On [XXX] ~[XXX] PM, at highway speed on [XXX] eastbound near Frisco, CO (a mountain interstate), returning to Denver from Salt Lake City, my 2021 Subaru Crosstrek suddenly and without warning dropped into reduced-power limp mode and began lurching and shaking while the engine kept running. ... tiple active safety systems automatically disabled: EyeSight, pre-collision braking, vehicle dynamics (traction/stability) control, and hill start assist. It took about two minutes to manage the lurching, power-limited vehicle across multiple lanes of traffic to reach the shoulder. My partner was in the vehicle. Had this happened on a bridge, a grade without a shoulder, a tunnel approach, or in heavier traffic, I believe a collision was a realistic outcome. The vehicle was towed to a Subaru dealer and diagnosed with a failed Thermo Control Valve. Sixteen trouble codes were stored, including P26A6, P0606, P060B, P2138, P2135, P2122, P2119, P2004, P2005, and P0191. Mileage was 49,983. There was no prior symptom. The vehicle had a full multi-point inspection at the same dealer only 11 days and ~1,400 miles earlier ([XXX]); nothing was flagged. This failure mode is already known to Subaru, addressed by Technical Service Bulletin 09-119-24 and a warranty extension to 15 years/150,000 miles; the dealer repair order confirms the TSB. I am concerned Subaru handles a safety-relevant failure, one that disables collision-avoidance systems at highway speed, through a quiet warranty extension rather than a recall or owner notification. Owners get no warning and discover it only when it fails dangerously. I request NHTSA open a formal safety defect investigation. I am submitting a complaint regarding premature exhaust system failure on my 2020 Buick Enclave involving the front exhaust pipe/exhaust manifold/catalytic converter assembly. The issue appears very similar to the conditions described in GM Special Coverage programs N212356540 and N242445280 involving exhaust pipe and bracket failures on related GM crossover platforms. ... ort brackets on the front exhaust pipe appear to fail or separate, which places excessive stress on the flex section of the exhaust system. This movement and stress can cause the flex pipe/manifold area to crack and may ultimately damage the catalytic converter assembly itself. My vehicle developed: Exhaust leak Loud ticking/exhaust noise Rich/lean oxygen sensor fault conditions Exhaust fumes entering or potentially entering the cabin Passenger dizziness while riding in the vehicle Concern for carbon monoxide exposure The repair facility documented that the exhaust leak should be repaired as soon as possible because exhaust fumes may enter the vehicle cabin. The estimated repair cost is approximately $1,700 for replacement of the integrated catalytic converter/exhaust manifold assembly. This appears to be more than ordinary wear and tear. The failure pattern is consistent with bracket/support failure causing abnormal stress and cracking in the exhaust system upstream near emissions components. Because these failures can allow exhaust gases and possible carbon monoxide exposure into the passenger compartment, I believe this issue represents a significant safety concern and warrants investigation for 2020 Buick Enclave vehicles and related GM crossover platforms.Read more

The failure caused engine oil to contaminate the engine coolant system, creating a brown/milky “milkshake” substance inside the coolant reservoir and throughout the cooling system. I had to add approximately one quart of engine oil. I am concerned this premature failure could create a safety risk due to possible overheating or engine failure while driving. Am having issues with my 22 Ram 1500 limited 4x4 with the 5.7 HEMI MDS. there are issues with this engine since 2009. The MDS lifters and cam shaft do not get enough oil. The P1326 code is a proprietary Kia/Hyundai specific code that can only be generated when the Knock Sensor Detection System (KSDS) software update is present on the vehicle. The presence of the P1326 code is confirmation by Kia's own proprietary detection system that this engine is experiencing the exact failure pattern associated with the Theta II engine defect. The dealership refused to perform a BTC test or enter the vehicle into Techline, citing that the VIN does not appear in the Theta II settlement database. Kia America customer care has also denied coverage. The vehicle remains inoperable. Safety risk is significant. The Theta II engine defect has been directly linked to engine fires in Kia and Hyundai vehicles. Limp mode and sudden loss of power while driving creates serious risk of accidents, injury, or death to the driver and other motorists. The 2012-2019 Kia Sorento is a model year range specifically identified in Theta II multidistrict litigation. ... ing an abnormal noise and observed white smoke coming from the rear engine compartment area. The operator safely evacuated passengers and emergency responders arrived quickly to suppress the fire. No injuries or fatalities occurred. A third-party fire investigation was conducted by Halsey King & Associates. Based on the preliminary findings, the fire is believed to have originated from a catastrophic failure of the Thermo King A/C compressor clutch bearing assembly. The bearing reportedly overheated, seized, and caused the compressor belt to ignite while still being driven by the engine. The investigation identified evidence consistent with: •A seized A/C compressor clutch bearing •Extreme heat generation within the clutch assembly •Belt ignition and progression of fire through engine compartment components At this time, our maintenance records do not indicate any previously documented defects associated with this specific Thermo King assembly prior to the incident. Our fleet currently has inspection and preventive maintenance procedures in place related to belt alignment, pulley alignment, belt tension, clutch air gap measurement, and related A/C system inspections. Complete loss of coolant in system disabled the vehicle putting driver at risk of being stranded, potential permanent damage of motor due to overheating, and loss of use of the vehicle on numerous occasions. Low coolant warning indicator only comes on when the coolant tank is dangerously low. Most leaks have been detected from inspection and diagnostic testing by authorized service shops. Sequence of Events: I was driving the vehicle at low speed down the ramp from Parking Level 2 to Level 1 in JFK Terminal 4 Short Term Parking. Ok so my first issues were with the vehicle stalling while driving in gear. Happened a few times intersections off-ramps etc…and the car would roll to a stop. Luckily no one killed. Again happened a few times. Went to dealer they “fixed”it. Then it happened a couple other times…scary shit…went to dealer then they“Fixed” it again. Happened again I didnt bring it back. And Those were my electrical problems …As far as mechanical problems I brought car in for the recall notice “ the pico test’ .
