I own a 2022 Nissan Rogue that has experienced multiple critical safety defects. The vehicle has repeatedly lost engine power without warning, exhibited delayed acceleration, and failed to engage gears—creating dangerous driving conditions that have put me at risk. Service History and Warranty Disputes: February 20, 2026: I brought the vehicle to the dealership after experiencing loss of engin
... e power. Diagnostics revealed that the EGR Pressure Sensor had internally shorted, causing a loss of communication diagnostic trouble code. The service department replaced the exhaust gas pressure sensor assembly. Despite the defect being a manufacturing fault and my purchase of a wraparound warranty, I was charged out of pocket for this repair. When I questioned the warranty coverage, I was transferred to consumer affairs and informed that only the dealership determines what is covered—an unsatisfactory response to a valid warranty claim. May 20, 2026: I returned to the dealership after the vehicle lost engine power again and lurched violently forward with jerking motions. The service department replaced the engine mounts. However, after only one day of driving, the same dangerous symptoms returned. May 29, 2026: I brought the vehicle back to the dealership for recurring loss of engine power, delayed acceleration, and violent lurching. The service department replaced the EGR system, which subsequently caused the spark plugs to warp. I was charged $400 for this additional repair, despite these being recurring manufacturing defects that should be covered under warranty. Conclusion: This vehicle demonstrates a pattern of dangerous, unresolved mechanical failures. The repeated issues—combined with warranty coverage disputes and repeated out-of-pocket charges for factory defects—indicate serious safety and reliability concerns with this 2022 Rogue.Read more

Supplemental Information to Existing NHTSA Complaint involving a 2017 Hyundai Sonata Plug-in Hybrid (VIN: [XXX] ) While traveling at approximately 70 MPH on a heavily traveled highway, the vehicle suddenly and completely lost propulsion without warning. The vehicle rapidly decelerated in traffic, creating a significant safety hazard for my family and surrounding motorists and requiring emergency
... maneuvering to reach the shoulder. The vehicle was towed to an authorized Hyundai dealership, where diagnostic testing identified multiple high-voltage battery system fault codes: • P0B68 • P0B6D • P0B72 The dealership diagnosed a high-voltage battery system failure requiring battery replacement. The vehicle is currently non-drivable. During my research, I found multiple owner reports involving Hyundai Sonata Hybrid and Plug-in Hybrid vehicles experiencing loss of power, hybrid system failures, limp mode conditions, and propulsion-related incidents. I also learned that Hyundai previously issued Recall 18V166000 involving 2016–2018 Sonata Plug-in Hybrid vehicles due to battery management system and voltage protection device concerns that could result in loss of power while driving. Dealer says does appply for my car. Although my vehicle’s fault codes may differ from those addressed in the recall, both involve battery-related failures capable of affecting propulsion and creating a highway safety risk. My concern extends beyond my individual vehicle. A high-voltage battery or battery management system failure that can cause a complete loss of propulsion at highway speed without warning presents a potentially serious safety risk. I respectfully request that NHTSA evaluate whether similar incidents involving Hyundai Sonata Hybrid and Plug-in Hybrid vehicles indicate a broader safety concern warranting further review. I can provide dealership diagnostic reports, repair estimates, towing records, photos, and videos upon request. 2020 Hyundai Sonata SEL Plus, VIN [XXX] . A/C system fails due to refrigerant leak that empties the system completely within 1-2 weeks of recharge. Leak cannot be located through standard external inspection. First noticed reduced cooling summer 2025. Driver's side loses cooling first, then passenger side fails days later — pattern consistent with evaporator core leak in dual-zone system. In Sep
... tember 2025, independent shop found system completely empty, recharged it, and attempted leak detection but could not locate source. Approximately two weeks later, system was empty again. Shop still unable to find leak. Symptom pattern — rapid total refrigerant loss, asymmetric failure (driver side before passenger), no externally detectable leak — points to evaporator core, which is enclosed in the HVAC housing behind the dashboard and invisible during standard inspection. Vehicle has 50,125 miles, still under the 60,000-mile warranty limit after 6+ years. Low-use vehicle averaging under 8,000 miles/year. 5-year calendar warranty expired approx. Jan 2025; issue first documented Sept 2025, only 8 months later. Defect was almost certainly present before warranty expiration. A class action investigation is underway (XXX) for premature A/C failures in 2018-2022 Sonata, Elantra, Palisade, Santa Fe, and Tucson vehicles. Loss of A/C in Texas climate poses risk of heat illness to occupants, especially children. This component should not fail at under 50,000 miles. I am submitting this complaint regarding a serious safety issue involving my Hyundai Sonata. While driving under normal conditions, my vehicle suddenly entered a low-power ("limp") mode without warning. The vehicle would not accelerate beyond approximately 30–35 mph, creating a potentially hazardous situation in traffic. At the same time, the check engine light began blinking. The sudden loss
... of power significantly reduced my ability to maintain traffic speed and safely maneuver the vehicle. This condition could have increased the risk of a rear-end collision or other accident. I brought the vehicle to an authorized Hyundai dealership (Fred Beans Honda Of Abington PH# 215-392-9020) for diagnosis. The dealership confirmed that the vehicle stored Diagnostic Trouble Code (DTC) P1326, related to the Knock Sensor Detection System (KSDS). According to the dealership, the vehicle entered limp mode as a result of this fault. The abrupt reduction in vehicle power and speed occurred without sufficient warning and created a safety concern. I am reporting this incident so that NHTSA can investigate whether this condition represents a broader safety defect affecting Hyundai Sonata vehicles equipped with the KSDS system. Vehicle Information: Make: Hyundai Model: Sonata Model Year: 2014 I request that NHTSA review this issue and evaluate whether additional action is warranted regarding vehicles experiencing DTC P1326 and associated limp-mode operation.Read more

