On April 27, 2025, in [XXX] , the vehicle displayed a low coolant warning in the morning. The owner replenished the coolant as instructed, the warning cleared, and the vehicle was driven to work without incident. Later that day, while traveling southbound on [XXX] at approximately 60 mph, the engine overheat warning activated, forcing the driver to stop and pull over. The vehicle required two sep
... arate tows — first to the residence, then to the dealership the following day. On April 30, 2025, the dealership completed a full warranty repair replacing the engine head gasket and seals under TSB 2071245/1, a Volkswagen-issued Technical Service Bulletin acknowledging a known manufacturing defect in the 2022–2023 Taos cooling system, for which there is no recall issued. A formal case was opened with VW Customer Care prior to the repair visit and is on record. This incident occurred just before the vehicle’s warranty expiration at 30,000 miles. What component or system failed or malfunctioned, and is it available for inspection upon request? The vehicle experienced an apparent engine failure involving the engine cooling system and possible head gasket and/or engine damage. Prior to failure, the vehicle had a strong sweet coolant smell and was later diagnosed by the servicing dealership as having a coolant leak and possible engine-related
... damage requiring engine replacement. The vehicle is currently located and available for inspection at Don Herring Mitsubishi. How was your safety or the safety of others put at risk? While driving after leaving the dealership, the vehicle began jerking, hesitating during acceleration, and losing power unexpectedly. A flashing check engine light briefly appeared and disappeared. The sudden loss of normal engine performance created a serious safety hazard because the vehicle could have stalled in traffic, increasing the risk of an accident or collision. I also have eight children, including [XXX] twins, and the loss of transportation has prevented me from reliably getting my children to school, medical appointments, and obtaining necessities. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Don Herring Mitsubishi inspected the vehicle and identified a coolant leak and possible major engine damage. The dealership advised that the engine may need replacement. The warranty company, CNA Extended Warranty, stated the dealership’s submitted notes referenced a blown head gasket and requested further teardown and inspection before approving repairs. The vehicle remains unrepaired while awaiting further action and/or engine replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Our 2017 Ford Edge Titanium had its engine check light go on at 61,000 miles on April 6, 2026. So we brought it into our local Ford dealership (where we purchased it new) the next day on April 7. We were told that diagnostics showed there was a coolant intrusion and we would need to basically rebuild the engine (long block) for $9,900! This is very frustrating since we bought an extended warrant
... y at the time of purchase and had our car regularly brought in for maintenance. We did everything expected as an owner. No surprise, our extended warranty was good up 60,000. Reading up about the problem with coolant intrusion, it seems a class action was taken with Ford but only on their 4 cylinder engines. However, I've found many other Edge owners (with the same 6 cylinder engine we have) who were dealing with coolant intrusion and costly repairs as well. Whether it's a 4 cylinder or a 6 cylinder it's apparent Ford had some manufacturing issues on their engine blocks that cause coolant to leak into the cylinders and I feel they should be held responsible for the 6 cylinders as well.Read more

On three different occasions this car’s engine shuts off on me sometimes at highway speeds. It is terrifying being in the left lane and losing all propulsion and power steering. They are saying because there are no check engine lights and they cannot replicate the problem they can’t do anything and want to return the car to us. We fought that and they started a techline report for engineering to help troubleshooting. Kia customer care was involved, but I was told I had 48 hours to pick my car up as is. Driving this car on long trips is now terrifying for me and my family, if you look online, nationwide Kia tellurides are having this same issue I’ve seen from the 2020 models to 2026. I am not the only person having this issue with a Kia telluride. Something needs to be done NATION WIDE before loss of life happens. I am submitting this complaint regarding ongoing mechanical and electrical issues with my 2021 GMC ACADIA that have been occurring intermittently for approximately the past year and a half. The primary issues involve the coolant temperature sensor, thermostat system, engine cooling fan, and dashboard warning messages. The vehicle repeatedly experiences problems related to the coolant temperature
... sensor and thermostat. During these episodes, the engine cooling fan continues running even after the vehicle has been turned off, sometimes for an extended period of time. In addition, the dashboard frequently displays the warning message: “Steering Assist is Reduced – Drive With Care.” These warning messages and symptoms occur off and on without consistent resolution. The vehicle has already undergone repairs, including replacement of the radiator, yet the problems continue to return. Despite previous service attempts, the underlying issue has not been permanently corrected. The recurring nature of these problems raises concerns about the reliability, safety, and dependability of the vehicle, especially considering the steering assist warning and possible overheating or electrical system concerns connected to the cooling system malfunction. The repeated failures have caused significant inconvenience, concern for safe operation of the vehicle, loss of confidence in the vehicle’s reliability, and ongoing disruption due to repeated repair needs. I am requesting a thorough diagnosis and permanent repair of the coolant temperature sensor, thermostat, cooling fan system, and any related electrical or control module issues contributing to these recurring problems.Read more

