Engine failure due to coolant intrusion.
Incident date: Dec 17, 2025
On September 3, 2023, the fuel pump failed while the truck was in the corner of an on-ramp for an interstate. The truck immediately lost power while in the corner causing a loss of control of steering. This resulted in a near collision with a concrete dividing wall while attempting to maintain control of the vehicle until the vehicle came to a stop. There were no warnings, check engine lights, or
... other issues with the truck prior to the incident. The truck was inspected by a diesel repair company where it was determined that the Bosch CP4 fuel pump failed resulting in a catastrophic fuel system failure with metal shavings contaminating the complete fuel system. I've had an oil leak for a couple of years, typically having to put in 5 - 7 quarts of oil in between oil changes. In 2025 I had my spark plugs and some oil seal replaced it was quite expensive to have done. 6ish months later my 4th cylinder started misfiring. I took it to the repair shop. My 2018 Ford Escape, which has experienced catastrophic engine failure due to the well-documented coolant intrusion defect in the 1.5L EcoBoost engine. In early March 2026, without any prewarning or engine light, my vehicle displayed an “Engine Fault Service Now” warning and stored diagnostic trouble code P2601 (Coolant Pump A Control Circuit Performance/Range). I immediately took the vehicle
... to Bidwell Automotive in [XXX] , where on March 10–11, 2026, technician Gannon performed a thorough inspection. The findings were as follows: the coolant level was critically low; the water pump showed signs of leaking; the top of the piston in the affected cylinder appeared washed clean (a hallmark of coolant entering the combustion chamber); and the technician detected an odor consistent with combustion gases in the cooling system. The diagnosis was a suspected failed head gasket consistent with internal coolant intrusion. I then brought the vehicle to Future Ford Lincoln of Roseville, your authorized dealership, on March 12, 2026 (RO #XXX, Service Consultant [XXX] , Certified Technician [XXX] ). Future Ford verified the coolant intrusion into Cylinder 2 by removing the spark plugs and performing a borescope inspection. The dealership’s own technician referenced TSB 22-2322 and recommended either a short block replacement ($9,396.45) or a long block replacement ($13,331.01). The dealership confirmed that my vehicle has no remaining warranty coverage and offered no assistance beyond the paid repair estimate. Ford’s Documented Knowledge of This Defect Ford Motor Company has long been aware that the 1.5L EcoBoost engine in 2017–2019 Escape vehicles suffers from a structural design defect in the engine block’s cooling passages that causes coolant to intrude into the cylinders. Incident Summary: On 2/2/2026, my vehicle was serviced under Safety Recall 25V-274 at an authorized GMC dealership. The service technician performed the mandated diagnostic protocol (including the PicoScope test) and informed me that the engine had "passed" inspection. No internal components were replaced; the remedy was limited to an oil change to a higher-viscosity oil (0W-40) and a new oil fill
... er cap. The Failure: Approximately 48 hours after this "successful" recall remedy, the vehicle suffered a catastrophic engine failure while in operation. The engine failed entirely. The vehicle lost all propulsion while in motion, creating an immediate safety hazard for myself and other motorists. Diagnostic and Risk: The failure has been confirmed by a GMC dealer, who subsequently performed a full engine and radiator replacement. In the 48-hour prior to failure, there were no warning lamps or symptoms; the vehicle had just been cleared by the manufacturer's own diagnostic tools as safe and free of defects. Systemic Concern: This incident demonstrates that the current diagnostic protocol for Recall 25V-274 is insufficient to identify terminal manufacturing defects in the connecting rods and crankshaft of the L87 6.2L V8 engine. The "remedy" of higher-viscosity oil failed to prevent the exact catastrophic loss of propulsion the recall was intended to address. The replaced engine is available for inspection upon request by the NHTSA Office of Defects Investigation.Read more

