First purchased this car from a dealership in 2023, used at128,051 miles. Did not have any problems with it in the first year I would say. Then suddenly while driving down the road the epc light would come on limiting my speed to 4,000 rpm. It would come on and off also causing my check engine light to come on and off. It has now become more frequent, the epc and check engine lights will come on a

On April 10th, while traveling approximately 7 hours from home on a very busy highway in [XXX] , my 2023 Chevrolet Tahoe (6.2L engine) suffered a complete and catastrophic engine failure, requiring a full engine replacement. The vehicle had to be towed and I was left stranded with no transportation. Prior to this failure, I had brought the vehicle to an authorized GM dealership for a recall inspe
... ction specifically related to the 6.2L engine on September 9, 2025 with 36,162 miles. The dealership technician inspected the vehicle and informed me that my engine was not defective and cleared it. Shortly after that inspection, the engine suffered a total failure at 46,6xx miles on the vehicle. I am currently leasing this vehicle at approximately $1,100 per month and have been without the vehicle for an extended period while it sits awaiting repair. GM customer care has been contracted and has indicated they cannot move forward with a buyback claim because the BBB is not taking my case because the vehicle is older then 2 years old which I know is a false legal claim. This is a serious safety concern, a catastrophic engine failure at highway speed poses significant risk to vehicle occupants and others on the road which appears consistent with other reported failures. I am aware that NHTSA has opened Recall Query RQ26001 investigating the adequacy of GM's remedy under recall 25V-274. My experience is consistent with the 36+ complaints that triggered this investigation, engine failure occurring after an authorized dealer performed the recall inspection and cleared the vehicle. I am requesting this complaint be added to the RQ26001 investigation file. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The failures of my engine should be considered in the NHTSA recall number 23V-751, Honda recall number XG1, GG0. In addition, my claim should be considered as part of NHTSA investigation PE25008 which I understand is a current investigation into many reports of connecting rod bearing failures in the 3.5L V6 engine used in numerous Honda vehicles, including the 2016 Honda Pilot. I experienced repeated stalling, loss of drive power while on the highway, limp mode, and all dashboard warning lights over several years. Based on dealership misdiagnoses, I paid out of pocket for multiple repairs including spark plugs and ignition coils which they claimed were the reason for my car acting up, that did not resolve the issue. Among other repairs I did on my own like replace the battery twice and what not. I have since received Honda’s official recall notice confirming that a faulty fuel pump causes the exact symptoms I reported. My vehicle is now completely inoperable it does not start and cannot be driven or transported. I have filed two claims with Honda Corporate, both denied based on mileage. Issue Description: The vehicle developed a check engine light and was diagnosed with a P0302 cylinder #2 misfire. The condition persisted after basic troubleshooting, and the vehicle was taken out of service to prevent further damage. The vehicle was subsequently brought to an authorized Lincoln dealership for diagnostic evaluation. The dealership confirmed that the vehicle requires a full engin
... e replacement due to internal engine failure. The estimated cost of repair is approximately $9,500–$10,000. This failure occurred at approximately 64,000 miles, which is significantly earlier than expected for a major engine failure. Safety Concern: The engine misfire and internal failure can lead to loss of power while driving, creating a potential safety hazard, particularly at highway speeds or during acceleration. Additional Information: Manufacturer technical service bulletins describe coolant intrusion issues in 2.0L EcoBoost engines that can lead to misfires and engine damage. The symptoms experienced in this vehicle are consistent with those described conditions. The manufacturer was contacted for assistance but declined to provide support. Summary: Premature engine failure at low mileage requiring full engine replacement, with symptoms consistent with documented issues affecting this engine type. My 2018 Honda Odyssey began exhibiting intermittent acceleration lag several years before the repair, and the condition progressively worsened. At the time of repair, the vehicle had 68,957 miles. The safety concern occurs when accelerating from a stop, such as at a red light, stop sign, or when merging into traffic. I depress the accelerator normally and the vehicle does not respond. There is a d
... elay where the engine and transmission fail to engage, leaving the vehicle stationary while surrounding traffic begins to move. To compensate, I must press the accelerator further. When the vehicle finally responds, it does so abruptly, causing a sudden surge of acceleration. This creates two safety hazards: (1) loss of control due to the unexpected “kick” when power finally engages, and (2) increased risk of collision when attempting to merge or cross traffic, because the vehicle does not accelerate when expected. I have been honked at by other drivers and have experienced near‑miss situations because the vehicle failed to move when it should have. Owner forums show many similar reports from other ~2018 Odyssey owners describing the same acceleration lag and abrupt engagement. Based on these reports, the commonly recommended fix is a throttle body cleaning and a transmission “teach‑in” procedure. An authorized Honda dealership performed these procedures on my vehicle at my expense (approximately $500–$550). These procedures are not listed in Honda’s factory maintenance schedule, yet appear necessary to correct a drivability issue that presents a safety risk. Given the frequency of similar owner reports and the safety implications of delayed or unpredictable acceleration, this issue may indicate a design or calibration defect in the throttle body, engine air‑management system, or transmission control logic.Read more

