1.XXX XX— GM Recall Notification (Recall No. N252494000) I received official notification from General Motors of Recall N252494000 affecting my vehicle. I brought the vehicle to [XXX] as directed to have the recall remedy performed. 2. XXX — First Report of Engine Propulsion Issues I reported engine propulsion concerns to Jim Ellis. This was my first formal complaint regarding the performanc
... e of the vehicle following the recall remedy. 3. XXX — Oil Type Change Performed by Jim Ellis Jim Ellis performed an engine oil type change, switching to 0W-40 oil, and advised me that this change would serve as the fix for Recall N252494000. I accepted this remedy in good faith. 4. XXX — Low Oil Warning Light Activated Approximately nine months after the recall remedy, and well before the end of the recommended oil life cycle, the low oil warning light activated on my dashboard. This is abnormal and indicative of excessive oil consumption. I returned to Jim Ellis, who performed a fresh oil change and placed my vehicle on a 6,000-mile oil consumption test — an acknowledgment that an oil consumption problem exists. 5. XXX — Active Engine Misfires, Jerking, and Flashing Check Engine Light While operating my vehicle on Sunday, May 31, 2026, I experienced severe engine jerking and misfires. The check engine light began flashing — not steady — which I understand to indicate an active and serious engine misfire event. I was genuinely concerned for my personal safety and immediately parked the vehicle. It has not been driven since. 6. NHTSA VEHICLE SAFETY COMPLAINT SUPPORTING STATEMENT Vehicle: 2013 Mercedes-Benz ML350 I am submitting this complaint regarding a continuing safety concern involving water intrusion and electronic system damage in my 2013 Mercedes-Benz ML350. My vehicle was subject to a Mercedes-Benz recall involving water intrusion and related components. Despite recall-related repairs being performed, moisture
... and water intrusion concerns remain unresolved. During the diagnostic process, Mercedes-Benz personnel identified evidence of water and humidity in areas associated with vehicle electronics and control modules. The vehicle has experienced repeated mechanical and electronic issues following discovery of the water intrusion condition. Multiple repair facilities have been involved, and I have incurred significant diagnostic and repair expenses while attempting to determine the full extent of the damage. The primary concern is not ordinary wear and tear. The concern is that water intrusion has affected vehicle systems and that the recall remedy has not resulted in a complete resolution of the condition. I am concerned that moisture exposure may continue to impact vehicle electronics, control modules, sensors, and other safety-related systems. Mercedes-Benz USA has been notified and a case has been opened. The dealership has also been involved. However, despite escalation requests and requests for management review, I have not received a meaningful resolution. The dealership continues directing me back to Mercedes-Benz USA while Mercedes-Benz USA has not provided a final management response. I respectfully request that NHTSA review whether the recall remedy adequately addressed the water intrusion condition and whether additional investigation is warranted regarding continuing moisture intrusion, electronic system exposure, and potential safety risks associated with this condition. Vehicle Information: 2024 Toyota Tundra 4x4, VIN: [XXX] livery: [XXX] Current Mileage: 24,147 Complaint Synopsis: My 2024 Toyota Tundra is subject to two active open safety recalls with no remedy available ("NO FIX YET" status per Toyota owner portal). 1. What component(s) or system failed or malfunctioned, and is it available for inspection upon request? Primary: Engine assembly (Recall 25TB14 /
... 25TA14) – machining debris left in the engine during manufacturing. This can cause engine knocking, rough running, no-start, and sudden loss of motive power. Secondary: Reverse lamp system (Recall 25TB06 / 25TA06) – moisture intrusion causing both reverse lamps to become inoperable. The vehicle remains available for immediate inspection at Clearwater Toyota or by Toyota representatives. 2. How was your safety or the safety of others put at risk? The engine defect creates a severe risk of sudden loss of motive power while driving at highway speeds, which can cause loss of vehicle control and increase the likelihood of a crash. The reverse lamp defect eliminates rear visibility and brake indication when backing up in low-light conditions, significantly increasing the risk of collisions with other vehicles or pedestrians. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The defects are formally admitted by Toyota through active NHTSA safety recalls. Clearwater Toyota has refused multiple requests for inspection and service documentation. No independent inspection has been performed due to Toyota’s refusal to schedule one. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Toyota has not conducted a physical inspection of my vehicle despite repeated requests. Clearwater Toyota provided a temporary loaner vehicle but has refused to inspect or document the recalled conditions. No police or insurance inspections have occur INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2022 Hyundai Palisade recently blew the engine due to very little oil (6 quarts). My wife stated that she was driving when it started to act weird. Very shortly after that she heard the vehicle making a loud knocking / banging sound. She stopped the vehicle and turned it off in the nearest parking lot. We had the vehicle towed to our certified mechanic to have it looked at. 6 quarts of oil was
... put into the engine before it was full. Both him and my wife stated the vehicle never showed a check engine light, low oil light or oil pressure problems. He stated the vehicle had no leaks. He advised us that Hyundai/Kia was having problems burning oil. This is a high risk and safety concerns since there was no warning, happened suddenly and could cause problems for people to find a safe place to pull off the roadway and interstate. So many things can happen if someone didn't hear the noise or have lights indicating a problem, therefor if this happened on the interstate people could struggle getting to the shoulder when engine locks up. Also, a piston or connecting rod can be ejected from an engine risking the safety of pedestrians on foot or driving down the road.Read more

