Almost all of the 1.5T engines (L15BE) are having head gasket issue where the gasket slowly fails over time resulting in slow coolant loss and at one point sudden overheating causing block and head warping die to design flaw ( groove in between the cylinders resulting in lack of effective and enough surface area for the head gasket to seal the combustion chambers). It has been confirmed by Honda t
... echs and by average 100k to 140k, all engines are failing and the dealers are raking up $ 6k to 9k charge for each change or even having to need new engines half the time. This process of slow leak results in sudden loss of power at highway speeds or acceleration and causing extreme danger of getting hit and having accidents. Took vehicle to mechanic and confirmed coolant leak inside cylinder 4 and 3.Read more

While driving, the vehicle experienced sudden engine malfunction. The car entered limp mode, lost power, and produced a loud knocking noise from the engine. The failure occurred without warning and created a safety risk because the vehicle could not accelerate properly in traffic. This increased the risk of being rear-ended or unable to move out of dangerous situations. The issue appears to invo
... lve the engine (possible internal failure). The vehicle is currently at an authorized dealership and is available for inspection upon request. The problem has been confirmed by the dealership during inspection. The vehicle has been under diagnosis for over three weeks. There were no prior major warning signs before the failure. At the time of the incident, the vehicle suddenly lost performance, entered limp mode, and began making abnormal engine noise. This issue is consistent with widely reported engine defects in similar vehicles. Despite this, the manufacturer has denied coverage and attributed the issue to maintenance, even though regular maintenance was performed. This is a serious safety concern due to sudden loss of power while driving.Read more

My son, [XXX] , has opened a complaint/Case #XXX with Kia. His 2023 KIA Forte had been idle for at least 24 hours when the engine compartment caught fire. He became aware of this when the Fire Department knocked on his apartment door around 4:00 PM 12/26/25 and said they were evacuating the building because a car was on fire. He went outside and was shocked, to say the least, that it was his car.
... The Fire Department had taken off the hood to extinguish the fire. He removed his belongings once the fire was out. He contacted his insurance company but since it was a holiday weekend, he did not get a call back until 12/29/25. He then filed a claim with State Farm Insurance which was required to get his car towed to a Kia repair location. On 1/9/25, [XXX] [XXX] t Castrucci Auto Shop in [XXX] [XXX] he car was totaled. I talked with [XXX] who was [XXX] between the auto shop and State Farm, told me that once the repair exceeded the value of the car, he was instructed by State Farm it was totaled and there was no need to continue. 2021 Hybrid CR-V - within the last 10 months the vehicle has twice lost engine power while driving on expressways with just enough time to pull off the road. In the same period of time, at least 6 times, when initially starting the engine, the engine runs roughly and will not accellerate. So far, this has happened in cold or cool weather but not extremely cold weather. In an effort to restart th
... e system, we then shut the system off, wait a few minutes and try restarting. Occaisionally, the engine will start normally after this "reset". More often, if the "reset" doesn't work, we shut off the heat/cooling system (to keep the system from calling for the engine power) and restart the system in EV Mode. The car will begin moving in EV Mode. The engine then starts and runs normally when accellerating beyond the EV system's capacity to accelerate. We understand this problem does not register in the car's computer as a "fault" once the car's engine operation returns to normal. Therefore, the dealers are unable to diagnose the problem. We have read of many other people having the same experience without any satisfactory correction from the dealerships or mechanics. Because this is an intermitant and unpredictable failure, and because the engine and system operates normally for long periods between incidents, the error must be computer/electrical and not related to the fuel pump. We have also replaced the aged engine battery and cleaned the cables. This did not correct the problem. Any assitance in resolving this issue is appreciated. We are concerned that the system will fail completely at the most inopportune time, leaving someone stranded on the highway or being a road hazard on a highway.Read more

