I WAS GOING UP A STEEP NARROW ROAD, A BIG TRUCK WAS COMING DOWN SO I STOPPED AND HONKED SO HE WOULD BACK UP. WHEN HE DID, I WAS UNABLE TO MOVE MY SUV. IT FROOZE ENTIRELY, WOULDNT GO UP, WOULDNT GO IN REVERSE, JUST STAYED ON BUT WOULD NOT MOVE. AFTER SEVERAL ATTEMPTS I WAS ABLE TO GO BACK A LITTLE AND MOVE TO A SIDE AND THEN I WAS ABLE TO DRIVE UP THE ROAD. THIS WAS THE 2ND TIME THIS HAS HAPPENED
... SO I ASKED THE SERVICE SHOP AT THE DEALER TO CHECK IN DRIVING THE SAME ROAD. THE SITUATION DID NOT REPLICATE, THEREFORE THEY SAY THERES NOTHING THEY CAN DO OR CHECK, ALTHOUGH I HAVE READ THAT THIS VEHICLE HAS BEEN KNOW TO DO THIS, BUT THEY DID NOT EVEN LOOK AT THE PRINTED EVIDENCE I TOOK TO THEM.. THERE WERE SUGGESTIONS ON WHAT HAD TO BE CHECKED BUT THE MANAGER REFUSED TO EVEN READ THE INFORMATION. THE MECHANIC SUGGESTED I STEP ON THE GAS UP TO THE END, ALTHOUGH THE MANUAL SAYS YOU SHOULD HIT GAS SLOWLY WHEN STOPING ON A HILL. EITHER WAY I TRIED BOTH WAYS....IT STILL FREEZES.Read more

The engine seems to be failing/affected. The vehicle is currently available for inspection at Huffines Kia in [XXX] where it has been since Saturday, March 14, 2026 and they have yet to perform the SC336 Safety Recall while trying to charge a diagnostic fee for a multipoint inspection that I did not ask for. The vehicle would not start which now I know is due to total oil consumption, which app
... ears to be a precursor to the recall. Having a vehicle that can catastrophically fail or stall without warning due to defective piston rings creates extreme risk of a crash or fire while my family is inside. The service department at Huffines Kia Mckinney confirmed to Kia Corporate on Wednesday, March 18, 2026 that the vehicle was completely depleted of oil. However, they have not officially confirmed the recall failure because they have not done the recall test. They told corporate that they haven't done the recall because I did not have oil in my car. They have had my car since Saturday, but have not topped it off with oil since it has been there for several business days. I have also opened a formal case with Kia Corporate (Case #XXX) due to the dealer not providing the free safety remedy required by law. There was no oil pressure warning light prior to the failure, however, when trying to start my car and it wouldn't start, the oil and engine light came on. A tow truck came to tow to the dealer. The tow driver was able to start my car using a jumper box. The dealership says my car ran fine and no oil or engine light was on. However, if they ran my car knowing it was depleted of oil, they have potentially further damaged my engine. I asked them to check the fuel system or the engine oil to see if there was any glitter, then, because the grinding noise we would have heard is metal on metal. And any small metal shavings in the wrong place would cause another catastrophic issue. I also asked them if they checked the cam lobe for damage. They did not inspect any of these and told me so. Yet, their paperwork stated they did a 21 point inspection. They told me all that was is checking codes. March 6, 2026 NHTSA CAMPAIGN NUMBER: DP26001 OPEN INVESTIGATION All wheel drive allegation NHTSA Action Number: DP26001 Components: POWER TRAIN Opened From: March 6, 2026 – Present Summary The Office of Defects Investigation (ODI) received a Petition dated January 9, 2026, requesting an investi
... gation into the loss of the optional all-wheel drive capability in model year (MY) 2021-2023 Acura TLX and MY 2022-2023 Acura MDX vehicles. The Petition contends that the interfacing splines between the transmission and the rear wheel drive power take off unit are failing. Failure of these splines prevents the transfer of drive power to the rear axle, making the vehicle front wheel drive only. The Petition further alleges that this failure results in a reduction of vehicle directional control. The Petitioner cites 48 complaints received by ODI alleging the same failure. ODI will evaluate these complaints to determine if they are related to the failure cited in the Petition and will conduct a technical review under 49 CFR Part 552. This Defect Petition is being opened to evaluate whether to grant or deny the Petition. The Petition can be viewed at NHTSA.gov under NHTSA Number 11712885.Read more

