What component or system failed or malfunctioned, and is it available for inspection upon request? Engine seized and vehicle was towed to dealership. It is available for inspection. How was your safety or the safety of others put at risk? Loss of propulstion, described below. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, dealership where originally
... purchased. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Inspected at dealership by GM/Chevrolet technician. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? None prior to failure Detailed description: On 5/21/2026, during normal driving, the engine in my truck lost propulsion due to a major internal failure. This is a 2023 General Motors 6.2L (L87) engine that was inspected at the dealership where it was originally purchased for the previously issued NHTSA "loss of propulsion" recall and passed without any noted issues. This engine only has ~20,000 miles on it. The engine failed in relatively heavy traffic and could have easily resulted in a serious accident due to inability to route my vehicle to a safe area while under power. This all happened without any prior warning, no check engine lights, noises or lamps or symptoms. The dash simply illuminated with a "Press Start" message. The truck was towed to the dealer for evaluation. The dealer called me today (5/26/2026) stating the engine was seized and a new engine would have to be ordered from GM for installation. This event occurred while the vehicle (2023 Silverado ZR2) is still under the original 36 month/36,000 mile warranty.Read more

My vehicle has the V6 twin turbo engine that’s been recalled on other years and models and was diagnosed with an internal engine failure. The mechanic at the dealership couldn’t figure out the problem so an expert TAS Lexus specialist was called in and couldn’t figure out the problem either. They’ve had the car for 3 months and since they don’t know how to fix it they are telling me to
... just replace the engine with a used engine at a cost of $25,000. The vehicle is just out of warranty and Toyota/Lexus is refusing to cover any of the costs. The vehicle was serviced regularly so this shouldn’t have happened. This is a common problem with this model of engine. I believe it’s a design flaw as this model engine is assembled at different plants/locations and over long periods of time that would indicate a design flaw and not due to Toyota’s official stance of debris left in engine block during assembly. Toyota should recall and replace all these engines at no charge to the owner of the vehicle.Read more

The last event ended in a a scary and close interaction with an 18 wheel truck that was unable to stop as we lost power. Dealer Confirmation: The dealer has confirmed, on two separate occasions, that the fuel injectors needed replaced. The vehicle has 3,000 miles on it. At 2,000 miles, damage was found to the fuel injectors. They were replaced and the car was placed back in my possession. At 3,000 miles, the incident occurred for the 3rd time and damage was again found with the fuel injectors. My wife and I had decided to empty our storage unit, so on Memorial Day we began moving everything into the garage. After five or six trips Honda, I pulled the car up beside the RV and close to the house for what turned out to be the last load. I unloaded my bicycle and nearly everything else, leaving only a few small items, and then removed the massage table from the back seat. The engine was off
... this entire time. That was when I noticed smoke coming from the engine compartment. At first, I thought the radiator had overheated, so I tried to open the hood, but it was too hot. I went into the garage for gloves and a fire extinguisher and tried again, but still could not get it open. Realizing the situation was getting worse and that I could not stop it there, I jumped into the car and tried to start it. To my surprise, it started immediately, and I drove it as fast as I could out to the street in front of the house. Only later did I realize that this likely saved my RV—and possibly the front of my house as well—from catching fire. We were so blessed that I was able to move the car to a safer location and no one was hurt. It was a miracle.Read more

I am submitting this complaint regarding a serious and dangerous brake failure involving my 2020 GMC Terrain SLT with approximately 150,000 miles. While driving, the vehicle suddenly experienced a vacuum pump failure that caused a severe loss of braking ability with no warning beforehand. The brake pedal became extremely hard, and the vehicle would barely stop. This created a very dangerous situa
... tion and could have easily resulted in a serious accident. Thankfully, I was not driving at a high speed when the failure occurred, and there were no vehicles directly in front of me at the time. However, only minutes earlier I had been traveling approximately 65–70 mph on the highway. If this brake failure had occurred while driving at highway speeds or in heavy traffic, I truly believe it could have caused a major crash resulting in serious injury or death. What concerns me most is that there was no warning before the brakes failed. No warning lights and no issues with my brakes. The sudden loss of braking power was extremely frightening and created an immediate safety hazard. After researching this issue, I discovered that other GMC and GM vehicle owners have reported similar vacuum pump failures and brake assist problems. I believe this is a significant safety issue that should be investigated further. Drivers should not experience sudden brake failure without warning, especially on a vehicle that otherwise appeared to be operating normally beforehand. I am very thankful no one was injured in my situation, but I am concerned that another driver may not be as fortunate. My vehicle had to be towed to a nearby shop and they suspected it was the vacuum pump failure and the car started smoking badly. so I had it towed to the GMC dealership and they called me today to confirm that was the problem and it also caused damage to the camshaft. This is a known issue with the terrain and there is currently a class action lawsuit.Read more

