Electronically controlled thermostat and sensor fail or get stuck at a much lower coolant temperature causing the dash to display the “steering assist is reduced drive with care.” It isn’t just a message, the steering is much harder to use potentially causing an accident. The ac completely doesn’t work causing potential for sensitive, elderly, or young children and pets to overheat inside
... , it’s significant risk of injury or death from the hot summer temperatures. The engine coolant temperature gauge is all the way down on C position which isn’t registering how hot/cold the engine gets potentially causing damage to the vehicle and a total loss of the vehicle entirely due to damage from the engine overheating. GM sent out letters to 2021 blazer owners which included a special coverage number N252508350 applying to people with this year having this problem and they have until 2036 or 150,009 mi to get the repairs made free of charge, they just bring the document with the special repair codes to the dealer and they fix it free of charge. After doing research online, this problem is affecting all years of the 2019+ blazers with the 3.6L V6 engine AND other known problems with other models and years of their other lineup of cars, it’s very widespread across their platforms. So what are people supposed to do when it isn’t the 2021 year? Pay an $800+ repair bill to GM when they KNOW it’s more widespread than just this year/make? This is a scam. I can’t even do the repair myself because I’m having trouble finding the correct parts online and not only that the repair is extensive, the whole top half of the engine needs to be taken apart just to get to the thermostat, we’re talking the airbox and intake manifold, all the plastic parts and shrouds and sensors covering engine, the fuel rail and injectors, pipes and clamps, engine coolant needs drained and refilled. It’s a major fix. I’m reporting this complaint to raise awareness and hopefully GM will do the right thing.Read more

On [XXX], I drove my 2018 Ford Fusion Titanium AWD to a doctor 37 miles away through rural PA. The car was a reliable workhorse, needing only regular shop visits, and had 108K miles by this date. Midway through the [XXX] drive , a red warning square popped up on the dash to tell me that the engine was overheating, and a pink warning square said the car would lessen electrical draw so that it coul
... d cool the engine. Both alerts disappeared quickly. There was no unusual sound or smell before the alerts nor the rest of the drive. I also had no prior dash alerts, only ?3 misfires months apart I now know were symptoms of [XXX]. During down time before/after the doctor, I Googled the error messages and read my owner's manual. I restarted the car w/o incident after it had sat for 45 minutes. Six miles into my way home, the same red square popped up, though no second alert did. I pulled over at a clearing 3 miles later. I sat by the side of the road without cell service for 45 minutes. I had no coolant or water with me. I was unable to remove the coolant lid after the wait, nor could I see if there were coolant in the reservoir. I needed to move the car because of having no cell service. After a total of 75 minutes, I restarted the vehicle and moved 2 miles up the road. A red triangle icon appeared and stayed on. I called my dealer, described what happened, and they told me to tow the car the 21 miles to them. My dealer found no issue beyond a cracked spark plug on cylinder 3 that they replaced 4K miles earlier. Not satisfied, my B-I-L and I researched the incident and found the 2.0L EcoBoost coolant intrusion issue. I had the car tested according to Ford's TSB #19-2346 at a separate dealer; they duplicated the intrusion in cylinder 3, and we had a diagnosis. I was fortunate that the car didn't stop mid-traffic, didn't catch fire, and I was stranded in daylight in an Amish community. The other side of that coin is grim. To whom it may concern, On [XXX] at around [XXX] PST, I was taking my car to get a car wash at Quick Quack Car Wash located in [XXX] . While waiting in line for the other cars to go in, all of sudden I see a huge fire flame shoot out from under the hood. My car was on full fire extremely rapidly, in front of the multiple cars and near the entrance of this car wash, while this car was in fire I ra
