In May 2026, the Emission Warning Light unexpectedly illuminated on my 2016 Honda Pilot. I immediately sought an inspection, and repairs were completed on 5/9/2026. Failed Component: The fuel injection system failed. Specifically, the injectors for cylinders #4 and #5. The replaced parts [are / are not] available for inspection. Safety Risk: This defect caused the engine to run dangerously rich, d
... umping raw fuel into the exhaust. This poses two severe safety hazards: 1) Sudden loss of motive power, hesitation, or stalling while driving in traffic, drastically increasing crash risk. 2) Extreme overheating of the catalytic converter, creating a significant under-vehicle fire hazard. Confirmation & Symptoms: The issue was confirmed by a professional service center. They pulled diagnostic code P0430 (Front Bank Catalyst System Efficiency Below Threshold Bank 2). Following Honda’s Service Bulletin 20-100, the technician performed a software update and a cylinder A/F test. Cylinders #4 and #5 failed the test (running rich), requiring immediate injector replacement to make the vehicle safe to drive. Inspection by Others & Manufacturer Awareness: The vehicle was inspected by the repairing facility. Honda is fully aware of this widespread defect, as evidenced by SB 20-100 and a related warranty extension. However, Honda chose to handle this severe defect via a limited warranty extension rather than a mandatory safety recall. My extended warranty expired in October 2025. When this known safety hazard manifested in May 2026, I was forced to pay for the repair entirely out of pocket. I urge the NHTSA to investigate this known stalling and fire hazard and compel Honda to issue a formal safety recall so consumers are not left at risk.Read more

Formal Consumer Complaint Statement — GM / Crivelli Buick I am filing a formal consumer complaint against Crivelli Chevrolet Buick GMC and General Motors/Buick regarding what I believe was discriminatory and unfair treatment in connection with a warranty-covered vehicle repair. The customer is [XXX] , owner of a 2017 Buick Encore. The vehicle was serviced by Crivelli Chevrolet Buick GMC in Mt.
... Pleasant, Pennsylvania, on or about May 20, 2026, for a turbocharger failure covered under GM Special Coverage Bulletin N232395330. The vehicle had recently passed Pennsylvania safety and emissions inspection on April 30, 2026. The turbo-related diagnostic code appeared on or about [XXX]. Crivelli completed the minimum turbocharger warranty repair but then informed the customer that additional intake/manifold-related repairs were needed and would not be covered. The customer disputed this because the additional failure appeared during the same repair sequence and appears directly related to the covered turbocharger failure. A written request was faxed asking Crivelli to review the invoice, compare the repair to GM’s covered parts list, and submit the related intake/manifold issue to GM for collateral coverage, goodwill review, or warranty consideration. Crivelli did not respond to the fax notice and did not return the customer’s calls. When the customer attempted to follow up with the service department, Scott from the service department refused to review the concern, refused to provide further service, and refused to contact GM regarding the related warranty issue. Based on the conduct, tone, refusal to review the documentation, refusal to escalate to GM, and refusal to provide equal service, I believe the customer was denied fair consumer service because of race. The customer is not asking for special treatment. I am reporting premature cooling system failure on my 2021 Audi SQ7 equipped with the 4.0L twin-turbo V8 (EA825 engine). The vehicle developed a coolant leak from the water pump and thermostat housing assembly at relatively low mileage (less than 27,000 miles). The failure caused coolant loss and low coolant warning messages, requiring major repair due to the engine’s integrated “hot-V” coo
... ling system design. Research shows many similar reports involving Audi EA825 4.0T vehicles (SQ7, SQ8, RS6, RS7, etc.) experiencing water pump and thermostat leaks, along with multiple revised OEM part numbers and redesigned components. This defect creates a potential safety risk because sudden coolant loss can lead to overheating, limp mode, loss of power, or breakdown while driving. I request that NHTSA investigate whether these recurring cooling system failures on EA825 4.0T engines represent a broader defect trend. Thank you for your attention to this matter.Read more

