I am writing to formally report a severe safety incident caused by a sudden head gasket failure in my vehicle. While driving on a busy highway, the car abruptly entered "limp mode" and stopped accelerating altogether, putting my family’s safety at immediate risk. Simultaneously, all dashboard warning lights illuminated. The vehicle was towed to an authorized Honda dealership, where technicians

My 2023 Lexus RX350 with 67,000 miles displayed a check engine light along with a Drive-Start Control Malfunction warning. The vehicle was brought to a Lexus dealership where it was diagnosed with fault code P25B372 — Turbocharger Wastegate A Actuator Stuck Open. The dealership confirmed the wastegate actuator has failed and requires replacement. The part is currently on backorder with over 800
... units backordered nationally, indicating this is a systemic issue affecting multiple vehicles. Toyota has issued Technical Service Bulletin T-SB-0084-23 covering this exact fault code on the same 2.4L turbocharged engine used in the 2023 Lexus RX350. The bulletin states the repair is covered under the Federal Emission Warranty for 96 months or 80,000 miles. However, the Lexus dealership has declined warranty coverage citing the absence of a Lexus-specific bulletin, despite the identical engine and fault code. The failure puts safety at risk as the Drive-Start Control Malfunction warning indicates loss of traction and stability control systems. The problem has been confirmed by the Lexus dealership. Warning lamps appeared on the dashboard prior to the visit. The vehicle is available for inspection upon request.Read more

The engine in my 2017 Ford Escape EcoBoost experienced a known “coolant intrusion” defect in which engine coolant leaks into the engine cylinders. This defect is widely associated with the 1.5L and 2.0L EcoBoost engines and can cause engine misfires, coolant loss, overheating, corrosion, and catastrophic engine failure requiring engine or short-block replacement. The vehicle is available for i
... nspection upon request. The vehicle is driven by a college student traveling away from home. The coolant intrusion defect creates a serious safety concern because the engine can suddenly misfire, lose power, overheat, or fail unexpectedly while driving. A sudden engine failure could place the driver and others at risk, especially during highway driving or in remote areas. The issue developed shortly after the vehicle was purchased from a dealership in June 2025. The symptoms are consistent with the widely documented Ford EcoBoost coolant intrusion defect acknowledged by Ford through Customer Satisfaction Program 21N12. The vehicle has exhibited symptoms including coolant loss and engine performance concerns consistent with this known defect. The dealership has been notified of the issue. The dealership has been contacted regarding the issue. Ford has previously acknowledged this defect through Customer Satisfaction Program 21N12 relating to coolant intrusion in affected EcoBoost engines. The vehicle experienced symptoms consistent with coolant intrusion, including coolant loss and engine performance concerns/misfires. These symptoms are widely reported with the affect EcoBoost engines and has progressed to severe engine failure.Read more

The vehicle has experienced repeated failures associated with diagnostic trouble code P06DD. When the condition occurs, the vehicle is unable to exceed approximately 3,500 RPM and experiences engine stalling and loss of power while driving. The issue is intermittent but has occurred repeatedly. The vehicle has been presented to authorized Dodge/Stellantis dealerships on five separate occasions fo
... r diagnosis and repair. The oil pump was previously replaced for this issue at approximately 5,000 miles, yet the condition continues to recur. Most recently, the dealership advised that the issue was resolved and returned the vehicle to me. Within approximately 30 minutes of leaving the dealership and driving on the freeway, the check engine light returned and the same stalling and loss-of-power condition immediately reappeared. The defect creates a significant safety concern because the vehicle loses the ability to accelerate normally while operating in traffic and on freeways. When the condition occurs, the vehicle cannot exceed approximately 3,500 RPM stalls while driving, creating a risk of collision. The vehicle currently has approximately 21,000 miles and remains under manufacturer warranty. Despite multiple repair attempts, the defect has not been permanently corrected.Read more

