The suspected failure involves the engine and emissions system. The vehicle has a history of engine-related issues and previously received engine-related repairs. The current engine has developed excessive oil consumption. This condition has resulted in fouled spark plugs and a catalytic converter becoming severely restricted/clogged from oil contamination, causing significant loss of engine performance. A dealership inspected the vehicle and documented the oil-burning complaint and catalytic converter-related fault. The dealership performed diagnostic work, including spark plug replacement, but the problem continued. An independent repair facility also inspected the vehicle and confirmed excessive oil consumption and catalytic converter restriction. The vehicle remains available for inspection. This condition presents a safety concern because the vehicle experienced a sudden loss of power while traveling at highway speeds and was unable to maintain normal traffic speed.Read more

WHAT HAPPENED / SAFETY RISK: On [XXX], I was driving at approximately 65 miles per hour on [XXX] southbound near Bath, New York, with four occupants aboard, when the engine failed suddenly and without warning — no warning lamps, messages, or symptoms preceded it. The vehicle immediately lost all motive power at highway speed. I was forced off the roadway and through brush and debris to bring the vehicle to a stop. The sudden total loss of power at highway speed created an immediate loss-of-control hazard to my occupants and to surrounding traffic. CONFIRMED BY OTHERS: The tow was dispatched through a 911 emergency call. A New York State Trooper was at the scene and observed the debris trail. The towing company's invoice (T&R Towing and Service, Call #[XXX], [XXX]) records "Break Down," winch recovery, vehicle not drivable, and the tow operator's handwritten condition description: "Motor go Boom." The vehicle is now in the possession of a Hyundai dealership (Niagara Hyundai), and Hyundai Motor America has an open case (#[XXX]); however, Hyundai has refused to inspect or diagnose the engine unless I first pay to repair unrelated damage, so the manufacturer has NOT yet inspected the failure — leaving this safety defect undiagnosed by Hyundai. I had to call a tow truck to get the vehicle towed to the local Ford dealership. 3 hours sitting on the side of the road and did not get picked up for 3 hours in the dark. Ford ran a diagnostic test the next day and informed me in writing that the engine needed to be replaced due to coolant leakage into the engine block. I would also need the cabin coolant heater pump and the sparkplugs replaced. The total would be over $8,200. I had the vehicle in for service three months ago due to a check engine light and did the requested repair. The informed me now that it was for a different issue. My 2018 Honda HR-V had the federally-mandated Safety Recall 24-015 (Fuel Pump Motor replacement) performed on August 16, 2025 at Honda Westborough, Westborough, MA. The vehicle had zero EVAP-related issues or warning lights before that recall work. Approximately 9 months later, in May 2026, a parking alert and check engine light appeared on the dash. I brought the vehicle back to the same dealers

The technician retrieved diagnostic codes P0455 (large EVAP leak) and P0456 (small EVAP leak), performed a smoke test, and confirmed the leak was coming specifically from the FUEL PUMP MOTOR AREA - the exact same location where the recall work had been performed. The technician's own notes state: "confirmed leak coming from the fuel pump motor area." After replacing the O-ring and lock ring, the leak persisted at the same spot. A new fuel pump assembly (part# 17045-T7X-A02) was ultimately installed to resolve it. Honda Westborough charged me $1,676.61 for this repair, including $870.50 in labor and $724.83 in parts. When I contacted American Honda Customer Relations, they declined to reimburse me, stating the repair was unrelated to the recall. I strongly dispute this. The recall involved opening the fuel pump housing to replace the motor. The subsequent EVAP leak was confirmed by smoke test to originate at that exact location. No such issues existed before the recall. This is a direct causal connection. Other Honda owners on public forums (HR-V Forum, CivicX Forum) have reported identical P0455/P0456 EVAP leaks appearing within days or months of the same recall being performed, suggesting a systemic issue with the recall repair procedure, specifically, improper resealing of the fuel pump housing after motor replacement. I am requesting: 1. An investigation into whether the Recall 24-015 repair procedure is causing downstream EVAP system failures. 2. This data does not match the events I experienced. I have formally requested full downloads from all other modules (Powertrain/PCM, ABS/EBCM, BCM, etc.) from both the insurance company (ESIS) and GM, but they have only provided the single SDM report. Since November 2025, this vehicle has experienced repeated loss of power, severe shaking during acceleration, jerking between gear shifts, abnormal humming/grinding noises, and multiple check engine light events. ... and would not respond normally to throttle input. During one occurrence, the vehicle had difficulty accelerating beyond approximately 55 mph while on the expressway. The shaking became severe enough that I feared mechanical failure could occur while driving. I was forced to pull off to the shoulder of the highway because I no longer felt it was safe. The inability to maintain speed and accelerate normally created a safety risk for both myself and surrounding traffic. The vehicle has been returned to the dealership six times for the same recurring issue. Repairs have included replacement of the ECU, catalytic converter, ignition components, fuel injectors, and other engine related components. Despite these repairs, the problem continues to return. The condition has been repoduced and documented by the dealership, and a manufacturer field technician was involved in the most recent diagnosis process. Most recently, after the vehicle was inspected and believed to be repaired, the check engine light illuminated again while the service manager was moving my vehicle to be washed inside of their garage, as I waited (today, June 11th 2026). The recurring loss of power, repeated check engine light events, and inability to permanently correct the defect raise significant safety concerns while operating the vehicle. ... for approximately five minutes and would not shut off as expected. No warning light, message, or other dashboard indication appeared before or during the event. This was unusual behavior compared with normal operation. Safety risk: The unexpected fan operation created concern that the cooling system was malfunctioning and that the vehicle could overheat, experience an electrical problem, or become unreliable while driving. A cooling system or electrical failure while in traffic could put me and other drivers at risk. Reproduction or confirmation: The issue occurred several time and was confirmed by a Chevrolet dealer, and was told that my warranty did not cover the issue. I took it to outside mechanic to fix the issue as Chevrolet wanted to charge over $1500 for the issue. No warning lamps, messages, overheating indicators, or other symptoms appeared before the fan remained on. The problem occurred suddenly back in 2025. Issue 2: Ignition Key Gets Stuck and Cannot Be Removed Ignition lock cylinder, ignition switch, transmission shift-interlock system, park-position switch, or steering column lock mechanism. The vehicle is available for inspection upon request. The ignition key intermittently becomes stuck and cannot be removed from the ignition. The problem occurs after parking and turning the vehicle off, and I sometimes need to move the shifter or steering wheel repeatedly before the key can be removed. A stuck ignition key can prevent me from properly shutting down and securing the vehicle. It may also indicate a failure in the ignition or shift-interlock system.Read more