I am reporting a serious safety defect in my 2019 Mercedes-Benz GLS450. My vehicle experienced low oil pressure warnings and engine warning lights, which were diagnosed as a faulty oil pump solenoid valve. The repair required removing major engine components, resulting in a cost of $5,000 that I paid personally. It is important to note that Mercedes-Benz updated the valve design in the following
... model year, indicating that the company was aware of a defect in the original design. This demonstrates that the failure is recurring and not isolated, creating a serious safety risk—low oil pressure can lead to catastrophic engine failure while driving, endangering the driver and passengers. I request that NHTSA: Document this as a potential safety defect. Investigate the prevalence of this issue. Consider initiating a recall or safety notice for affected GLS450 vehicles.Read more

Toyota made a defective coolant valve in their 2019-2021 cars that leaked spontaneously in these vehicles. Over 950,000 cars have been affected to date, including my 2020 Rav4. They are aware of the issue and made a customer support program for this issue which led to engines overheating/electrical code problems, which can cause a severe safety issue if left unrepaired. The program however has a l
... imitation until November 2025 or 100,000 miles whichever comes first. When I was alerted of this, I brought my car to the dealer for service, but they wouldn't replace the part because it wasn't leaking too much at the time to need repair. However, just 4 months after the primary coverage expired, my engine code went off and when I brought my car, the coolant valve turned out to be defective and leaked into the electrical wire harness which then also had to be replaced in addition to the coolant valve. The repair cost was $1200 and both were required to keep the car driving safely and reliably on the road. Because my car had 120,000 miles and the known issue presented after Nov2025, Toyota refuses to cover the service, although it is a known manufacturing defect. Toyota is committing fraud to avoid covering this safety issue. The repair is expensive and if left unrepaired, the cars can spontaneously catch on fire on highways and in parks leading to wild fires and endanger others on the road. The toyota technician specifically informed me the issue was similar to other cars and known and the defect led to the destruction of the wire harness. To avoid a safety recall, Toyota reached a private settlement against class action lawsuit and created a limited CSP to avoid covering the defect in cars like mine to maintain their reliability status in consumer reports so cars like mine with little higher mileage won't be reported. It is fraud. I believe they are skipping on consumer repair bills that consumers on entitled to for their manufacturing defect.Read more

NHTSA Safety Complaint – 2016 Mercedes-Benz C300 Vehicle: 2016 Mercedes-Benz C300 | Mileage: 74,662 miles What Happened I’m filing this complaint because my 2016 Mercedes-Benz C300 suffered a sudden, complete engine failure at 74,662 miles, and I believe it’s a safety risk that deserves investigation. I’ve owned this car since 2016 and followed every recommended service interval. All a
... vailable warranties were purchased and maintained. The car showed absolutely no signs of trouble before this happened. Last week before filing, I was driving normally when the check engine light came on without any prior warning — no oil pressure light, no temperature alert, nothing else. Within minutes, the engine shut off completely. The car rolled to a stop and I noticed oil leaking from the front of the engine. It was towed to an authorized Mercedes-Benz dealership, where they confirmed the engine suffered a catastrophic internal failure and needs to be fully replaced. Why This Is a Safety Concern No advance warning. The check engine light appeared only minutes before the engine died. That’s not enough time to safely exit a highway or busy road. A sudden total engine failure at speed affects steering, braking assist, and reaction time — it could easily cause a serious accident. Engine failure at 74,662 miles is not normal. A complete engine failure on a well-maintained luxury vehicle at under 75,000 miles strongly suggests a manufacturing defect or design flaw, possibly affecting other vehicles of the same model year. Oil leak at engine front. The leak observed at failure likely points to a specific component or seal failure. If this is a design weakness, other 2016 C300 owners may be at risk without knowing it. What I’m Requesting 1.Open an investigation into the 2016 Mercedes-Benz C300 for sudden engine failure and inadequate warning systems. 2.Determine whether other owners have reported similar incidents. 3.Evaluate whether a safety recall is warranted.Read more

