I am reporting a serious safety issue involving a 2017 Hyundai Sonata Plug-in Hybrid (VIN: [XXX] ), originally purchased in California. While driving at approximately 70 mph on a highway, the vehicle suddenly lost all propulsion without warning and rapidly decelerated. This created a dangerous situation with surrounding traffic and put me and my family at risk. I had to quickly maneuver to the sh
... oulder to avoid a potential collision. The vehicle was towed to an authorized Hyundai dealership in Utah, where it was diagnosed with a failure of the high-voltage battery system. I was informed that the battery requires replacement. This type of sudden loss of propulsion at highway speed is extremely hazardous and could result in a crash, injury, or fatality. There were no prior warnings indicating an imminent failure of this severity. Given the safety risk associated with unexpected loss of propulsion, I am concerned this may represent a broader issue affecting similar vehicles. The vehicle is equipped with an interior camera, and video of the incident can be provided if needed. I respectfully request that this issue be investigated as a potential safety defect. The vehicle involved is a 2017 Dodge Durango. The primary systems malfunctioning include the engine cooling system, braking system, and electrical system. The engine repeatedly overheats while driving and has produced smoke from under the hood, creating a risk of breakdown or fire. The braking system is unreliable, with the brake pedal going down too far and not providing proper stopping power, i
... ncreasing the risk of a crash. The electrical system malfunctions unpredictably, including windshield wipers and interior controls turning on and off without input. These issues have occurred multiple times and have left the vehicle stranded on the side of the road, including at night. This has placed the driver and passengers at risk in unsafe conditions and traffic environments. The problems began shortly after purchase and have been ongoing. The issues have not been properly resolved. A mechanic was supposed to inspect the vehicle, but it is UNKNOWN if a full inspection was completed. Warning signs included overheating temperature readings, smoke, weak braking response, and electrical irregularities prior to failure. The vehicle is currently not reliable or safe to operate due to these ongoing safety defects.Read more

Dear NHTSA Safety Team, I am submitting a formal safety-related complaint regarding my 2023 Mercedes-Benz EQB 250, which is currently under an active high-voltage battery recall. The recall imposes significant safety restrictions, including an 80% charging limit, a requirement to park the vehicle outside due to fire risk, and the inability to safely charge the vehicle at home. Additionally, I have
... been advised by my dealership that DC fast charging should not be used, further limiting the vehicle’s usability. Mercedes-Benz has informed me that the remedy requires a full high-voltage battery replacement, which may take many months (until at least December). In the meantime, the vehicle is not usable as a daily vehicle due to the combined safety and charging restrictions. I have attempted to work with Mercedes-Benz USA to obtain an appropriate remedy for this loss of use, but I have been told that only minimal options are available, and my request for executive-level review has been denied. I am requesting NHTSA’s assistance in reviewing this matter as a safety-related loss-of-use issue. The current restrictions materially affect my ability to safely operate and maintain the vehicle. Thank you for your attention to this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am submitting this complaint on behalf of the vehicle owner. On March 31, 2025 (136,962 miles), the vehicle was brought to Fred Anderson Nissan of [XXX] for Voluntary Emission Recall PD128 (NTB24-061), which required a new Particulate Matter Sensor and ECM reprogramming. One day later, the Check Engine Light illuminated. On April 2, 2025 (137,083 miles), the dealer diagnosed code P2002-00 (DPF
... Efficiency Below Specs). Repair Order #XXX states: "The new sensor that is now monitoring DPF is able to see this and is setting the code as designed." The dealer's own documentation confirms the fault code was triggered by the recall — not a pre-existing failure. The vehicle had no warning lights before the recall was performed. Nissan Consumer Affairs (Case #XXX) declined goodwill assistance citing mileage, without addressing the documented causal link between the recall and the fault code. The owner now faces a DPF replacement costing several thousand dollars as a direct result of complying with a manufacturer recall. Safety concern: A DPF in confirmed fault condition can trigger unexpected limp mode, including at highway speeds. If other PD128 recipients are experiencing the same recall-induced fault and cannot absorb the repair cost, those vehicles may be operating in a compromised condition. We request NHTSA investigate whether Recall PD128 is creating safety and repair liability for consumers who complied in good faith. I am reporting a serious safety defect involving my 2013 Hyundai Santa Fe Sport equipped with a Theta II engine While driving and exiting the freeway with my wife as a passenger, the engine suddenly failed after knocking symptoms and completely seized/locked while the vehicle was still in motion. The vehicle immediately lost motive power and came to a stop, creating an immediate and dangerous tra
... ffic safety hazard. Had this engine seizure occurred moments earlier while traveling at freeway speed, it could have caused a catastrophic accident involving serious injury or death. The Hyundai dealership diagnosed the vehicle with rod bearing failure, which is the known defect associated with Hyundai Theta II engine failures. My VIN falls under the Theta II extended engine warranty program, and the dealership confirmed eligibility and accepted the vehicle for inspection under that coverage. Despite this, Hyundai denied the repair claim, first alleging maintenance-related issues and later stating the vehicle was out of warranty. I maintain that the vehicle was properly maintained with oil changes performed approximately every 6,000/8,000 miles. Some calendar-time gaps occurred because the vehicle was stored and driven very little during my recovery from a personal injury surgery and recovery, but the vehicle was maintained on mileage intervals. This engine defect resulted in sudden engine seizure and total loss of motive power while driving, creating a serious safety risk to vehicle occupants and surrounding motorists. Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

