Vehicle: 2019 Acura RDX (Technology Package) I am reporting a serious safety issue involving sudden loss of engine power while driving. On March 14, 2026, I filled my gas tank before traveling approximately 1.5 hours out of town. The vehicle operated normally that day. The following morning, during snowy and icy conditions, while accelerating in traffic, the vehicle suddenly lost power and stall
... ed. Multiple warning lights illuminated on the dashboard (“Christmas tree” effect), and the steering became very stiff, making it difficult to safely maneuver the vehicle out of traffic. I was able to restart the vehicle, but warning lights remained on. The next day, while driving on the highway under normal conditions, the same issue occurred again. While accelerating, the vehicle suddenly lost power and stalled in traffic, creating a dangerous situation. In response, the fuel pump inside the gas tank was replaced; however, the problem persists. The vehicle continues to display warning lights and still loses power when accelerating. I contacted an Acura dealership after seeing reports of a fuel pump recall for similar vehicles, but I was told my VIN was not included. Despite this, the symptoms match known fuel pump-related issues. This defect poses a serious safety risk, especially when driving at highway speeds or in traffic, as sudden loss of power and steering assist can lead to a crash. No injuries have occurred yet, but the issue is ongoing and unresolved. I am requesting investigation into this issue as a potential safety defect.Read more

The original B58 engine (B58B30M0, 2015–2017) utilized a conventional mechanically robust gerotor oil pump with predominantly metallic internal construction and demonstrated strong reliability in field operation. Beginning with the B58 Technical Update introduced for 2018 production (B58B30O1), BMW redesigned the lubrication system by integrating the engine vacuum pump and oil pump into a combi
... ned variable-displacement assembly. This redesign introduced a thermoplastic internal adjusting ring (carrier) responsible for regulating oil pump displacement through rotor eccentricity control. This component, constructed from reinforced thermoplastics including PA66-GF materials, operates within a chemically aggressive and thermally cyclic environment involving continuous exposure to engine oil additives, fuel dilution, condensation, and repeated heat cycling. While acceptable under accelerated validation testing, real-world operating conditions increasingly demonstrate premature degradation consistent with polymer embrittlement and fracture. Observed Failure Mode Documented owner and technician reports indicate that under cold-start conditions — particularly in sub-freezing climates — elevated oil viscosity and startup load impose increased mechanical stress on the thermoplastic carrier. Fracture of this internal component allows debris intrusion into the pump mechanism, resulting in unstable oil pressure regulation. Commonly reported precursor symptoms include: • Inability to perform electronic oil level measurement via iDrive • Erratic or oscillating oil pressure behavior • Intermittent lubrication warnings or absence of warning prior to failure • Rapid progression to bearing damage and engine seizure In many cases, drivers receive insufficient warning before catastrophic lubrication loss occurs. Vehicles may become disabled without notice, including during highway operation or winter driving conditions, leaving occupants stranded in potentially dangerous environRead more

Ford Escape has experienced coolant intrusion into the engine cylinders, a known issue affecting this model year. The vehicle began misfiring and consuming coolant without external leaks. The dealership confirmed a Technical Service Bulletin applies to my VIN. This defect can cause engine failure while driving, creating a serious safety risk. The engine shudders so bad I cannot drive the car. I
... have also had to replace the transmission a few years back. It became a safety hazard. I was driving it and it wouldn't get up to speed and it was stalling on the road with traffic. This vehicle has several know issues, the cost to upkeep and constantly replace things to maintain safety is unaffordable for anyone. Children and people ride in these cars. These cars are unsafe to be on the road for those in them and around them. I still owe money on mine and can't even drive it due to its safety hazards with the engine and the coolant intrusion. I do not have a repair invoice as I decided to not drop anymore money into this car. Its unfortunate I am still paying on the loan for it.Read more