... ay years ago. No big deal apparently. The conclusion gets it wrong of course. Part of the ruling claiming no safety risk is based on implying the car starts back right after engine loss and that's simply not true. Twice mine wouldn't start back after the incident, not immediately not within 30 minutes. Only once did it fail, then restart after. I've read through the hundreds of similar complaints and many are stranded after it happens as well. But there's definitely one thing in common EVERYONE'S DRIVING DOWN THE ROAD WHEN IT HAPPENS. This is a severely dangerous issue, putting peoples lives at risk. This is 100% on the manufacturer to bring to light and shamefully haven't. This started last summer for me and I didn't post about it but seeing brand new report still being added irks me. It didn't happen over the winter. I own a 2022 Nissan Rogue that has experienced multiple critical safety defects. The vehicle has repeatedly lost engine power without warning, exhibited delayed acceleration, and failed to engage gears—creating dangerous driving conditions that have put me at risk. Service History and Warranty Disputes: February 20, 2026: I brought the vehicle to the dealership after experiencing loss of engin
... e power. Diagnostics revealed that the EGR Pressure Sensor had internally shorted, causing a loss of communication diagnostic trouble code. The service department replaced the exhaust gas pressure sensor assembly. Despite the defect being a manufacturing fault and my purchase of a wraparound warranty, I was charged out of pocket for this repair. When I questioned the warranty coverage, I was transferred to consumer affairs and informed that only the dealership determines what is covered—an unsatisfactory response to a valid warranty claim. May 20, 2026: I returned to the dealership after the vehicle lost engine power again and lurched violently forward with jerking motions. The service department replaced the engine mounts. However, after only one day of driving, the same dangerous symptoms returned. May 29, 2026: I brought the vehicle back to the dealership for recurring loss of engine power, delayed acceleration, and violent lurching. The service department replaced the EGR system, which subsequently caused the spark plugs to warp. I was charged $400 for this additional repair, despite these being recurring manufacturing defects that should be covered under warranty. Conclusion: This vehicle demonstrates a pattern of dangerous, unresolved mechanical failures. The repeated issues—combined with warranty coverage disputes and repeated out-of-pocket charges for factory defects—indicate serious safety and reliability concerns with this 2022 Rogue.Read more