Incident date: Apr 14, 2026
My vehicle recently idled roughly and shut off at low speeds and while stopped at intersections on my way to work. I brought it to an authorized Kia dealership the very next day. After researching my VIN, I discovered Kia Safety Recall #SC336 (NHTSA Campaign 25V099), which identifies a piston oil ring defect known to cause excessive oil consumption and internal engine damage. I promptly brought my
... vehicle to the dealership specifically to address this recall. However, the dealership did not complete recall-related repairs. Instead, they determined—without sufficient justification—that my vehicle’s issues were not related to the recall, but to a "crush washer". My vehicle exhibits the exact symptoms outlined in the recall, including excessive oil consumption, rough engine operation, and stalling. The dealership then attempted to attribute the oil consumption to a crush washer on the oil drain plug, despite there being no visible signs of oil leakage under my vehicle at any time. This explanation does not reasonably account for the level of oil loss I have experienced or the internal engine condition. According to official recall documentation filed with the NHTSA, the defect involves improperly manufactured piston oil rings that can damage cylinder walls, leading to excessive oil consumption, abnormal engine noise, rough operation, and ultimately engine failure or stalling. The recall further warns that continued operation may result in engine seizure and loss of drive power. Independent automotive reports confirms that this defect has affected over 137,000 vehicles, including 2021 Kia Seltos models, and in many cases requires full engine replacement due to internal damage caused by faulty piston rings. Additionally, documented consumer complaints describe the same pattern I have experienced—excessive oil consumption with no external leaks, worsening engine performance, and dealerships attributing the issue elsewhere despite clear alignment with the recall defect.Read more

While operating my vehicle under normal driving conditions, I experienced a sudden and complete loss of engine power without warning. The engine shut off abruptly in active traffic, creating an immediate safety hazard and increasing the risk of a collision for myself and surrounding motorists. Following the stall, the vehicle would not restart. The engine cranks but does not start (a “dry crank
... ” condition). There were no prior warnings, indicators, or drivability issues suggesting an impending failure. This vehicle is equipped with the L87 6.2L V8 engine, which is subject to a known recall involving internal engine defects that may result in sudden failure. The symptoms and failure mode observed are consistent with reported cases of internal engine damage, including potential bearing or rotating assembly failure. At the direction of GM and the dealership, the vehicle was previously brought in for recall inspection. The recall was subsequently marked as completed. However, I am now being advised that the recall cannot be reopened and that I am responsible for the repair costs. A completed recall inspection does not eliminate manufacturer responsibility when the underlying defect later results in failure. I understand there is an ongoing and recently reopened NHTSA investigation into GM’s “Pico test” inspection process, as multiple engines have reportedly failed after passing this procedure. Given that this failure occurred after the recall inspection was completed, this incident raises concerns regarding the adequacy and effectiveness of the prescribed remedy. This event represents a serious safety issue due to the sudden loss of power in traffic without warning. I am submitting this complaint to document the failure and to request further review of the recall remedy and associated inspection process.Read more