I reported problems with my truck to Beck and Masten South GMC. I was put off a number of times with ridiculous excuses. My vehicle sat in the shop for a month to get my OnStar safety equipment repaired. I also had arguments with them regarding the oil light coming on and it burning excessive amounts of oil. I was told this was normal. I know better. Also, I needed to get it back into the shop for
... service and inspection for a squeeling noise under the hood. And to have them check the oul consumption. I was told it idle too often, and this was the reason oil was dissapearing. I was told they would put me on the loaner list so I could have means to get to my oncologist for cancer care. Well, I called numerous times because they never furnished a loaner for these issues I was told a third, then forth time I was next in line for a loaner so I could drop it off for repairs and recall work. After 8 months of these lies I finally went to Monument Chevrolet. Beck and Masten never intended to help me. Total I had tried and was told by newer service advisors, the manager, and GMC this would be remedied over 10 times in over 6 months. I finally gave up and moved on to never return to this dealer I had purchased two brand new GMC 1500 trucks from in a two year time frame. I did get my OnStar gateway repaired by replacement. It had stopped working again, shortly after getting it back. I kept getting put off. They stated it needed another Gateway for OnStar. I kept calling, and was told they would contact me ince it came in. I ended up troubleshooting the entire modem/router assy. I found inconsitant configuration parametrs. So, I reconfigured it all from start to finish after clearing the memory by taking battery cables off and allowing it to power down and clear memory. Then I did perform a reconfiguration and username/password change as well. I repaired it myself. The Onstar has never acted up again. These were my first issues. Sudden loss of propulsion at highway speeds - 2023 Escalade. We easily could have been killed. I drive a 2023 Cadillac Escalade that I purchased from Motor Werks Cadillac in [XXX] . It was very expensive. The car has under 33k miles on it, off memory.I informed the dealer I would be taking the car on a family vacation road trip from [XXX] to [XXX] (for spring break), and requested they perform a
... ny necessary preventative service on it to ensure it was in perfect shape to make the trip. They performed the service (basic air filter change etc) and returned the car to me two days before I packed my family [XXX] and [XXX] daughters and wife) into the car and began the journey. After driving less than three hours, the engine completely and suddenly died, without warning, while we were traveling at 75 miles an hour on the highway in the left lane in the middle of rural [XXX] . At the same time all of the electronics in the vehicle died as well. It was an extremely dangerous situation because the road is heavily trafficked by semi trucks, which I was barely able to avoid as I attempted to coast across all the lanes of traffic to get onto the shoulder of the road on the right hand side. Because the car was without propulsion, I was not able to get fully onto the shoulder, but mostly. This left the car in a precarious position because the semi's that we're driving by could easily have hit the car had they not been careful and veered slightly to the left. We easily could have been killed. There's more to the story, like the dealer telling me Cadillac is aware that catastrophic engine failure at highway speeds is a known issue impacting Escalades around the country, and including significant mental trauma that my children experienced by being stranded on the side of the road for several hours in the dark in a car that had no engine, no electronics and no heat. After the recall work was performed, the vehicle began consuming engine oil at an abnormal and excessive rate between oil changes. Most concerning, during a recent check, the engine oil level was found to be extremely low, with no oil registering on the dipstick. This condition presents a significant risk of engine damage or failure and raises serious safety concerns. Prior to the recall service,
... the vehicle did not exhibit any abnormal oil consumption. The issue has only occurred after the recall was completed, which strongly suggests a direct connection to the repair.Read more

I own a 2023 Ford F-150 Lightning, with 48,200 miles and 2.5 years of ownership. Recently, my truck began displaying a “Powertrain / Reduced Power” warning while driving, and the air conditioning stopped producing cold air. The dealer scanned the vehicle and found DTC P0534 – A/C System Charge Loss. Further diagnostics indicated that multiple vehicle modules may require reprogramming and sof
... tware updates. The dealer stated they need to evacuate and recharge the AC system, add dye, and remove front trunk components to locate the leak. This AC system is part of the high-voltage battery and eDrive thermal management system, meaning that failure of the HVAC system affects battery cooling and powertrain performance. Loss of cabin cooling is a symptom of a thermal system malfunction, which can lead to battery overheating, powertrain derating, and reduced driving performance. The combination of unexpected reduced power and loss of cabin cooling creates a safety risk, particularly during highway driving, merging, or situations requiring acceleration. These failures occurred at relatively low mileage and are associated with the high-voltage EV system, which should be functioning properly to ensure safe vehicle operation. I am submitting this complaint to report a potential safety defect with the 2023 Ford F-150 Lightning affecting the battery thermal management, high-voltage AC system, and eDrive components, which can result in sudden power loss and compromised vehicle safety.Read more