I am submitting a complaint regarding a 2019 Honda CB300R with approximately 1,225 miles. This vehicle is subject to an open recall (NHTSA Recall 19V-412) involving a transmission mainshaft circlip that can detach and cause gear misalignment and shifting issues. Prior to service, the motorcycle exhibited a symptom where it would not shift or operate properly beyond 4th gear. The motorcycle was
... brought to a Honda dealership for recall service. During inspection, the dealer disassembled the engine and identified internal engine damage, including cylinder scoring and compression loss. The dealer attributed the cause to carbon buildup from old fuel and did not clearly establish whether the transmission issue described in the recall was evaluated as the root cause of the shifting limitation. Given that the recall specifically identifies shifting issues as a symptom of the defect, I am concerned that the original transmission-related issue may not have been properly diagnosed or addressed before recommending extensive engine repairs. I am submitting this complaint to document a potential issue involving recall-related symptoms not being properly evaluated or resolved.Read more

I am reporting a serious safety concern involving a 2018 Hyundai Santa Fe Sport with approximately 132K miles. While driving at highway speed, the vehicle suddenly lost power/velocity and the engine light flashed. This created an immediate safety hazard, as maintaining speed is critical in highway traffic. Within seconds, the vehicle regained power and the warning light turned off, giving the impr
... ession that the issue had resolved. Shortly afterward, while exiting the highway, the vehicle began shaking and felt as though it would stall but was able to drive home less than one mile. A mechanic retrieved misfire codes for cylinders 2 and 4. Further inspection revealed internal engine damage in cylinder 4 requiring major rebuild. A full deconstruction revealed that one of the valves in cylinder 4 had partially broken. This failure occurred with little to no warning and involved a sudden loss of power at highway speed, which could have resulted in a serious accident. The vehicle’s warning systems did not provide adequate or persistent alerts prior to significant engine failure. Over the last 6 months it began consuming oil frequently requiring weekly checks, however no warning systems or errors to indicate an underlining concern. Based on research, similar Hyundai vehicles have documented issues involving excessive oil consumption and internal engine damage without sufficient warning. This raises concerns about a potential safety defect affecting engine performance and failure detection.Read more

I am reporting a safety concern involving BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this internal component fract
... ures, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or severe mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions the cold is when they fail the most, where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 for just the pump, assuming the motor is unharmed. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components in the car even when no replacement has occurred and the old components are actually in the car. Also, BMW has actively removed the plastic part number out of all dealer networks as well.Read more

Multiple safety systems on my 2019 Honda Odyssey Elite have simultaneously failed, including LKAS, ACC, Collision Mitigation Braking System, Auto High-Beam Headlights, and other driver-assist features. The dashboard displays warnings for nearly every safety-related system, and the vehicle disables these features without warning. **Detailed Description of the Problem:** Whenever I start my vehicle,
... multiple error messages are displayed related to the Lane Keeping Assist System (LKAS), Adaptive Cruise Control (ACC), Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Auto Headlights, and other safety features. These warnings appear together and persist even after restarting the vehicle. Apparently, this issue is widespread among 2018–2020 Honda Odyssey owners, with many reporting the same cluster of system failures. The problem appears to be related to a malfunction in the vehicle’s electrical system, sensors/ cameras, or control modules. In my case, the failures occur without any external damage, weather event, or driver action that would explain the sudden loss of safety functionality. When these systems fail, the vehicle disables critical safety features such as automatic emergency braking, lane keeping, adaptive cruise control, and automatic headlights. This creates a significant safety risk, especially at highway speeds or in low‑visibility conditions. **Frequency:** The issue occurs whenever the vehicle is started, affecting my driving everyday.Read more

After an hour from incident to inspection at the local Honda dealership, the vehicle powered on normally of which no codes showed and the vehicle started with no issues. The manufacturer and dealer state they cannot replicate the problem and found no historical or pending diagnostic trouble codes (DTCs). Despite the lack of codes, these symptoms are a direct match for NHSTA Recall Campaign 25V-031 (Honda Recall AL0) which was officially announced to address Torque Control Failures caused by the FI-ECU software. According to Honda’s safety report, improper programming in the fuel injection unit causes the engine to stall during "sudden changes in throttle opening"—which is exactly what I experienced. As of now, my VIN has not been identified on the current recall list. This loss of power during acceleration creates an immediate safety risk of rear-end collisions and leaves the driver stranded in active traffic.Read more

On December 24, 2025, while driving in traffic, my Kia Sportage experienced severe lurching, bucking, and jerking, causing the engine to shut down. All dashboard warning lights illuminated, including a message prompting me to "turn engine back on." The vehicle would not restart, necessitating a tow to Horne Kia. After several days of diagnostics, it was determined that a blown ignition fuse and a