The overheating cat. converter presents a significant safety hazard, including potential engine seizure. I purchased the vehicle used in March 2025. I am the third owner. Despite maintaining the vehicle responsibly (Regular oil changes), the engine failure progressed rapidly. Hyundai Motor America has denied a goodwill repair solely on the basis of mileage, even though the defect clearly originated long before my ownership and aligns with previously identified safety-related engine issues. Additionally, Hyundai's records show that this vehicle had an engine-related recall active during prior ownership period; the recall was never performed by the Hyundai dealership that sold the vehicle to the previous owner. This raises concerns about improper recall handling and whether the defect should have been corrected before the vehicle was resold. Given the safety implications, the documented defect pattern, and the unresolved recall history, I am requesting NHTSA's review of this matter. The engine failure poses a serious safety risk; Hyundai's refusal to address a known defect, the vehicle is unsafe to operate. Timeline: 2014 Hatchett Hyundai West, KS 17,665 mi.; 2017-H. I am reporting a safety defect involving my 2023 Kia Carnival SX Prestige. On June 6, 2025, I brought the vehicle to Yonkers Kia Service Center because of a strong gasoline odor. The dealership told me the fuel tank was swollen and that the vehicle was unsafe to drive. The vehicle remained at the dealership from June 6, 2025 through October 17, 2025. Kia later indicated that the fuel tank, caniste
... r, and vent valve had been replaced. When the vehicle was returned to me on October 17, 2025, it still smelled like gas. Within 24–48 hours, the same or related safety issues returned, including strong fuel odor, loss of engine power, check engine light, abnormal fuel consumption, and later a visible fuel leak, which I documented on video. Because the vehicle did not feel safe, I stopped driving it on October 19, 2025. In March 2026, the vehicle was towed back to Kia for renewed diagnosis. Kia later reported that one fuel injector was leaking and performed additional repairs. My VIN now shows open unrepaired Recall SC368 / NHTSA Recall 26V232. That recall states that the high-pressure cross-over fuel pipe nuts may loosen over time, resulting in a fuel leak and fire risk. The recall warning signs include fuel smell and/or check engine light, which directly overlap with the exact symptoms I repeatedly experienced and reported. I am reporting that this vehicle had repeat fuel-related safety failures, a prior extended repair attempt that did not resolve the issue, and was previously returned to me in what I believe was an unsafe condition.Read more

Went to park car in a park to drop daughter off and it sputtered out and stalled out of nowhere with no warning. I had received a lamp warning very sporadically prior to this and on occasion that stated 'reduced steering assist drive with care' this resulted in heavy steering for that drive until car was shut off and then it would go away and be fine again, fixed the car through Knudtsen Chevy due
... to a recall about a month before on the issue with the engine being turned off and it thinking it was still hot and still running the fan (even though the car was not running). Knudtsen fixed that issue. Low battery voltage light did not come on however battery was older so had battery installed at park to ensure the issue was not due to the battery. The oil was fresh and showed no issues, changed 2 weeks before fresh oil no issues found during service. When tried to restart the car it would think it would try to start and then sputter out and not stay in idle. Les Schwab left and called a mobile mechanic to bring the computer. Computer was hooked up and it would try to start, however, it would not know it was already running and try to start again on it's own...the codes that came up were the PO101 code came up for Mass Air Flow issue...and then the codes would go in a wild sequence of fail.....mechanic checked for loose connections around air filter, took air filter out and it was clean no issues, checked pipe no visible issues. We walked away and walked around the park to give it some time to sit and then we tried it again and it tried and sputtered out and died again would not stay idling. Then all of a sudden we heard it try to start itself without us trying to start it and it clanked. We hauled it to Knudtsen for a diagnosis. They stated "the motor will not turn this way or that way and it is seized or locked up". This is a 2021 with only 89,000 miles it should have a longer life than this. Please investigate and is there anything we can do to fix it?Read more