My 2014 Jeep Grand Cherokee Summit EcoDiesel (VIN: [XXX] ) experienced a catastrophic engine failure at approximately 116,000 miles while driving. The vehicle suddenly displayed an oil pressure warning, and I immediately shut the engine down to avoid further damage and potential safety risks. At the time I shut the engine off, the engine was not making catastrophic knocking noises and was not lea
... king oil heavily. The vehicle was towed to Astorg Dodge Chrysler Jeep in Parkersburg, WV. Before inspection, I specifically instructed dealership personnel not to start the engine because I suspected a serious oil-related issue and wanted to avoid causing additional damage. Despite these instructions, the dealership started and ran the engine anyway. After the dealership started the vehicle, the engine condition reportedly worsened significantly, including severe engine noises and much heavier oil leakage. I was later informed the vehicle would require complete engine replacement, with estimated repair costs ranging from approximately $10,000 to $24,000. At the time of failure, the vehicle was also subject to the unresolved Z46 High Pressure Fuel Pump (HPFP) recall, which had not yet been completed. I had previously been informed that recall parts were available and the repair would be scheduled. I am concerned that the unresolved recall and/or related fuel system issues may have contributed to this catastrophic engine failure. I am also concerned about the potential safety risk associated with sudden oil pressure loss and catastrophic engine damage while operating the vehicle. I am submitting this complaint so the failure and unresolved recall involvement are documented and reviewed. I am writing to formally log a safety defect complaint regarding a severe mechanical failure in my 2014 Hyundai Elantra, equipped with the Nu 1.8L engine. The vehicle has been professionally diagnosed with severe internal engine knocking stemming from catastrophic piston slap, a manufacturing defect inherent to this engine family block that causes premature cylinder wall scuffing, piston ring degr
... adation, and unmetered oil loss. The National Highway Traffic Safety Administration has previously acknowledged that manufacturing issues in Hyundai engine blocks can cause rotating internal components to fail, leading directly to sudden, un-alerted engine stalling at highway speeds, a severe and immediate safety hazard to drivers, passengers, and surrounding traffic. Despite this being a well-documented mechanical defect (originally addressed under the [XXX] , et al. v. My 2019 BMW X3 equipped with a B46 engine developed a significant coolant leak at approximately 80,000 miles. The vehicle displayed a low coolant warning, and coolant was visibly leaking underneath the vehicle. Around the same time, I began noticing excessive white exhaust/steam during cold starts and, for the first time since owning the vehicle, had to add engine oil between scheduled oil changes
... . The vehicle is currently undergoing inspection by an independent repair facility. Preliminary findings indicate concerns involving premature failure of critical plastic cooling system and/or oil filter housing components. Based on my research, these failures appear consistent with widespread complaints involving BMW B46/B48 engine cooling system and oil filter housing defects. I am concerned this defect could lead to sudden coolant loss, overheating, engine failure, or unsafe driving conditions. Prior to the coolant warning appearing, there were no significant alerts indicating a major or imminent cooling system failure. The apparent failure of critical plastic engine cooling components under normal driving conditions raises serious concerns regarding the durability and safety of these parts. I have photographs documenting the coolant leak, as well as detailed notes and diagnostic information from the repair facility, available upon request. The vehicle is available for inspection.Read more

The vehicle was brought to an authorized GM dealership for a scheduled appointment to address a check engine condition related to a purge valve and an open safety recall for the lower control arm. The VIN was provided and confirmed at the time the appointment was made; however, the recall was not completed during the visit. The dealership replaced the purge valve without documenting diagnostic te
... sting to confirm the cause or verify the repair. The vehicle was returned with the same check engine light still present. Additional issues were communicated through technician video and text, including a potential oil leak and possible fluid mixing, but none were documented or diagnosed in the service record. A dealership manager later stated that the vehicle was “not safe to drive,” but no written explanation or safety guidance was provided, and the vehicle was released without clarification. No diagnostic records, technician notes, or findings have been provided by either the dealership or GM despite escalation. The consumer has worked through appropriate dealership and manufacturer channels for approximately one month without resolution. The consumer is now being directed to return the vehicle for additional diagnostics at the consumer’s expense without defined scope or explanation. The vehicle remains with an unresolved condition and an open safety recall, without clear guidance regarding safe operation. This lack of clear, documented information is particularly concerning for senior-aged owners who rely on accurate guidance to make safe decisions.Read more

... my vehicle was out of warranty, I had to pay the entire repair out of pocket ($7500), plus be without my vehicle for about 2 weeks. After, doing some research, I learned that had the incident happened 6 months sooner, the repair would have been covered in full by a CSR. I would like to know if there is anything that can be done to reimburse my expenses regarding this repair.Read more

Apparent coolant leak identified in my 2014 Ford Escape 1.6L (Ecoboost).