... n inside and asked the personnel for a fire extinguisher, to consume the flames coming out of the hood, as I was extremely concerned about the safety and personnel of this location. As soon as I got the extinguisher, I ran towards the car and started spraying powder everywhere I saw flames coming out from top of the hood and under the car, as flames and melted material dropped down from under the car. When I saw the flames got contained, I saw a water hose near the entrance in which I went and turn on and ran to continue pouring water into the engine bay compartment in order to make sure the fire was fully contained. All I could say is that I was very scared and extremely worried that this car could explode in front of this business with all the people working there. As soon I put the fire out, my friend help me push it out of the way into the much larger empty parking lot in order for the business to continue to conduct their business. Component/System Failed: Engine — excessive oil consumption and oil leaks leading to complete engine failure requiring full engine replacement. Vehicle is currently at my home and available for inspection upon request. Safety Risk: My vehicle became completely inoperable while in use as my primary family transportation. As a parent of three children this has left my family without safe reliable
... transportation. An inoperable vehicle also creates safety risks related to emergency situations where transportation is critical. Confirmed by Dealer: Yes. An authorized Kia dealership confirmed the engine defect and approved a full engine replacement. The repair has not yet been performed. Inspections: Vehicle has been inspected exclusively at an authorized Kia dealership where all service and maintenance has been performed. Warning Symptoms: This is the third time this vehicle has become inoperable due to what was previously identified as an oil issue but is now confirmed to be an engine defect. Prior to complete failure the vehicle exhibited ongoing oil leaks and excessive oil consumption over an extended period. AAA roadside assistance was contacted and identified possible starter and battery failure believed to be connected to the engine condition. Additional Note: I have identified NHTSA complaints from other 2020 Kia Telluride owners reporting identical engine oil consumption and failure issues suggesting a pattern defect.Read more

My 1.5L EcoBoost engine experienced coolant intrusion into the cylinders, a known and documented defect associated with this engine family across multiple Ford model years. Prior to the failure, the vehicle displayed symptoms consistent with internal coolant loss, including white exhaust smoke, coolant level dropping without visible external leaks, and rough running. No external coolant leak was e
... ver identified. The vehicle was subsequently diagnosed by an independent service center, which confirmed coolant intrusion into the engine internals consistent with a head gasket or coolant jacket failure. The component is available for inspection upon request. The failure rendered the vehicle completely undriveable. The vehicle has been out of service for approximately 12 months and has not been driven since the failure occurred. The repair was quoted at approximately $12,000 for a full engine replacement, exceeding the current market value of the vehicle. Safety was put at risk due to sudden loss of engine performance and drivability while the vehicle was in use on public roads. Coolant intrusion can cause hydrolock, catastrophic engine seizure, and sudden loss of power, all of which create risk of collision, particularly at highway speeds. This failure is consistent with a well-documented defect in the 1.5L EcoBoost engine design, which has been the subject of technical service bulletins and consumer complaints regarding coolant entering the combustion chamber. The failure occurred with no prior accident, overheating event, or owner-caused damage.Read more

The next morning I drove it to the Oxmoor Hyundai Service Center. I told relayed the timeline of all symptoms I had experienced over the 2 month lifetime of my ownership. They did not attempt to reproduce the issue. They only entered "shakes in reverse" as the customer provided issue on the intake information. Service Center performed diagnostic, found cylinder misfires on cylinder 2 and cylinder 4. Replaced spark plugs to resolve Cylinder 2. Cylinder 4 continued to misfire. Used Borescope and found broken valve. They did not address the scoring on the cylinder of the valve. Recommended engine replacement. Advised it would not be covered under a warranty because the rod did not break. The oil consumption was proven with documentation from multiple Valvoline visits and the vertical scoring was present on the cylinder with the broken valve in the borescope photo. The Service Center refused to do a Boring Clearance Test or any other test that would prove the engine should be covered under an extended warranty or class action settlement. It wasn't until I reviewed HMA documents uploaded to the BBB Arbitration, that I realized that the information I initially gave to the Service Center of all the symptoms with timelines had not been entered into the system for diagnostic review by Techline. 