Incident date: Jan 3, 2026
The vehicle has experienced persistent engine performance issues that have not been resolved through standard repairs. Initial diagnostics indicated knock sensor faults, which were replaced at significant cost. However, the issue persisted and additional codes related to air fuel imbalance (P219B) appeared. The vehicle continues to exhibit reduced power, hesitation, and inconsistent acceleration
... while driving. This creates a safety concern, particularly when merging into traffic or attempting to accelerate in situations requiring reliable engine response. The dealership has quoted a minimum repair cost exceeding $7,000, with the possibility of increasing to $10,000 depending on further findings once the engine is opened. Due to the high cost and uncertainty of repair, the vehicle may remain in operation with an unresolved defect. Research indicates that similar issues have been reported by other owners of this vehicle generation, suggesting a potential broader defect that may impact vehicle safety. This issue affects the vehicle’s ability to deliver consistent power and safe acceleration. Given the relatively low mileage and the severity of the repair required, this may represent a defect that warrants further investigation. [Incident Description & Manufacturer's Bad Faith] On April 6, 2026, an authorized Honda dealership (McConnell Honda in Montgomery, AL) diagnosed my vehicle (2019 Honda Odyssey EX-L, 143,097 miles) with DTC P219A (Rec replacing fuel injector set and fuel feed pipe at this time), requiring a fuel injector replacement. This exact mechanical failure is documented in Honda’s official Service Bullet
... in 21-010. The bulletin acknowledges that defective fuel injectors can cause engine misfires, loss of motive power, and engine stalling while driving. These conditions present an immediate and severe safety hazard. Honda established a 150,000-mile warranty extension limit for this defect in TSB 21-010. My vehicle has 143,097 miles and is well within this limit. However, American Honda Corporation has arbitrarily refused to repair the vehicle. Furthermore, on April 21, 2026, the assigned Honda Case Manager left a voicemail officially denying the repair. He explicitly refused my repeated requests to provide this denial in writing. In the voicemail, he falsely stated that my vehicle at 143,000 miles is "well outside the range" for assistance, directly contradicting Honda's own written 150,000-mile bulletin. Please review the attached documents, including the official dealership diagnostic report (DTC P219A), the repair estimate, and a copy of TSB 21-010. I have also secured the audio recording of the case manager's voicemail to prove their bad faith. Honda is knowingly leaving a dangerous vehicle with a confirmed, documented fuel system defect on the road, while actively providing false information to the consumer to avoid repair costs. I strongly request the NHTSA to investigate Honda's deceptive and dangerous practice.Read more

I am filing a safety complaint regarding a catastrophic timing chain failure on my 2014 BMW X3 xDrive28i equipped with the N20 engine. While driving, the timing chain and oil pump drive chain failed without warning. This is a well-documented defect on BMW N20/N26 engines that has been the subject of extended warranties and numerous consumer complaints. This failure can cause sudden engine seizur
... e, immediate loss of power steering and braking assist, and creates a serious risk of crash while the vehicle is in motion. BMW issued an Extended Limited Parts Warranty for this exact defect (timing chain and oil pump drive chain), acknowledging a manufacturing defect. However, BMW is refusing to cover my repair even though this is a known safety issue affecting thousands of vehicles. The vehicle had been properly maintained, and there were no warning signs before the failure. I purchased the vehicle in October 2021, and this defect occurred during normal use commuting to work. The cost of repair is extremely high due to the internal engine damage caused by the timing chain defect. This is not normal wear and tear. This is a known design/manufacturing defect that BMW has been aware of for years. Also, when I purchased this vehicle, the headlights, and taillights on the hatchback were not working. I had brought this up to the dealership they replaced the headlights, but did not address the tail lights. I am requesting that NHTSA investigate this defect further, and the tail lights as these failures create a significant safety hazard for drivers and other vehicles on the road.Read more

My 2020 Hyundai Palisade has an excessive and dangerous oil consumption defect. The vehicle consumes approximately 3/4 of a quart of oil every 1,000 miles. At a standard 8,000-mile oil change interval, the engine loses more oil than it holds between services. At approximately 185,000 miles, my vehicle stalled on the interstate with my family inside the vehicle. Upon inspection, the engine was cri
... tically low on oil. I added 5 quarts of oil to restore proper level. Critically, at NO point prior to the stall did the low oil pressure warning light illuminate. The vehicle's oil warning system completely failed to alert me that the engine was dangerously low on oil. This is a dual failure: the engine is consuming oil at an excessive and unsafe rate, and the warning system designed to protect the driver failed to activate when the engine was in a critical state. I have reported this issue to my dealership and filed a formal complaint with Hyundai Motor America. Neither has resulted in a meaningful resolution. Hyundai's internal "acceptable" oil consumption threshold of 1 quart per 1,000 miles is significantly outside mainstream industry standards set by other major manufacturers and appears designed to deny warranty claims rather than protect consumers. This defect poses a direct and serious safety risk. A vehicle stalling on the interstate due to oil starvation, with no warning to the driver, is a life-threatening failure. I am requesting that NHTSA open a formal investigation into the 2020 Hyundai Palisade for excessive oil consumption and oil pressure warning system failure.Read more