Supplemental Information to Existing NHTSA Complaint involving a 2017 Hyundai Sonata Plug-in Hybrid (VIN: [XXX] ) While traveling at approximately 70 MPH on a heavily traveled highway, the vehicle suddenly and completely lost propulsion without warning. ... maneuvering to reach the shoulder. The vehicle was towed to an authorized Hyundai dealership, where diagnostic testing identified multiple high-voltage battery system fault codes: • P0B68 • P0B6D • P0B72 The dealership diagnosed a high-voltage battery system failure requiring battery replacement. The vehicle is currently non-drivable. During my research, I found multiple owner reports involving Hyundai Sonata Hybrid and Plug-in Hybrid vehicles experiencing loss of power, hybrid system failures, limp mode conditions, and propulsion-related incidents. I also learned that Hyundai previously issued Recall 18V166000 involving 2016–2018 Sonata Plug-in Hybrid vehicles due to battery management system and voltage protection device concerns that could result in loss of power while driving. Dealer says does appply for my car. Although my vehicle’s fault codes may differ from those addressed in the recall, both involve battery-related failures capable of affecting propulsion and creating a highway safety risk. My concern extends beyond my individual vehicle. A high-voltage battery or battery management system failure that can cause a complete loss of propulsion at highway speed without warning presents a potentially serious safety risk. I respectfully request that NHTSA evaluate whether similar incidents involving Hyundai Sonata Hybrid and Plug-in Hybrid vehicles indicate a broader safety concern warranting further review. 2020 Hyundai Sonata SEL Plus, VIN [XXX] . A/C system fails due to refrigerant leak that empties the system completely within 1-2 weeks of recharge. Shop still unable to find leak. Symptom pattern — rapid total refrigerant loss, asymmetric failure (driver side before passenger), no externally detectable leak — points to evaporator core, which is enclosed in the HVAC housing behind the dashboard and invisible during standard inspection. Vehicle has 50,125 miles, still under the 60,000-mile warranty limit after 6+ years. Low-use vehicle averaging under 8,000 miles/year. 5-year calendar warranty expired approx. Jan 2025; issue first documented Sept 2025, only 8 months later. Defect was almost certainly present before warranty expiration. I am submitting this complaint regarding a serious safety issue involving my Hyundai Sonata. While driving under normal conditions, my vehicle suddenly entered a low-power ("limp") mode without warning. The vehicle would not accelerate beyond approximately 30–35 mph, creating a potentially hazardous situation in traffic. At the same time, the check engine light began blinking. The sudden loss
... of power significantly reduced my ability to maintain traffic speed and safely maneuver the vehicle. This condition could have increased the risk of a rear-end collision or other accident. I brought the vehicle to an authorized Hyundai dealership (Fred Beans Honda Of Abington PH# 215-392-9020) for diagnosis. The dealership confirmed that the vehicle stored Diagnostic Trouble Code (DTC) P1326, related to the Knock Sensor Detection System (KSDS). According to the dealership, the vehicle entered limp mode as a result of this fault. The abrupt reduction in vehicle power and speed occurred without sufficient warning and created a safety concern. I am reporting this incident so that NHTSA can investigate whether this condition represents a broader safety defect affecting Hyundai Sonata vehicles equipped with the KSDS system. Vehicle Information: Make: Hyundai Model: Sonata Model Year: 2014 I request that NHTSA review this issue and evaluate whether additional action is warranted regarding vehicles experiencing DTC P1326 and associated limp-mode operation.Read more

Shortly after purchase of the vehicle on November 26, 2021, the vehicle began experiencing repeated DEF (Diesel Exhaust Fluid), SCR, and DPF system failures, accompanied by dashboard warning indicators, drivability instability, reduced power warnings, and eventual shutdown conditions. The most serious safety concern is that when the DEF/emissions warning activates, the vehicle may enter reduced-p

Experiencing a stuck lifter in the DFM module. After doing some research, it appears there are a bunch of other people that are dealing with the same issue. The only recall was for the 6.2L engine but not the 5.3L. It appears that both engines are experiencing the exact same issue and GM hasn’t done anything to help with it.