NHTSA Complaint Description What Happened & Safety Risk: While driving a 2024 Jeep Wrangler 4xe at highway speeds on Interstate 84, the vehicle suffered a catastrophic, unrecoverable loss of motive power. Without any warning, the vehicle completely seized up, shut down, and turned off entirely while moving in active traffic. This exact life-threatening failure has now occurred twice. Operating a v
... ehicle that completely dies at highway speeds places the driver, passengers, and all surrounding traffic at an extreme risk of a high-speed rear-end collision, as the vehicle loses all ability to accelerate or safely clear the roadway. Component/System Malfunction: The failure lies within the hybrid propulsion system and electrical/software communication network (specifically relating to active Federal Recalls 25V710 for Hybrid Control Processor software shutdown and 25V766 for engine sand contamination). The vehicle is absolutely available for inspection upon request. Dealer Confirmation & Inspection: The problem is currently being evaluated. The vehicle is currently held at an authorized Stellantis dealership service department, where it was brought immediately following the most recent highway shutdown. Warning Symptoms: There were no advance warning lamps, diagnostic messages, or performance symptoms prior to the failure. The loss of propulsion and complete vehicle shutdown occurred instantaneously and without warning while traveling at highway speeds.Read more

This Forte was not neglected and it was not abused. Nearly 90% of its miles came from highway commuting and interstate travel. It was completely stock, never modified, never driven hard. More importantly, it received fully synthetic oil changes every 3k-4k miles since new, well ahead of what Kia or the oil manufacturer even recommends, because I intended to keep this car long term. All of that is
... verifiable through Carfax. The engine failure came without meaningful warning. While driving, I noticed an unusual noise and pulled over as quickly as safely possible. I checked the dash expecting some kind of indicator and found nothing. No check engine light, no oil pressure warning. A nearby mechanic found the engine more than 2.5 quarts low on oil despite the vehicle being only about 3,300 miles into a fresh synthetic oil change. There were no external leaks. No oil spots at home, none at work. Nothing to explain where that oil went. After topping it off, the noise briefly stopped. With no warning lights on and the noise stopped, I thought we had caught it in time. I even felt confident enough to also have the mechanic change the transmission fluid as it was about due again. However, a few miles later, the engine suffered complete failure. Violent shaking, severe knock, then the check engine light came on. I shut it down immediately and pushed it home. Once home, I checked the oil again and found it had unusually darkened in an extremely short period of time. The vehicle has not been driven since and is currently inoperable. What troubles me most, beyond the financial loss, is that this vehicle gave me no real opportunity to save it. The oil pressure warning light never illuminated. More than two quarts of oil vanished within a single service interval with no external source and no indication whatsoever, even with checking the level periodically. I went above and beyond in maintaining this vehicle and still lost the engine. This could have been a catastrophe on the roadRead more

The vehicle is currently under review with Volkswagen Customer Care, but the safety-related drivability concern has not yet been fully resolved. Supporting documents include the 2025 written report of jerking from a stop, the 2026 repair order documenting hesitation on acceleration, and post-repair emails showing the issue remained unresolved. I own a 2015 Chevrolet Silverado equipped with a GM V8 engine that experienced significant engine failure symptoms consistent with known GM engine defect issues and related recall/service campaign concerns. Prior to failure, the vehicle began exhibiting abnormal engine noises, warning lights, performance issues, and symptoms indicating internal engine damage. The condition worsened and resulted i
... n major engine failure requiring extensive repair or engine replacement. The failure created safety concerns due to the potential for sudden loss of power and loss of propulsion while driving. Based on the symptoms, model year, and engine configuration, I believe the failure may be related to known GM engine issues involving internal engine components, lifter failures, AFM/DOD system failures, bearing damage, or related defects that have been the subject of recalls, special coverage adjustments, technical service bulletins, or investigations affecting GM trucks and SUVs. I believe the vehicle should qualify for coverage, repair assistance, recall-related repairs, or special policy consideration from General Motors due to the nature of the engine failure and the known history of similar failures affecting comparable GM vehicles. He also said Ford changed the cooling design after 2019 due to the number of problems they have had with the 2.0L engine head gasket. The first warning light that came on was after the engine stalled the second time.Read more