My Tesla Cybertruck has experienced a failure of the Power Conversion System (PCS), which prevents the vehicle from charging at home using the installed residential charging equipment. Tesla diagnosed the issue and advised that the required replacement part is unavailable, with no estimated date for repair. As a result, I have been forced to rely exclusively on public Superchargers for several mon

While Tesla has provided free Supercharging, this does not restore the vehicle’s intended functionality and requires significant additional travel, waiting time, and disruption to my daily activities and business operations. The vehicle was purchased and represented as capable of convenient home charging, and the prolonged inability to repair a critical charging system due to parts unavailability substantially impairs its use, value, and practicality. I am submitting this complaint because Tesla has been unable to provide a repair timeline, and I am concerned that other owners may be experiencing similar failures and extended delays in obtaining repairs. The offered loaner vehicle is a Tesla Model Y, which is not comparable to a Cybertruck and does not meet the hauling and cargo needs associated with my business operations.Read more

Shortly after purchase of the vehicle on November 26, 2021, the vehicle began experiencing repeated DEF (Diesel Exhaust Fluid), SCR, and DPF system failures, accompanied by dashboard warning indicators, drivability instability, reduced power warnings, and eventual shutdown conditions. The most serious safety concern is that when the DEF/emissions warning activates, the vehicle may enter reduced-p
... ower mode and ultimately become non-operable, creating a significant roadway safety hazard. The vehicle has repeatedly experienced conditions where it could lose power, become unreliable in traffic, or shut down unexpectedly after emissions-related warnings appeared. The vehicle required repeated repair attempts beginning almost immediately after delivery, including diagnostics and repairs involving: DEF system malfunction SCR/DPF restriction codes Reduced power warnings Fuel pressure-related faults Emissions-system failures Drivability instability Suspension-related concerns Despite multiple repair attempts by authorized facilities and warranty-related interventions, the same categories of failures continued to recur. This condition creates a serious safety concern because the vehicle can become unreliable or effectively inoperable while being driven, particularly after activation of emissions-related warning systems. The repeated loss-of-power and shutdown-related behavior creates risk during normal roadway operation. I am requesting review of the recurring emissions-system failure pattern, drivability instability, and operational shutdown behavior associated with this vehicle.Read more

Vehicle: 2015 Audi Q3 | Engine: 2.0L TFSI 4-Cylinder Turbo Component: Engine Complaint Description: While driving on the interstate at highway speed, the red oil pressure warning light illuminated suddenly and without prior warning. The standard low oil level warning light did NOT illuminate prior to the failure. Oil level was verified as correct and within specification at the time of failure.
... The vehicle suffered a complete and catastrophic engine failure due to oil starvation and rod knock, despite a verified proper oil level and oil pressure. The engine requires full replacement. This failure occurred at only 3,432 miles since the most recent oil change. The vehicle has a documented history dating back to 2021 of recurring engine oil consumption concerns, fuel contamination of engine oil, and high-pressure fuel pump failure — all serviced at authorized dealerships. A multipoint inspection performed approximately four months prior to this failure returned a fully satisfactory result with no engine concerns flagged. This is believed to be related to the known piston ring defect in Audi/Volkswagen 2.0L TFSI engines documented in federal class action litigation (Rieger v. Volkswagen Group of America, Case No. 1:21-cv-10546-NLH-EAP), which explicitly named the 2012–2017 Audi Q3 as an affected vehicle. The engine failure occurred without warning while at highway speed, posing a significant and immediate safety risk to the driver and other motorists.Read more

