2021 GMC Sierra AT4, VIN [XXX] , NHTSA Campaign 25V-274, GM Case XXX. Brake pad monitor failed at 128,923 miles -- 24 miles after leaving Penske Chevrolet (released at 128,899, RO XXX, 04/17/2026). Brakes became spongy immediately following service. Monitor disabled entirely to stop recurring red Service System alert. Root cause is a burnt fuse block ground fault confirmed in writing by two GM-aut
... horized dealers. Ray Skillman (RO XXX, 06/05/2025) documented the fault then deleted Labor Operation 14BUZ ENGINE ELECTRICAL from the recall repair work order. Penske documented verbatim: "all of these other faults share this same ground circuit." The recall repair at Ray Skillman used a remanufactured short block instead of a complete engine per N252494002. The original seized-engine oil pan has never been replaced across three dealerships and 128,000 miles. GM's own Part 573 filing states this defect "increases the risk of a crash." I drove 300 miles on an interstate on 04/22/2026 with active brake warning and no loaner because GM Executive Resolution refused to authorize one. Three GM dealers have inspected this vehicle. GM Executive Resolution Team [XXX] ) has not returned calls or emails and has not confirmed in writing this vehicle is safe to operate. OnStar issued a Critical Alert for P0617 on 04/15/2026. Zero brake faults existed in the 13 months prior to the Penske visit. This is a direct continuation of unresolved electrical damage from a botched federal safety recall repair at 384 days and counting. This appears to be a widespread issue affecting many Infiniti and Nissan vehicles equipped with the VR30 3.0 twin-turbo engine. Numerous owners online report similar symptoms including engine knocking, turbo failure, metal contamination, loss of power, and premature engine failure at relatively low mileage. The issue creates a serious safety concern because the vehicle can lose power, hesitate during acceleration, or potentially fail while driving in traffic or on highways. I tow truck came about two hours later only to realize it could not tow it since the vehicle was stuck in park and could not be powered to move it to neutral. They had to call a new tow truck. I was sitting in my car stuck in the entrance of my neighborhood impeding traffic for close to 4.5 hours. A new tow truck finally arrived at close to 9:45pm. It was towed on dollies to the dealership. They inspected it today and said the starter and the engine had both failed/blown and needed full replacement. I am waiting to hear back from them on the estimated time for replacement biut could take weeks which is devastating. My vehicle is the 2025 Chevy Tahoe High Country with the 6.2 L V8 engine that I now understand has issues prior to 2025 and was recalled but nothing yet for the 2025. I purchased a 2023 Hyundai Elantra Hybrid 2023 on December 15, 2023, and since early ownership I have experienced persistent jerking/clunking while slowing to a stop, harsh downshifts, hesitation during braking, and rough transitions between electric and engine power. The issue was first documented around 5,744 miles and continued through approximately six to eight repair/work orders, where dealer
... ships repeatedly described the condition as normal hybrid/DCT behavior while performing software resets, adaptive relearns, and diagnostic checks. After continued complaints, Hyundai Techline became involved and the dealership concluded the vehicle “jerks pretty bad,” leading to a full transmission replacement under warranty. Despite the transmission replacement, the issue persisted and the dealership later confirmed slight clunking during downshifts and discovered a torn engine mount, which was also replaced. The dealership additionally noted that the issue did not occur while driving another Elantra Hybrid or the loaner vehicle, suggesting the condition was specific to my vehicle. I also opened a case with Hyundai Motor America Consumer Affairs regarding the recurring defect and repeated failed repair attempts, but the issue continues despite major drivetrain repairs. Hyundai Motors America Consumer Affairs has not been able to help me with this either. They also didn't provide me with the Federal Agency number when I requested them in order to reach out and seek help from the agencies. Hyundai didn't help me in any way. There were no warnings. They have been able to reproduce the issue but do not want to resolve the issue. I strongly believe no vehicle is designed in a way that it jerks and downshifts in a harsh manner causing distress to the driver/owner.Read more

I was unable to use the push button start for a few minutes up to 15 minutes and in 2025 I was unable to use the push button at all on most days. I had to research a work around to be able to start my Jeep and even after that depending on the day, it takes several seconds to minutes to start it this way. On 2 different occasions as I was driving, I had no control over my steering for approx. 10 seconds. I held on to the wheel and slowed down. The control returned and I was able to drive home. I researched and found out that 2014-2018 had reports of hacking and the hacker taking control of the vehicle in multiple ways including steering, brakes, alarm system, and hacking FOB. My radio has been turned off and the stations changed. One of my FOBs was tampered with, including switching the lock and unlock button's capability, panic button was disabled, and now the FOB does not work at all. There is a loud grinding noise behind my glove box that can be heard at different times including when Jeep is turned off. Loud noises that sound like a rock hitting the window happen randomly to my front windshield, driver and passenger windows happens multiple times while I am driving and when vehicle is turned off. I am filing a safety complaint regarding a 2015 Kia Sorento equipped with the 2.4L Theta II engine. The vehicle developed progressive engine knock, reduced power, and check engine warnings while driving. Diagnostic trouble codes include P0017, P0014, and P0340 relating to crankshaft/camshaft timing correlation and camshaft position faults. A Kia dealership performed diagnostic testing using Kia