However, just 19 days later, while attempting to merge onto the freeway, the vehicle again bucked violently, and the engine shut down with all warning lights flashing. The police had to assist in moving my vehicle out of traffic, and it was towed to Courtesy Kia in Mesa, AZ. Once again, no error codes were found. After further diagnostics, it was discovered that the same ignition fuse had blown and that the catalytic converter had "shattered." The catalytic converter was replaced. Upon picking up the vehicle from the dealership, and driving less then 5 miles on freeway, the vehicle was bucking and hard shifting once again resulting in a third tow to dealership just 48 hours later after the previous service. Subject: URGENT SAFETY COMPLAINT - 2022 Nissan Rogue (VIN: XXX) - Recall Remedy Failure I am filing a formal complaint regarding a life-threatening defect in my 2022 Nissan Rogue. This vehicle has experienced three (3) total losses of motive power at highway speeds, nearly causing multiple accidents. I am now afraid to drive this vehicle as it remains a substantial safety hazard. Recall & Repair F
... ailure: The vehicle is subject to Recalls R25E2 and R25E3 for engine bearing and throttle body failure. The dealership performed an ECM software update which effectively removed the "open recall" status from my VIN history. However, the dealer refused to perform the mandatory physical oil pan inspection for metal debris required by the recall protocol. The car still exhibits severe bucking, hesitation, and gurgling noises. By only performing a software "fix," the manufacturer has cleared the safety alert while leaving a catastrophic mechanical defect unaddressed. This is a complete remedy failure. 30-Day Out-of-Service & Misdiagnosis: In January 2025, the dealership retained my vehicle for over 30 days. During this stay, the service department initially informed me via text that the engine required a full replacement. The following day, they abruptly changed the diagnosis to spark plugs. I have text records of this contradiction. This 30-day stay meets the statutory presumption of a "Lemon" under Florida Statute 681.104. Unresolved Issues: Despite replacing the throttle body, EGR valve, water pump, and various mounts, the engine still stumbles, especially from a stop. The dealer manager previously discussed a vehicle swap but attempted to force a new credit application, treating a safety-defect remedy as a new sale. Dealer/Manufacturer Response: The vehicle was recently in the possession of McGovern Buick GMC Westborough for service. Despite the vehicle exhibiting an active Check Engine Light (CEL), the dealership staff pressured the consumer to take delivery of the vehicle and drive it on public roads. The service advisor admitted in writing that the CEL was active but insisted the vehicle was "feel free to drive." Furthermore, the dealership attempted to falsify internal corporate safety notes to claim the consumer "insisted" on taking the vehicle, masking a known mechanical defect. This represents a willful suppression of a safety defect by a franchised dealer, which led directly to a hazardous operating condition. To the National Highway Traffic Safety Administration, I am filing this formal safety complaint regarding a dangerous engine failure incident involving my 2017 Hyundai Sonata Plug-in Hybrid. Description of Incident: While traveling on CA-85 near Mountain View, California, at approximately 65 miles per hour, the engine suddenly and without warning lost all power. The vehicle began to slow down i
... nvoluntarily in active highway traffic, creating an immediate and serious risk of rear-end collision. The driver had no ability to maintain safe highway speed or accelerate out of danger. The vehicle had to be towed from the scene to the dealership. There was no warning light or prior indication that engine failure was imminent. The failure was sudden, complete, and occurred at highway speed in heavy traffic conditions. Safety Risk to the Public: This incident represents a significant and ongoing public safety hazard. A sudden, unpredictable loss of engine power at highway speed — with no prior warning — places the driver, passengers, and surrounding vehicles at serious risk of collision. This is not an isolated mechanical inconvenience; it is a life-threatening failure mode. Known and Documented Defects: The 2017 Hyundai Sonata Plug-in Hybrid has been the subject of multiple recalls and class action litigation related to engine defects. My incident is consistent with the known failure patterns documented in those proceedings. Despite this history, Hyundai Motor America has denied my warranty claim, citing missed services — a characterization I dispute, as the vehicle was serviced exclusively at an authorized Hyundai dealership whenever the onboard system indicated service was due, and no technician ever identified or disclosed any engine damage during those visits. Request to NHTSA: I respectfully request that NHTSA: 1. Open or expand an investigation into engine failure incidents in the 2018 Hyundai Sonata Plug-in Hybrid 2. Investigate whether a recall is warrantedRead more

Vehicle experienced a sudden and total loss of propulsion while driving due to an internal failure of the rear electric motor. The dealer diagnosed it as 'coolant intrusion' into the motor unit. This is a known manufacturing defect in the 2019 e-tron (TSB 2073858/2) where internal seals fail, allowing coolant to destroy the motor. This created a high safety risk on the highway. This car has major oil consumption that affects the engine and catalytic converter. This can cause engine damage and stalls in driving that affect safety on the roads because the car doesn't accelerate properly. There were no warning signals to indicate this being a problem with the car, especially at purchase. This car will get an oil change and in less than a week, the oil is gone and bone dry.

