I always turn off the engine auto stop while driving and am sure this feature was turned off at the time of the incident. I was stopped at a drive thru idling in drive when my RPM sounded like it lowered and the the vehicle started shaking. Since it sounded like it was going to stall, I instinctively put the vehicle in park and that naturally bumped up the RPM avoiding a stall. I tried to rep
... licate it and could not in the 15 minutes I sat in idle afterwards. I would like to note that we did the software emissions recall update at the dealership recently and ever since then the vehicle is consistently idling 100-200 RPM lower than it used to, usually around 500 RPM in drive and 700 RPM in park. I'm grateful this has only happened at a drive thru so far but I'm afraid to stall in traffic as we drive in busy and dangerous areas and it could be deadly to stall in traffic. Since so far I cannot get this to replicate we have not set an appointment at the dealership as all the other complaints on this matter show the dealership is unable to replicate the problem. Please require KIA to fix this as it is incredibly scary to fear stalling in dangerous location due to traffic speeds and/or getting stuck in a crime riddled area. Thank you for your help.Read more

The starter on a new 2026 Chevrolet Equinox RS failed at approximately 6,500 miles. While the vehicle was being driven home during evening rush hour, it shut off at a red light and would not restart. The vehicle was stranded in the left lane of a four-lane highway. It could not be moved because the engine would not start, and the vehicle would not even roll in neutral, as the wheels were locked an
... d would not turn. The disabled vehicle blocked a live travel lane in heavy traffic. The driver was a [XXX] new driver. Being stranded in a moving traffic lane during rush hour created a serious risk of a rear-end collision to the occupant and to surrounding vehicles forced to maneuver around the disabled car. Police responded because the vehicle was creating an active roadway hazard and arranged for a tow. The tow operator had to drag the immobilized vehicle onto the flatbed because the wheels would not turn. The failure was confirmed by an authorized Chevrolet dealer, which diagnosed a failed starter and replaced it under the manufacturer's warranty. There were no warning lamps, messages, or symptoms prior to the failure. The vehicle shut off and failed to restart without any advance indication. The vehicle has been repaired and is available for inspection. The failed starter was replaced by the dealer. Component Affected: ENGINE AND ENGINE COOLING Description of the Defect & Incident: My 2024 Nissan Rogue equipped with the 1.5L VC-Turbo engine is exhibiting severe progressive mechanical failure, matching the symptoms described in NHTSA Campaign Numbers 26V080000 (Engine Bearing Seizure) and 26V081000 (Throttle Body Gear Failure). The vehicle is currently experiencing excessive engine shaking, a
... violently rough idle, abnormal mechanical noise from the engine bay, and a dangerous drop in motive power/acceleration. History of the Issue & Dealer Denial: I originally identified and proactively reported an abnormal engine noise to an authorized Nissan dealership in May 2024. The service department issued a "could not duplicate" finding and turned me away. In June 2024, I provided a digital audio/video recording of the symptom at that time. Following the closure of the initial Nissan dealership, I transferred the care and servicing of my 2024 Nissan Rogue to Future Nissan of Roseville. I explicitly reported the recurring, abnormal, loud engine noise and excessive engine shaking with no remedy offered. Following the official February 2026 recall announcements, the symptoms have severely worsened, progressing to violent engine shaking. I returned to the dealership for the recall remedy. The dealership service department is actively denying recall coverage, claiming the current failure is an unrelated "secondary issue" rather than a consequence of the recalled components. Because Nissan's own NHTSA filings explicitly state that these defects are progressive and give advanced warning through "unusual noises or vibration/rough running" before complete engine failure, the dealership's refusal to honor the recall is a violation of federal safety standards. The alleged secondary damage is direct consequential damage from a primary safety defect that I documented within the initial warranty period. Nissan must take responsibility for this failure.Read more