My 2020 Kia Soul had its engine replaced under safety recall SC209 (NHTSA Campaign 21V259). At 136,202 miles, this replacement engine suffered a secondary catastrophic failure, dropping heavy metal shavings into the oil lubrication system and causing the engine to stall. Kia of Orange Park (Repair Order 183972) diagnosed the failure but refused to honor the 15-year/150,000-mile Class Action Engine

Vehicle: 2015 Audi Q3 | Engine: 2.0L TFSI 4-Cylinder Turbo Component: Engine Complaint Description: While driving on the interstate at highway speed, the red oil pressure warning light illuminated suddenly and without prior warning. The standard low oil level warning light did NOT illuminate prior to the failure. Oil level was verified as correct and within specification at the time of failure.
... The vehicle suffered a complete and catastrophic engine failure due to oil starvation and rod knock, despite a verified proper oil level and oil pressure. The engine requires full replacement. This failure occurred at only 3,432 miles since the most recent oil change. The vehicle has a documented history dating back to 2021 of recurring engine oil consumption concerns, fuel contamination of engine oil, and high-pressure fuel pump failure — all serviced at authorized dealerships. A multipoint inspection performed approximately four months prior to this failure returned a fully satisfactory result with no engine concerns flagged. This is believed to be related to the known piston ring defect in Audi/Volkswagen 2.0L TFSI engines documented in federal class action litigation (Rieger v. Volkswagen Group of America, Case No. 1:21-cv-10546-NLH-EAP), which explicitly named the 2012–2017 Audi Q3 as an affected vehicle. The engine failure occurred without warning while at highway speed, posing a significant and immediate safety risk to the driver and other motorists.Read more

The vehicle is experiencing a dangerous hesitation during acceleration at low speeds, immediately followed by a sudden, violent forward lunging motion. When attempting to accelerate from a complete stop, or when rolling at low speeds (such as yielding at a roundabout, turning through an intersection, or merging into slow-moving traffic), the vehicle suffers from a distinct **lag or complete fail
... ure to accelerate** for several critical seconds. In an attempt to get the vehicle to move out of harm's way, the driver naturally applies more pressure to the gas pedal. The vehicle's transmission then randomly responds after a few seconds with a violent lunge forward, spinning the tires and causing the vehicle to surge unpredictably. This unpredictable behavior creates an immediate and severe safety risk to the occupants of my vehicle and others on the road: 1. Intersection Collisions: The sudden lag leaves the vehicle stranded in oncoming traffic during turns, while the subsequent lunge threatens to overshoot the intended lane. 2. Pedestrian Hazards: The sudden, uncontrollable lunging makes low-speed maneuvers in school zones, crosswalks, and parking lots incredibly hazardous. 3. Drivers behind behind me do not expect a vehicle to suddenly stop accelerating after it has begun to move forward. I have taken the vehicle to the dealership we're we were able to reproduce the issue and Honda would not offer a fix. I thought I was going to get my entire family killed the other day when I pulled out at a stop the van hesitated in the middle of the highway for a few seconds before lunging forward. This is an intermittent issueand the severity varies each time, but is commonly cited online.Read more

The vehicle has repeatedly lost electrical power while driving. During one incident at approximately 50 mph, the vehicle’s electronics shut off and power steering assist stuttered/lost assist for several seconds, causing reduced vehicle control. During another incident while turning/in a turning lane, the engine and electronics shut off and the vehicle had to be restarted. Afterward, the vehicle
... struggled to restart and displayed a warning to drive cautiously due to power steering loss. The vehicle also has recurring hot-start/no-start symptoms and electrical loss after shutting the engine off. Based on my research, GM Bulletin 19-NA-052 appears to describe a related condition on 2016–2019 Chevrolet Camaro 6.2L vehicles where circuit 1 may short to ground at the starter heat shield, causing loss of electrical power or failure to start, with repair involving the starter solenoid cable. This appears to be a safety-related manufacturing/design defect because the vehicle can lose electrical power and steering assist while in motion. The vehicle/component is available for inspection upon request.Read more
