Coolant Intrusion – Engine Defect Concern “Coolant intrusion” is a known engine design defect in which coolant leaks into the cylinders, leading to internal damage, misfires, and eventual engine failure. In our case, we own a 2017 Ford Escape 1.5L EcoBoost, which is now inoperable. The vehicle was properly maintained, with no prior issues. At approximately 103,000 miles, the engine began t
... o fail unexpectedly. There was no overheating condition. Outdoor temperatures were normal (around 75°F), and the vehicle had been running properly up to that point. We have always used good quality fuel and kept up with regular maintenance. The first signs of failure included a check engine light with a cylinder misfire code, followed by noticeable white smoke from the exhaust. At the same time, coolant began rapidly disappearing despite being refilled. There are no external leaks, indicating the coolant is entering the engine internally. The vehicle now will not operate properly and cannot be driven. This failure occurred without warning and without any owner-related cause. Given the widespread reports of identical issues affecting Ford vehicles with this engine, this is clearly not an isolated incident but a systemic design problem. The pattern of coolant intrusion, white smoke, misfires, and sudden engine failure is well-documented among Ford customers. This issue requires immediate attention. It should be addressed as a recall, as it stems from a defective engine design rather than normal wear, misuse, or neglect.Read more

On two separate occasions, my 2023 Nissan Rogue has experienced major loss of power while driving 80 mph. The first time, all of the dash lights were blinking and there was an awful noise coming from the engine. I could not drive over 10 mph. The vehicle had to be towed 200 miles to the nearest Nissan dealer in Billings, MT. They cleared the codes, test drove it and said everything was fine. The s
... econd time my vehicle had to be towed. 115 miles from the nearest Nissan dealer. Same issues, except this time there were no dash lights. Prior to all of this happening, my car had an open recall for the 1.5L VC-Turbo engine that my car has. Since the first time this occurred and Nissan "fixed" the problem, there has been no open recall. With the second occurance, Nissan had to replace the Throttle Actuator and Gaskets according to the invoice. Luckily, I had purchased my own extended warranty and that warranty covered $1,104.20 of the $1,204.20 (I paid $100 co-pay). I was charged $715 for towing from Nissan Roadside Assistance. The invoice claims the Nissan found the following diagnostic results: DTC P0507-00 Wwhich is directly linked to a fractured internal gear in the Electric Throttle Control Actuator, the subject of NHTSA Recall 26V081 and DTC P0196 which is directly linked to engine oil temperature performance, the precursor to engine bearing seizures addressed in NHTSA Recall 26V080. Nissan is currently excluding many 2023 models from the "official" recall list. My car has 67,350 miles on it, but this failure should be covered by Nissan's 10-year/120,000-mile extended powertrain warranty. The parts replaced (throttle actuator and gaskets) and the diagnostic codes are identical to those in the 2026 expanded recall populations. Denying coverage based on an "open recall" statis is a violation of the spirit of the NHTSA safety mandates for this engine. I am formally requesting/demanding reimbursment from Nissan North America for the $815 that I have been charged.Read more

I was driving at approximately 75 to 80MPH on the highway when my engine shutoff and the vehicle died while driving it. I was able to coast to the side of the road but had to cross two lanes to the left to do so while weaving in traffic to get to the shoulder of the road. That occured on Saturday March 14th 2026, that Monday, I have had all regular and scheduled maintenance done to my vehicle either on time or prior to the recommended time including oil changes and the like. I went to the dealership and they told me that the engine had seized up and that this was a very well documented and common issue with the 6.2L L87 V8 Engine dating back to 2019 but that GM had only issued recalls on the 2021 - 2024 years to correct the issue. I asked the Service manager at Hendricks Chevrolet to file for a Goodwill Coverage request on my behalf to cover the repairs of the engine and he refused and stated that I had to do so through GM customer service line. I have done so since and my case number with GM is 92108267. My vehicle is currently at the dealership awaiting the go ahead from GM to replace the engine. I have an extended warranty on my vehicle that goes to 120,000 miles and the issue occured at 114,113 miles. While driving on the 60 East highway (approximately 65-68 mph), the vehicle did not accelarate and became completely non-responsive. Each time I pressed on the gas pedal, the RPM needle started trembling between 600-1100, which is much lower than where it should be while the vehicle was in Drive and traveling at this speed. This was an extremely dangerous situation as I had my wife and 5 month old
... daughter in the vehicle, and other vehicles were traveling significantly faster than me. I noticed some engine vibrations, and the digital gauge cluster screen presented information on the screen as if the vehicle was "off." It instructed me to push the gear lever into Park. It then instructed me to press the brake pedal and push the iginition button. All of this occurred as the vehicle was still coasting (likely around 40 mph). I pulled over on to the shoulder to see what occurred. After coming to a complete stop the engine cut out. I was able to restart the vehicle and drove approximately 10 minutes before it occurred again on the surface streets. At that point the ignition did not catch and it sounded very much like the engine was not able to turn on at all. The check engine light turned on at this point. I called AAA for tow services shortly thereafter, and the vehicle was dropped off at the dealership. I have not received any followup from the dearlership yet.Read more

