The vehicle has intermittent hesitation/delayed acceleration from a stop and during low-speed stop-and-go driving. At times, engine RPM increases before the vehicle responds or accelerates normally. This creates a safety concern when entering traffic, turning, merging, or accelerating from a stopped position because the vehicle does not always respond predictably to throttle input. The concern wa
... s reported to a Volkswagen dealer in writing during a prior repair process and was later formally documented on a repair order as hesitation on acceleration. The vehicle has prior Volkswagen repair history involving a coolant warning/low coolant concern, head gasket repair, and later camshaft seal/belt cover repair. After the most recent repair, the delayed acceleration/hesitation concern remained unresolved. The vehicle is currently under review with Volkswagen Customer Care, but the safety-related drivability concern has not yet been fully resolved. Supporting documents include the 2025 written report of jerking from a stop, the 2026 repair order documenting hesitation on acceleration, and post-repair emails showing the issue remained unresolved. After I notified the dealer in writing that the hesitation/delayed acceleration concern remained after the most recent repair, I did not receive a documented response or resolution, and the concern remains unresolved with Volkswagen Customer Care.Read more

I own a 2015 Chevrolet Silverado equipped with a GM V8 engine that experienced significant engine failure symptoms consistent with known GM engine defect issues and related recall/service campaign concerns. Prior to failure, the vehicle began exhibiting abnormal engine noises, warning lights, performance issues, and symptoms indicating internal engine damage. The condition worsened and resulted i
... n major engine failure requiring extensive repair or engine replacement. The failure created safety concerns due to the potential for sudden loss of power and loss of propulsion while driving. Based on the symptoms, model year, and engine configuration, I believe the failure may be related to known GM engine issues involving internal engine components, lifter failures, AFM/DOD system failures, bearing damage, or related defects that have been the subject of recalls, special coverage adjustments, technical service bulletins, or investigations affecting GM trucks and SUVs. I believe the vehicle should qualify for coverage, repair assistance, recall-related repairs, or special policy consideration from General Motors due to the nature of the engine failure and the known history of similar failures affecting comparable GM vehicles. I am requesting that GM review the vehicle history, diagnostics, and failure condition to determine eligibility for recall coverage, special coverage adjustment, warranty assistance, or reimbursement related to the engine failure.Read more

I am filing a vehicle safety complaint regarding a 2024 Hyundai Palisade Calligraphy due to repeated safety-related mechanical failures and ongoing defects that create dangerous and unpredictable driving conditions. Since purchase in August 2024, the vehicle has experienced multiple serious issues, including steering rack problems causing sticking, resistance, abnormal noises, and inconsistent st
... eering response while driving. The vehicle has also experienced rear automatic braking/sensor malfunctions causing sudden stopping and grinding during operation, creating unsafe driving conditions and concerns about unintended braking behavior. Additional issues include transmission and gear engagement concerns, engine replacement under recall, infotainment system replacement, ongoing recall-related concerns, and excessive tire wear requiring replacement of all four tires at approximately 29,000 miles. The premature tire wear raises concerns regarding possible steering, suspension, alignment, or drivetrain defects. There are also concerns involving unusually fragile windshield and window glass, which appears to be a commonly reported issue among Hyundai Palisade owners. Despite repeated dealership visits, diagnostic evaluations, repairs, and major component replacements, the problems continue. The vehicle has spent an estimated total of approximately five months in repair facilities or undergoing service evaluations since purchase. The steering issues and rear automatic braking/sensor malfunctions are the most serious concerns because they directly impact the safe operation of the vehicle. The vehicle has demonstrated unpredictable behavior, including grinding, sticking steering response, and sudden stopping. These ongoing defects have caused significant safety concerns and loss of confidence in the reliability and safe operation of the vehicle. I believe these issues warrant further investigation due to the potential risk to drivers, passengers, and others on the roadRead more

While operating my truck under normal driving conditions, the check engine warning light suddenly illuminated without warning. Due to concerns about potential vehicle reliability and safety, I immediately brought the vehicle to the dealership for diagnosis and inspection. After evaluating the vehicle, the dealership stated that diagnostic trouble codes were present and that additional analysis of
... the system would be required. The vehicle remained at the dealership for approximately one week before being returned to me. Upon release of the vehicle, the dealership advised that the issue was related to multiple erroneous codes and that a software update had been performed. However, the dealership did not provide a clear explanation as to why the codes were generated, what systems were affected, or whether the condition could reoccur. I was also not provided with a complete accounting of all diagnostic trouble codes identified during the inspection. I am concerned that the dealership failed to provide full transparency regarding the diagnosis and repairs performed on this vehicle. The repair order documentation was limited in detail and appeared to omit important information regarding the discovered fault codes and the full scope of work completed. Because modern vehicle systems rely heavily on electronic control modules and software for engine operation and safety-related functions, I am requesting that NHTSA document and review this issue for potential underlying electronic or software-related defects that may affect vehicle reliability and safety.Read more