The vehicle is experiencing a dangerous hesitation during acceleration at low speeds, immediately followed by a sudden, violent forward lunging motion. When attempting to accelerate from a complete stop, or when rolling at low speeds (such as yielding at a roundabout, turning through an intersection, or merging into slow-moving traffic), the vehicle suffers from a distinct **lag or complete fail
... ure to accelerate** for several critical seconds. In an attempt to get the vehicle to move out of harm's way, the driver naturally applies more pressure to the gas pedal. The vehicle's transmission then randomly responds after a few seconds with a violent lunge forward, spinning the tires and causing the vehicle to surge unpredictably. This unpredictable behavior creates an immediate and severe safety risk to the occupants of my vehicle and others on the road: 1. Intersection Collisions: The sudden lag leaves the vehicle stranded in oncoming traffic during turns, while the subsequent lunge threatens to overshoot the intended lane. 2. Pedestrian Hazards: The sudden, uncontrollable lunging makes low-speed maneuvers in school zones, crosswalks, and parking lots incredibly hazardous. 3. Drivers behind behind me do not expect a vehicle to suddenly stop accelerating after it has begun to move forward. I have taken the vehicle to the dealership we're we were able to reproduce the issue and Honda would not offer a fix. I thought I was going to get my entire family killed the other day when I pulled out at a stop the van hesitated in the middle of the highway for a few seconds before lunging forward. The vehicle has repeatedly lost electrical power while driving. During one incident at approximately 50 mph, the vehicle’s electronics shut off and power steering assist stuttered/lost assist for several seconds, causing reduced vehicle control. During another incident while turning/in a turning lane, the engine and electronics shut off and the vehicle had to be restarted. Afterward, the vehicle
... struggled to restart and displayed a warning to drive cautiously due to power steering loss. The vehicle also has recurring hot-start/no-start symptoms and electrical loss after shutting the engine off. Based on my research, GM Bulletin 19-NA-052 appears to describe a related condition on 2016–2019 Chevrolet Camaro 6.2L vehicles where circuit 1 may short to ground at the starter heat shield, causing loss of electrical power or failure to start, with repair involving the starter solenoid cable. This appears to be a safety-related manufacturing/design defect because the vehicle can lose electrical power and steering assist while in motion. At that point I had call a towing service and arranged to have the vehicle taken to Autonation Jeep Chrysler the next morning where I had purchased the vehicle. This was a newly purchased vehicle and only had a total of 78 miles on it. Surprisingly the next morning I went into the garage to check on what was happening with the vehicle and to my surprise the vehicle had stopped running. I then disconnected The 12 Volt battery and left it disconnected for an hour or two. After this time I reconnected the battery. I again attempted to start the vehicle and surprisingly the vehicle started up and all functions in the car seem to be performing normally. At this point I drove the car about a mile to the Autonation Jeep Chrysler dealership and had conversation with service technician. Surprisingly he told me that Jeep has had this problem with other vehicles and the solution is to disconnect the 12 Volt battery for 20 or 30 minutes and then reconnect the battery. At that point the car seems to reset itself and it will run normally. End This car of mine only had 78 miles on it This is totally unacceptable. Furthermore this seems to be a problem with other Jeep vehicles. Model: Sierra Denali Ultimate Engine: 6.2L V8 VIN: [XXX] Mileage at Most Recent Incident: Approximately 52,000 miles Safety Complaint Summary I am submitting this complaint to report a recurring loss-of-propulsion and engine power event involving my 2023 GMC Sierra Denali Ultimate equipped with the 6.2L V8 engine. This vehicle has experienced two separate incidents in which it suddenly lost power
... while being operated at highway speeds. During both events, the vehicle unexpectedly lost propulsion without warning and without any apparent driver input or mechanical indication beforehand. No warning lights, check-engine lights, reduced-engine-power messages, service transmission messages, or other warning indicators were displayed prior to the failure. The vehicle simply lost power while traveling in traffic. During both occurrences, I was forced to safely maneuver the vehicle to the shoulder of the roadway while rapidly losing speed among surrounding traffic. Once stopped, the vehicle would not resume normal operation until it was restarted. After restarting, the vehicle appeared to operate normally; however, the fact that this condition has now occurred twice raises significant concerns regarding the reliability and safety of the vehicle. Safety Risk This condition presents a significant and immediate safety risk because sudden loss of propulsion during highway operation can leave the driver unable to maintain speed, safely merge, maneuver, or avoid surrounding traffic. The unexpected reduction in speed creates the potential for rear-end collisions, loss of vehicle control, or other traffic accidents, particularly when operating on high-speed roadways. Because the vehicle provided no warning before losing power, there is little opportunity for a driver to take preventive action. Car completely turns off when I’m stopped and idle stop has engaged. Seems to happen when the AC is running hard. Sometimes, the car turns back on by itself in seconds. Other times, I have to turn the car back on myself by pressing the “engine on” button, and I struggle to get the car to turn back on. Very dangerous to be obstructing traffic at busy intersections or on a busy highway when
... stopped while another car ahead is making a turn. I’m scared to drive this car with my children in it. It is a minivan! I understand this is a known issue under investigation by NHTSA, but I’m struggling to get my local Honda dealer to acknowledge and address it. This issue happens at least 1x a day when I’m running the AC.Read more

