What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE97AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is a
... vailable for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset by standard non-dealership methods like unplugging the battery.Read more

While slowing to a near-complete stop and preparing to shift into Park at a low-speed private property location, the vehicle suddenly and violently accelerated without any driver input. The operator, a [XXX] experienced driver, had no time to react. The vehicle travelled approximately 200-300 meters uncontrolled, narrowly missed a grounds keeper operating a tractor on the paved road, left the pav
... ed road, drove over a fallen log and between trees, and stopped autonomously just before hitting a tree directly ahead. The driver did not brake at any point. The vehicle stopped itself. Physical evidence at the scene shows deep tire marks dug into soft ground consistent with autonomous emergency braking activation directed straight toward the tree. Four witnesses were present including a bystander who nearly was struck by the vehicle. A formal safety case has been opened with the manufacturer's corporate office. Federal safety complaint also filed with Transport Canada. A nearly identical sudden unintended acceleration incident involving a 2023 Porsche Macan while parking is already on record with NHTSA. This appears to represent a pattern on the same platform and warrants formal investigation. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2022 Nissan Rogue with approx 71,139 miles on. While driving on the freeway at 65mph, the vehicle suddenly lost power and became disabled. Prior to the failure, the vehicle did not dispaly an overheating warning, coolant warning, or other indication the engine was overheating or experiencing coolant loss. The vehicle was being driven normally and was on the way to routine maintenance and f
... uel stop when the failure occurred. An independent repair facility diagnosed coolant intrusion into the combustion chamber and determined the engine had overheated and required replacement. The vehicle's sudden loss of propulsion occurred without warning creating a safety hazard for myself and my daughter as I was forced to coast to a stop in the Gore Area of the freeway just as you entered the [XXX] from [XXX] due to the inability to maintain speed on the freeway. This was the safest area for us to park but it was terrifying as cars zoomed past us from both sides. I am concerned that the engine failed without adequate warning to the driver and that similar complaints have been noted by several others describing the same issue with the same year, make and model as my car. Multiple reports have said it occurred while on the freeway, making this a greater concern for safety. I took my car to Nissan who stated they see the coolant intrusion, but that it was not covered under recall. I asked what caused the internal leak, to which they replied that would require them to take apart the engine to examine further at my cost. I advised the issue with the car not indicating it was even out of coolant to cause it to overheat was concerning because even if I replace the engine, if the electrical failure isn't addressed I could run into the same issue and this time actually get into an accident that could cause death or serious injury. My car is still at Nissan Mission Hills Dealership and available for inspection upon request. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am submitting this complaint regarding an apparent head gasket failure involving my 2022 Honda Accord Sport 1.5T equipped with Honda's 1.5-liter turbocharged engine. At approximately 143,000 miles, I began experiencing coolant loss without any visible external leak. I also noticed rough operation and shaking during startup. Concerned about the symptoms, I consulted a Honda specialist who advised
... that the vehicle is exhibiting signs consistent with a failing head gasket and will likely require head gasket replacement. This vehicle has been meticulously maintained and serviced throughout ownership. I retain maintenance records and have taken exceptional care of the vehicle. There has been no abuse, neglect, overheating event, or failure to maintain the vehicle. Through my research, I have found numerous reports from owners of Honda vehicles equipped with the 1.5-liter turbocharged engine describing similar symptoms, including coolant loss, startup misfires, rough cold starts, coolant intrusion into cylinders, and premature head gasket failures. The consistency of these reports suggests a possible design or manufacturing defect affecting the durability of the engine. A failing head gasket can lead to loss of engine power, engine damage, misfires, overheating, and potentially unsafe driving conditions. Given the volume of owner complaints involving Honda's 1.5-liter turbocharged engines, I request that NHTSA investigate whether a safety-related defect exists and whether corrective action, including a recall or service campaign, is warranted.Read more