The dealer repair order specifically states: * “Found data PIDs for additional knock noise” * “All registering knock on all cylinders” * “Recommend replacement of engine sub assy based on knock detection” The vehicle experiences engine knocking and loss of power that create concern for sudden engine failure or stalling while driving, which could increase crash risk. This complaint is being submitted because the symptoms appear consistent with the known Theta II engine defect involving connecting rod bearing failure, engine knock, seizure, and potential engine failure.Read more

COMPONENT FAILED: Left transmission mount bracket and its fasteners on a 2017 RAM ProMaster with 3.6L V6 Pentastar (sales code ERB) and 62TE 6-speed automatic (sales code DG2). At least one bracket-to-transaxle bolt sheared. The bracket separated from the transmission, the powertrain dropped on the driver side, the left front CV axle pulled out of the transaxle, and transmission fluid loss occurre
... d. The transmission case casting also appears cracked at the bracket mounting boss. All failed components are preserved and available for inspection. SAFETY RISK: Failure occurred during LOW-SPEED MANEUVERING. Had it occurred at highway speed, the dropped powertrain and separated front drive axle could have caused loss of vehicle control, sudden disabling of the vehicle in traffic, and roadway debris from dragging components. Industry-published cases describe identical failures at 40+ mph (Certified Transmission, May 2019). Multiple owner reports of the same failure exist for 2015-2021 ProMasters. CONFIRMED BY: Failure confirmed by visual inspection at an independent service facility. The failure mode matches FCA Service Bulletin 23-022-19 (8/27/2019), which warns the bracket fasteners are torque-to-yield, single-stage, one-time use, and that reuse can lead to bracket bolt failure. The TSB is classified Information Only — no recall has been issued. INSPECTED BY: Inspected by the owner and an independent repair technician. Not yet inspected by the manufacturer, police, or insurance. The vehicle had recent service in the transmission area; whether the one-time-use fasteners specified by the TSB were replaced during that service is under investigation. WARNING SIGNS: None. No warning lamps, messages, or symptoms preceded the failure. Given the documented failure mode, manufacturer service bulletin, and multiple owner reports of identical failures across model years, a formal investigation into a potential safety recall is requested.Read more

But The dealer's initial assessment from 05.13.2026 (estimates $9325.25) indicates the fire was caused by a failure of the Exhaust Gas Recirculation (EGR) cooler, which is a known issue (similar to recall VB1/19V-757) where the cooler cracks, leaks coolant, and causes a fire in the intake manifold. This is a component failure resulting in an explosion/fire, not typical wear. I need a diagnostic report that specifically mentions the EGR cooler failure as the source of the fire. I was driving down the interstate when all of a sudden, I hear a bang, and my power went crazy, couldn't push the gas at all, my car was suddenly shut off. We coasted to the side of the interstate and got out to see a pile of oil underneath my car. Called a tow truck to help us and he found a massive hole in the engine block / oil pan - catastrophic engine failure. I have 117k miles on my car BUT
... I have kept up with maintenance on this vehicle. There should be no reason for this to happen. This put my family in immediate danger.Read more

Description of Incident: While stopped at a red light at a busy intersection near a shopping district, the vehicle's Auto Idle Stop (AIS) system engaged and shut off the engine. When I attempted to restart by pressing the brake pedal — as instructed — the vehicle failed to restart. The dashboard displayed an error message reading "Idle Stop Function: Press Brake to Restart." My foot was on th
... e brake pedal at the time; pressing it did not restart the engine. I then manually shut off the vehicle using the Engine Start/Stop button and attempted to restart it. The vehicle would not start. I remained stranded at the intersection for approximately 10 minutes. I had to call the cops for a patrol car because other cars were piling up behind me and I had my 6 month old son in the vehicle. Safety Impact: During this time, I was unable to move the vehicle from a live traffic intersection. This created an immediate safety hazard to myself, my family, and other drivers.Read more

I am reporting a safety concern involving BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this internal component fractu
... res, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or severe mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions the cold is when they fail the most, where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 for just the pump, assuming the motor is unharmed. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components in the car even when no replacement has occurred and the old components are actually in the car. Also, BMW has actively removed the plastic part number out of all dealer networks as well.Read more