We bought this vehicle from Carvana in February 2023. Since very early on owning this vehicle, the engine hesitated and stuttered when trying to shift from first to second gear, and would lunge forward slightly. In December 2024, I received a letter from Ford Motor Company about a customer satisfaction program, Campaign 24N07. The letter addressed the symptoms my vehicle exhibited, including engin
... e hesitation, engine running rough, and engine stalling at low speeds, though the check engine light was not on. The letter states that Ford Motor Company provides a one-time replacement of the canister purge valve, given these symptoms and the check engine light being on. When I called all the local Ford dealerships, they refused to do it because the check engine light was not on, and that Ford Motor Company issued this as a customer satisfaction program, rather than a safety recall.Everything that was listed in Campaign 24N07 is MOST DEFINITELY a safety issue, and should not be taken lightly. It is highly concerning that Ford Motor Company didn’t think to consider ENGINE ISSUES a safety recall. The class action lawsuit Vargas et al v. Ford Motor Company had a deadline of November 4, 2020, which was 2 years and 3 months prior to us buying the vehicle, so any vehicles of the affected years and models should have been either repaired, or not allowed to be sold unless the repairs were done by an authorized Ford dealership. This information was not told to us when we bought the vehicle. On May 25, 2026, while driving, the engine fault and steering assist fault warnings came on, as well as the check engine light. When we were able to safely park the car, we discovered the entire power train is non operational. The engine no longer turns over, and we are now having to buy a new car.Read more

Electronically controlled thermostat and sensor fail or get stuck at a much lower coolant temperature causing the dash to display the “steering assist is reduced drive with care.” It isn’t just a message, the steering is much harder to use potentially causing an accident. The ac completely doesn’t work causing potential for sensitive, elderly, or young children and pets to overheat inside
... , it’s significant risk of injury or death from the hot summer temperatures. The engine coolant temperature gauge is all the way down on C position which isn’t registering how hot/cold the engine gets potentially causing damage to the vehicle and a total loss of the vehicle entirely due to damage from the engine overheating. GM sent out letters to 2021 blazer owners which included a special coverage number N252508350 applying to people with this year having this problem and they have until 2036 or 150,009 mi to get the repairs made free of charge, they just bring the document with the special repair codes to the dealer and they fix it free of charge. After doing research online, this problem is affecting all years of the 2019+ blazers with the 3.6L V6 engine AND other known problems with other models and years of their other lineup of cars, it’s very widespread across their platforms. So what are people supposed to do when it isn’t the 2021 year? Pay an $800+ repair bill to GM when they KNOW it’s more widespread than just this year/make? This is a scam. I can’t even do the repair myself because I’m having trouble finding the correct parts online and not only that the repair is extensive, the whole top half of the engine needs to be taken apart just to get to the thermostat, we’re talking the airbox and intake manifold, all the plastic parts and shrouds and sensors covering engine, the fuel rail and injectors, pipes and clamps, engine coolant needs drained and refilled. It’s a major fix. I’m reporting this complaint to raise awareness and hopefully GM will do the right thing.Read more