2022 Volkswagen Tiguan SE, ~70,000 miles. The vehicle experienced a cylinder head failure in the EA888 2.0L TSI engine, causing the check-engine light to illuminate. On at least one visit I was discouraged from requesting a deeper diagnostic by being told it would require a significant out-of-pocket fee, despite the vehicle being under the manufacturer's warranty. The vehicle was dropped off for the current repair on [DATE] after the check-engine light came on. The dealership confirmed the cylinder head failure is covered under Volkswagen's New Vehicle Limited Warranty. However, the required cylinder head part has been on backorder for over 60 days with no confirmed ETA. No loaner or rental vehicle has been provided despite the extraordinary duration of the repair. The dealership's own service advisor stated that in the state of Texas alone, approximately 7,000 cylinder heads are currently pending replacement across Volkswagen vehicles — indicating this is a widespread, systemic defect actively being managed within the dealer network, not an isolated incident. The dealership has also proposed that I bear 50% of the repair cost, despite the vehicle being under warranty and having a complete and consistent maintenance record. This cost-sharing proposal was framed as a courtesy, which I do not accept. I own a second 2022 Volkswagen Tiguan SE with the same engine and model year, which I am also concerned may be affected by the same defect. On [XXX], my 2017 Land Rover Range Rover HSE Td6 diesel (VIN: [XXX] ) suffered a complete, catastrophic engine failure without warning at approximately 70 mph on [XXX] . The engine stopped instantaneously, eliminating power steering and power brakes while I was overtaking a sem
... i-truck in the passing lane. We narrowly reached the shoulder safely. The odometer read 79,018 miles — inside both limits of the federally mandated 8-year/80,000-mile emissions warranty under 40 CFR §85.2103. The vehicle was 7 years old at time of failure. Hoehn Land Rover Carlsbad diagnosed complete engine failure caused by timing chain guide failure — a known defect pattern in the Ingenium diesel engine. The service manager stated failures like this "aren't that rare" with these engines. Repair estimate: $28,000–$30,000. Jaguar Land Rover North America (Case #[XXX]) denied the warranty claim entirely, offering $800, without ever evaluating the claim under the active federal emissions warranty. Their denial cited only the expired extended CPO warranty and a maintenance gap — neither of which constitutes valid grounds for denying an emissions warranty claim under 40 CFR §85.2103(d), as the manufacturer cannot demonstrate the maintenance gap caused a documented design defect. This failure pattern is documented in JLR Technical Service Bulletins LTB00735 and SSM74184. Land Rover Korea issued a recall for the same timing chain failure in 2020. No equivalent recall was issued in the United States for identical vehicles with identical engines. JLR has refused all requests for review or reconsideration for 24 months. I own a 2021 Kia Telluride with approximately 111,000 miles. I initially brought the vehicle to a Kia dealership for an oil consumption issue and engine noise. The dealership diagnosed and repaired a timing chain and tensioner issue, stating the vehicle was “working properly” upon return. After picking up the vehicle, I immediately began experiencing electrical warning lights, including the b
... attery warning light and system alerts (blind spot and general warning indicators). I returned to the dealership, where they performed diagnostic testing and indicated a potential alternator issue, but stated it was in a “borderline” range and that the vehicle was safe to drive. Shortly after leaving the dealership, while driving with my family in the vehicle, the electrical system began to fail. Multiple warning lights illuminated, and the vehicle lost electrical function while in motion. The vehicle ultimately became completely inoperable and shut down in a parking lot, creating a serious safety concern. Prior to the dealership visit and repairs, the vehicle had no history of electrical issues, warning lights, or charging system problems. The dealership later acknowledged that the alternator issue was not present when the vehicle was initially brought in. I am submitting this complaint due to the sudden and complete electrical system failure while driving, which posed a significant safety risk. The progression from no prior symptoms to total failure immediately following dealership service and diagnosis raises concerns about vehicle reliability and potential safety defects related to the charging system.Read more