I am writing to formally express my dissatisfaction with the handling of repairs on my Chrysler Pacifica and the significant financial and personal impact this situation has caused my family. The issue began when the engine suddenly stopped, leaving us stranded. The vehicle was first taken to an insurance-recommended shop that could not diagnose the issue, then transferred to Wischsnewsky Dodge in
... Baytown, TX. There, we were told the problem was the starter and fuse box. Based on their professional assessment, we approved costly repairs, which required financing. However, just one day after picking up the vehicle, it failed to start again. This not only showed a lack of proper diagnosis, but also placed my wife and children in an unsafe situation. We were also forced to cancel a prepaid family trip due to lack of transportation. The dealership retrieved the vehicle and promised a loaner, but none has been provided after more than two weeks. We have since incurred additional expenses for rentals and rideshare services. We are now being told the issue may be a head gasket failure—something that could have been identified earlier with a basic test and is reportedly a known issue with this model. The estimated cost to replace the engine per the dealership is $16,000. At this point, we have taken on debt for ineffective repairs and continue to face mounting costs without a clear resolution. I am requesting immediate assistance from Chrysler to address this situation and provide a fair outcome.Read more

The vehicle is experiencing sudden engine stalling while in motion, a known safety hazard. Despite a documented maintenance history with no issues, the dealer (Bayside Kia) is attempting to deny a Theta II Settlement claim by shifting the narrative to 'engine sludge,' which only appeared as a symptom in June 2025( Walmart oil change) alongside engine performance issues. The vehicle has active code
... s P0014 and P0017, which are secondary symptoms of the internal bearing wear and metallic debris contaminating the oiling system. KDS Diagnostic tests showed Cylinder 1 at 0.16mm (approaching the 0.20mm failure limit). This clearance proves the connecting rod bearings are worn and reaching the failure threshold; this mechanical wear is the root cause of the engine's inability to operate safely. The presence of metallic debris in the oil is the definitive signature of the Theta II manufacturing defect. Furthermore, the dealer acted in bad faith by attempting to charge for Airbag repairs (Codes B1346/B1481) that are covered under the 15-year Clock Spring Warranty Extension (CS1102). The dealer has dismissed my qualifying maintenance documents, claiming they are 'not official,' which is a direct violation of the Magnuson-Moss Warranty Act. A manufacturer cannot deny a warranty claim because services were performed at third-party facilities, nor can they deny a claim without proving 'exceptional neglect.' I am reporting a refusal to honor a court-ordered safety settlement and an attempt to return an unsafe, stalling vehicle to the road. There is no evidence of engine abandonment or exceptional neglect. I’m considered "in compliance" if I can show I didn't go more than 14 months without servicing or a certain mileage between oil changes. The dealer's dismissal of verified third-party receipts and bank statements is a bad-faith attempt to ignore an inherent manufacturing defect documented by a 0.16mm BCT result and metallic debris.Read more

My car is a 2019 Audi Q8 3.0L V6 with only 41,831 miles. My emergency light went on and I brought it my locat Audi Dealer. The inspection revealed that the coolant pump was leaking internally, contaminating the entire vacuum system with coolant. This is not a maintenance failure or the result of owner neglect. It is a well-documented, inherent design defect in the EA839 3.0T engine platform that
... powers the 2019 Q8, a defect that Audi of America has acknowledged through multiple Technical Service Bulletins. Specifically, the EA839 water pump uses a vacuum-operated diaphragm to control coolant flow. When the pump's internal seals fail — as they predictably do — coolant migrates directly into the engine's vacuum system, contaminating components far beyond the pump itself. Industry experts and Audi forums alike describe this failure mode as inevitable on this platform: not a matter of if, but when. This is a design defect manifesting at a fraction of the mileage at which any reasonable owner would expect a cooling system component to fail. Audi of America has issued multiple TSBs addressing coolant pump failures on this exact engine platform, and previously extended warranty coverage for Q7 and Q8 vehicles, including those built during the same production window as the vehicle in question. Our concern is this issue can cause the engine to seize while driving and Audi's repair costs for this known issue have been quoted minimally at $5,519.76Read more