I am writing to formally report a recurring and serious mechanical issue involving the head gasket in the 2018 Honda Accord equipped with the 1.5L turbocharged engine. This issue appears to be widespread and poses both safety and financial concerns for vehicle owners. In my experience, the vehicle began exhibiting symptoms consistent with head gasket failure at relatively low mileage. These sympt
... oms included unexplained coolant loss, engine overheating, white exhaust smoke, rough engine performance, and in some cases misfires. Upon inspection and diagnosis, the root cause was identified as a failed or failing head gasket. This failure is particularly concerning because it occurs prematurely and without warning, despite regular maintenance and responsible vehicle use. A head gasket is a critical engine component, and such failures can lead to severe engine damage, sudden loss of power, and unsafe driving conditions—especially if overheating occurs while the vehicle is in motion. Additionally, the cost of repair is substantial, often ranging from several thousand dollars, placing an unexpected financial burden on owners. Many drivers, including myself, rely on these vehicles for daily transportation, and this defect undermines the reliability that is expected from a modern vehicle. There is growing evidence through consumer reports, online forums, and mechanic feedback that this is not an isolated incident but rather a pattern affecting multiple 2018 Honda Accord 1.5L vehicles. The consistency of these reports suggests a potential design or manufacturing defect related to the engine or head gasket system. Given the safety implications and frequency of occurrence, I respectfully request that the NHTSA investigate this issue further to determine whether a recall or manufacturer accountability action is warranted. Addressing this defect would help prevent further financial hardship and reduce the risk of unsafe driving conditions for affected vehicle owners.Read more

I am reporting a pattern of systemic failures on a 2012 Hyundai Sonata Hybrid with approximately 131,000 miles. This is my second NHTSA complaint regarding this vehicle and manufacturer. Primary Safety Concern — Engine Wiring Harness: During a recent engine replacement at an authorized Hyundai dealership, technicians identified damage to the engine wiring harness and documented in writing that t
... he owner “might have issues with drive ability.” The dealership declined to repair it under the same goodwill program used to replace the engine and returned the vehicle with this known defect unresolved. The harness directly controls engine operation, hybrid system communication, and overall vehicle control. On a hybrid vehicle, a compromised harness presents a serious and ongoing safety risk. This component is available for inspection upon request. Engine Failure: The hybrid system warning light appeared prior to failure. The vehicle was towed to an authorized dealership where diagnostic code P1326 confirmed engine failure. The manufacturer acknowledged this and replaced the engine under goodwill. Additional System Failures: A pre-scan diagnostic revealed simultaneous fault codes across six systems — engine, transmission, ABS, active hydraulic booster, battery management, and motor control unit — including codes U0293 and U1004 indicating lost communication with the hybrid powertrain control module across multiple systems simultaneously. Independent Safety Advisory: A separate independent repair facility documented in writing that the vehicle presented a severe wheel hub bearing noise and formally advised of an extremely dangerous driving condition. Prior NHTSA Complaint: My first complaint to NHTSA involved this same vehicle when the manufacturer refused to provide a loaner during an extended recall-related repair hold, requiring formal escalation before they complied. The wiring harness defect remains unrepaired. The vehicle is driven daily out of necessity.Read more