On [XXX], I drove my 2018 Ford Fusion Titanium AWD to a doctor 37 miles away through rural PA. The car was a reliable workhorse, needing only regular shop visits, and had 108K miles by this date. I also had no prior dash alerts, only ?3 misfires months apart I now know were symptoms of [XXX]. During down time before/after the doctor, I Googled the error messages and read my owner's manual. I restarted the car w/o incident after it had sat for 45 minutes. Six miles into my way home, the same red square popped up, though no second alert did. I pulled over at a clearing 3 miles later. I sat by the side of the road without cell service for 45 minutes. I had no coolant or water with me. I was unable to remove the coolant lid after the wait, nor could I see if there were coolant in the reservoir. I needed to move the car because of having no cell service. After a total of 75 minutes, I restarted the vehicle and moved 2 miles up the road. A red triangle icon appeared and stayed on. I called my dealer, described what happened, and they told me to tow the car the 21 miles to them. My dealer found no issue beyond a cracked spark plug on cylinder 3 that they replaced 4K miles earlier. Not satisfied, my B-I-L and I researched the incident and found the 2.0L EcoBoost coolant intrusion issue. I had the car tested according to Ford's TSB #19-2346 at a separate dealer; they duplicated the intrusion in cylinder 3, and we had a diagnosis. I was fortunate that the car didn't stop mid-traffic, didn't catch fire, and I was stranded in daylight in an Amish community. The other side of that coin is grim. An authorized Kia dealership confirmed the engine defect and approved a full engine replacement. The repair has not yet been performed. Inspections: Vehicle has been inspected exclusively at an authorized Kia dealership where all service and maintenance has been performed. Warning Symptoms: This is the third time this vehicle has become inoperable due to what was previously identified as an oil issue but is now confirmed to be an engine defect. Prior to complete failure the vehicle exhibited ongoing oil leaks and excessive oil consumption over an extended period. AAA roadside assistance was contacted and identified possible starter and battery failure believed to be connected to the engine condition. My 1.5L EcoBoost engine experienced coolant intrusion into the cylinders, a known and documented defect associated with this engine family across multiple Ford model years. Prior to the failure, the vehicle displayed symptoms consistent with internal coolant loss, including white exhaust smoke, coolant level dropping without visible external leaks, and rough running. No external coolant leak was e
... ver identified. The vehicle was subsequently diagnosed by an independent service center, which confirmed coolant intrusion into the engine internals consistent with a head gasket or coolant jacket failure. The component is available for inspection upon request. The failure rendered the vehicle completely undriveable. The vehicle has been out of service for approximately 12 months and has not been driven since the failure occurred. The repair was quoted at approximately $12,000 for a full engine replacement, exceeding the current market value of the vehicle. Safety was put at risk due to sudden loss of engine performance and drivability while the vehicle was in use on public roads. Coolant intrusion can cause hydrolock, catastrophic engine seizure, and sudden loss of power, all of which create risk of collision, particularly at highway speeds. This failure is consistent with a well-documented defect in the 1.5L EcoBoost engine design, which has been the subject of technical service bulletins and consumer complaints regarding coolant entering the combustion chamber. The failure occurred with no prior accident, overheating event, or owner-caused damage.Read more

The dealership confirmed the cylinder head failure is covered under Volkswagen's New Vehicle Limited Warranty. However, the required cylinder head part has been on backorder for over 60 days with no confirmed ETA. No loaner or rental vehicle has been provided despite the extraordinary duration of the repair. The dealership's own service advisor stated that in the state of Texas alone, approximately 7,000 cylinder heads are currently pending replacement across Volkswagen vehicles — indicating this is a widespread, systemic defect actively being managed within the dealer network, not an isolated incident. The dealership has also proposed that I bear 50% of the repair cost, despite the vehicle being under warranty and having a complete and consistent maintenance record. This cost-sharing proposal was framed as a courtesy, which I do not accept. I own a second 2022 Volkswagen Tiguan SE with the same engine and model year, which I am also concerned may be affected by the same defect. Public records, owner forums, NHTSA complaint databases, and ongoing class action filings confirm this is a known, widespread issue affecting numerous 2022Read more