The motor experienced a catastrophic failure causing the rod to blow through the top of the engine, cutting a fuel line and causing a fire. While driving, the check engine light began flashing and the vehicle shook severely due to engine misfiring. The vehicle has had a recurring cylinder 3 misfire throughout ownership. Spark plugs and ignition coils were first replaced around 40,000 miles and replaced again in December 2025 for the same recurring issue. A Hyundai dealership inspected the vehicle and found codes P0300 random misfire a
... nd P0303 cylinder 3 misfire. The dealership technician documented that the piston rings did not look good and confirmed the catalytic converter was damaged as a result of the misfires. The vehicle has had an engine rattle at idle, oil consumption, and blow-by since early in ownership. The engine defect is progressive and predates the current mileage. The flashing check engine light and severe shaking while driving at highway speeds created an unsafe condition with other vehicles on the road. The problem has been confirmed by a Hyundai dealership. While driving a brand new Kia Carnival Hybrid (purchased yesterday), I experienced a severe, life-threatening powertrain acceleration lag from a rolling start. When attempting to accelerate into oncoming traffic on a wet road surface, the vehicle completely hesitated and dropped power for approximately 1.5 to 2 seconds, leaving the vehicle stran

My 2025 Cadillac Escalade ESV lost power and shut down while I was driving on the highway. The gas pedal stopped responding, and the vehicle would not restart afterward. This created a serious safety risk because the vehicle suddenly lost propulsion while driving in traffic. The vehicle is less than one year old and is already going to be on its third engine. The original engine was already repla
... ced once before. The previous replacement engine was a brand-new GM engine with 0 miles, but it failed after only about 20,000 miles. Pepe Cadillac confirmed that this is the second replacement engine, which means the third engine counting the original engine. The dealer also confirmed that the engine locked up due to internal bearing material failure and needs to be replaced again. The repair order number is 140134. I am concerned this is a serious safety-related engine defect. A brand-new GM replacement engine should not lock up after only about 22,000 miles, especially while driving on the highway. I am also concerned that simply replacing the engine again may not fully address the safety and reliability issue with this vehicle. Cadillac Customer Care case number: 100539041.Read more

Approximately 8 months ago, I brought my car to the BMW dealership Keeler's in Latham, NY 12110 for a Recall 24V-608:474, the dealer held my car for a day. Two weeks later after taking my car in for the recall, my check engine light started going off and on, off and on, then it comes on and stayed on. On certain occasions I smelt burning wiring and antifreeze smell, yet I assumed that those partic

It wasn't until the weather was getting warm that I try to use the Cold air in my car that I notice that there was nothing but hot air coming out and not the ice-cold air that I was getting before I took it to the BMW dealer in Latham. I connected a code reader to my car and the codes that come back were U019F and U0079 which stated that is was a lost communication with the Engin coolant pump. I call BMW and tell them about my complaints; they told me to bring in my car to get it look at. I brought the car to Keeler. The Service Advisor Daniel Purpura told me that my car has be reset and a shelter cap replace and the shop mechanic drove it, and now the check engine light is off. When I went to start the car the engine light was back on, and I told him that. He told me that the code they found was code 003B97. The 003B97 is not the recall code, but it was caused by the recall work. Under federal recall law, any defect created during recall repair must be corrected at no charge. On May 27, 2026 at approximately 11:00 PM, my 2020 Jeep Gladiator Overland (VIN: [XXX] ) suffered catastrophic engine failure during normal operation at 78,190 miles. The alternator seized, causing the harmonic balancer (crankshaft damper) to fracture. The broken balancer fell into the engine block, causing extensive internal damage requiring complete engine replacement at significant cost. This
... failure sequence points to a design and/or manufacturing defect in the OEM alternator and/or its mounting system on the 3.6L Pentastar V6. A properly designed alternator should not seize with enough force to fracture the harmonic balancer and deposit debris into the engine block under normal operating conditions. The vehicle was not involved in any accident or operator abuse. It was being driven normally with no prior warning — no dashboard lights, no unusual sounds, no prior related mechanical issues. I request NHTSA investigate whether this failure pattern is systemic across 2020 Jeep Gladiator vehicles and other 3.6L Pentastar V6 platforms, including the Jeep Wrangler, Ram 1500, and Dodge Durango. Sudden complete engine failure at highway speeds is an immediate crash hazard to the driver and surrounding motorists. I am reporting a catastrophic engine failure on a 2023 Jeep Wagoneer (VIN: [XXX] ) that occurred within 163 miles of consumer delivery, suggesting a latent manufacturing defect present at the time of sale. The vehicle was delivered February 13, 2026 at approximately 29,750 miles. Engine noise was noted the same evening. By March 10, 2026, the vehicle was grounded at Farris Motor Company [XXX] ) a
... t 29,913 miles — 163 miles after delivery. The vehicle remains inoperable after 60+ days. Stellantis STAR Case #XXX documents the following confirmed failures: collapsed lifters, failed camshaft, damaged rockers, and scored cylinder walls. Technician notes in the official STAR Case record state the engine oil contained metal contamination so severe it "looked like paint." Borescope imaging confirmed scoring on cylinder walls. Stellantis has thus far refused to authorize a full engine replacement, instead directing the servicing dealer to replace only individually identified failed components — despite the fact that metal contamination of this severity compromises the entire lubrication circuit, including both turbochargers. The servicing dealer (Farris Motor Company) has independently advocated to Stellantis for full engine replacement and agrees the partial repair is insufficient. I am reporting this failure to NHTSA to document the defect pattern and request investigation into whether other 2023 Wagoneer vehicles with the 3.0L Hurricane engine may be experiencing similar latent valvetrain failures. I am submitting this complaint regarding a serious mechanical and safety issue involving my 2019 BMW X3. On or around January 21, 2024, while driving from my home, a warning signal suddenly appeared on my dashboard related to the vehicle’s cooling system/engine. Due to safety concerns, I pulled into a nearby Wawa parking lot, which was the safest available place to stop at the time. I turned the