I am writing regarding my BMW X5 xDrive40i, model year 2019, engine code B58TU. The vehicle has been serviced at authorized BMW dealerships. For this engine type B58TU especially years 2018-2021, repeated oil pump failures have been reported due to a known design issue: an internal plastic component can develop cracks, potentially leading to a complete oil pump failure. BMW silently replaced the
... oil pump with the updated version by keeping the same part number but featuring metal internals. Once there is a fail on the oil pumps with plastic components, engine loses access to get the oil an immediate stop is needed. This creates a huge safety concern cause: 1) If you are on a highway at highway speeds it might not be possible to move to the shoulder safely with no engine power or even worse: with a locked engine. 2) If there is a maintenance on the road with no shoulder access or if this is a bridge where there is no shoulder, it creates a huge risk to safely move the vehicle. An unavoidable sudden stop can create a huge accident risk for everybody. Of course any car can get a engine problem on those occasions but this is a very known and very common problem for BMW cars equipped with B58TU engine especially years 2018-2021. BMW is also definitely aware of that and this safety concern cause they updated the design silently. But, when contacted with BMW NA, they refused to help, refused to admit there is a design problem. They also refused to change the oil pump with the updated oil pump even with my proposal of goodwill pricing.Read more

2021 Mazda CX‑5 (VIN: [XXX] ). First low‑oil warning at 17,800 miles. ([XXX]) Mazda dealership in La Crosse, WI performed an oil change and fully refilled the oil. Second low‑oil warning at 18,700 miles — only 900 miles later. ([XXX]) This indicates rapid, abnormal oil loss consistent with the known valve‑stem‑seal defect for this model year. Sudden low‑oil conditions pose a safety

While driving at a speed just under 45 mph, my Mazda CX-70 suddenly displayed and chimed “Hybrid System Malfunction. Have Vehicle Inspected.“. This warning startled me because I am familiar with issues since I had briefly owned a CX-90 PHEV the first year this model came out and it had the same exact problem. I discovered that this issue carries serious safety concerns and ended up convincing
... the dealer to swap the vehicle for a CX-90 inline 6 gas only model after hours of pleading. I ended up signing up for another lease a couple of years later with this current CX-70 PHEV, as I was told that the “bugs” have pretty much been worked out so I would give it another try. This warning continued to stay on as I drove for a few minutes so I immediately took the car straight to the Mazda dealership service department where it was kept 24 hours. The dealer said that the issue resolved itself - something to do with not driving the vehicle interchangeably enough between gas and EV mode. The photo was taken once I slowed down to stop at an intersection since it took me 15 minutes to drive to the dealer.Read more