While traveling on the highway, the vehicle suddenly lost power without warning, creating a hazardous situation in active traffic and increasing the risk of a collision. The presence of a young child in the vehicle further heightened the severity of the safety risk. The issue was preceded by a light tapping noise that began approximately 10 miles prior to the failure. Within about one minute before total failure, a loud squealing noise occurred, followed immediately by complete engine shutdown and loss of power. There were no meaningful warnings that would indicate imminent catastrophic failure in time to safely prevent the incident. The problem has been confirmed by an independent repair facility, which diagnosed the engine as seized due to internal failure consistent with rod bearing damage. A Kia dealership and Kia America have both been contacted and declined responsibility for repair. The vehicle has been regularly maintained, with routine oil changes performed as expected. The most recent oil change was completed approximately 2,000 miles prior to the failure. Beginning in January 2026, the vehicle exhibited progressive symptoms including repeated lurching and difficulty shifting gears. On or around March 25–26, 2026, while operating the vehicle at approximately 35 mph on a surface street, the foll
... owing failure sequence occurred: the vehicle began producing a repetitive thudding sensation (thud-thud-thud), followed by violent shaking of the steering wheel, followed by a complete loss of steering response. The vehicle drifted uncontrollably into an adjacent lane of active traffic under clear, dry weather conditions — a loss of control consistent with brake hose failure causing uneven braking force, compounded by the known transmission and PCM defects. Collision with another vehicle was narrowly avoided. The vehicle currently has five open, incomplete recalls: (1) front brake flexible hose leak, (2) rear brake flexible hose leak, (3) transmission shifter cable bushing, (4) rear-view camera inspection, (5) powertrain control module reprogram. None have been completed. The described failure is consistent with and likely caused by these unresolved manufacturing defects. I am submitting this complaint regarding my 2025 Mazda CX-90 PHEV, leased in April 2025. Since early in my lease, the vehicle has had ongoing drivability issues that have worsened over time and now present a serious safety concern. From the beginning, the vehicle exhibited jerking and hesitation while driving. Initially minor, this has become more frequent and now includes stalling behavior during
... normal driving conditions, including acceleration and traffic flow. This creates a risk of collision, especially when merging or at intersections.Approximately six months into my lease, the radiator failed and had to be replaced. More recently, while driving, multiple warnings appeared at once: Hybrid System Malfunction, Battery System Malfunction, Oil Monitoring Malfunction, and i-ACTIVSENSE partially disabled. These warnings were documented. Two weeks prior, the dealership reported no diagnostic codes. However, on my most recent visit, I was told via video that the active air shutter is now the issue. The required part is on backorder until August. I was told I could either remain in a non-comparable loaner while still making full lease payments, or continue driving the vehicle in its current condition until the part arrives. I find this unacceptable given the safety concerns.I expressed that I do not feel safe driving this vehicle, but the service advisor stated this is “not a safety concern.” I disagree. A vehicle that jerks, hesitates, or stalls while driving, combined with multiple system malfunctions and disabled safety features, presents a clear safety risk.I also disclosed prior to leasing that I do not have access to charging. I was told this would not be an issue. I am now being told that not charging the vehicle may be contributing to the problem. Service advisors have also indicated these vehicles have known hybrid battery issues.I no longer feel safe driving this vehicle and am concerned for my safety and my family’s safety.Read more

The vehicle experienced multiple electrical and safety-related malfunctions while driving. On several occasions the instrument cluster and vehicle screens suddenly went black while the vehicle was in motion, causing loss of critical driving information such as speed and warnings. In another incident the vehicle displayed a “High Voltage Malfunction” warning while driving and the vehicle shut
... down shortly after. This resulted in a sudden loss of propulsion and created a dangerous situation on the road. The vehicle has also displayed a “Coolant Too Hot – Stop Vehicle” warning which required replacement of the coolant pump at the dealership. In a separate event the vehicle lost battery charge unexpectedly after indicating a full charge and had to be towed to a charging station. The vehicle is also subject to Safety Recall 26V073 involving the high-voltage battery. According to the recall notice, the battery may fail due to an internal short circuit which could lead to a vehicle fire while parked or driving. The manufacturer currently does not have a repair available and advises owners to limit the battery charge to 80% and park the vehicle outside when not in use due to the potential fire risk. These repeated failures combined with the unresolved safety recall raise serious concerns about the safety and reliability of the vehicle. The issues began around September 2024 and have continued through multiple repair visits into March 2026.Read more

The internal core of the Exhaust Heat Recovery System (EHRS), part of the center muffler/front pipe assembly (#28600-L5210), suffered a catastrophic failure. This allowed approximately 9 gallons of engine coolant to leak directly into the hot exhaust stream. Safety Risk: This failure creates a severe visibility hazard. During acceleration from stop lights and stop signs, the vehicle emits massive,
... thick clouds of white glycol smoke. The smoke is so dense that it completely obscures the driver's view of trailing vehicles and prevents following drivers from seeing the roadway or my vehicle's brake lights, creating a high risk of rear-end collisions. Additionally, the rapid loss of coolant caused the engine to overheat and lose power while in motion, nearly causing a stall in active traffic. Confirmation: The failure was confirmed by the service department at Washington Hyundai (Invoice #174281). This is the second failure of this specific component (previously failed in Jan 2025). The manufacturer issued TSB 23-EM-009H for this defect, acknowledging it affects 2020-2023 Sonata Hybrids, but no safety recall has been issued. Symptoms: The issue was preceded by a "sweet" burning smell and a sudden "Engine Overheating" warning on the dashboard. The failed component has been replaced by the dealer, but the manufacturer refused to replace the coolant-poisoned Oxygen (O2) sensors, leaving the vehicle in a technically compromised state. The failed unit is likely held by the dealer for warranty parts return.Read more

I could not get it out of park to be able to get it off the road, causing a traffic delay and safety hazard to me and