After inspection and diagnosis, I was informed that the vehicle required extensive repairs, including an engine replacement and cooling system repairs. At the time the coolant warning appeared, the replacement engine reportedly had approximately 59 miles on it. I believe a sudden engine or cooling system malfunction while operating a vehicle presents a serious safety concern for drivers, passengers, and others on the roadway. In addition, I experienced issues involving the shark fin antenna on the vehicle, which I understand may be connected to broader complaints involving BMW vehicles. I am requesting that NHTSA review this matter to determine whether these issues may indicate a broader defect or safety-related concern affecting other consumers. My 2011 Hyundai Sonata (VIN [XXX] ) was brought to Gateway Hyundai in [XXX] for the mandatory P1326/KSDS engine recall inspection. I had a confirmed appointment on XXX at 7:30 AM. The dealership acknowledged the P1326 code and that the KSDS test was required.Instead of performing the recall, the dealership refused to inspect the vehicle. They contacted a third party who was not in possession of t
... he vehicle and released the vehicle to that person without my authorization. The plates were removed and the vehicle was later sold without my consent. The recall was never performed and the engine defect was never inspected. This is a failure to perform a required safety recall and a violation of chain-of-custody procedures. A vehicle with a known engine failure risk was put back into public circulation without inspection. This creates a safety hazard for the public.I also suffered financial harm including $1,500 in towing, parts, labor, and storage fees. I have a signed tow invoice showing the VIN and the total amount paid.I am requesting that NHTSA investigate Gateway Hyundai’s handling of recall vehicles, review their authorization and release procedures, and take enforcement action if violations are found. I again have noticed intermittent pulsing at low to medium speeds. At roughly 1k-2k rpm the engine at or about 10-20 mph is pulsating or hesitating with an obvious pulse which you can see on the tachometer as well (bouncing.) It seems like there is either a computer, fuel flow or electric issue. Also, at speeds up to the 40 mph range, this is happening as well. Yesterday while towing a small 7'x12
... ' empty dual axle trailer I noticed it again, I occasionally use my trailer to haul small loads up to 4500lbs. This has happened before but this time I decided to notate this on your site, since reading about ongoing problems with the 2020 6.2L GMC Sierra 1500. I was aware of the other following years, 2021-2024? issues and that is why I purchased this 2020 year model. My truck will be available for inspection when the request is sent to me. Safety was not an issue, I ran my truck most of the day. I have not yet notified my Chevrolet dealer, Roger Dean, Cape Coral, FL. My truck has not yet been inspected for this issue. There were not any lights or warnings on display at time of incidents. Thank you, [XXX] [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

... issue, I became concerned that this failure appears very similar to the widely reported connecting rod bearing failures associated with Acura/Honda 3.5L V6 engines and Recall 23V-751. The symptoms in my vehicle closely match the issues described in the recall and related investigations, including: connecting rod bearing failure, spun bearing, internal engine damage, metal contamination, cylinder misfire, and complete engine failure. Additionally, I understand that the National Highway Traffic Safety Administration (NHTSA) has expanded investigations into similar engine failures affecting Acura and Honda vehicles outside the original recall VIN population.Read more