2025 CADILLAC XT6 AWD SPORT, VIN [XXX] , PURCHASED NEW FROM CENTRAL [XXX] CADILLAC ON FEBRUARY 20, 2026 WITH 3,950 MILES. WITHIN APPROXIMATELY TWO MONTHS OF DELIVERY THE VEHICLE DEVELOPED AN ABNORMAL ENGINE NOISE AT COLD START AND AN INTERMITTENT VIBRATION DURING DRIVING. I REPORTED BOTH SYMPTOMS TO CENTRAL HOUSTON CADILLAC BY PHONE AND TEXT MESSAGE. DEALER PERSONNEL TOLD ME THE NOISE WAS "NORM
... AL" AND THAT "ALL CADILLAC/GM VEHICLES HAVE THAT ISSUE." BASED ON THAT ASSURANCE I CONTINUED TO DRIVE THE VEHICLE AS FAMILY TRANSPORTATION, INCLUDING WITH MY MINOR CHILDREN ONBOARD, ONE OF WHOM IS A SPECIAL-NEEDS CHILD. IN APRIL 2026, APPROXIMATELY TWO MONTHS AFTER PURCHASE, I BROUGHT THE VEHICLE TO CENTRAL HOUSTON CADILLAC FOR AN UNRELATED CONCERN. AT THAT VISIT THE DEALER DIAGNOSED A CATASTROPHIC INTERNAL ENGINE DEFECT AND ADVISED THAT COMPLETE ENGINE REPLACEMENT IS REQUIRED. THE VEHICLE IS CURRENTLY OUT OF SERVICE FOR APPROXIMATELY 30 DAYS WHILE THE ENGINE IS REPLACED UNDER WARRANTY. A CATASTROPHIC ENGINE FAILURE IN A NEARLY-NEW VEHICLE CREATES A SUBSTANTIAL RISK OF SUDDEN LOSS OF POWER, STALLING, LOSS OF CONTROL, OR UNDERHOOD FIRE. THE DEALER'S INITIAL MISCHARACTERIZATION OF THE WARNING SYMPTOM (COLD-START KNOCK) AS "NORMAL" IS PARTICULARLY CONCERNING AND SUGGESTS A POTENTIAL PATTERN ACROSS 2024–2025 CADILLAC XT6 VEHICLES THAT NHTSA SHOULD INVESTIGATE. NO CRASH HAS OCCURRED. NO INJURIES HAVE OCCURRED. NO FIRE HAS OCCURRED. HOWEVER, THE VEHICLE WAS OPERATED WITH CHILDREN ONBOARD AFTER THE DEFECT BEGAN MANIFESTING AND BEFORE THE DEALER PROPERLY DIAGNOSED IT. COMPONENT/SYSTEM: High-voltage battery system (traction battery). The vehicle is subject to a manufacturer recall for risk of internal short circuit and potential thermal event. The vehicle and battery are available for inspection upon request. SAFETY RISK: According to the manufacturer and dealer, the defect presents a risk of battery fire, including while the vehicle is parked and not in operat

These precautions materially limit driving range and require the vehicle to be parked away from the residence, which increases risk exposure to occupants, visitors, and surrounding property. REPRODUCTION/CONFIRMATION: The defect has been confirmed by the manufacturer through a formal recall applicable to this vehicle. The dealer has acknowledged the recall and the associated safety instructions. INSPECTIONS: There has been no individualized inspection specific to this vehicle because the issue is a known, manufacturer-identified defect affecting recalled vehicles of this model. The condition and risk have been acknowledged by both the manufacturer and dealer. WARNINGS/SYMPTOMS: There were no prior warning lights, messages, or observable symptoms before receiving notice of the recall. The safety issue was not detectable to the driver absent the manufacturer’s notification. REPAIR STATUS/TIMING: The manufacturer has advised that replacement batteries are not expected to be available for several months (estimated beginning December 2026 or later). March of 2026, my car started to shake and eventually stall out again and this prompted me to check my oil levels where there was little to no oil. An oil consumption test was performed and it was noted my car is consuming 0.5 quarts of oil per 1,000 miles. Per Hyundai, this does not meet the oil consumption warranty (1 quart per 1,000 miles) and there is nothing to do for this issue at this time. My concerns are that this issue arises prior to my next recommended oil change. Also, there are no warning signs that prompt me that my oil is actually low; no dashboard warnings or notifications. I really only know this problem is occurring when my vehicle physically shows signs while I am driving. System/Component Involved: Fuel System / Fuel Injectors / High-Pressure Fuel Joint Pipe Description of Incident and Safety Risk: My 2017 Honda CR-V (1.5T engine) experienced a fuel system failure evidenced by DTC P0172 and a failed AF cylinder test, confirmed by an authorized Honda dealer. This failure leads to engine hesitation and power loss, creating a safety hazard during operation. More cri
... tically, during the repair (Case #16244235), the dealer and manufacturer refused to replace the High-Pressure Fuel Joint Pipe. According to Honda’s official Service Manual and TSBs for the Earth Dreams 1.5T engine, this pipe is a mandatory, one-time-use component that must be replaced whenever fuel injectors are serviced. Reusing this component violates Honda’s own safety protocols and poses a severe fire risk due to potential fuel leakage under high pressure ($20\text{ MPa}$+). Safety Risk to Others: Reinstalling a non-reusable high-pressure fuel line creates a high probability of fuel spraying onto hot engine components, leading to an under-hood fire while driving. This endangers the occupants and others on the road. Reproduction and Inspection: The injector failure was confirmed by [Insert Dealer Name] in Houston, TX. The subsequent safety violation (failure to replace the mandatory fuel joint pipe) is documented on my repair invoice dated April 2026. The vehicle and the old part (if still on the car) are available for inspection. Symptoms and Warning Lamps: The Check Engine Light (DTC P0172) appeared shortly before the failure, accompanied by a strong gasoline odor and engine stalling symptoms.Read more

