very prominent issue with 2017-2018 Pacificas, more notably Cylinders 1, 3, & 5. The catalytic converter sits above the firewall cylinder bank and possibly is causing this possible engine failure due to excessive heat and poor heat dissipation capability over time. If not caught quickly, coolant can leak into cylinder affected and then overheat engine causing irreversible damage. This situation can also create a very hazardous situation if driving into speeding traffic from a turn and engine misfires, my vehicle could be rear ended if acceleration does not happen quickly enough.Read more

Dear NHTSA, I am submitting this complaint to report a recurring issue with oil consumption in my Hyundai vehicle, which I understand is a known problem affecting this particular engine type and model. I am the original owner and have been a loyal Hyundai customer for many years. I have followed all recommended maintenance, including regular oil changes. Despite this, my vehicle has developed ex
... cessive oil consumption that has not been resolved through standard service procedures. The dealership has contacted Hyundai for approval to perform a combustion chamber cleaning, but based on my research and feedback from other owners, this procedure does not effectively address the root cause of the issue. My concern is that this problem may be indicative of a broader mechanical defect that could impact engine performance and safety. My 10-year warranty is nearing expiration, and I am worried that delays in resolving this issue may result in the loss of warranty coverage for necessary repairs. I believe this issue warrants further investigation, as it appears to be affecting multiple vehicles with the same engine configuration. I respectfully request that NHTSA review this matter and consider whether a broader safety investigation is warranted. Thank you for your attention. Incident date: Dec 5, 2022
The incident happened in December 2022; the vehicle is available for inspection still. Without warning, the engine light started blinking, and engine ran very rough, so I ended up pulled over and stuck until we could tow to the local Honda dealership. Then was without a car for several days. The dealership found error code P0302 (misfire on cylinder 2), due to oil gumming up on the spark plugs, c
... aused by the eco system design flaw in the 2014 Odyssey. The dealer quoted $5640 to replace the 1-3 piston rings. I could not afford this, so they replaced all 6 plugs and cleared the codes for a short term fix ($465). Which brings us to now, when Honda has offered a warranty extension and to reimburse for previous repairs related to this problem. I submitted my paperwork for the $465 reimbursement a month ago and have had multiple phone calls with Honda, and several "escalations" to supposed supervisors, each of whom seem to have not even looked at the incident details. Honda has denied the claim. The paperwork I submitted to them is from the dealer repair in Dec. 2022 and states "misfire on cylinder 2, need piston ring repair on cylinders 1-3, P0302 was the dtc error code, replaced all 6 plugs and cleared codes for short term fix, for long term fix, vehicle needs the 1-3 piston rings replaced." The Honda reps claim the dealer service did not use the right wording; it should have the words "fouled by oil" in the service report (but they don't provide appropriate wording for service departments to use). The last voicemail I received from them said there was no error code provided (which there clearly was). Honda is not interested in fulfilling the reimbursement their latest campaign promises. Beyond that, the long term fix is not covered on the warranty extension, because the vehicle is not currently broken. So I am left with a van that I can't drive on the highway or out of town, since it can happen again without warning. This is a safety risk for my family. Shame!!Read more

My vehicle’s fuel delivery system failed after a recall repair. The in-tank fuel pump was replaced under recall by the dealer, even though I had no drivability complaints, no warning lights, and a full inspection that day confirmed no issues with my vehicle. Immediately after the recall pump replacement, the vehicle developed drivability problems including sputtering, hesitation, and failure to
... start. On one occasion, the underside of the car overheated and exhaust fumes, including carbon monoxide, entered the cabin. The interior filled with heat and fumes, I became lightheaded, dizzy, and shaky, and nearly lost consciousness while driving. My dog, who was with me, vomited and panted heavily from the exposure. I had to pull over and exit the vehicle immediately. This incident occurred only after the recall pump was replaced and points to faulty installation or malfunction of the recall component, which disrupted the fuel system and caused improper combustion and exhaust handling. On another occasion, the vehicle stalled in the middle of an intersection, leaving me at serious risk of being hit by cross traffic. The car now shows a high-pressure fuel pump fault. Although the in-tank pump and the high-pressure pump are in different locations, they are part of the same system: the in-tank pump supplies steady fuel volume and pressure directly to the high-pressure pump. If the in-tank pump is defective, mis-installed, or inconsistent, it causes the high-pressure pump to fail. This made my vehicle unsafe to operate, placing me, my passengers, and other drivers at risk. The problem has been confirmed by the dealership, which documented a high-pressure fuel pump fault, and reviewed by Honda Corporate. Components and service records are available for inspection. There were no warning lamps, messages, or drivability issues prior to the recall service, all symptoms appeared immediately afterward.Read more