This complaint documents a recurring automatic transmission failure resulting in three complete vehicle shutdowns during active operation — two at highway speed and one in an active turning lane. The most recent failure occurred within 15 minutes of the vehicle being released from an authorized Hyundai dealership in College Park, Maryland, whose general manager admitted in a recorded call that t
... he vehicle was released knowing the transmission was unrepaired and unsafe. The first failure occurred in 2025 on a highway with no prior warning. The vehicle was towed to the dealership, where the transmission was confirmed defective and repaired under warranty. The second failure occurred in January 2026, again on a highway with no warning. The dealership held the vehicle from January through May 2026 — four months — with no updates despite multiple owner inquiries. In April 2026 the dealership confirmed the transmission had failed again and was seeking Hyundai corporate authorization for the warranty repair. On May 21, 2026 I was told the vehicle was ready and the transmission had been fully repaired under warranty. This was false. On [XXX], within 15 minutes of leaving the dealership, the transmission failed a third time, completely shutting the vehicle down in an active turning lane with no warning. The general manager at Hyundai College Park later admitted in a recorded call that the transmission was never repaired and the vehicle was released only to recover the dealership's rental car, with full knowledge it was unsafe. All three failures were confirmed by the authorized dealer. Component is available for inspection. NHTSA investigation and recall consideration requested. Once I got there, I parked in the parking space the vehicle did the automatic shut off to conserve energy once I parked I press the button to shut that feature off when I did it the vehicle shut off again. I lost power to everything and had to restart the engine. This has happened multiple times as I believe it’s related to the electrical system and the automatic shut off. The engine in my 2017 Hyundai Santa Fe Sport equipped with the 2.4L Theta II GDI engine is experiencing severe excessive oil consumption. The vehicle consumes approximately 2–2.5 quarts of oil every 1,000 miles despite multiple repair attempts and dealership-performed combustion cleaning procedures. Hyundai Motor America and the dealership documented the condition under PA #[XXX] and recommended
... engine replacement. Hyundai Motor America advised me in writing that the vehicle should not be operated in its current condition because of the excessive oil consumption and pending engine replacement. Excessive oil consumption creates a risk of sudden engine failure, engine seizure, stalling while driving, and potential engine fire if oil levels drop unexpectedly. Because the engine rapidly loses oil between normal service intervals, the vehicle may become unsafe to operate without warning, placing myself, passengers, and other motorists at risk. The defect was confirmed multiple times by Capital Hyundai in San Jose, CA through two official Hyundai oil consumption tests. The dealership documented oil consumption of approximately 2–2.5 quarts per 1,000 miles and submitted the results to Hyundai Motor America under PA #[XXX].The dealership also performed multiple repair attempts, including spark plug replacement, fuel induction cleaning, combustion chamber cleaning, and repeated oil changes, but the excessive oil consumption continued. Prior symptoms included: Check engine light illumination, Cylinder #4 misfire condition, Engine noise, Excessive oil loss between oil changes, and Oil level dropping significantly within short mileage intervals. My car caught on fire on [CCC] and it's completely burned down, a total loss. Following is information about my car and some summary of what happened. Car information: - Mazda CX5 2024 - Bought in June or July 2024(car payment only made 22 times so far) - Odometer 24,445mi(as of 05/15/2026) What happened: - In [XXX] Friday afternoon, I was in Bellevue and noticed an engine overheat warning. So,
... I stopped immediately to cool down and called Mazda Dealership(Lee Johnson Mazda) to schedule a service. - Then after 20-30 minutes, I started the car again and the warning was gone. So I drove slowly and got on [XXX] to head home in Kirkland. I was already driving slowly. - After a few minutes, overheat warning as well as engine warning started firing and noticed that smoke coming out of bonnet. - So, I pulled over on the shoulder near [XXX] and called roadside assistance. But even before I get connected to the roadside service, engine caught on fire and I had to call 911. - The car is completely burned down and it's a total loss. - State patrol wrote me a "Exchange of Information" and "Tow/Impound and Inventory Record". Why I am submitting this form: - This car is less than 2years old and has been seeing Mazda