My 2020 Ford F-150 5.0L Coyote V8 had its engine replaced under factory warranty on 4/30/25 by an authorized Ford dealer. The replacement engine was installed with my original 50,000 mile fuel injectors reinstalled by Ford's technician — I was not informed new injectors were not included. The vehicle is now diagnosed with a failed direct fuel injector causing a cylinder 3 misfire at approximatel
... y 10,000-12,000 miles on the replacement engine. Additional injectors show signs of imminent failure. The shop also documented the replacement engine running 2 quarts low on oil at only 10,000-12,000 miles of service. This level of oil consumption on a recently replaced engine is abnormal and unexplained. The safety concern is twofold. First, a vehicle running chronically low on oil due to abnormal consumption on a replacement engine risks catastrophic engine failure without adequate warning to the driver. Second, misfiring cylinders affect vehicle driveability and create unpredictable power delivery that constitutes a safety risk particularly at highway speeds. Symptoms including rough running and the check engine light first appeared in November 2025. The vehicle has been at an authorized Ford dealer for diagnosis since May 2026. Ford Customer Relations has declined goodwill assistance despite the failure occurring on components reinstalled by Ford's own technician during a warranty repair, symptoms originating within the warranty window, and the vehicle being only 8 days outside the 12-month parts warranty expiration. I am requesting NHTSA investigate whether Ford's practice of reusing high-mileage fuel injectors during engine replacements under warranty constitutes an unsafe practice, and whether abnormal oil consumption on replacement engines represents a systemic defect.Read more

I purchased a Kia Optima 2016, in December of 2025. At the time the car wouldn't go over 60 mph. I hauled the car to Kia to have it inspected. The Kia dealership informed me that there is a recall on that particular car for an engine replacement. They tested the car and later informed me that they believed I purchased the car knowing it had a recall and wanted the engine replaced for free so I cou
... ld sell the car. The car is not for sale, and I had/have no intentions on selling the car. I went ahead and paid $6100 for the engine to be replaced. I later did research and learned that if a car has a recall, it doesn't matter my intentions with the car. I felt that Kia denied a replacement engine because of their opinion of me and not for the recall. The car is registered and insured in my name and is being driven daily. At this time, I feel that I deserve a refund of the $6100. Can you guide me on which steps to take on this matter?Read more

The 2019 vehicle experienced a significant engine failure consistent with widely reported issues involving GM 6.2L V8 engines in GMC Sierra and Chevrolet Silverado platforms. Prior to failure, the vehicle developed abnormal engine noise including ticking/knocking sounds, followed by loss of engine performance and mechanical failure. Vehicle diagnosis indicated internal engine damage requiring maj
... or repair or full engine replacement. The failure occurred despite normal maintenance and relatively low mileage for a modern full-size truck. The vehicle had no prior abuse, modification, or racing use. Based on technician feedback from December 2025 service call and publicly reported incidents, the failure appears consistent with commonly reported GM 6.2L issues involving: connecting rod bearing failure crankshaft/bottom-end damage lifter or valvetrain failure internal metal contamination sudden engine seizure or loss of propulsion This issue presents a serious safety concern because sudden engine failure can occur during normal roadway operation, potentially resulting in: sudden loss of power inability to accelerate vehicle shutdown while driving increased crash risk in traffic conditions I am aware of numerous similar complaints involving GM 6.2L V8 engines across multiple model years, including investigations and recalls affecting related vehicles and powertrains. My vehicle appears to exhibit the same or substantially similar failure characteristics. I request that NHTSA continue investigation into 2019 model-year GMC Sierra 6.2L engine failures and evaluate whether these earlier vehicles should be included in broader recall or warranty coverage.Read more