I am filing this complaint regarding Hyundai Motor America's failure to complete a vehicle repair in a timely manner on my 2022 Hyundai Ioniq 5 (VIN: [XXX] ), which has been at Hyundai of Leander, an authorized Hyundai service center in Leander, Texas, since September 2025 — more than six months — awaiting a replacement high-voltage battery. (Case No. XXX) I purchased this vehicle in Septembe
... r 2025 from Apple Sport Imports in [XXX] . The vehicle was sent immediately to Hyundai of Leander for an open repair and I never took full delivery. I was informed the replacement battery was initially allocated to a different dealership and a new order could not be placed until approximately January 2026. As of March 2026, Hyundai's own case manager confirmed in writing on March 30, 2026 only that they are working to determine the battery's arrival date with no confirmed completion date provided. Since November 2025, I have made 5 monthly payments of $513.92 totaling $2,569.60 on a vehicle I have never driven, with a sixth payment of $513.92 due in April 2026. I am also flagging a potential records discrepancy for NHTSA's awareness. When I checked my VIN on Hyundai's recall lookup website, Recall 272 — covering an ICCU software update, fuse replacement, and ICCU replacement on 2022-2024 MY IONIQ 5 vehicles — is listed as a Closed Campaign with a completion date of October 24, 2025. My vehicle was physically at Hyundai of Leander awaiting parts on that date and remains there today. I do not know whether this recall is related to the current open repair, but I am flagging the potential discrepancy for investigation. When I raised this with Hyundai of Leander's service department, I received no clear explanation. I am requesting that NHTSA investigate Hyundai Motor America's recall fulfillment practices and the accuracy of their recall completion records for this vehicle. ... om the Techline Case Narrative. Evidence of Non-Compliance: 1. Improper Component Installation: Despite the documented timing failures, the dealership appears to have installed a Short Block (Part # 21102) instead of the required Long Block Assembly (Part # 21101-QQK). This leaves my oil-contaminated camshafts, valves, and VVT solenoids in the vehicle, posing a continued stalling risk. 2. Failure to Address Consequential Damage: The dealership is attempting to charge me a $99 "Customer Pay" fee for a catalytic converter back-pressure test. Under SC336, testing for consequential damage caused by a defective engine is the manufacturer's responsibility. 3. Withholding Documentation: The dealership has refused to provide the Techline Case Number or the Engine Part Number upon request, preventing me from verifying that the safety remedy was performed correctly. I will not be taking possession of the vehicle until Kia Corporate verifies that a Long Block Assembly was installed and that all diagnostic fees related to the recall symptoms are waived. The component that failed is the coolant bypass valve, which is part of the engine cooling system. The vehicle is available for inspection upon request. The issue was first indicated by an "Engine Maintenance Required" warning message that appeared on the dashboard. No prior symptoms were noticeable before the warning appeared. The warning came on suddenly during normal driving conditions. This
... failure poses a potential safety risk because the coolant bypass valve regulates engine temperature. If it malfunctions, it may lead to improper engine cooling, overheating, reduced engine performance, or sudden breakdown while driving. This could increase the risk of being stranded in traffic or experiencing a hazardous situation on the road. The problem has been diagnosed and confirmed by an authorized Toyota dealership, which identified the coolant bypass valve as failed and in need of replacement. The dealership provided a repair estimate of $775 prior to taxes and fees. The manager shared that it would be around $890+ after it is all calculated. The vehicle has not been involved in any accident related to this issue, but the malfunction was verified through dealer inspection. No police or insurance reports were filed, as this was a mechanical failure. Based on personal research, this appears to be a widespread issue affecting similar Toyota vehicles, including other Corollas and RAV4 models, where the same component has failed under similar conditions. In some cases, repairs have been covered by the manufacturer, indicating awareness of the issue. There are many groups on Facebook and Reddit where customers with the same make and model 2022 Toyota Corolla SE have had the same issue after 3 years of having the vehicle. In summary, while driving under normal conditions, the warning message appeared without prior indication. The dealership later confirmed that the coolant bypass valve had failed and required replacement.Read more