Repairs included CVT diagnostics ($95.00), CVT transmission service ($386.39), water pump replacement ($610.65), and a cooling system service ($259.95) — all cascading failures tied to the engine and transmission defect. On November 13, 2025, my vehicle was towed from my residence due to complete engine failure. A full engine replacement was required at an additional cost of approximately $5,958.22, plus a diagnostic fee of $95.00. Total out-of-pocket expenses: approximately $7,405.21. All repairs were completed prior to Nissan's official recall notification to owners, scheduled for March 27, 2026. Reimbursement Claim Denied: I submitted a formal reimbursement claim to Nissan Consumer Affairs. My claim was denied on the basis of an incorrect diagnostic code. I believe this denial is improper. The underlying failure — engine bearing degradation caused by the KR15DDT's lubrication deficiency — is precisely what these recalls address. Denying reimbursement on a technicality of code classification, while the physical failure is covered under the recall, is deceptive and harmful to consumers. Due to a recall not being under the vin yet, i have now incured a $16k expense to add an engine to it after i purchased it in Nov of 25 at $97,000. This is a saftey issue and luckily no one was hurt when the car started shaking and couldn't go over a certain speed. I want this reviewed and looked at for possible reimbursement as there have been several other vehicles with the same known issue.Read more

The 12-volt battery was also destroyed as a consequence of the 48V pack failure — the 12V charges exclusively via a DC-to-DC converter fed by the 48V loop; when the pack failed, the converter lost power and the 12V battery was starved of charge. NOTE: This incident is consistent with the active NHTSA ODI preliminary evaluation of 2022 Jeep Wagoneer and Ram 1500 eTorque electrical failures. FCA/Stellantis has acknowledged these failures are likely caused by an electrical concern. My P0B32 event represents a catastrophic failure of the exact system under investigation, with the additional safety consequence of complete loss of power steering at highway speed. Stellantis Corporate Case #96719164 is open. The vehicle has been repaired (48V battery pack replaced) but Stellantis has declined to address the warranty access barrier created by their diagnostic system architecture. The dealer's equipment required a new 12-volt battery installation before it could communicate with the covered hybrid system — a $645 mandatory prerequisite to accessing a Federal Emissions Warranty repair.Read more

Both were dispatched to the scene. I called my husband for help while we waited for police & fire to arrive. My husband popped the hood and we saw large blue flames coming out from the transmission or the engine and we notice that transmission fluid started to leak under the vehicle. My husband put the fire out with the fire extinguisher. A police report was made. I contacted the dealership immediately. I contacted my auto insurance agent the next morning. The fire dept. used heat guns to test the heat level in the engine area. Prior to this, I did not have any issues with the vehicle. I believe this to be some type of mechanical or electrical malfunction or failure, based on the white smoke and blue flames. My daughter and I were put at extreme safety risk. If my husband and the YMCA staff did not assist, I feel that we would have lost our lives. With the large amount of smoke rolling into the air, children entering & leaving the YMCA were exposed to smoke/oder.