I then pulled onto the shoulder after nearly being rear ended by another vehicle approaching on the left. The vehicle would not react to the gas pedal. I pulled onto the shoulder and turned the vehicle off. Tried to restart but again nothing but mutliple lights and warnings. I checked my app for Chevrolet and it stated that the vehicle needed engine and transmission system service. Variable valve timing system needs serviced. Ingition system needed serviced. The onstar diagnostic alert indicated P0016 was detected. I opened the hood to inspect and the coolant overflow was spewing coolant. I checked the oil level at that point and it was just a little below full. I had taken this vehicle into the dealership two other times stating that the vehicle had stalled before on the highway but not to this degree. The other two times it stalled when accelerating it would allow me to pull over and restart. But both of those times I never had any flashing lights or diagnostic codes. Vehicle entered limp mode at highway speed (approximately 70 mph) on [XXX] freeway in Washington State, it felt like I had rear ended a vehicle it shut down so hard. This was due to mis diagnosed inspection and service repair from a local Subaru Dealer. Vehicle lost power rapidly, creating a dangerous safety situation. Root cause traced to TCV/PCV system defects previously documented by authoriz
... ed Subaru dealers. Previous Subaru authorized dealer replaced a torn PCV hose but did not diagnose the problem causing the tear. The dash warning lights were still illuminated after the diagnostic and repair when vehicle was picked up after service. I have attached a more detailed overview of the dangerous situation. The car finally lost power on the freeway while I was in the fast lane pushing 70 miles an hour. I was in the fast lane and with no power I had to cut across both lanes of traffic and get to the shoulder. I was 45 minutes from my home and it was dark, raining and the Subaru would not start. I called a mobile mechanic I found online who came out to the freeway, roadside, where I was stranded. He worked on the car for a couple of hours in the rain. He got the car to start but it would not stay running. I had the car towed to a friend's house in Vancouver, WA about 6 miles from where it had broken down. The same mobile mechanic came out and changed out one of the oil actuators and cleaned and replaced two of the OCVs then flushed the oil system and put new oil in the car. Cleared all the codes. The car starts, idles and will drive up to about 1500 rpm’s but any more than 1500 rpm’s and it dies. It's in limp mode and will move past those rpm’s. The warning lights and the PCV house splitting must have been signs of something else being wrong. That is the discussion I had with Wilsonville Subaru. I am filing this complaint regarding Honda's reimbursement program associated with the piston ring warranty extension related to engine misfire issues on certain vehicles. Prior to Honda issuing the warranty extension, I paid out of pocket to have the repair completed at an authorized Honda dealership. After Honda announced the warranty extension and reimbursement program, I submitted a reimburse

Since submitting the claim, I have contacted Honda numerous times seeking an update. On multiple occasions, Honda representatives advised me that the reimbursement had been approved and was simply awaiting issuance of payment. However, no reimbursement has been received. Due to the lack of progress and conflicting information, I filed a complaint with my state's Attorney General's Office. In response to that complaint, Honda stated that the reimbursement claim was still under review, which directly contradicts what I had previously been told by Honda representatives. I have complied with all documentation requirements and have made repeated attempts to obtain a clear answer regarding the status of my claim. Despite this, I have not received reimbursement, a definitive status update, or a reasonable explanation for why a reimbursement claim related to a known warranty extension issue has remained unresolved for nearly a year. The 2.0L EcoBoost engine in my 2018 Ford Edge Titanium experienced head gasket failure due to Ford’s known cylinder channel design defect present in 2015–2019 EcoBoost 2.0 engines. Ford scored channels between cylinders in an attempt to improve cooling, which allows coolant to degrade the head gasket and mix with engine oil. Symptoms included white smoke from the exhaust, coolant loss, rough i
... dle, and a check engine light prior to failure. The coolant-oil contamination caused engine performance degradation and risk of sudden failure while driving, endangering myself and other motorists. The engine drained all coolant unbeknownst to me while driving, causing the car to overheat and causing damage to the head gasket. Car had to be pulled over and towed to a shop. Head gasket was repaired, but the issue continued and more coolant was found in the engine oil in 2026. Coolant constantly has to be replenished after driving. The failure was confirmed by an independent mechanic. The vehicle has not been inspected by Ford or NHTSA. Ford has not issued a recall for this known defect despite widespread reports from 2015–2019 Edge, Fusion, and other EcoBoost 2.0 owners experiencing identical failures. I am filing this complaint to contribute to the pattern of evidence needed to compel a formal NHTSA investigation and potential recall action.Read more

