I am writing to formally report a safety concern regarding my 2018 Hyundai Elantra. The vehicle has experienced a failure of the heater hose/pipe, which appears to be made of plastic and connected to a metal component. This failure has resulted in a coolant leak, creating a serious risk of engine overheating. This issue is not only inconvenient but also dangerous. A sudden loss of coolant while d
... riving can cause the engine to overheat without warning, potentially leading to engine failure, loss of vehicle control, or leaving the driver stranded in unsafe conditions. This is especially concerning while driving on busy roads or highways. Based on my experience and discussions with repair professionals, this appears to be a known issue with this model, as the part is not widely available outside of Hyundai/Kia and may indicate a design flaw. A critical cooling system component should not fail prematurely under normal driving conditions. Given the safety risks associated with this defect, I believe this issue warrants further investigation and consideration for a recall or extended warranty coverage. Consumers should not be responsible for costly repairs caused by a potentially defective design. I respectfully request that Hyundai review this issue and provide assistance with the repair, as well as take appropriate action to ensure the safety and reliability of affected vehicles. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

failing component: Transmission / Powertrain. The transmission is jerking aggressively when shifting into gear. yes, the vehicle is available for inspection. I currently have the vehicle in my possession. How was safety put at risk? The vehicle jerked violently into gear while accelerating from a stopped position at an intersection. The force was strong enough to physically jolt both the driver a
... nd passenger. This creates a safety risk due to unpredictable vehicle behavior during acceleration and could lead to loss of control or increased risk of collision. Has the problem been reproduced or confirmed? YES – the issue has occurred after the vehicle was returned following transmission replacement. The dealership has attempted multiple transmission replacements, indicating an ongoing unresolved issue. Has the vehicle been inspected by anyone? YES – inspected and repaired multiple times by dealership service department. Transmission has been replaced multiple times, including a failed replacement while still at the dealership. Warning lights or prior symptoms? NO warning lights were present. Prior to the failure, a whining noise was present, which was previously reported to the dealership as a suspected transmission issue. The vehicle was brought to a dealership for a transmission-related issue and has undergone multiple transmission replacements. After the most recent repair, the vehicle was returned with the issue unresolved. On March 22, 2026, while accelerating from a stopped position at an intersection, the vehicle jerked aggressively into gear. The force was significant enough to physically jolt both the driver and passenger. Despite multiple repair attempts, the transmission is still not operating correctly. This creates a safety concern due to unpredictable shifting behavior. Additionally, the dealership drove the vehicle approximately 3,000 miles while in their possession, yet the issue remains unresolved.Read more

Incident Description: On multiple occasions in January and February 2026, while operating a 2024 Mazda CX-5 in reverse on low-friction surfaces (snow/ice) with a slight incline, the vehicle suffered a total systemic failure. System Failures: While reversing at low speeds, the engine spontaneously shut off without warning. This resulted in an immediate loss of power braking and power steering. Si
... multaneously, the electronic gear selector (shifter) became non-functional, preventing the vehicle from being placed into 'Park' to stop the uncontrolled rearward slide. Safety Risk: The vehicle became a "runaway" object on several occasions. In one instance, the vehicle narrowly avoided crushing a pedestrian against another parked car, stopping only when the tires reached a different surface texture. In a separate incident, the vehicle slid uncontrolled down a residential hill, striking snowbanks. There is a grave concern regarding whether the Supplemental Restraint System (airbags) remains functional during these engine-stall events, as the dealer could not confirm occupant protection status during a total power loss. Inspection & Reproduction: The vehicle was presented to an authorized Mazda dealer on March 3, 2026. The service department attempted to replicate the issue by reversing over soapy water on a gravel surface. However, due to the high friction of the gravel, they were unable to induce the specific tire-slip/low-traction conditions required to trigger the malfunction. No error codes were found during an informal scan, and no warning lamps or messages appeared prior to or during the failures. Component for Inspection: The vehicle is available for inspection. I believe the failure is rooted in the traction control or engine management software, which appears to improperly shut down the powertrain when it detects specific wheel-spin patterns in reverse, subsequently disabling critical safety systems including brakes and transmission locks.Read more

I am writing to formally request reimbursement for a timing chain replacement performed on my vehicle, which was the subject of a specific safety recall issued by BMW. The timing chain defect in this vehicle represents a severe safety hazard, as a failure can cause an immediate and total engine stall while the vehicle is in motion. This loss of power in traffic creates an 100% avoidable risk of co
... llision. Despite BMW recognizing this defect through a prior reimbursement program, they are refusing to compensate for a safety-critical repair on a low-mileage vehicle (58,XXX miles) simply based on the car's age. At the time of repair, the vehicle had approximately 58,XXX miles. Although the vehicle is older, it was in storage for several years because the previous owner was unaware of the recall and lacked the financial means to address it. Despite the period of storage, the car remained in excellent condition, with the timing chain being the only mechanical requirement at the time. The repair was completed by a local independent shop and paid for out-of-pocket. While BMW has recently honored a different recall for an electronics panel on this same VIN, my request for timing chain reimbursement was denied based on the vehicle's age. I believe this denial is inconsistent, as the safety defect identified in the recall existed regardless of the car's age or storage history. I am seeking compensation for the parts and labor costs associated with this safety-critical repairRead more