2019 Ford Escape with a documented history of powertrain failures. White smoke on cold start and engine stalling began around 48,000 miles; the dealer noted this engine is known for coolant intrusion. At about 62,000 miles the engine short block was replaced under Ford Field Service Action 21N12 / TSB 22-2322. Smoke, shaking, and stalling continued, and the engine intermittently cuts out when shif
... ting into reverse and must be restarted. At about 87,000 miles the transmission failed and the full transmission assembly was replaced (work order Jan 2026, vehicle returned end of March 2026). The owner repeatedly told the Ford dealer the vehicle felt unsafe. On [XXX] the owner reported renewed exhaust smoke, an illuminated engine light, and a clicking sound at startup and asked to bring it in; instead of inspecting it, the dealer advised taking the vehicle straight to CarMax. About 9 days later, on [XXX], while driving in Los Angeles about 60 miles from home with young children aboard, the transmission would not stay in gear and the engine warning lights began blinking. The owner called the Ford dealership for guidance but no one could advise and the service desk would not answer. After consulting a mechanic, the family agreed it was unsafe to keep driving given the noises and pulled over in Anaheim, about 30 miles from home, at a mechanic shop to read the codes. A paid tow (~$600) was required. Loss of gear engagement and stalling in traffic with children aboard is a serious crash and stranding hazard. The scan returned codes P0012 and P0014 (camshaft timing, stored as permanent), P0300 (random/multiple misfire), and P0301, P0302, P0304 (cylinder misfires); the dash showed "Auto Start/Stop not ready - Stall Alert - Restart Engine." The defect has been reproduced and confirmed repeatedly by the Ford dealer, which performed the engine and transmission replacements. A repurchase request was denied. 2022 Audi Q5 (VIN: [XXX] ). Engine lubrication system failure — persistent abnormal oil consumption across two separate engines over 14 months. Beginning March 2025, the low engine oil warning light began activating repeatedly, sometimes after as few as 300 miles. AutoNation Audi Bellevue confirmed 1 quart low and initiated oil consumption tests. Their technician noted in writing that oil found
... externally was "not enough to cause a 1 quart low oil light" — meaning the root cause was never identified. A PCV valve replacement was attempted July 2025; the warning returned after only 300 miles — same as before the fix. Audi of America approved a full engine replacement in September 2025, completed December 2025. Within weeks of pickup in January 2026, the low oil warning activated again on the brand new engine, and again after 4,000 miles in April 2026. The defect has persisted across two engines despite multiple repair attempts. Audi now claims consumption is "within spec" at 0.16 qt/1,000 miles but has not explained why the warning light keeps activating. Safety risk: unpredictable oil loss risks sudden engine seizure at highway speeds with insufficient warning to stop safely. Confirmed and inspected by AutoNation Audi Bellevue and Audi of America. Vehicle available for inspection. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

10th Gen Honda Accord has a major engine design flaw allowing coolant to pass through the gasket mixing with fuel in the cylinders causing premature head gasket repair which also puts the injectors under immense stress due to high compression. While driving my 2019 Honda accord to the dealership for inspection. All the lights started flashing on the dash. Highlighting - Critical failure detected
... please stop driving when safe. After a few mins the car’s infotainment system shut off no electrical activity and the car shut off mid drive. Luckily, it was in the morning and not during peak traffic congestion. Had I been going on the expressway I could’ve harmed myself, or someone else. This is highly unacceptable that a car with only 96k miles has a serious engine issue like this. I have maintained the oil changes at the required intervals, changed the fuel pump under recall, tires, brakes etc. After towing to the dealership. Diagnostics were performed and it was confirmed that the coolant was empty and radiator low. Which then prompted the dealer to perform a borescope test for the cylinders and it was found that cylinder 3 had a misfire and was leaking coolant into the cylinders causing premature wear on the gasket as well. The car is out of warranty and the service advisor said to me that they had another Honda just like mine with the same issue. This isn’t an isolated incident and many owners with the 10th gens have the same exact issue nationwide and Honda turns a blind eye and expects us to pay over 6,000 for the repair it’s a total scam! They are making money hand over fist without any recall and just scamming their customers. This is truly unacceptable and should be investigated immediately and Honda reprimanded!Read more

I bought the truck brand new in July 2022. May 2, 2026 I hit 46,461 miles and started hearing a clanking sound and caused vibration in low speeds. After 30 MPH the vibration went away. I took it straight to Toyota dealer and with hardly any questions asked they are rebuilding the whole engine and still have it since today May 29,2026.