Incident date: Sep 15, 2025
On 9/15/2025, my 2020 Ford Escape SE began experiencing serious electrical and shift system issues: hard downshifting, “Shift System Fault” warning, inability to go into park, and failure to shut off without disconnecting the battery. I confirmed a related PCM recall was active and had the vehicle towed to Art Hill Ford for diagnostic and recall service. The dealership refused to perform the
... PCM recall, claiming my issue “didn’t match” the symptoms — despite clear electrical failure. Instead, they diagnosed a park pawl solenoid issue without scanning or physically inspecting the vehicle. I was told the issue was resolved, paid ~$1,400, and picked up the vehicle. Within 24 hours, new and dangerous transmission issues arose: harsh downshifting, loss of power, sudden high RPMs, and dashboard warnings. I became stranded in the middle of the road and had to have the car towed again. After this second visit, the dealer claimed there were metal shavings in the transmission and now says I need a full transmission replacement (~$10,000). These shavings were not reported during their first transmission disassembly — which is highly suspicious and may indicate the damage was caused during or after their service. Replacing transmission fluid on high-mileage vehicles (mine has ~132,000 miles) is known to be risky and can cause internal failure if not done carefully. My transmission had no mechanical problems prior to this visit. I now believe the dealership’s misdiagnosis, failure to perform the PCM safety recall 1st, and service caused a safety-critical failure that they are refusing to take responsibility for. I am a veteran with a pregnant wife — we depend on this vehicle. Ford corporate has not provided assistance. This situation has left me without transportation and potentially with catastrophic repair costs due to service-related damage.Read more

My safety and the safety of my children were severely at risk. Being in a vehicle that is unable to accelerate or even maintain speed on a major highway creates an immediate and extreme hazard for being rear-ended by other traffic moving at 60+ mph. We were a vulnerable, slow-moving obstacle with no ability to power ourselves out of a dangerous situation. The vehicle was towed to the Palm Springs Kia dealership in Cathedral City, California, where it was inspected by their service department. A certified Kia technician confirmed the cause of failure was a cracked engine valve, requiring a full engine replacement. I am the original owner of this vehicle and have a complete and documented history of performing all scheduled maintenance. This incident represents a severe safety defect, as a sudden loss of motive power in traffic places the vehicle occupants and surrounding drivers at high risk of a serious collision. Had a Honda fixed for a well known piston ring problem that did not have a recall, but should have at the time repaired. In July, Honda sent us an extended warranty for the very problem we had repaired. I sent in all documentation and claim was denied twice. My dealer has sent me a voice mail confirming the repair is the exact one mentioned in the warranty extension. American Honda is claiming cer

We first noticed that the car was sputtering and jerking when accelerating at highway speeds. It wasn't consistent at first but continued to get worse over time and at more regular occurrences. White/grey smoke was spotted from the tailpipe whenever the vehicle was accelerating, either from a stop or when "resuming" on the cruise control. We brought the vehicle in to the dealership as the "serv
... ice engine" light came on and explained what was happening with the car. They told us that there was an "intake valve" that was plugged and need to be replaced and that the on-board computer needed to be updated. We also had some routine maintenance done as well and they sent us on our way. Quickly after getting the car home we noticed that the original problem persisted and actually got worse. We checked the recalls on our car and noticed that there are other recalls on other VINs for the same Make, Model, Year and engine type as our car and the symptoms of those recalls were exactly the same as our symptoms in our car. We returned the vehicle to the dealership and again the "service engine" light had come on. This time they got the code that says something to the effect of "cylinder scoring" and that we needed to have the engine replaced. The recall for the 2.0L Nu engine for 2023 Kia Seltos' that doesn't currently include the VIN for our vehicle is SC336. We had to replace the engine out of our own pocket and would like to see this VIN included in the recall that affects other vehicles of this similar Make, Model, year and engine type.Read more