COMPONENT FAILURE: The engine, power train, and speed control systems failed due to the known Nu 2.0L GDI connecting rod bearing defect. SAFETY RISK: My [XXX] daughter was driving in heavy rush-hour traffic when the vehicle suddenly lost motive power and entered a 20 MPH "limp mode." This sudden deceleration created an immediate and severe risk of a rear-end collision, endangering my daughter a
... nd surrounding motorists. DEALER CONFIRMATION: The failure was officially confirmed by Flow Kia of Charlottesville (RO #XXX). On Dec 3, 2025, the dealer confirmed active DTC P1326 (KSDS) and documented failed cylinder compression (as low as 150 PSI; min spec is 178 PSI). The dealer issued a written diagnosis stating: "engine replacement is required." MANUFACTURER CONDUCT: Despite their own dealer's written diagnosis and the active P1326 safety code, Kia Corporate issued a bad-faith warranty denial. The dealer also unlawfully charged me $165 for the mandatory KSDS warranty diagnostics. SUMMARY: This is a textbook safety failure under NHTSA Investigation EA 21-003. Kia's refusal to replace the engine under the court-supervised Engine Litigation II settlement represents a systemic failure by the manufacturer to remedy a known safety hazard. 51-page evidence packet attached. High voltage battery pack, Power Inverter Module (PIM), and HV charging system. PIM replaced twice (p/n 68381781AD, 8/4/25 and 8/18/25) after being found melted with arcing damage at HV connections. Battery pack has active BPCM fault codes and a recorded cell voltage excursion. Vehicle available for inspection. Vehicle lost all motive power without warning at highway speed on multiple occasions,

A melted inverter and arcing at HV connections were found on a vehicle still being driven daily. BPCM data shows Cell 79 recorded a voltage excursion to 1,526 mV — less than half minimum safe operating voltage — stored in permanent memory. Yes. Lawless Inc. (Woburn, MA), authorized Chrysler dealer, confirmed fault codes PA121-00 and P0A43-00 on 8/4/25 and replaced the PIM. Vehicle returned 8/18/25 with same failure — dealer confirmed arcing damage and melted inverter. Three visits total; defect unresolved after two repair attempts on the same component. All three visits performed by authorized Chrysler dealer under 10-year/100,000-mile HV battery warranty. Stellantis warranty claim opened each visit. No police or insurance involvement. I am writing to report a serious, premature failure of a primary emissions and safety-related component (the catalytic converter) that has occurred as a direct consequence of a known, unaddressed manufacturing defect in Hyundai's Theta II engine. My vehicle has a documented history of excessive engine oil consumption. This inherent manufacturing flaw causes the engine to burn motor oil at an extre
... me rate, forcing unburned oil directly into the exhaust system. Over time, this oil has fouled and destroyed the catalytic converter substrate, triggering trouble code P0420 and causing a complete component failure. A dealership inspection at Heritage Hyundai Towson (RO #XXX) confirmed "excessive wear" and failure of the catalytic converter. Because the catalytic converter is failing prematurely due to a liquid defect (excessive oil burning) rather than standard wear and tear, it poses an environmental hazard, compromises vehicle emissions control, and can lead to sudden engine stalling or reduced power while driving, presenting an active safety risk. Hyundai Motor America (Case #XXX) has formally denied assistance to rectify this consequential damage, despite full awareness of the underlying engine defect. I request that NHTSA investigate how Hyundai’s ongoing oil consumption manufacturing issues are causing cascading safety and emissions component failures in these vehicles. While driving my 2023 Nissan Rogue on Interstate xxx from [XXX] to [XXX] with my xxx daughter in the vehicle, the vehicle suddenly displayed multiple warnings including collision assist/sensor malfunction warnings, start/stop system failure warnings, and “check owner’s manual” messages. Immediately afterward, the vehicle experienced a sudden loss of power and refusal to accelerate. At the
... time of failure, I was traveling approximately 78 mph in the fast lane of the interstate. The vehicle rapidly lost speed and dropped into what appeared to be limp mode, eventually slowing into the upper 50 mph range despite pressing the accelerator. I had to fight across multiple lanes of freeway traffic while unable to maintain highway speed. This created an extremely dangerous situation due to surrounding high-speed traffic and limited safe shoulder access in a remote desert area. The vehicle became unsafe to continue driving and required a tow of over 70 miles to the nearest Nissan dealership. The tow expense alone exceeded $600. Prior to this incident, the vehicle had already experienced a separate unexplained no-power/no-start event. At that time, the battery, starter, and alternator reportedly tested normal. The vehicle remains within the manufacturer’s 5 year/60,000 mile powertrain warranty and currently has active recalls associated with the vehicle. A Nissan case has been opened and the vehicle is pending dealer diagnosis and inspection. After posting publicly about this incident in Nissan owner groups, I identified numerous other owners reporting nearly identical symptoms involving warning light cascades, sudden loss of acceleration, limp mode events, and highway safety concerns. I am filing this complaint regarding an active safety recall on my 2024 Toyota Tundra involving manufacturing debris left in the V35A 3.4L twin-turbocharged V6 engine during production. This recall has been open for multiple years and spans over 250,000 vehicles across the 2022–2024 Tundra, Lexus LX 600, and 2024 Lexus GX 550. The manufacturing debris causes damage to the engine’s main bearing
... s, resulting in engine knocking, rough running, failure to start, and sudden loss of motive power while driving at speed — a condition Toyota itself acknowledges increases the risk of a crash. Toyota deployed a remedy consisting of full engine replacement; however, Toyota has since acknowledged that the replacement engines are experiencing the same failures, meaning there is currently no confirmed, working fix. Despite this, Toyota continued selling new vehicles with this known defect. Remedy deadlines have been repeatedly promised and repeatedly missed, with the latest expected resolution pushed to July/August 2026 — a deadline that has already slipped multiple times. As an owner, I am left driving a vehicle that Toyota acknowledges is dangerous, with no confirmed timeline for a legitimate repair, and suffering significant financial harm due to the ongoing depreciation and diminished resale value caused directly by this unresolved defect.Read more

