Based on my experience and additional research, the issue appears to originate from the engine’s oil pump design. Specifically, the oil pump contains an internal pressure-regulating component originally manufactured from plastic. This component is prone to fracturing, particularly during cold starts or low ambient temperatures. When this failure occurs, oil pressure becomes erratic or is lost entirely, often without sufficient warning to prevent engine damage or vehicle shutdown. My vehicle had this occur on January 20th 2026, as it was 5 Degrees outside. I suddenly lost oil pressure and had my vehicle’s engine catastrophically fail on a highway with my two small children ages (3) and (7 months) in the vehicle. In some cases, drivers may observe warning signs such as inability to measure oil level electronically, abnormal oil pressure readings, or drivetrain malfunction warnings. However, these indicators are inconsistent and often appear only moments before complete failure, leaving insufficient time for corrective action. This issue represents a significant safety concern for the following reasons: * Sudden loss of oil pressure can result in immediate engine shutdown or loss of propulsion while driving * Failures frequently occur under predictable conditions (cold starts in low temperatures), indicating a repeatable defect * Drivers are not provided with adequate warning to safely respond before damage.Read more

Our 2018 Pacifica (3.6L Pentastar) has a blown head gasket at 160K miles. I didn’t realize this was a common problem until I researched youtube and Chrysler forums and found 100’ of conplaints and examples. Ford reported that this condition can cause the transmission to unexpectedly downshift and create a loss-of-control hazard. (NHTSA) My vehicle appears to use the same Ford-based engine, transmission, and control architecture family and is exhibiting the same dangerous fault pattern. Diagnostic scans show PCM code P0720 and P0722. Freeze-frame data captured the transmission commanded in 3rd gear with turbine shaft speed present at 1041 rpm, but output shaft speed at 0 rpm. This indicates loss of the output shaft speed signal while driving. The transmission tag reads CK4P-7000-FB. The likely fault area is the output shaft speed sensor circuit, internal transmission lead frame, internal harness, connector, or related transmission signal path. We contacted Mobility Ventures, but they have not meaningfully responded or helped resolve the defect. We also contacted a local Ford dealer, and they stated they do not know how to handle or repair this defect on this vehicle. I am not the first owner, so the required recall inspections and updates were never completed on this vehicle before I bought it. The sudden engine crack and loss of oil created a serious safety risk, especially because there was no warning before it happened and it almost left our family stranded two hours from home. This failure matches the symptoms described in Hyundai’s Theta II engine defect investigations. While at the mechanic shop, we were quoted $10,000 to get a new remanufactured engine put into this car. I certainly (and I know most families) don't have $10k sitting around waiting to be used for car repairs. I am requesting that NHTSA investigate whether this vehicle should be included in the affected group, and whether this engine failure is part of the broader safety defect already identified in many Hyundai engines from the same model years. It was fixed with new spark plugs and wires. It's cost me thousands of dollars for repairs each time. But that's not why I'm reporting this to NHTSA. The issue is safety. When the lifters fail, you get no warning. The truck rattles and rumbles. It becomes unstable on the road, and sometimes wobbles as if it's about to go out of control. The problem causes the loss of electronic stability control (ESC) and renders the electronic parking brake useless. Lifter failure can also cause the engine to stall while driving, especially at a stoplight. This happened to me. I had to press the gas pedal gently to keep the engine running at a stop light just to get off the road. On the highway, when the lifters fail, the truck slows to a point where it cannot keep up with traffic. That presents a dangerous situation. It's not safe. GM knows about it, but as far as I understand, the company has done nothing to fix the safety problem. Internet forums are full of comments about the problem, the safety issues, and the enormous cost of repairs. Surely your agency can compel GM to fix the lifter failure problem before someone suffers a major accident due to an uncontrollable vehicle. I'll never buy another vehicle with AFM. On December 13, 2025, we drove to Flagstaff, Arizona from Tucson, Arizona, where we live. As we came into town, the Bronco began to lurch and seemed to have difficulty shifting. The second time it died completely and required a special tow truck because we couldn't even get it into neutral to roll. It was towed to Holmes Tuttle Ford where they kept it for a week but were able to eventually restart it. They had it for 5 days, ran a diagnostic and reset the system. They drove it for over 80 miles and couldn’t recreate the problem. Then on February 25, I drove to Phoenix. While in Phoenix, it again inextricably had trouble shifting, died and wouldn’t start. I then had it towed to Camelback Ford. On March 6, Camelback Ford called and said they ran numerous tests and still can’t pinpoint the problem and the vehicle still won’t start or shift into neutral.Read more

... worker attempt a jump. The vehicle would not restart. I called Moses GMC of Huntington, WV, and they dispatched a tow through GMC Roadside Assistance. I waited approximately 2 hours on the shoulder for the tow. After the tow, Pete, the Moses GMC service manager, called and said nothing was wrong with the vehicle, that it only needed an oil change. He said it started right up and he drove it into the bay. Moses performed the oil change. When the technician test drove it afterward, the engine fully shut down on the hill leaving the dealership property. My husband Zach happened to drive past and saw the Yukon stopped with hazard lights on at the top of the exit. Pete called that evening and said, "I think there actually is something wrong with it." On April 10, 2026, Pete confirmed the failure is the GM 6.2L V8 L87 engine defect covered under NHTSA Recall 25V-274 (connecting rod/crankshaft manufacturing defect). Full engine replacement required, estimated 6-7 weeks. This vehicle was previously inspected by Moses GMC under Recall 25V-274 in August 2025 and cleared at that time, approximately 8 months before the April 7 shutdown. Vehicle had ~38,000 miles, garage kept, full maintenance records, no prior engine issues or warning signs. No crash, no injury, no other vehicles involved in any of the three failure events.Read more