In October of 2025 my car stalled in slow traffic after a 100 mile car ride. After 20 minutes, it started again. The engine code was P0507. Two weeks later, without warning as I was accelerating on the highway the car stopped in the middle lane. This was incredibly dangerous. I had to wait 30 minutes in busy am traffic for a police escort. And the event was costly as I had to pay $200 to be towed
... off the highway. Once the car arrived at the service center, it started again. The mechanic convinced me to replace the purge solenoid as a starting place to address the problem. The car ran normally from November to March. In March 2026, after almost two hours in stop and go traffic, the car engine stalled out again. We pushed the car to the side of the road and waited 20 minutes. The engine started again. The coolant was below minimum and the computer code was P1217, engine overheat. My mechanic found a faulty cooling fan and replaced it on 3/17/26. This experience of the engine suddenly stopping while the car is moving with no warning or lights on the dash until the stallout has been extremely dangerous and costly. I believe Nissan knows of the cooling fan issue given online forums. Not recalling this faulty part has put me and my children in danger. In Summer 2024, the air conditioner of this car stopped working in traffic. It would blow hot air. The dealership could not replicate the issue and instead charged the air conditioner costing me a lot of money. The air conditioning problem has been intermittent since. I believe this is connected to the defective cooling fan and the Nissan dealership did not do their due diligence to investigate the issue.Read more

As my [XXX] son was driving, his car, out of nowhere, stopped working and the steering wheel became incredibly hard to turn. It put him and other vehicles around him at risk as he was doing 50mph and his car suddenly stalled. No warning. No dash lights. It was hard for him to pull over to safety. We replaced the battery and tried to restart the car, then the engine started smoking. We immediately
... called for a tow. Our local shop said his engine seized. We then towed it to Hyundai and they confirmed the same thing, "it definitely locked right up." The car has been well taken care of, no warning signs prior to engine failure, no warning lights, etc. His car is also under 100k miles. There is literally no reason for an engine failure. In 2019 there was a NHTSA Recall 19V204, Hyundai Recall #182 ECU Software update; this was supposed to fix the issue but here we are... the engine STILL BLEW. Hyundai is refusing to help. They won't do anything. There's known issues with Hyundai's 1.6 GDI turbo engine. They should be held responsible. My 2018 Mazda CX-5 AWD (2.5L SKYACTIV-G, non-turbo) was diagnosed by a Mazda dealer with a cracked/leaking cylinder head, a known manufacturing defect in the casting of the aluminum cylinder head. Engine oil is leaking from the rear of the cylinder head onto the exhaust manifold, creating a burning oil smell and visible oil accumulation at the rear of the engine. This is a documented fire hazard.
... The Mazda dealer confirmed the diagnosis matches Technical Service Bulletin SA-031-21, which Mazda issued in May 2021 specifically for this defective cylinder head design on 2018–2019 CX-5 models. The dealer additionally informed me that Mazda has internally acknowledged this as a growing, widespread problem across the 2018–2021 CX-5 model years, and that a recall may be forthcoming. Mazda customer service denied my goodwill repair claim despite acknowledging the TSB. The estimated repair cost is $7,000. The vehicle is currently not being driven due to the fire risk of oil contacting the exhaust manifold. This defect appears to affect a significant number of 2018–2021 Mazda CX-5 vehicles. I am requesting NHTSA open a formal safety defect investigation and compel Mazda to issue a recall. Multiple other owners have reported the same issue on NHTSA and in owner forums. This is a manufacturing defect not normal wear and tear and poses a serious fire safety risk to consumers.Read more

Vehicle: 2016 Hyundai Tucson Issue Category: Engine / Powertrain / Vehicle Safety Complaint: I am reporting a serious safety issue involving my 2016 Hyundai Tucson equipped with a Theta II engine. My vehicle first displayed an engine oil warning light. I immediately took the vehicle to an authorized Hyundai dealership, where I was charged approximately $500 for an oil change and diagnostic test
... ing. I was told there were no issues with the vehicle. Within approximately 20 miles of leaving the dealership, the vehicle suddenly entered limp mode while driving and displayed diagnostic code P1326. This created a dangerous situation as the vehicle lost power unexpectedly. I returned the vehicle to the dealership, where a software update was performed. However, within another 20–30 miles, the exact same issue occurred again, with the vehicle entering limp mode and triggering code P1326. Hyundai has now refused to repair the engine under warranty and is attempting to attribute the issue to a catalytic converter problem, despite the presence of code P1326, which is associated with engine rod bearing failure and is part of a known defect affecting these vehicles. This condition presents a significant safety risk, as the vehicle can lose power suddenly while driving, increasing the risk of a crash. I believe this issue is directly related to the known Hyundai Theta II engine defect, which has been the subject of recalls and a class action settlement. I am requesting that NHTSA investigate Hyundai’s handling of these failures and their refusal to repair vehicles experiencing known engine defects that pose a safety riskRead more