Year: 2022 Make: Chevrolet Model: Silverado 1500 Engine: 2.7L TurboMax (L3B) VIN: [XXX] Mileage at Failure: 119,139 Description of Problem: 2022 Chevrolet Silverado 1500 2.7L L3B TurboMax engine is completely inoperable due to Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Vehicle shakes severely, will not accelerate, check engine light on,
... and has gone into limp mode. Diagnostic codes P0304 (Cylinder 4 misfire) and P2101 (electronic throttle actuator fault triggered by engine shaking and limp mode) were recorded. All external components including spark plugs, coils, and injector harnesses were inspected and ruled out as causes. Failure is confirmed mechanical and internal. This failure was diagnosed and documented in writing by an authorized Chevrolet dealership on March 12, 2026 (Repair Order XXX, Service Advisor [XXX] ). The dealer's written estimate states verbatim: "Failed Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Cylinder head replacement required to correct this concern. Direct cause of engine noise and misfire condition." Dealer repair quote: $10,371.34. A second independent shop confirmed the same failure and cited Active Fuel Management (AFM) and Direct Fuel Injection (DFI) as known contributing design factors in this engine's oil consumption and internal wear pattern. That shop quoted between $11,876.88 and $14,897.35. This is a known pattern in the L3B engine platform. Consumer Reports has documented a 4% catastrophic failure rate in this engine. Multiple NHTSA complaints exist for this same failure across 2019-2022 model years. GM has issued technical service bulletins but has not issued a recall. I am reporting a safety concern involving BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this internal component fractu
... res, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or severe mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions the cold is when they fail the most, where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 for just the pump, assuming the motor is unharmed. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components in the car even when no replacement has occurred and the old components are actually in the car. Also, BMW has actively removed the plastic part number out of all dealer networks as well. They are aware.Read more

My vehicle was inspected by a mechanic after experiencing sudden loss of power and stalling while merging onto the expressway in active traffic. At the time of the incident, the vehicle became unable to accelerate properly, creating a serious safety hazard due to surrounding vehicles traveling at highway speeds. The mechanic determined that the fuel pump had failed. During inspection prior to rep
... lacement, the mechanic identified metal shavings present in the fuel system, which appear to have originated from the fuel pump. These metal fragments contaminated the system and pose a risk of further engine damage. This failure appears to be related to a known issue involving fuel pump defects in Ford vehicles. The design and/or function of the fuel pump may allow internal deterioration that leads to system-wide contamination and potential engine failure. The 2011 to 2019 Ford Explorers have a huge issue with them. The internal water pump on my 2017 Ford Explorer 3.5L engine failed. The vehicle has already been brought to a dealership for service and is available for inspecti
... on upon request. This failure created a serious safety risk because the engine could have seized suddenly while the vehicle was in motion. If I had not caught the issue early, it could have resulted in a complete engine shutdown without warning, leading to loss of power, reduced steering control, and inability to accelerate in traffic. I have a strong understanding of vehicles and was able to recognize the warning signs, but the average driver would likely not identify these symptoms. Without that knowledge, this type of failure could easily occur without warning and put both the driver and others on the road at significant risk. Yes. The issue has been confirmed by both an independent mechanic and the dealership as a failed internal water pump. Coolant was beginning to leak internally into the engine and oil pan, where it mixed with the engine oil. The symptoms—coolant leaking near the oil pan, loss of coolant, lack of heat, and eventual engine failure—are consistent with this type of failure on this engine. The vehicle has been inspected by both the dealership and my independent mechanic. It is currently at the dealership for service and remains available for further inspection if needed. There were early symptoms, including a coolant leak, bubbling in the coolant reservoir, and loss of heat inside the vehicle. The vehicle is experiencing excessive engine oil consumption. The engine was found by the dealership to be low by 3 quarts of oil after about 1000 miles during an oil consumption test, indicating abnormal oil loss. The suspected failure involves internal engine components, possibly piston rings, allowing oil to enter the combustion chamber and burn. This condition creates a safety risk because the
... engine can become critically low on oil, without warning, potentially leading to engine damage, loss of power, or engine failure while driving. The problem has been reproduced and documented by the dealership during oil consumption testing. However, there are inconsistencies in the dealership’s reporting, as later documentation stated that no oil consumption was found despite earlier records showing excessive oil loss. This vehicle has been inspected multiple times by an authorized dealership and is currently part of an open case with the manufacture. The vehicle was also held at the dealership for approximately three weeks without clear documentation of work performed. There were minimal warnings indicators prior to the issue. The vehicle showed rough engine performance, which was consistent with oil fouling the spark plugs. No clear warning light reliably indicated the severity of the oil loss before the condition was discovered during testing.Read more