The vehicle will randomly shut off while driving. A message will appear stating "Check the Starting System" Attempts to restart the engine are unsuccessful. The engine will start, stall after about 2 seconds and the message "Check Starting System" will display. The first time it happened, I was turning a corner at a rather dangerous intersection and the vehicle stalled. It took about three attempt
... s before the engine would remain running. When the engine would stall the dash would go dark and the engine would not remain running this happened about a month ago in hot weather. It occurred again on April 25th, 2026 way worse this time. The temperature in the area reached 91 Degrees Fahrenheit. My vehicle was sitting outside in the heat for at least 5 hours, same issue, The vehicle would then proceed to stall in the middle of the freeway, I was able to coast to the breakdown lane where the vehicle would remain unable to start and the message "Check Starter System" would display. Looking on NHTSA webpage before filing a complaint, it seems that this is a rather common issue. VW had a recall for other models (Campaign 28H7) for models such as the Jetta, Golf and other like models. This is a really dangerous issue and could end up getting someone seriously injured or worse killed in a collision if my vehicle stalls like this. The vehicle is scheduled for a service appointment in the near future (Local VW dealer is at least 2 weeks out before they can even look at my vehicle) After viewing this information, I can't help but believe it is possible the 28H7 recall is affecting more models than anticipated.Read more

The vehicle's auto idle stop system is malfunctioning, resulting in the engine failing to restart from a complete stop. This total failure has occurred on five separate occasions, including while stopped in active traffic intersections. Prior to these total restart failures, the vehicle showed the following symptoms: 1) Initially, the vehicle experienced rough engine idling, abnormal vibrations,
... and a raw gasoline odor after driving. 2) This progressed to a noticeable delay between releasing the brake pedal and the engine attempting to ignite, during which the starter sometimes sounded strained. 3) This has culminated in the current defect where the engine shuts off normally when stopped at an intersection, but completely fails to immediately restart when the brake is released. This defect completely immobilizes the vehicle in active traffic lanes, exposing us to an immediate and severe risk of a rear-end collision. When the engine fails to restart, the vehicle locks the transmission out of drive and triggers a warning on the dashboard. It is a highly hazardous situation, especially with a lot of distracted drivers on the roadways.Read more

Subject: Request for Buyback or Trade Assistance – 2020 Ford Escape SE To Whom It May Concern, I am writing regarding my 2020 Ford Escape equipped with the 1.5L engine, which is currently subject to multiple open safety recalls. Of particular concern is the active fuel injector recall involving a risk of engine compartment fire. I understand that the current remedy does not fully address the
... underlying defect and that additional actions may still be pending. In addition, my vehicle is affected by other safety-related recalls, compounding concerns about reliability and safe operation. Given the nature of these issues—specifically the fire risk and the lack of a complete and permanent repair— as well as a history of over 15 recalls on this vehicle, I am increasingly concerned about the safety, usability, and value of my vehicle. This situation has significantly impacted my confidence in operating the vehicle. Based on these circumstances, I am formally requesting that Ford Motor Company review my case for: • A manufacturer buyback, or • Substantial trade assistance toward a new Ford vehicle, or • Appropriate goodwill compensation reflecting the ongoing safety concerns and diminished value I would appreciate prompt review by a case manager and a written response outlining Ford’s proposed resolution. Please contact me at your earliest convenience to discuss next steps. Thank you.Read more