I am reporting a serious safety defect in my 2023 Ford F-150 Platinum SuperCrew (EcoBoost engine. The oil pressure warning light suddenly illuminated, indicating a loss of oil pressure. Immediately after, the engine suffered catastrophic failure (it "blew the engine"). At the same moment, with no input from me, the gear shifter automatically shifted from Drive into Park. This caused the transmissi
... on to lock up, resulting in violent skidding and the truck veering off the road. I was able to regain control and avoid a collision or rollover, but the incident created an imminent risk of serious injury or fatality, especially while towing. I have maintained it properly, never missed a payment on the $74,000 truck, and drove it cross-country from Arizona to Florida without prior issues. Ford Motor Company has claimed there were "70 warnings" prior to the failure, but I received **zero** dashboard warnings, lights, messages, or indications at any point leading up to or during the event. This is the second Ford F-150 I have owned with unexplained powertrain problems. I traded a 2018 F-150 for the platinum & it repeatedly stalled without reason & required the engine to cool before restarting; dealerships (FL & TN) could not fully diagnose it, with one suggesting an oil-pump-related overheating issue despite no warning lights. Repair estimates are extremely high: Gator Ford quoted $17,000, Brandon Ford quoted $14,000, Veterans Ford quoted $12,000, and an independent shop quoted $12,000 (notably, the independent shop knew the dealership quotes without me providing them). I believe this points to a potential defect in the EcoBoost engine's oil pump system (possibly excessive pressure or failure under load/towing) combined with an unintended shift-to-Park function in the transmission/shifter electronics, creating a dangerous loss of control at highway speeds while towing. This combination of sudden engine seizure and automatic shift to Park poses an unreasonable risk of crash.Read more

I am filing a complaint regarding a catastrophic engine failure in my 2021 GMC Sierra 1500 equipped with the 6.2L V8 engine. While driving under normal conditions, the engine suddenly failed without warning and threw a connecting rod, rendering the vehicle inoperable. There were no prior warning signs such as knocking, overheating, oil pressure issues or even a check engine light. The vehicle has
... been properly maintained, including regular oil changes using manufacturer-recommended 0W-40 oil. This failure created a serious safety concern due to the sudden loss of power while driving, which could have resulted in a dangerous situation, especially at highway speeds or in traffic. I am aware that General Motors has issued a recall and/or is aware of similar failures involving the 6.2L engine in certain 2021 vehicles. However, my vehicle’s build date falls just outside the currently defined recall range, despite experiencing the exact same type of catastrophic failure described in those cases. My build date is 12/20 and the recall was issued for 3/21. Given the similarity of this failure to known issues, I believe this defect extends beyond the currently identified recall population. This raises concerns that additional vehicles, including mine, are at risk of sudden engine failure without warning. I am requesting that National Highway Traffic Safety Administration investigate this issue further and consider expanding the recall to include additional affected vehicles outside the current build date range.Read more

The failure involved the engine (6.2L L87 V8), specifically suspected internal components such as the crankshaft and/or connecting rods, as identified in a current recall. The vehicle is currently located at Dennis GM in Corner Brook, Newfoundland and is available for inspection. On March 9, while driving the vehicle normally on a main highway, the engine suddenly lost all power without warning.
... The accelerator became completely unresponsive and the vehicle rolled to a stop in the middle of a main highway. The engine would not restart. Due to the electronic gear selector, the vehicle could not be shifted into neutral, leaving both the truck and trailer immobilized in a live traffic lane. This created a serious safety risk to myself and other motorists, especially as I was in an area without cellular service and could not immediately call for assistance. The problem has not yet been formally diagnosed, as the vehicle remains at the dealership awaiting inspection. However, the symptoms are consistent with known failures described in the recall affecting these engines. The vehicle had previously undergone recall inspection on September 1, 2025 and was cleared at that time. The vehicle has been inspected previously by an authorized GM dealer as part of the recall process, but no defect was identified. It has since been towed to Dennis GM following the failure and is pending further inspection. No police or insurance inspections have been conducted. Incident date: Mar 4, 2026
The vehicle developed a progressive engine failure beginning in early 2024, including loud knocking noises at cold start and visible white/gray/black exhaust smoke. The condition worsened over time. In February 2026, the failure escalated to active oil discharge from the exhaust pipe while the engine was running. Oil exiting through the exhaust indicates internal engine failure (such as piston ri
... ngs, cylinder walls, or valve seal failure), not an external leak. This creates a safety risk including potential loss of power while driving, risk of engine seizure, and possible fire hazard due to oil in the exhaust system. The condition has been documented on video multiple times and was shown to dealership personnel. The vehicle was taken to a Hyundai dealership for evaluation and remains available for inspection. Despite this, no written diagnostic findings or repair determination have been provided. The issue has been persistent, progressive, and reproducible, and represents a potential defect affecting engine integrity and safe operation of the vehicle.Read more