service regularly. The odometer checked by dealership on 05/15 is 24,445mi which is slightly more than 24,000 which is stated in the Lemon LAW. But still, the car is pretty brand new and around 24,000 ish. So, I'm wondering if this still qualifies for Lemon Law. - Even before this happened, I had a service history for engine warning and get the engine checked on 05/15/2026 which is exactly a week ago before the accident. Also, they said the engine thermostat had an issue and replaced and covered by Mazda's warranty. - Overheat and engine warning was on and off. As soon as I pull over, it caught on fire. - This happened on [XXX]. It was very dangerous and lucky that we pulled over on right before it caught on fire INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Error code, cylinder 2. This is 2018 Ford Edge 2L ecoboost and personal mechanic looked in cylinder and saw small amount of coolant. The car is sitting at Ford dealership in Albemarle NC at this time for full diagnostics. The car has mostly been parked since getting error code but husband said coolant is full, oil looks normal, is not cloudy and car drove fine to dealership. We are hoping this wil
... l be a minor (not replace the whole engine) fix. But I am bracing for worst, and scared it is the "well known to Ford" coolant intrusion issue due to defective engines in models 2015-2018. If so I hope Ford will do the right thing. My husband and I have been life long Ford customers, but if I have to pay for that engine with no assistance from them I will never owl another Ford. This seems to be a chronic issue with these engines. Im not sure exact date we got the error code but sometime in April. I can probably get exact date my husband had it checked from parts store if need be. I bought this car brand new (although it was technically NOT new. It was advertised and sold as new but I later found out it had been previously titled to dealership...but this is another story). I changed oil regularly, with full synthetic and I am in my late [XXX] and drive accaccordingly. This vehicle has been well taken care of. My vehicle is suffering from a severe coolant intrusion to cylinders 2 and 3 and I’m constantly getting bad misfire codes while driving.
Incident date: Jul 1, 2025
The vehicle exhibits two distinct and dangerous behaviors: 1. Engine stalling upon deceleration: When slowing to a stop, the engine stalls abruptly before the vehicle comes to a complete stop, causing a sudden, unexpected deceleration that throws occupants forward with significant force. 2. Unintended surge upon brake release: When releasing the brake pedal from a complete stop, the vehicle surg
... es forward unexpectedly and with excessive force, causing occupants to be jolted harshly without warning. Both incidents occur without any prior indication to the driver, leaving no opportunity to prevent the danger to occupants or others nearby. SAFETY RISK: Passengers were subjected to sudden, unexpected forward jolting without warning, posing a risk of injury. The vehicle's behavior also creates a hazard for other drivers and pedestrians in the vicinity. DEALER / SERVICE CENTER: The problem has not yet been confirmed by a dealer or service center. INSPECTION BY MANUFACTURER, POLICE, OR INSURANCE: The vehicle has not been inspected by the manufacturer, police, or insurance representatives at this time. WARNING LAMPS OR PRIOR SYMPTOMS: No warning lamps, dashboard messages, or prior symptoms appeared before or during these incidents. The failures occur without any advance notice to the driver, leaving no opportunity to take corrective action.Read more

While driving a drivetrain warning ⚠️ was detected and code was displayed on the I drive, along with a CEL. The vehicle is in limp mode so I was unable to accelerate past 35-40 mph. Took the car into the dealership and was informed that my waste gate actuator failed @59,388 miles. The part is an electric controller and can fail without warning. In my opinion 59,000 miles is extremely early fo

I bought this vehicle in mid April 2026 with 9,500 miles. It has the 1.5L Ecoboost engine. I drove the car for 3,000 miles and noticed the coolant was at ‘minimum’ and wondered why a car with so few miles is using coolant. I took the car to my dealer and they confirmed coolant intrusion into the engine due to a bad head gasket. The vehicle is now at the dealer waiting on either a new head gask

While accelerating onto the highway in my 2016 Kia Optima 2.4L, the vehicle suddenly lost all engine power without warning. The vehicle would no longer accelerate and I was forced to coast and brake to safety. Immediately after the engine shut off, the check engine light, oil light, and battery light illuminated on the dashboard. After the incident, the vehicle would not restart or crank at all.