My Ecosport prior to going to Celebrity Ford had no mechanical and electricial issues. It went to into Celebrity Ford of Toms River for the Engine Pump Recall on Dec. 23, 2025. I did not get the vehicle back until Feb 4, 2026 after major engine work was completed by Celebrity Ford. Substantial engine parts replacement—was performed as part of this recall repair (can been seen on service invoice)
... . Since that installation, the vehicle has had continuous mechanical problems that did not exist beforehand. Since then, the vehicle has had multiple mechanical and electrical failures- from "Full Accessory Power Message" along with numerous times failing to start with multiple warning lights illuminated as well as the most recent incident on March 5th. While driving home from work on the Garden State Parkway at 70 mph, my vehicle began to shudder/ shake violently almost stalling with lose of power along with the check engine light blinking from an engine that had significant work done according to the first service invoice from 12/23/25 to 2/4/26. At the time, I was in the center lane. I had to carefully maneuver across parkway work traffic to the right lane while my vehicle continued operating abnormally. This was a frightening and dangerous experience that could have resulted in a serious accident.Since bringing my vehicle in for this recall, it has experienced repeated failures and serious safety concerns that have made it unreliable and unsafe to drive due to Celebrity Ford's liability issue for negligence and improper recall repair. Celebrity Ford performed the recall repair, installed/ replaced major engine components, caused repeated breakdowns and now is disrepair to a vehicle that was in perfect working condition prior. I am seeking accountability for the damage caused and a resolution to this ongoing issue. I do not have my vehicle currently as they are still trying to duplicate the concern and in contact with a "Hotline" team with no ETA of completion.Read more

While driving on the freeway without warning, my engine stopped working. My vehicle was towed to the Chevy dealership where they confirmed that I was the latest victim of the ongoing engine failure problem with the 6.2 L engine. There was no warning, no sign that I had a problem. My engine just simply stopped working on a busy freeway. I was barely able to make it to the shoulder where I had mer

The check engine light is on my Chrysler Pacifica 2017 Limited with error codes P0302 and P0306, indicating misfires in the second and sixth cylinders. The vehicle was purchased brand new in 2017 from a Chrysler dealership in Olathe, Kansas and currently has approximately 97,000 miles. So far, the spark plugs and valve cover gasket have been replaced, but the same error codes persist. The mechanic
... is now recommending head gasket replacement, with an estimated repair cost of up to $5,000, which is extremely high. After researching this issue online, I found that thousands of Chrysler owners have experienced the same problem, yet no recall or corrective action has been issued by Chrysler. I contacted Chrysler customer support directly, but was informed that no assistance could be provided because the vehicle is out of warranty. We chose to purchase this vehicle based on the trust, reliability, and customer service reputation of Chrysler, but unfortunately, this experience has not reflected those values. An engine failure of this magnitude at this mileage is unacceptable, especially when it appears to be a known and widespread issue. I strongly believe Chrysler should take responsibility and provide assistance for this engine defect.Read more

I was driving about 10mph around a turn and the motor turned off and I had to park to start the motor again. When I came to a complete stop the car engaged it's parking break and suddenly jerked forward. This has happened several times in the last year and this time I was the driver of the vehicle. It's a extreme safety issue and my wife has had it happen to her at a busy intersection more than on

My complaint is that I was sold a dangerously defective vehicle with a known, unrepaired “stop driving” airbag safety recall, was not warned about the deadly risk, and am now being told to move and tow the car in direct contradiction to official safety instructions. ### Factual background - The vehicle I purchased has a Takata‑type airbag safety recall that carries a formal “do not drive
... ” or “stop driving” warning because the airbag can explode and kill or severely injure occupants, even in a minor crash.[1][2][3][4] - Federal safety authorities have confirmed that at least 28–29 people in the United States have been killed and more than 400 injured by exploding Takata airbag inflators.[3][4][5][1] - I received the recall notice about two months ago, but I was told the repair parts were back‑ordered, which delayed fixing the deadly defect through no fault of my own.[1][3] - During this delay, the vehicle’s battery became completely dead, and I am now being told by the recall and towing company that I must make sure the car is taken out of the garage, which would require entering and moving a vehicle I have been legally and explicitly told not to drive. ### Legal and safety concerns - Official guidance from the National Highway Traffic Safety Administration (NHTSA) and the manufacturer is that vehicles under this Takata “do not drive” recall must not be driven at all until the defective airbag is replaced, because even a minor collision can cause the inflator to explode and send metal shrapnel into the cabin.[2][4][3][1] - The instruction to move or drive the car out of the garage conflicts with this clear safety warning and effectively asks me to assume a risk that has already caused multiple deaths and serious injuries nationwide.[4][5][2][3][1] - Federal law prohibits franchised dealers from selling new vehicles with unrepaired safety recalls, and legal experts warn that selling used vehicles with undisclosed safety defects can expose sellers and dealers toRead more