The engine powertrain system, specifically the internal engine lubrication, timing chain, and camshaft sensors. The vehicle's Theta II engine suffers from systemic, excessive oil consumption, which starved the engine of lubrication. This caused the timing chain to stretch and mechanically fail, and compromised the camshaft sensors. The vehicle is currently un-repaired and is available for inspec
... tion upon request at Hyundai of Downtown LA. While driving, the engine suddenly stalled and completely shut off mid-drive without warning. This caused an immediate loss of motive power and a dangerous reduction in steering and braking control while surrounded by active traffic. Operating a vehicle that randomly shuts down on public roads presents an extreme, life-threatening safety hazard to myself, my passengers, and other drivers. Furthermore, the dealership is attempting to force me to drive the vehicle for an additional 1,000 miles in this stalling, un-repaired condition to complete a corporate oil test, compounding the safety risk. The failure and safety hazard have been physically confirmed and diagnosed by an authorized dealership: Hyundai of Downtown LA (3850 S Figueroa St, Los Angeles, CA 90037). Their service department mechanically diagnosed the physical failure of the camshaft sensors, a stretched timing chain, and severe engine oil starvation. There were absolutely no dashboard warning lights, check engine lights, low oil pressure lamps, or messages prior to the engine stalling and shutting off. The vehicle computer failed to trigger any diagnostic trouble codes (DTCs) or dashboard warnings to alert me to the low oil level or the impending mechanical timing failure before the vehicle completely shut down mid-drive.Read more

I bought my 2018 Acura TLX a-spec V6 in Nov. 2021, certified pre-owned from an Acura dealer. It had warranty through 100k miles. At just under 70k miles, around mid-2023 I believe, the engine seized and would not start. Warranty covered engine replacement. This was just around the time of the 3.6L V6 engine recall. ... later, deal did inspect the engine for recall, but determined it was not part of the recall. Come current to late April 2026, just before engine #2 hit 70k miles (total miles on car ~139,000), engine has seized again. Driving on freeway and car went into limp mode, thankfully close to home and made it safely. Had car towed next day to mechanic who sent a video of the knocking noise (can send video if requested; unable to upload here) and refused to work on it. He later drove the car down the block and that is when engine completely failed; no longer would turn on. Towed home, towed to dealer a week ½ later when they had opening. Dealer performed diagnostic and confirmed crankshaft seized. However, now, well past warranty coverage. I called Acura customer relations and opened a case; they will only cover "half" of the engine replacement, at a cost of $13,817 to me. I own a 2018 Honda Accord with approximately 108,000 miles. My vehicle has experienced ongoing fuel system and drivability issues connected to the fuel pump recall. It took over two years for Honda to complete the recall repair due to delays and lack of available parts. During that time, my vehicle developed additional issues including failed fuel injectors that required replacement. Even after t
... hose repairs, the vehicle continues going into limp mode, causing loss of power and creating a serious safety concern while driving. When I brought the vehicle to Honda for diagnosis, I was charged approximately $180 just to scan the vehicle and then quoted nearly $4,000 in additional repairs. In addition, my vehicle experiences what many Honda owners refer to as the “Christmas lights” issue, where multiple warning lights suddenly illuminate across the dashboard at the same time. This appears to be a widespread complaint among owners of this model and is often associated with ongoing fuel system, electrical, or sensor-related problems. Honda vehicles are marketed as reliable, yet many owners of this model have reported similar problems related to the fuel system, injectors, limp mode, and dashboard warning light failures after the recall issues. I believe the prolonged delay in addressing the recall contributed to further damage to my vehicle. Honda should take responsibility for these known issues and provide assistance to affected customers.Read more