My 2024 Ford Ranger Raptor has been suddenly losing power or stumbling upon initial acceleration. This has been occurring intermittently since it was nearly new. My truck now has approximately 10,500 miles. The problem has become increasingly frequent. It seems to occur most often when accelerating from a complete stop. The loss of power can last for several seconds, then afterwards, normal power
... is regained. It poses a safety risk since it has nearly caused collisions. Several times, the truck has lost power while making a left turn in front of oncoming traffic. It simply will not accelerate, and I find that I am stuck, unable to complete my turn and clear the oncoming traffic. Once when accelerating away from a stoplight, the truck lost power, and I was nearly struck by following vehicle. I have taken my truck to a local dealer who attempted to diagnose the problem and they didn’t offer a solution since they were unable to reproduce the problem during their road test. It has happened many times since then and I am scheduled to return the vehicle to the same dealership to attempt to diagnose and fix the problem again soon. The problem seems to occur, regardless of which drive mode the truck is in and also occurs with Electronic Stability Control both enabled and disabled. I am filing a safety complaint regarding my 2019 Infiniti QX50 experiencing engine failure consistent with known bearing defects. On February 13, 2026, while driving from [XXX] to [XXX] on the highway, my vehicle began shaking severely, producing a rattling/knocking noise from the engine, and losing power while in motion. This created a dangerous driving condition and required the vehicle to be
... towed. The vehicle was taken to a repair shop in [XXX] , where it was diagnosed with engine bearing failure. The shop indicated that this issue was consistent with a known defect and that there was an active recall related to engine bearings at that time. Based on that information, the vehicle was towed to Bob Moore Infiniti in [XXX] for recall inspection and repair. Initially, the dealership indicated that there was an issue and that it was associated with a recall. However, they later reversed their position and stated that the vehicle was “fine” and did not perform any recall repair, despite the ongoing symptoms and prior diagnosis. The vehicle continues to exhibit the same dangerous symptoms, including shaking, engine knocking, and loss of power, and remains diagnosed by another repair facility as having engine bearing failure. This is a serious safety concern because the vehicle can lose power unexpectedly while driving at highway speeds. Additionally, there appears to be inconsistency in how this known defect is being handled, as my vehicle was initially identified as affected but later denied repair. I am reporting a Failure of the Remedy for NHTSA Recall 23V634000 (Kia Recall SC281). The original recall attempted to "fix" a High-Pressure Fuel Pump (HPFP) issue via a software update to the Engine Control Unit (ECU). This remedy has failed. My vehicle is currently experiencing the exact safety hazard described in the recall: loss of motive power and stalling at various speeds due to HPFP failur
... e. Despite the software "patch" performed in 2024, the physical hardware (HPFP) has now suffered a mechanical failure, rendering the vehicle unsafe to drive. Description of the Incident: On or about March 15, 2026, the vehicle began lose power and stuttered while in motion, posing a significant risk of a rear-end collision. Diagnostic scans (OBD-II) confirmed code [Insert Code if known, e.g., P0087 for Low Fuel Rail Pressure]. A secondary diagnosis at Courtesy Kia (Mesa, AZ) on March 18, 2026, confirmed the HPFP has failed. Failure of Manufacturer to Act: The manufacturer (Kia America, Case #XXX) is currently refusing to replace the failed component under the recall mandate. They are categorizing the repair as "Goodwill" because the recall was "closed" by the unsuccessful software update. The manufacturer’s refusal to remediate a known mechanical defect that causes stalling in traffic is a direct violation of the spirit and legal requirement of the National Traffic and Motor Vehicle Safety Act. Two days later dealership calls me and says new software has arrived and they need to run diagnostics for recall again, this time on May 16th 2025 car passes. NOW, almost one year later and after warranty has expi
... red, the car has a DTC P1327 which is noted in the recall letter and the car will NOT start. Had it towed to nearest dealer and they say since the original dealer "Passed" the recall, I have to pay to get car looked at again. It is obvious to me their original diagnosis missed something and now I just want my vehicle back, but it won't run because of the P1327 DTC and of course the dealership where car is now says since I did not buy it there, nothing they can do? KIA service is TERRIBLE.Read more