The contact owns a 2014 Ram 1500. The contact stated that the ABS, BRAKE, and an unknown warning light were illuminated. The contact stated that approximately one year later, while driving approximately 5 MPH in the rain, the steering wheel jerked left and right continuously, and the brake pedal was depressed and the vehicle stopped facing on-coming traffic. The vehicle was driven to the residence
... . The vehicle was not diagnosed or repaired. The contact stated that the vehicle was then taken to a dealer for an oil change and spark plugs replacement; however, after driving away, the check engine warning light illuminated. The vehicle was taken back to the same dealer where it was diagnosed and determined that the ABS module needed to be replaced. The vehicle was not repaired because the parts were on back order. The manufacturer was notified of the failure. The failure mileage was 68,000.Read more

Repeated concerns involving the reliable response to pressing the gas pedal. Directly after purchase notified dealership the vehicle felt unsafe to drive. That pressing the gas pedal did not guarantee the vehicle would predictably move forward. Response ranges from high rpm with no forward propulsion, delayed response with eventual slow or regular movement, high rpm with no forward movement when t
... aking tight turn, feeling of incorrect gear. Most important when passing and accelerating or moving into traffic from stop you do not know how the vehicle will respond. Two Volkswagen dealers have inspected and state there is nothing wrong. Vehicle has recently had vacuum seals replaced which gave a temporary improvement of response feel but has degraded and left vehicle with a rough idle at stopping. Dealership has replaced based on check engine light: Faulty Breather Valve EVAP Solenoid Vavle Upper Timing Cover Safety is put at risk when car presents an unpredictable response in entering traffic or initiating a maneuver requiring acceleration. Check engine light has appeared only on the most recent repair.Read more

Unknown. I was driving and my jeep started making g a very loud rattling sound it kept getting louder. Eventually it made a screeching sound and the dash board lit up and it stopped and the engine began to smoke really bad. The engine light came on but I tried my best to make it home. It was towed to the dealership and they said that the engine needed to be replaced because there was a hole in it.

I am reporting two separate stall/loss of propulsion events on my 2022 GMC Sierra 1500 Denali Limited (VIN: [XXX] ), which is subject to NHTSA Recall #25V274000 and GM Special Coverage XXX (L87 Engine Loss of Propulsion). INCIDENT 1 — April 16, 2026: While decelerating off a highway exit ramp, the vehicle stalled without warning. The dashboard prompted me to shift to neutral and restart. I perf
... ormed a rolling restart successfully and continued driving. This stall occurred while decelerating on a highway exit ramp with vehicles traveling at highway speeds behind me. The sudden and unexpected loss of propulsion created a significant rear-end collision risk. INCIDENT 2 — April 17, 2026 (next day): While traveling at approximately 40 MPH and climbing a hill under load, the vehicle stalled a second time. The dashboard again prompted a neutral restart. This time the vehicle would not restart. All I heard was a single click from the starter, followed by a complete dash reboot. The vehicle lost all power repeatedly upon restart attempts. I was stranded on a narrow, high-speed rural road in Standish, Maine during rush hour, with vehicles passing at speed. I was in serious danger. The vehicle was towed to Weirs GMC in Arundel, Maine (the selling dealer), where it currently remains for diagnosis under RO# XXX. Both stall concerns are documented as Warranty Pay. Additional concern: Since the day after purchase (March 28, 2026), I have noticed a pronounced cold-start tick originating from the bottom end of the engine, confirmed by an independent mechanic as coming from inside the engine. This vehicle passed a Recall #25V274000 inspection on December 28, 2025 at 98,233 miles. The tick and stall events began after that inspection. I am requesting this be formally documented as a safety complaint tied to Recall #25V274000 and Special Coverage XXX.