I am filing a vehicle safety complaint regarding a 2024 Hyundai Palisade Calligraphy due to repeated safety-related mechanical failures and ongoing defects that create dangerous and unpredictable driving conditions. Since purchase in August 2024, the vehicle has experienced multiple serious issues, including steering rack problems causing sticking, resistance, abnormal noises, and inconsistent st
... eering response while driving. The vehicle has also experienced rear automatic braking/sensor malfunctions causing sudden stopping and grinding during operation, creating unsafe driving conditions and concerns about unintended braking behavior. Additional issues include transmission and gear engagement concerns, engine replacement under recall, infotainment system replacement, ongoing recall-related concerns, and excessive tire wear requiring replacement of all four tires at approximately 29,000 miles. The premature tire wear raises concerns regarding possible steering, suspension, alignment, or drivetrain defects. There are also concerns involving unusually fragile windshield and window glass, which appears to be a commonly reported issue among Hyundai Palisade owners. Despite repeated dealership visits, diagnostic evaluations, repairs, and major component replacements, the problems continue. The vehicle has spent an estimated total of approximately five months in repair facilities or undergoing service evaluations since purchase. The steering issues and rear automatic braking/sensor malfunctions are the most serious concerns because they directly impact the safe operation of the vehicle. The vehicle has demonstrated unpredictable behavior, including grinding, sticking steering response, and sudden stopping. These ongoing defects have caused significant safety concerns and loss of confidence in the reliability and safe operation of the vehicle. I believe these issues warrant further investigation due to the potential risk to drivers, passengers, and others on the roadRead more

While operating my truck under normal driving conditions, the check engine warning light suddenly illuminated without warning. Due to concerns about potential vehicle reliability and safety, I immediately brought the vehicle to the dealership for diagnosis and inspection. After evaluating the vehicle, the dealership stated that diagnostic trouble codes were present and that additional analysis of
... the system would be required. The vehicle remained at the dealership for approximately one week before being returned to me. Upon release of the vehicle, the dealership advised that the issue was related to multiple erroneous codes and that a software update had been performed. However, the dealership did not provide a clear explanation as to why the codes were generated, what systems were affected, or whether the condition could reoccur. I was also not provided with a complete accounting of all diagnostic trouble codes identified during the inspection. I am concerned that the dealership failed to provide full transparency regarding the diagnosis and repairs performed on this vehicle. The repair order documentation was limited in detail and appeared to omit important information regarding the discovered fault codes and the full scope of work completed. Because modern vehicle systems rely heavily on electronic control modules and software for engine operation and safety-related functions, I am requesting that NHTSA document and review this issue for potential underlying electronic or software-related defects that may affect vehicle reliability and safety.Read more

We bought this vehicle from Carvana in February 2023. Since very early on owning this vehicle, the engine hesitated and stuttered when trying to shift from first to second gear, and would lunge forward slightly. In December 2024, I received a letter from Ford Motor Company about a customer satisfaction program, Campaign 24N07. The letter addressed the symptoms my vehicle exhibited, including engin
... e hesitation, engine running rough, and engine stalling at low speeds, though the check engine light was not on. The letter states that Ford Motor Company provides a one-time replacement of the canister purge valve, given these symptoms and the check engine light being on. When I called all the local Ford dealerships, they refused to do it because the check engine light was not on, and that Ford Motor Company issued this as a customer satisfaction program, rather than a safety recall.Everything that was listed in Campaign 24N07 is MOST DEFINITELY a safety issue, and should not be taken lightly. It is highly concerning that Ford Motor Company didn’t think to consider ENGINE ISSUES a safety recall. The class action lawsuit Vargas et al v. Ford Motor Company had a deadline of November 4, 2020, which was 2 years and 3 months prior to us buying the vehicle, so any vehicles of the affected years and models should have been either repaired, or not allowed to be sold unless the repairs were done by an authorized Ford dealership. This information was not told to us when we bought the vehicle. On May 25, 2026, while driving, the engine fault and steering assist fault warnings came on, as well as the check engine light. When we were able to safely park the car, we discovered the entire power train is non operational. The engine no longer turns over, and we are now having to buy a new car.Read more

Purchased a used Santa Fe in January 2026 with 129k mile and at 131k my vehicle's CEL came on and did not function properly while operating. ... ed and I drove home and parked it. I ran CEL diagnostic and received the code P0021. I had the vehicle towed to the local Hyundai dealer and they also pulled the same code and indicated I have an oil consumption issue explained as follows "Timing over advanced. Timing components driven by oil pressure. Inspected cylinders and borescope and found cylinder wall scoring and faulty piston rings. Radiator has crack causing coolant leak. Engine will have to be replaced to remedy oil consumption. Radiator will need to be replaced to remedy leak." resulting in over $12k of repair work. The vehicle's temperature gauge did not indicate any abnormalities nor did the oil light come on to indicate any oil consumption issues. The vehicle was driven from the dealer to my home within city limits/speed and has not replicated the acceleration issue but the CEL remains on as the repair is too costly and is more than what was paid for the vehicle.Read more