HEAD GASKET FAILING, common problem in car. Broke down in the middle of busy Main Street and started smoking out of exhaust pipes. It has been confirmed by my service center that they now have 3 ford fusions, including mine, that they are replacing a head gasket on right now. They were no warning signs, it happened out of nowhere.

Component/System: Fuel System – Fuel Filler / Left-Side Radiator and Cooling Fan Assembly (subject to NHTSA Recall 25V536). What happened: On June 15, 2024, I was refueling my 2023 Chevrolet Corvette Z06 at a BP gas station in Phillipsburg, New Jersey. As I removed the fuel pump nozzle from the vehicle, a violent explosion occurred. I was immediately engulfed in a fireball. My clothing caught fi
... re and was burning me. I jumped clear of the flames and extinguished the fire on my clothing. I saw that the vehicle remained fully engulfed in flames with the pump nozzle still lodged in the filler. I ran to the adjacent pump island where a station attendant was attempting but was unable to open a locked fire extinguisher cabinet. I broke the glass with both of my bare hands to remove the fire extinguisher. I sustained a severe laceration to my forehead that was approximately 3 inches long and required plastic surgery of over 50 stitches to close. I then ran to the car that was burning with approximately 10 foot flames and I used the fire extinguisher to put out the fire on the vehicle and pump. I then pulled the pump handle from the fuel filler opening and called the police. The police and fire department arrived and confirmed the fire was extinguished. The flames singed the hair on my head and legs. Component availability: Documentation including photographs, police report, and medical records are available. Safety risk: A fuel-fed explosion at an active gasoline pump created an extreme and immediate risk of death or serious injury to me the station attendants and others. Dealer/manufacturer confirmation: GM subsequently issued NHTSA Recall 25V536, acknowledging the defect — fuel spillage during refueling can contact the left-side radiator cooling fan, which remains active even with the ignition off, causing ignition. This recall directly confirms the defect that caused my incident. Insurance inspection: The police and Geico inspected the vehicle. No warnings at all.Read more

These vehicles are not suitable to be on the road, and you NHTSA and Hyundai are doing NOTHING ABOUT IT. These vehicles are EATING oil, a quart every 200 miles. My daughter(college) can't even go on a short road trip to another city without adding oil. The engines in these crap cars are blowing up left & right. There are numerous Facebook page groups for these and sister Kia vehicles with the sa
... me 2.4 engines. Thousands upon thousands of posts about engine oil and engines blowing up, no one doing anything about it. My daughter's car is beautiful, but worth NOTHING due to this issue, so we are out thousands of dollars on this. We are about to go to WAR with Hyundai on this and I'm sure we'll get nowhere. If we're lucky, they'll install a rebuilt engine...BUT IT"S THE SAME CRAP ENGINE!!!!!!! So we are ALL back to square ONE and NHTSA is doing NOTHING. This is a well known FLAW for years now.Read more

