The vehicle remained with the dealer. The dealer then advised the contact to inform the insurance company of the failure; however, the insurance company referred the contact back to the dealer because the failure was not caused by a crash. The dealer was advised accordingly and threatened to tow the vehicle at the contact's expense and to charge the contact for vehicle rental if the contact did not return the rental. The contact advised the dealer that the vehicle was not repaired and was unsafe to drive. The vehicle was not repaired. The Volkswagen dealer and manufacturer were not contacted. The vehicle experienced a catastrophic engine failure at approximately 50,230 miles, after which both the engine and turbochargers were replaced under warranty with the cause listed as undetermined. Both the original and replacement engines have now failed in the same manner, with turbochargers leaking oil and producing visible smoke, indicating a likely recurring defect or unresolved underlying issue. The replacement engine failed after only 40,000 miles, further raising reliability concerns. Infiniti quality actions (P4A05 and P4A06) identify turbocharger oil leakage as a known issue, aligning with the current failure. Dear American Honda Motor Co., Inc. ,I am writing to formally document my experience with a premature failure of the Continuously Variable Transmission (CVT) in my Honda HR-V. The vehicle was covered under the extended warranty program Honda implemented specifically for the known CVT issues affecting 2016–2020 model years. In On 3/10/26 the CVT failed at approximately 96,000 miles. The vehicle w
... as towed to an authorized Honda dealership, where technicians confirmed premature belt deterioration and breakage. Prior to the failure, I had the vehicle serviced at the dealership for the Powertrain Control Module (PCM) software update outlined in Honda Service Bulletin 21-047. During that visit, I reported a noticeable burning smell, which I now believe originated from the failing transmission belt. The dealership assured me there were no issues and stated that the software update had adjusted the gear ratio to reduce strain on the transmission. The vehicle was returned to me with no further action recommended. Subsequent communication with American Honda revealed that the company had been aware of widespread CVT belt issues in these model years. Rather than issue a full recall, Honda extended the powertrain warranty to 7 years or 150,000 miles (whichever occurs first). My repeated attempts to escalate the matter with Honda Customer Service, including multiple requests for contact information for a regional manager, were unsuccessful. After several weeks of delays—during which my vehicle remained at the dealership for over 20 +days—Honda Corporate initially offered to cover a remanufactured CVT replacement at a cost of $4,500 to me. The dealership ultimately assisted by reducing my out-of-pocket expense to $2,900.I find it unacceptable that a vehicle with only 96,000 miles required a major transmission replacement costing nearly $3,000, especially after Honda had already acknowledged the defect through a technical service bulletin and an extended warranty. The vehicle would not restart. The vehicle was towed and required a complete engine replacement under warranty. After that replacement, additional issues followed. There was an HVAC/defrost concern. In June 2025, the vehicle again became inoperable without warning while parked at a store. It would not start despite repeated attempts, including by a dealership representative. A diagnostic scan produced over a page of error codes, but the dealership could not identify a cause and stated the vehicle was safe. In Dec. 2025, the vehicle engine was inspected under a recall and I was told there were no issues. In Feb. 2026, the vehicle lost heat and was found to have no coolant despite regular maintenance and recent inspection. In Mar. 2026, at about 37,700 miles, the vehicle developed a loud clunking noise while driving. I stopped driving and returned it to the dealership, where I was told the engine had failed again and required another full replacement. The vehicle has now had two engine failures and three engines installed in less than three years. There were no warning lights or prior indications before these failures. Original letter to Hyundai below. The car is currently at a dealer again for repair of the same issue. Fuel pump replaced in Nov 2025 but issue has returned in March 2026. Videos of the issue recently occurring are available. Good morning, We had our 2024 Santa Fe Hybrid lose power while merging onto the freeway in November 2025. I was pressing the accelerator and it did not do anything, just k
... ept rapidly decelerating. Pulled the car to the shoulder and turned it off completely and back on, and the car started up with the engine running which was unusual- typically the car starts up in silent electric mode. It would not accelerate past a few miles per hour. This has now happened on multiple occasions, disappearing for a few days only to return. It has gotten very dangerous to lose speed and acceleration into a busy freeway and almost caused an accident yesterday. We were thankfully close to a dealer (Glassman Hyundai in Southfield, MI) and were able to limp it a mile down the shoulder at low speed to drop it off. However, they are telling us they have no loaners available to provide us which seems unacceptable with a car under warranty with 18k miles on it. Is there anything you’re able to do to assist? Vehicle: 2026 Ram 1500 Limited I am filing a safety complaint regarding a brand new vehicle that experienced a dangerous failure in live traffic and continues to have unresolved issues. The vehicle was purchased on March 31, 2026. On April 3, 2026, just three days after purchase, the truck broke down in active traffic. The situation was serious enough that local police had to respond to direct
... traffic while the vehicle was towed. This created a clear safety hazard for both the occupants and other drivers on the road. The vehicle was taken to the dealership on April 4, 2026 for repair. At that time, the dealership replaced the battery and reset a radio frequency module. We were notified the vehicle was repaired and ready for pickup on April 7, 2026. However, immediately after leaving the dealership, the vehicle began experiencing additional failures. The key fob would not function, the doors would not lock or unlock electronically, and the vehicle struggled to start. Within minutes, the check engine light illuminated. Due to safety concerns, the vehicle was returned to the dealership the same day. After returning the vehicle, we were informed that there is a short in the main PDC (power distribution center / fuse box). This critical issue was not identified or addressed during the initial service visit. A fault in the main fuse box presents a serious safety concern, including the potential risk of an electrical fire. Additionally, the dealership failed to properly verify the repair before releasing the vehicle. The truck was not adequately tested or driven after the initial repair, resulting in us being sent to pick up a vehicle that was still unsafe and malfunctioning. This vehicle has been in our possession for approximately one week and has already experienced multiple serious failures, including a breakdown in live traffic, repeat electrical failures, and now a confirmed issue within the main power distribution system. I don't know if this qualifies as an immediate safety issue, however, if the vehicle is stopped and placed into park on a busy freeway, or in traffic in any way, then this absolutely is a safety issue after the fact because the vehicle cannot be moved due to not being able to get it into gear. This involves the shift linkage component and not being able to get the car out of park and into any othe
... r gear. It's already an active recall but it's VIN specific and my VIN was not included. It really needs to be though. It should be all Fusions at this point. I drive a 2020 Ford Fusion and the shift linkage bushing broke. Initially I got the "shift system error" message back in January and could not get my car into gear. It would just bounce right back into park. I had to call a tow truck but eventually the driver was able to somehow reset the system using the hidden reset button in the center console. It was enough to keep me running for a couple of months. However, two weeks ago it happened again and the reset option was no longer working. The car was towed to my dealership and they replaced the "missing" bushing and I was charged $511.00 for the repairs. Considering how widespread this issue this seems to be, I would like to see a full model-specific recall issued for these Fusions and possibly reimbursement for the costs I was subjected to due to the issue. I can provide the service receipts from the dealership showing that it was replaced and the total cost. Thank you!Read more