Vehicle Information Year/Make/Model: 2018 Mazda CX-5 Engine: 2.5L Mileage at Failure: Approximately 115,000 miles Component: Engine Complaint Summary: I am reporting a sudden and unsafe engine failure involving a cracked cylinder head in my 2018 Mazda CX-5. This failure occurred without warning at approximately 115,000 miles and resulted in significant oil leakage from the engine. Description
... of the Failure and Safety Risk: While operating the vehicle, I detected a strong burning oil odor followed by visible smoke coming from the engine compartment. Upon inspection, it was determined that the cylinder head had cracked, allowing engine oil to leak onto hot engine and exhaust components. This condition presents a serious safety hazard, including the risk of fire due to oil contacting high-temperature surfaces. Notably, there were no warning lights, messages, or prior indicators of any issue before the failure occurred. The vehicle has been properly maintained and has not experienced overheating or misuse that would reasonably lead to this type of failure. Repair Information: The vehicle was diagnosed by an authorized Mazda dealership, which confirmed a cracked cylinder head. The estimated cost of repair is approximately $5,130. Additional Information / Defect Pattern: Through independent research, I have identified multiple reports of similar failures involving Mazda 2.5L engines in comparable vehicles. These reports describe nearly identical symptoms, including sudden oil leakage, smoke, and cracked cylinder heads at relatively low mileage. This pattern suggests a potential manufacturing or material defect rather than normal wear and tear. Manufacturer Response: I have contacted Mazda Customer Experience and escalated the matter. Mazda has declined to provide any assistance, citing the vehicle being out of warranty, despite the nature of the failure and associated safety concerns. These fumes are noticeable while driving and create a hazardous environment for occupants, raising concerns of toxic exposure and making the vehicle unsafe to operate. The vehicle also exhibits drivability issues, including jerking and performance irregularities when accelerating. Despite multiple attempts to have the vehicle diagnosed and repaired under warranty at authorized Kia dealerships, including All Star Kia East and All Star Kia Seigen Lane, the dealerships have refused to properly diagnose or repair the vehicle. I have been informed that my vehicle or account is “flagged in all systems,” and service has been denied on that basis. Vehicle is subject to two active safety recalls, both listed as “remedy not available.” One recall involves a potential engine defect related to manufacturing debris, which may result in engine damage and sudden loss of motive power. Manufacturer recall documentation indicates this condition may increase the risk of a stall, particularly under sustained load such as towing or highway operatio

However, both safety defects remain unresolved, and no repair or timeline for remedy has been provided. The vehicle is used for towing and hauling, which requires sustained engine load. Due to the risk of engine stall under load and the absence of manufacturer guidance regarding safe operation, use of the vehicle for towing and long-distance travel has been limited. These conditions have been confirmed through manufacturer recall notices. The vehicle has not been repaired because a remedy is not yet available. No warning lights or symptoms have been observed. ... these repairs, the same throttle failure has occurred repeatedly, including within hours of being returned as “repaired.” Most recently, the throttle failed again while driving on a public street, and I was only able to move the vehicle into a driveway with minimal acceleration. The vehicle is currently not drivable and remains unsafe to operate. The dealership has not resolved the issue and has released the vehicle multiple times in an unsafe condition. This is an ongoing safety defect involving loss of acceleration and vehicle control.Read more

While driving on the interstate I was trying to pass a vehicle and the vehicle all systems but the brake system issue. Check engine, emissions, power steering, hill start assist, traction assist. The vehicle would not accelerate. When I got on the shoulder the engine was struggling to stay on and had a loud knocking sound. Had the vehicle towed to nearest exit and had someone come pick me up with

The oil consumption issue has begun. At about 75k miles, my 2018 Hyundai Tucson 1.6L Turbo is burning oil at an insane rate. And there is no indicator of it occurring (no codes or anything on the dash) until it's too late. Hyundai is now WELL KNOWN, and aware, for this problem and it is a catastrophic safety issue because of the possibility of the engine seizing and disabling the vehicle. It isn't
... negligence on the consumers part either, especially when timely and consistent oil changes are being done. *NOTE* My vehicle has had the transmission (6 speed dual clutch) replaced at around 30k miles and is having the same issue again (at 75k miles). So first the transmission and now the engine!Read more

Safety Hazard: Oil Pump Failure Leading to Sudden Engine Stall (BMW B58TU engines) Description of Defect: The oil pump module in the 2020 BMW X5 (G05) contains internal plastic components that are prone to thermal degradation and premature failure. These plastic parts can crack or disintegrate, causing a sudden loss of oil pressure. Safety Risk: A failure of the oil pump leads to insufficient lu
... brication of critical engine components, which can result in a sudden and complete engine seizure while the vehicle is in motion. This causes an immediate loss of propulsion and power steering assist, significantly increasing the risk of a high-speed collision or being stranded in dangerous traffic conditions. Additional Note: This is a known mechanical defect where a critical engine component (oil pump) relies on non-durable plastic materials, creating an unreasonable safety risk for drivers and passengers due to potential catastrophic engine failure without prior warning.Read more