On [XXX], my 2017 Land Rover Range Rover HSE Td6 diesel (VIN: [XXX] ) suffered a complete, catastrophic engine failure without warning at approximately 70 mph on [XXX] . My wife and our two infant sons (ages [XXX] and [XXX] ) were passengers. There were no prior warning lights or symptoms. The engine stopped instantaneously, eliminating power steering and power brakes while I was overtaking a sem
... i-truck in the passing lane. We narrowly reached the shoulder safely. The odometer read 79,018 miles — inside both limits of the federally mandated 8-year/80,000-mile emissions warranty under 40 CFR §85.2103. The vehicle was 7 years old at time of failure. Hoehn Land Rover Carlsbad diagnosed complete engine failure caused by timing chain guide failure — a known defect pattern in the Ingenium diesel engine. The service manager stated failures like this "aren't that rare" with these engines. Repair estimate: $28,000–$30,000. Jaguar Land Rover North America (Case #[XXX]) denied the warranty claim entirely, offering $800, without ever evaluating the claim under the active federal emissions warranty. Their denial cited only the expired extended CPO warranty and a maintenance gap — neither of which constitutes valid grounds for denying an emissions warranty claim under 40 CFR §85.2103(d), as the manufacturer cannot demonstrate the maintenance gap caused a documented design defect. This failure pattern is documented in JLR Technical Service Bulletins LTB00735 and SSM74184. Land Rover Korea issued a recall for the same timing chain failure in 2020. No equivalent recall was issued in the United States for identical vehicles with identical engines. JLR has refused all requests for review or reconsideration for 24 months. UPDATE to previous complaint – Reference Number: [XXX] VIN: [XXX]a [XXX] UPDATE to my previous NHTSA complaint regarding sudden unintended acceleration in my Tesla Model Y (VIN: [XXX] ) I previ[XXX]sudden acceleration incident on [XXX], at [XXX] 4 parking garage ramp. Tesla has now provided the Vehicle Data Report and EDR data (Case # [XXX]). The data shows accelerator pedal position increasing
... to 100%, with brake input occurring late. However, I am 100% certain that I did not press the accelerator pedal at any time. I have a clean driving record of over 27 years. This discrepancy between the logged data and actual driver input strongly suggests a potential safety defect in the accelerator pedal sensor system (both redundant sensors), drive-by-wire system, or brake override / Automatic Emergency Braking functionality. Attachments included with this update: Tesla Vehicle Data Report (VDR) Tesla EDR Report High-resolution photos of vehicle damage and airbag deployment I request that NHTSA review this additional technical information as part of their evaluation for a possible safety defect investigation. BTW, it is really difficult to contact Tesla to report such incidents. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The engine was burning oil, so did a consumption test. The dealer took apart the engine and found the rings for the pistons were leaking and scaring was resulting. This caused me to buy part of a new engine because it was 6days past warranty with only 44,000mi on it. Also, at 18,000mi, a lifter bent and needed that replaced. There are numerous reports for this engine. It should be recalled! I shou

I own a 2021 Kia Telluride with approximately 111,000 miles. I initially brought the vehicle to a Kia dealership for an oil consumption issue and engine noise. The dealership diagnosed and repaired a timing chain and tensioner issue, stating the vehicle was “working properly” upon return. After picking up the vehicle, I immediately began experiencing electrical warning lights, including the b
... attery warning light and system alerts (blind spot and general warning indicators). I returned to the dealership, where they performed diagnostic testing and indicated a potential alternator issue, but stated it was in a “borderline” range and that the vehicle was safe to drive. Shortly after leaving the dealership, while driving with my family in the vehicle, the electrical system began to fail. Multiple warning lights illuminated, and the vehicle lost electrical function while in motion. The vehicle ultimately became completely inoperable and shut down in a parking lot, creating a serious safety concern. Prior to the dealership visit and repairs, the vehicle had no history of electrical issues, warning lights, or charging system problems. The dealership later acknowledged that the alternator issue was not present when the vehicle was initially brought in. I am submitting this complaint due to the sudden and complete electrical system failure while driving, which posed a significant safety risk. The progression from no prior symptoms to total failure immediately following dealership service and diagnosis raises concerns about vehicle reliability and potential safety defects related to the charging system.Read more

I set out to make a quick trip to the store after work and the car was hesitating and I looked at the dash and noticed the check engine light was on. I stopped was in the store about 10 minutes when I came out I restarted it and it sounded fine and then engine light was not on. I was about 2 miles in towards home when the braking system failure message came on, steering failure message came on and
... many other messages. The engine started sounding very loud and knocking. I immediately pulled over and called a tow truck. I had no previous messages come up and the vehicle was running fine before this incident. The auto repair shop that I had it towed to listened to the engine and thinks it sounds like a crank shaft failure.Read more

I was driving my brand new 2025 Kia Telluride that was purchased from the dealership 3 weeks before the incident. The engine died while driving approximately 40 mph on a flat road. The engine died and then there was a loud beeping and a flashing alert on the dashboard in the circle where the righthand indicator camera view appears that said, “Engine Off-Shift to P or N then start engine.” This

The car had been running for about 3 minutes before the engine died. The power steering was also affected. Once I pulled over and put the car in park the engine started again as normal and there were no alerts on the dash or when I checked the Kia app. I dropped the car at the dealership the following morning, they have had it for 6 days so far and let me know that their diagnostics and test drives did not reveal any issues or defects with the vehicle.Read more