An antifreeze leak is very hard to locate because Ford’s design placed the water pump INside the engine rather than on the OUTside, which would be more the norm. If antifreeze leaks enough, it gets into the oil and the entire engine stands to be destroyed. Our heater quit blowing warm air recently and after intently searching we discovered that we had an antifreeze leak inside of our engine, an
... d antifreeze was slowly leaking out through a weephole in the engine. The water pump had gone bad and needed replacing. The hours required to replace this water pump are intense and they are long. Almost the entire engine needs to be dismantled in order to perform the work. Not only does the pump need changed, but so does the timing chain, hoses, seals, gaskets, tensioners and other engine parts, all at the same time, making this a VERY costly out-of-pocket expense for us elderly people who have long relied on Ford’s standard of doing what’s right by its people. In our research we found that the water pumps in Flex engines are failing, regularly, once the vehicle is close to having 80,000 miles on it. This repair most DEFINITELY should be covered by recall since it is happening so regularly. We have just been forced to pay for this repair on our own and I would like to please ask for your help in recovering what we’ve had to spend. Will you please provide contact information as to where we can reach out for assistance in this matter? Your help is VERY much appreciated! Thank you!Read more

While driving my 2025 Chevrolet Equinox, the vehicle experienced a sudden power and drivability issue. The vehicle began to decelerate on its own while in motion, even though I was actively driving. A warning notification appeared on the driver dashboard, and shortly after, the vehicle power shut off. This creates a serious safety concern because the vehicle can unexpectedly lose speed and/or pow
... er while driving in traffic. The issue appears to involve the transmission, powertrain, or vehicle electrical/power management system. The loss of acceleration and sudden deceleration could increase the risk of a crash, especially at highway speeds or in heavy traffic. This incident occurred while the vehicle was being driven, not while parked. The warning message appeared on the dash before or during the power loss. I am reporting this because the vehicle’s sudden deceleration and power shutdown while driving could be a safety defect. I am requesting that NHTSA review whether this issue is related to a known defect, recall, transmission issue, powertrain control issue, or electrical system failure affecting the 2025 Chevrolet Equinox.Read more

While driving, the vehicle suddenly began running rough, losing power, and displaying engine warning lights. Due to the severity of the issue, the vehicle was towed to a GMC dealership for diagnosis. The dealership diagnosed lifter failure and a cracked cylinder head. The dealership also documented coolant intrusion, oil/coolant contamination, and evidence of internal engine damage. Repairs were
... attempted, including replacement of the cylinder head and lifter. After the repairs were completed, the vehicle never left the dealership because it continued misfiring on the same cylinder. GM later determined the engine had a failed piston ring and scored engine block requiring complete engine replacement. The vehicle has approximately 55,000 miles, has been properly maintained, and was purchased new. This catastrophic engine failure resulted in loss of power and major internal engine damage at relatively low mileage. I also opened a case directly with GM Customer Assistance regarding this failure. The case involved multiple escalations after the initial repair attempt failed and the vehicle ultimately required full engine replacement. GM ultimately advised that only partial assistance toward the engine replacement would be provided, leaving the customer responsible for a significant portion of the repair cost. I am submitting this complaint because this vehicle appears consistent with the ongoing NHTSA investigation involving GM 6.2L L87 engine failures and loss of motive power.Read more

For powertrain: failure of right front CV axle before 100k miles on a certified pre-owned vehicle; associated failure of shocks and struts in right front axle. The problem was inspected by an ASE certified VW mechanic, attested to be true and shown to us, and repaired. There were no warning lamps/lights, and the finding was made on a full vehicle inspection when brought in for a brake pad change.
... The issue appeared at approx 79000 mi on the odometer, 3 months after purchasing the vehicle. Obviously failure of the CV joints and components provide a risk to drivers and other motorists with the potential for injury due to steering and suspension failure during operation. Engine: the water pump was discovered to have begun to fail at approx 89000mi during a routine oil change service in May 2026. The mechanic states the issue is not noticeable without removal of the brush guard from under the engine compartment, and therefore may be missed by owners. A warning for “low coolant” is only shown once the level has dropped sufficiently. There is not currently a recall open for this part, confirmed with VW service center at a dealership May 27, 2026. This is harmful physically and financially for owners, passengers, and motorists as it can lead to catastrophic engine failure.Read more

While driving the car it went in to limp mode, restricting the ability to maintain normal driving. It did this because the head gasket needs to replaced because coolant is leaking into # 3 cylinder. My wife and others were put at risk when she had to pull over in an area that was unsafe according to a Summerville police officer who came to her aid and helped her get the car to a safe area.