I am reporting a safety defect involving a GM 6.2L L87 engine. My vehicle was included in the GM recall related to engine failure (loss of propulsion). The recall remedy was performed, and the engine was replaced under recall/warranty in August 2025. After the recall repair and engine replacement, the vehicle experienced a second catastrophic engine failure, on April 21st. The engine failed agai

The vehicle has now been out of service for an extended period (approximately 2.5 months total across both events - and ongoing), and I am continuing to make payments on a vehicle that is not operable. This appears to be a failure of the recall remedy itself, not just the original defect. I am concerned this represents a broader safety issue involving post-recall failures of the L87 engine, as currently under investigation. Grinding noise on startup. Goes away after warmup. Ford Dealer just reported that I need a $6,500 engine repair. I’ve owned this truck since brand new in 2019. Always have done routine maintenance. This clearly is a quality issue.

I own a 2021 Ford Bronco Sport Base (VIN: [XXX] ) equipped with the 1.5L EcoBoost 3-cylinder engine. I am experiencing a complete and repeated loss of engine coolant with no visible external leak. Upon inspection, air bubbles were observed rising through the coolant reservoir when coolant was added, indicating combustion gases are entering the cooling system through a compromised seal. On multiple
... occasions upon startup, sweet-smelling white smoke has been emitted from the exhaust, consistent with coolant burning in the combustion chamber. The vehicle has overheated as a result of coolant loss. These symptoms are consistent with the known 1.5L EcoBoost cracked engine block or head gasket failure defect affecting 2021 Ford Bronco Sport and 2020-2022 Ford Escape vehicles. I have been advised by a ford dealership mechanic that the vehicle is unsafe to drive due to the risk of engine failure and potential engine fire. The vehicle has been parked and is not being driven. I am filing this complaint as a safety concern given Ford’s history of engine fires related to this defect, which was the basis of recall 22S43 and ongoing federal class action litigation. I am requesting NHTSA investigate whether an expanded recall or additional consumer remedy is warranted for this vehicle. Since moving here to Nevada in 2023, I’ve had several oil changes. After about 1200 I have to add mother oil to the car. I’ve gone back three times to 5 min oil change to get free refills since they offer free top offs after your initial oil change. I than took a deep dive on the internet and discovered that the Volvo XC90, 2016 had issues with oil consumption, but never informed me of this i
... ssue. I’ve been to the dealership several times and not once did the inform me either. I mentioned to Bailey Anderson at Findlay Volvo in Las Vegas that this car consumes a lot of oil. He said no it didn’t without even checking for facts. I read that Volvo knew about this issue and we’re fixing the issue, however when I inquired they wanted me to pay for their mistake. How is that so when they knew there is and was an oil consumption matter and did not inform me of the matter. I’m furious with Volvo for hiding or concealing this issue. How can I get them to correct the matter, at this point?Read more

While driving, the truck suddenly Boggs down and gets sluggish. A message appeared once that said reduced acceleration drive with care. As I was driving down the tollway in Dallas going 75 miles an hour, my engine completely shut down and went into neutral in rush hour traffic. I had to slowly get over from being in the left lane as my car was slowing and slowing. I was able to get over on the side of the tollway as cars zoomed past me. By the grace of God, I didn't get hit. Once it was towed, the service department confirmed
... that it is my engine that shut down and the entire engine needs to be replaced. Now the problem is they cannot find an engine or parts to fix it. No warning came on at all before this happened. Very dangerous situation.Read more