I turned the engine off for a few minutes. I turned it back on and still could not shift the car into gear. All of the error messages scrolled across the dash and all of the error lights came on. I had never seen a light on the dash that day or any other. FHP arrived. We pushed the car to the left shoulder. He said the cars were traveling at 60-70 mph at this point. Prior to that I was trapped in my car as I could not get out of my car with the high speed traffic passing on the left shoulder and swerving around to the right lane too. I called for a tow truck and was towed to a VW dealership. It was in service for 8 days. I was told it was a glitch in the software update. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The fuel delivery system and engine power management. The vehicle suffers from sudden "Limp Mode" activation and total loss of acceleration. The vehicle is available for inspection. 2. How was your safety or the safety of others put at risk? My safety was severely compromised when the vehicle sud

This occurred twice in high-speed traffic, creating an immediate risk of a rear-end collision as the vehicle could no longer maintain the flow of traffic or accelerate away from danger. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? No. The issue was reported to a dealer in 2024, but because the malfunction indicator lamp (MIL) turned off after a restart, the dealer was unable to reproduce the failure or find stored error codes at that time. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, it was inspected by an authorized Honda dealer in 2024, but no repairs or actions were taken as they could not replicate the fault. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No. There were no prior symptoms or early warnings. The Service center suspected a head gasket issue, so they began to address it; what they found was casting sand sludge throughout the engine block. An issue which is an inherent defect of manufacturing, which is unknowable to a unsuspecting owner - until a total failure occurs. This issue would also coincidentally constitute a breach of the "implied Warranty Of Merchantability", as the issue makes affected vehicles not fit for their intended purpose. The Service Center that found the defect is an Authorized FCA Stellantis Dealer/Service Provider. The Service Manager has had an "Conference Call" with an Area Service Manager & A Service Technical Advisor. The Service Manager has indicated that it is casting sand & has told me that if the manufacturer has him send them the engine that i what he is putting on the report. He has told me that he feels that the manufacturer likely will try to bury the issue by attempting a "Forced Buyout". Cursory, research by myself has found that Casting Sand in the presence of H.O.A.T or O.A.T. coolant & heat forms a sludge which over time {40,000 to 55,000 miles} renders an engine unusable, risking catastrophic failure or fire. LX Platform vehicles of the era 2005-2009 were considered "Durable for intended purpose for 150,000 to 300,000 miles". Casting Sand left in the engine block, during manufacture, violates that durabilityRead more

I have a 2021 GMC Sierra 1500 AT4 3.0L diesel pickup truck with 98,500 miles. The coolant control valve failed on my vehicle and for some reason it is not covered under GM's special bulletin that covers this part 15yr/150,000. Ridiculous.

I am submitting a safety complaint regarding a serious incident involving my 2025 Kia EV9. The vehicle was serviced on March 1 at World Kia in Joliet, Illinois. On April 4, only four days after that service, the vehicle experienced a sudden and unexpected ICCU failure while I was driving. During my normal travel, in active traffic and at speed, the vehicle abruptly entered limp mode and displayed
... multiple system warnings. This caused an immediate and significant reduction in power, creating a hazardous situation for myself and surrounding drivers. I was forced to maneuver the vehicle out of traffic while it was rapidly losing propulsion capability. The vehicle is currently at Berwyn Kia in Berwyn, Illinois for diagnosis and repair. I am reporting this incident due to the clear safety implications of a sudden loss of power during highway driving operation. A failure of this nature poses a risk of collision, especially in high‑speed or congested traffic conditions. I am requesting that NHTSA review this issue as part of its ongoing monitoring of Kia/Hyundai/Genesis Integrated Charging Control Unit (ICCU) failures and related safety concerns. Thank you for your attention to this matter.Read more