The first time I heard the engine noise was on [XXX]. I brought the vehicle to the Schaumburg Muller Acura dealership and dropped it off on May 20. On May 21, the dealership performed an initial inspection and informed me that they needed authorization to further disassemble the engine in order to determine the cause of the problem. I provided that authorization, and by May 22 the engine had been
... torn down to the point where the failure could be identified. On May 22, I spoke with my service advisor, Ken, who informed me that the connecting rod bearings were worn and that the engine would need to be replaced. I was sent photographs, a video, and an estimate for the repair totaling $12,943.83. While I was shocked by the cost, Ken advised me that the dealership would be contacting Acura to request goodwill assistance on my behalf. Because this occurred immediately before the Memorial Day weekend, several days passed before I received an update. On May 30, a different service advisor informed me that Acura had approved $8,000 in goodwill assistance. During that conversation, he asked me, “The original estimate was around $16,000, right?” I knew that was incorrect, as the estimate I had received was $12,943.83. Concerned by the discrepancy, I contacted Acura Customer Relations on June 1 and opened a case for review. Later that same day, I spoke with Ken and provided him with the case number. During that conversation, he informed me that the original estimate had been incorrect because certain parts and/or labor had allegedly been omitted. I was then told that the actual repair cost would be approximately $18,000 to $19,000 plus tax. While I can understand minor adjustments to an estimate, a difference of several hundred dollars or even $1,000 might be reasonable. However, an increase of approximately $6,000 from the original estimate is extremely concerning and raises serious questions about the accuracy of the repair assessment and cost estimate. The vehicle suffered a sudden, severe engine malfunction/misfire while in operation, presenting a serious safety hazard on the road. Following the incident, the vehicle was taken to an authorized dealership service center (Repair Order #XXX), where technicians verbally confirmed that the engine requires a complete replacement due to internal mechanical failure. The catastrophic mechanical failure
... of the engine directly caused consequential damage to the emissions system, entirely clogging the catalytic converter with unburned fuel/debris. Despite the clear link to the known, systemic defects inherent to the Theta II engine line—which is subject to active federal consent orders, safety recalls, and lifetime warranty extensions—both the dealership and Kia Corporate are refusing to honor the 8-year/80,000-mile Federal Emissions Defect Warranty. The manufacturer is attempting to use the catastrophic failure of their own defective, recalled engine line to evade their legal environmental and safety warranty obligations, leaving the consumer stranded without alternative transportation. This represents a systemic effort by the manufacturer to deflect liability for documented powertrain safety failures onto individual consumers. Post-Recall Low Oil Warning + Performance Issues on 2021 GMC Yukon Denali – Campaign 25V274000 Vehicle: 2021 GMC Yukon Denali, 6.2L V8 Recall: 25V274000 Owner: One owner since new (purchased 2021) I had recall campaign 25V274000 performed on my vehicle. This was the first low oil warning I have ever received since purchasing the SUV new in 2021. I have always followed the vehicle’s oil life mo
... nitor with no prior issues. The change oil light appeared at roughly 7,500 miles after the recall service (when I already had a service appointment scheduled). About one week later, the low oil warning came on. I had the vehicle towed to the dealership and did not drive it after the warning. Since the recall service, I have also experienced performance issues including noticeable stuttering that was really evident before I towed it in. The dealership initially agreed to a consumption test but later acted like it was not needed, said everything was fine, and were dismissive — blaming me for not following their sticker. I am concerned the recall remedy is not fully effective. Please investigate this matter. The connecting rod is failing. Loud knocking is happening just as outlined in the recall on other 2016 Pilots. Poses a great safety risk as failure will result in total engine destruction.

The vehicle has an unresolved electrical/propulsion failure involving sudden shutdowns while driving and CAN C bus/lost-communication faults. Since the high-voltage battery was replaced through a recall-related repair, the vehicle has had repeated shutdown/loss-of-communication events, occurring dozens of times. In the most serious incidents, while driving at speed, the dash flickered, windshield
... wipers activated unexpectedly, headlights turned off, turn signals would not work, and steering locked or became difficult to control. The vehicle then completely shut off while still moving. After shutdown, the hazard lights would not work. One documented incident occurred on the interstate at about 70 mph. This puts occupants and others at risk because the vehicle can suddenly lose motive power and critical electrical/control functions at the same time, including lighting, signals, hazards, instrument display, and steering/control assistance. The vehicle has been inspected multiple times by a Chrysler dealership. Dealer records documented multiple modules losing communication on the CAN C bus. The dealer replaced the shifter module as a possible contributor, but the problem continued. The dealer later corrected loose pin tension at a communication/star connector, but the vehicle continued to shut down and later scan reports still showed widespread active/stored CAN and lost-communication faults across powertrain, hybrid control, body control, braking/ABS, steering, and shifter-related systems. The vehicle is available for inspection upon request. The problem has been confirmed by dealer scan reports and repair records, but the root cause remains unresolved. The manufacturer has been contacted through customer care and repair/scan documentation was submitted. The exact root cause is UNKNOWN, but dealer findings indicate an unresolved major electrical/CAN communication problem possibly involving the harness, connector network, or communication system.Read more