The vehicle has a recurring P0420 code (“Catalyst System Efficiency Below Threshold Bank 1”). A boroscope inspection at Ricardo Caballero Kia Santa Isabel (invoice #[XXX] dated December 5, 2025) confirmed internal disintegration of the catalytic converter, with physical pieces of the catalyst falling out when the exhaust pipe was removed. This failure appears related to Recall 23V534 (SC276) a
... nd TSB PS709 (overheating / coolant boiling issues in 2017-2022 Niro Hybrid models). Motorambar, Inc. (official Kia distributor in Puerto Rico) has ignored my warranty claim since December 2 and December 22, 2025 (more than four months). Motorambar is now refusing to authorize a courtesy loaner vehicle at the dealer Ricardo Caballero Kia Santa Isabel, claiming the tests will take less than 5 days. This leaves me without transportation. The catalytic converter is fully covered under the Federal Emissions Warranty (8 years / 80,000 miles). I request that NHTSA investigate this as a potential safety and emissions defect and require Kia / Motorambar to: 1. Immediately repair or replace the catalytic converter under warranty at no cost. 2. Provide a courtesy loaner vehicle during the repair. 3. Reimburse previous diagnostic costs of $449.17. My wife drove back home from work around 5:45 PM EST on March 2, 2026 and parked the car in the garage as always. She parked and turned off the car and got out to check the mail from the mailbox across the street. The music stopped when she turned off the car ignition after putting the transmission in park position and opened the door to get out. She walked to the mailbox and heard a loud bang. Sh

Office of Defects Investigation National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 Re: Safety Defect Complaint — Ram ProMaster (2018–2025) — Cowl Water Intrusion into Engine Compartment To Whom It May Concern, I am writing to formally report what I believe to be a serious, systemic, and unresolved design defect in the Ram ProMaster van, affecting
... model years 2018 through at least 2025. I am requesting that the Office of Defects Investigation open or expand a formal investigation into this matter. Nature of the Defect: The windshield cowl on the Ram ProMaster is designed in a manner that permits significant water intrusion directly into the engine compartment. Water enters through the cowl seam and surrounding gaps, bypassing any drainage system, and flows onto critical engine and electrical components including but not limited to: the serpentine belt and pulley system, the engine fuse box, transmission components, and wiring harnesses. I brought my vehicle to two separate authorized Ram dealerships. The second dealer confirmed — based on testing conducted on multiple ProMaster vans dating back to 2018 — that this water intrusion behavior is consistent across the model line. In other words, the dealer has effectively confirmed that this is not a defect isolated to my vehicle, but a design characteristic of the ProMaster platform itself. This confirmation does not reassure me. It alarms me. A design that has permitted consistent water intrusion onto safety-critical engine and electrical components across a major commercial vehicle platform for over a decade is not an acceptable engineering standard. Given the size and weight of these vehicles — many operated commercially and carrying passengers or heavy cargo — the consequences of power steering loss, electrical failure, or mechanical failure are severe. We have tried dealing with Stellantis as well. Any help would be greatly appreciated. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIRead more

On or about 02/09/2026, the vehicle experienced a catastrophic failure directly related to a known defect identified in Technical Service Bulletin 19-NA-066. The engine "sucked" in the oil filter due to a poor design on GM's part. The stand pipe was not present apparently, and must have broken off at some point. This bulletin acknowledges an issue affecting this vehicle platform. This should have
... been recalled to address this engineering flaw that GM acknowledged in their TSB. Despite multiple attempts and ongoing communication with authorized representatives to have the vehicles engine replaced under an extended warranty, the vehicle was inoperable and unusable, causing significant financial hardship. I owned the vehicle for approximately 5 months and had the vehicle in the shop multiple times for other repair issues that were known per forums from other owners of this model/year of vehicle. I have exhausted all reasonable efforts to resolve this matter through dealership, GM customer service channels, and GM's executive team without satisfactory resolution. As a direct result of this defect and resulting failure, I have incurred the following damages: Loss of use of the vehicle ($24,578 purchase price on 09/20/2025) Rental and alternative transportation expenses ($90.58 rental vehicle from 02/12/2026 - 02/14/2026) Diminished vehicle value ($24,578 - $12,000 (Selling Price) = $12,578) Out-of-pocket diagnostic and related expenses ($2,036.50) Total loss and seeking reimbursement: $14,705.08Read more

When stopped at a red light, the vehicle’s auto off system turned the engine off. When the light turned green and I removed my foot from the brake, the engine stalled and would not restart. Luckily, this happened leaving my neighborhood and not in a more prec

They aren’t willing to fix it until there is a recall on the vehicles. In the meantime, I have a one year old vehicle that sputters and struggles to restart more often than not. I have had the vehicle stall like this after the auto off system turned the engine off when trying to merge into traffic on the interstate. I had a near miss to an accident at that time because the vehicle stalled and would not restart.Read more