... The electrical systems, dashboard lights, and chimes still function normally, and the battery is new. The vehicle had proper oil level at the time of failure. No warning signs were present prior to the sudden loss of power. This failure created a serious safety hazard because it occurred during highway acceleration and left the vehicle unable to maintain speed or move out of traffic safely. Based on the symptoms and sudden engine shutdown, I am concerned this may be related to the known Theta II engine issues associated with Kia Optima vehicles.Read more

The vehicle suddenly began vibrating noticeably when idling and at low speeds. About 30 minutes after this started happening a warning lamp lit up. We drove the vehicle about 2 miles to a local auto parts store to get an initial computer diagnosis of the issue. The test resulted in a code of "P0304" which detected a cylinder #4 misfire and recommended a fuel injector replacement. We brought
... the vehicle to a local mechanic to get the issue further evaluated. After this evaluation, it was determined that the cylinder #4 misfire was related to a lifter failure. The mechanic explained that this situation could possibly have caused more damage, specifically to the camshaft, but that job was bigger than he was willing to take on. He recommended we get the vehicle towed to the dealership for further evaluation. He also informed us that this situation was common with the engine in this vehicle and GMC is aware of it. He directed us to some articles online that elaborated on the potential known issues related to the automatic fuel management (AFM) system and the related engine damage. The vehicle was towed to a local GMC dealership for evaluation. They found the failed cylinder #4 with associated lifter failure and damaged camshaft. We went forward with the repair at the dealership. We asked the dealership about the known engine issues with this vehicle. They acknowledged the widespread issues but informed us that our particular VIN has not been included in any recalls or safety notifications, so we were responsible for the repairs. The vehicle spent nearly 3 weeks at the dealership. One of the parts was on backorder due to a national shortage, which delayed the repair by about one week. That seems to imply that the parts involved in this repair are in high demand, which raises the question of this being a manufacturing/parts issue that may need to be investigated further.Read more

I own a 2019 BMW X5 xDrive40i with the B58 3.0L turbocharged inline-six engine. While driving in Minnesota, the vehicle suddenly displayed an urgent warning stating “oil pressure too low” and instructed me to stop the engine. This occurred while the vehicle was being operated on the road, requiring me to stop driving immediately. The vehicle was later inspected by an authorized BMW dealer. Th
... e dealer diagnosed the failure as a defective oil pump and stated that the oil pump must be replaced. The estimated repair cost was approximately $7,500 or more. This is a serious safety concern because a sudden loss of engine oil pressure can lead to engine seizure, loss of motive power, and loss of vehicle control risk depending on traffic conditions. The oil pump is a critical lubrication component and should not fail as a normal wear item. I have found numerous similar owner reports involving BMW B58 oil pump failures, including reports suggesting that cold-weather operation may contribute to or expose the failure. My vehicle is operated in Minnesota, a cold-weather state. The vehicle was inspected by Park Place BMW in Rochester, MN. I am requesting that NHTSA investigate whether this is a safety-related defect affecting 2019 BMW X5 xDrive40i vehicles and other BMW models equipped with the B58 engine.Read more

Good evening, I want to formally document what happened today regarding my GMC Sierra after I brought it to the dealership on Friday morning for transmission concerns that I had already reported. Unfortunat
... ely, today my family and I experienced a very serious and frightening incident on the expressway. While driving, the truck suddenly lost power, barely moved forward, and began shaking violently. We were placed in a very dangerous situation in traffic and struggled with great difficulty and fear just to make it home safely. Once we arrived home, the vehicle completely shut off and would not restart again. This situation was extremely alarming, especially because I had already reported the transmission problems and trusted the guidance given to me that the vehicle was still drivable. My family’s safety was put at risk, and I need this matter to be treated with urgency and seriousness. Please advise immediately on the next steps regarding the vehicle, towing assistance if necessary, and how this situation will be properly addressed. Thank you.Read more