### **NHTSA Complaint Statement of Facts** **RE: Recall Y43 (NHTSA Campaign 22V-406) – Failure of Recall Remedy** **Summary:** The manufacturer (Stellantis/FCA) is refusing to provide a complete and effective remedy for **Recall Y43**. My 2022 Ram 3500 was manufactured without the **Particulate Matter (PM) Sensor**, a defect officially acknowledged in Recall Y43. This factory omission has caus
... ed terminal blockage of the **Diesel Particulate Filter (DPF)**—a specific consequential failure documented in the manufacturer's own recall filing. **Technical Argument:** 1. **Causal Link:** The official Y43 recall notice states: *"The PM sensor... may not have been installed... This could increase vehicle emissions and **block the diesel particulate filter**."* My vehicle currently suffers from the exact "blocked DPF" condition described. 2. **Ineffective Remedy:** Stellantis has offered to install the missing sensor (the Y43 part) but refuses to replace the ruined DPF. Installing a sensor on a filter already terminally blocked due to the sensor's absence is not an "effective remedy" under the **National Traffic and Motor Vehicle Safety Act**. 3. **Warranty Admission:** Per **TSB 09-014-22 (X97)**, the manufacturer has issued a 1-year unlimited mile warranty extension for the DPF specifically for vehicles affected by Y43. Refusing to replace a filter that failed *before* the recall was performed is an attempt to circumvent this warranty obligation. **Safety Risk:** The vehicle is currently in "Limp Mode" with a "Filter Full - Power Reduced" message. This creates an unreasonable safety risk of **stalling or loss of power** while operating on public highways, a condition explicitly mentioned in the Y43 safety report as increasing the risk of an accident. I am seeking an order for the manufacturer to replace the DPF as part of the Y43 recall remedy at no cost to the consumer. The manufacturer’s Case Manager has denied the claim and refused further communicationRead more

On March 19, 2026, I was driving to work when traffic began to slow, and I reduced my speed accordingly. As traffic started to return to normal speed, I pressed the accelerator; however, although the engine revved, the vehicle did not accelerate beyond approximately 15 mph. I safely moved to the side of the road and turned the car off. After restarting the vehicle, shifting into drive, and pressin
... g the accelerator, the same issue occurred. I subsequently took the car to two separate mechanics, both of whom reported that the transmission needs to be replaced. This is unexpected, as the vehicle has fewer than 100,000 miles and has received regular maintenance.Read more

2020 Volkswagen Passat SE 2.0L VIN: [XXX] Complaint Summary: I am reporting a persistent and unresolved fault involving the secondary air injection system (DTC P0491 – insufficient airflow) that has resulted in repeated, conflicting diagnoses across multiple repair facilities. This raises concerns about a potential systemic defect affecting this vehicle model. Detailed Description: The issue
... began prior to March 2025 while the vehicle was still under warranty. The vehicle displayed a Check Engine Light associated with diagnostic trouble code P0491 (secondary air injection system insufficient airflow). On April 3, 2025, a repair facility diagnosed the issue as a failed secondary air injection pump and replaced the component. However, the Check Engine Light returned within 24 hours with the exact same fault code, indicating that the repair did not resolve the issue. A second independent diagnostic evaluation was performed later in April 2025. This inspection confirmed that: The secondary air injection pump was functioning correctly Airflow through the system was present Electrical controls and PCM commands were operating properly The fault was attributed instead to a mechanically stuck secondary air injection combination valve Despite this revised diagnosis, the issue has remained unresolved. As of 2026, a dealership evaluation has now attributed the same fault condition to a potential failure within the cylinder head assembly, indicating a significantly more serious underlying condition. Safety Concern: This issue involves the emissions and engine air management system and has resulted in: Persistent Check Engine Light Repeated failure of emissions-related components Conflicting and escalating repair diagnoses across multiple qualified service providers I am requesting that the National Highway Traffic Safety Administration review this issue for potential investigation into a systemic defect. “The vehicle, a 2017 Ford F-150, is experiencing an active engine oil leak originating from the oil pan assembly. Upon inspection, the leak appears to be due to a defect in the oil pan sealing surface integrity. There is no evidence of external damage, impact, or improper maintenance that would otherwise explain the failure. The condition is consistent with a manufacturing defect, potentially i
... nvolving improper gasket material, insufficient sealant application, or warping of the oil pan flange. Oil residue is visible along the oil pan perimeter and lower engine components, with active dripping observed after engine operation.” Safety Concerns: Leaking oil can hit hot exhaust components → potential smoke or fire hazard.