My 2022 Volkswagen Tiguan (VIN: [XXX] ) is experiencing excessive engine oil consumption. The vehicle requires oil replenishment every less than 1,000 miles, which is far beyond what any reasonable consumer would expect from a properly functioning engine. I reported this issue to the Volkswagen dealership while the vehicle was still under the manufacturer’s warranty. The dealership stated that t
... his level of oil consumption is “normal” and only added oil without performing any diagnostic testing or repairs. No repair order documenting a defect was issued, and no corrective action was taken. This problem is consistent with the known defect in the EA888 2.0L TSI engine, specifically the defective piston rings that fail to create a proper seal, allowing oil to leak into the combustion chamber and burn off during operation. A class action lawsuit has been filed against Volkswagen regarding this exact defect in 2022–2023 Tiguan models. The excessive oil consumption poses a serious safety risk. If oil levels drop critically low without warning, the engine could suffer catastrophic failure, stall unexpectedly, or overheat while driving — all of which significantly increase the risk of a crash. Despite this being a warranty-covered vehicle and a known manufacturing defect, Volkswagen and its authorized dealership have refused to repair the issue or acknowledge it as a defect. I am requesting that NHTSA open a formal investigation into this matter and consider a safety recall for all affected 2022–2023 Volkswagen Tiguan vehicles. I purchased this vehicle used in 2024, and since I bought it have had a variety of issues. I have already invested over 5k in repairs. I have replaced the head gasket and now the water pump twice. This vehicle will not stop overheating. Ford has a known issue with these engines and should correct these issues. They are making working families go broke and we still owed 8k on the car loan.

The contact stated that while driving in the rain, when passing another vehicle, the vehicle began shuddering and experienced a loss of power. The vehicle's original engine suffered severe oil consumption and a blown head gasket, which contaminated the exhaust system. The Kia dealership replaced the engine under warranty but refused to replace the damaged catalytic converter claiming it wasn’t a part of the warranty even though it was consequently damaged. The dealership explicitly stated the vehicle should not be started or driven due
... to the blocked exhaust, but then did repair and release after I pushed for the catalytic converter replacement. They said I would have to follow up with Kia corporate. They released the unsafe vehicle anyway and I have been following up with Kia customer service for 2 months to request a CAT replacement. While driving, the blocked exhaust caused a severe rattling noise, immediate engine overheating, and complete mechanical engine seizure. The engine completely cut power and locked up while in motion, creating an immediate crash hazard on the roadway. The vehicle is now completely inoperable and will not turn over. This is a severe safety defect where a known blocked exhaust system directly caused catastrophic engine failure while operating.Read more

2019 Flex , Odometer reading 58,190. Coolant seen on lower part of engine along with low coolant level. Took to dealership who confirmed the water pump failed and was leaking. We were lucky it was caught early before coolant leaked into the engine, causing the engine to stop while driving and permanently damage engine. There are no warnings for this when it happens (coolant leaking into engine),we
... were just fortunate to notice low coolant. Because there are no coolant warnings, it can lead to catastrophic engine damage while operating vehicle. There are no guarantees that this will not happen again, only parts are under two year warranty. After research, Ford knew this was an issue with this engine when we bought it, not only on the Flex but same engine on other models, yet continued to sell them. Cost $3,391.71 to repair. Because it is over 5 year warranty no help offered, yet is under 60K miles. There are other complaints regarding this same issue yet Ford continues to not acknowledge faulty design of engine. 2) still waiting for rrecall/repair of backup camera, 3) also waiting-recall/repair for plastic pillar covers. Both of these are under a recall for over a year. NHTSA has other complaints of this from consumers.Read more

Coolant intrusion in the engine which needs replacement. Car was misfiring and white smoke coming out took the auto shop for service. Its about $9400 to fix the car which is now more than the car is worth. Dealership never warned me that this was a potential issue.