The vehicle experiences a transmission/drivetrain defect involving delayed gear engagement, hesitation, and apparent slipping. The vehicle jerks and shakes violently and is unable to gain speed. This issue is intermittent but highly repeatable and occurs al
... most every time when there is a slope immediately after a stop sign. It results in loss of acceleration, violent shaking, and inability to maintain or build speed, creating a serious safety risk in traffic. It first happened around 1 year (early 2026) after the new car purchase (Nov 2025). There are no warning lights or messages. The dealer claimed they were unable to reproduce the issue during a brief test drive without me present and incorrectly attributed it to a bulletin without performing a detailed diagnosis or repair or looking at the video we recorded on the issue. The issue persists and affects safe operation of the vehicle. No inspection has been done by police or insurance. In one instance, this occurred while other vehicles were approaching from behind, creating a dangerous situation where I could not accelerate to match traffic flow. This resulted in a high risk of being rear-ended due to the unexpected loss of acceleration. I am aware of other consumer reports describing similar transmission-related symptoms such as slipping, hard shifting, and hesitation in this model. I am reporting a significant safety compliance issue involving a Certified Pre-Owned (CPO) vehicle and the fraudulent reporting of a safety recall completion. On March 21, 2026, I attempted to purchase the vehicle listed above. This vehicle is subject to NHTSA Recall 22V753 (VW Campaign 13i4) regarding engine connecting rod failure. Before signing, I requested documentation proving the recall had
... been performed. The dealership staff repeatedly insisted that the recall "had to have been completed" as part of the CPO process. Upon my further insistence, the dealership finally admitted that the recall work had NOT been performed and stated it would "be completed on Monday." However, a check of the VIN on [XXX] and VW’s public portal shows "0 Open Recalls," indicating the dealership has officially marked this safety recall as "Closed" or "Completed" in the system despite the remedy not being applied. This dealership is actively marketing and attempting to deliver a vehicle with a known engine failure risk as "Certified" while falsifying federal safety records. This is a clear violation of the National Traffic and Motor Vehicle Safety Act and poses an immediate risk to any consumer who might purchase this vehicle believing the safety defect has been remedied. Vin number is [XXX] I can't personally verify that the vin number falls within the lot described, however given their response was "our bad we're doing this on Monday" I can only come to the safe conclusion in my purchase decision that it does recall this recall procedure and that they fraudulently marked this recall as completed. To the best of my knowledge… My 2020 GMC Sierra Denali with a 6.2L engine and 74,000 miles had a complete engine replacement. One lifter on bank 2 was stuck, and the push rod in the cylinder was bent. The engine made a popping noise when starting. I took it to a GM dealership, where they quoted $9,200 to repair the issue or $9,600 for a brand-new engine with a 3-year/100,000-mile warranty. I k
... now there is open recalls for 21’-24’ Models, but I would like some help with mine. This kind of problem shouldn’t happen on a GMC - loaded truck with only 74,000 miles. Thank you!Read more

The vehicle has 82.000 miles on it, and the engine started to rattle as I entered the street to my house, and the motor quit running. I had the vehicle hauled to a repair garage for analysis, and they said the piston rings failed. The vehicle is in perfect shape except for the broken engine.

So, the N20 2.0L motors are known to have prematurely failing plastic chain guides and a weak hydraulic tensioner, leading to whine, rattles, and potential engine failure. BMW improved the issue in the 2015 or newer models which have more robust timing chain guides and Tensioners. The fix involves replacing the entire timing chain assembly (guides, chain, tensioner) with updated, more durable pa

I am filing a safety complaint regarding a progressive engine defect. Beginning as early as 2019 (approximately 84,000 miles), I repeatedly reported excessive oil consumption and related engine concerns to authorized Audi dealerships. These complaints continued over several years. At no point was I informed that these symptoms could be associated with a known defect pattern or progressive engine
... failure. The condition has now escalated to a severe engine malfunction. The vehicle began exhibiting engine misfires, flashing check engine light, EPC warning, and unstable operation. The dealership confirmed misfire fault codes and oil contamination affecting the ignition system. Engine replacement was initially recommended. Excessive oil consumption leading to internal contamination and misfires presents a significant safety hazard. Loss of engine power or sudden failure while driving can create dangerous conditions, particularly at highway speeds or in traffic. Despite the long history of reported symptoms, Audi of America has declined to provide assistance, citing only warranty status and failing to address the defect progression. The vehicle has now been inoperable at an Audi dealership for approximately 60 days (nearly two months), with no resolution. This situation raises concerns regarding a safety-related defect, failure to disclose a known issue, and a potential broader pattern affecting similar vehicles equipped with this engine. I am requesting that this issue be reviewed for potential defect investigation. The dealer provided diagnostic findings with Failure Codes via a temporary link that is no longer accessible. I am requesting a copy of the full report and will upload this (once received), along with other documentation.Read more
