Vehicle stated "PARKING BRAKE FAULT". Driver (myself) discovered electric parking brake was stuck in the ON position and would not disengage. Vehicle could not be moved on its wheels. Vehicle was in parking garage, and driver became stranded for over 24 hours waiting for a low-clearance tow truck with a dolly that could move the vehicle. Ford dealer (Bill Brown Ford) inspected vehicle and discove
... red wiring immediately behind BOTH rear electric parking brake connectors were corroded and broken. Ford dealer stated there was an active recall for this issue (CSP 22P31 or TSB 22-2143), but that my vehicle's build date was not included. CSP 22P31 only includes build dates before 15 March 2022; Driver's vehicle has a build date of 24 June 2022. Ford dealer stated they repaired the vehicle by completing the procedures defined in recall CSP 22P31, but Ford did not cover the costs or acknoledge it as a recall repair due to the build date. tl;dr: The critical safety defect defined in Ford recall CSP 22P31 affects vehicles built in July 2022, but Ford has not expanded the recall to cover these vehicles. In this case, the corrosion led to DUAL failure of both rear electrical parking brake connectors, causing two exposed electrical circuits and lock-up of the parking brake.Read more

While driving, the Forward Collision Warning system activated unexpectedly without any apparent hazard or obstruction in front of my vehicle. Immediately after the warning appeared, the vehicle suddenly lost speed and became very slow to accelerate. I was unable to safely increase my speed while traveling on a busy roadway, creating a dangerous situation and nearly resulting in a collision with su
... rrounding traffic. Due to the sudden loss of power and inability to accelerate normally, I activated my hazard lights and was forced to pull onto the shoulder of the road for my safety and the safety of other drivers. There were no visible obstacles, vehicles, or road conditions that appeared to justify the activation of the Forward Collision Warning system. This unexpected activation and resulting loss of vehicle performance created a significant safety risk. I am submitting this complaint because a malfunction of the Forward Collision Warning system or related vehicle systems could potentially lead to a serious accident or injury. I have experienced recurring brake issues despite recently having the brakes replaced. The problem continues to occur, raising concerns that there may be an underlying electrical or electronic malfunction affecting vehicle performance. I was recently pulled over shortly after the brake replacement due to concerns regarding the vehicle's braking system. Because the issue persists after repairs, I am concerned about the safety of the vehicle and the potential risk of a crash or loss of control.Read more

My 2023 Ford Escape is equipped with a factory-installed rearview backup camera system and SYNC infotainment system. Beginning on or around February 2026. I began experiencing a series of recurring, intermittent, and worsening failures with the rearview camera and the Accessory Protocol Interface Module (APIM) that controls it. These failures render the backup camera system unreliable or completel
... y non-functional during vehicle operation. Specific Symptoms Observed •BLUE / BLANK SCREEN: When the vehicle is placed in reverse, the rearview camera display shows a solid blue screen or goes completely blank rather than displaying the rear camera image. This failure occurs with no warning and leaves me unable to see what is behind the vehicle while backing up. •INVERTED / UPSIDE-DOWN IMAGE: The rearview camera image displays upside-down and/or mirrored, rendering the image confusing and unsafe. Guidelines, directional aids, and proximity warnings displayed on screen are also inverted, making them unusable. •DISTORTED / CORRUPTED IMAGE: The camera feed displays visual distortion, artifacts, freezing, flickering, or corruption that makes it impossible to accurately interpret what is behind the vehicle. •APIM FAILURE: The Accessory Protocol Interface Module (APIM) — the core software module controlling the SYNC 3 display and camera feed — has failed or malfunctioned. Dealership diagnosis has identified the APIM as requiring repair or replacement. This module failure is the root cause of the camera display issues described above. The failures occur intermittently and unpredictably. The camera may function correctly for a period, then fail on the next use. In some cases, the failure is persistent across multiple ignition cycles. There is no consistent triggering condition — the defect occurs in varied weather conditions, temperatures, and after varying periods of use. Turning the vehicle off and back on sometimes temporarily restores the camera, but the defect returns.Read more

For my 2019 BMW X5 SDRIVE40i, The A/C evaporator core failed and began leaking refrigerant, resulting in complete loss of cold air from the A/C system. The vehicle had approximately 50,000 miles. The repair required full dashboard removal and cost approximately $4,500 including a rental vehicle. This appears to be a widespread issue on 2019–2025 BMW X5/X6/X7 models, suspected to be caused by R-
... 1234yf refrigerant incompatibility with the aluminum evaporator core. The leaking R-1234yf refrigerant is also an environmental concern. R-1234yf is a hydrofluorocarbon (HFC) greenhouse gas with a global warming potential significantly higher than CO2. An uncontrolled leak of this refrigerant directly into the atmosphere — due to a defective factory-installed evaporator core — contributes to environmental harm and violates the spirit of EPA regulations governing refrigerant emissions. Consumers should not bear the financial burden of a known manufacturing defect that also results in the uncontrolled release of a regulated substance. BMW has not issued a recall or extended warranty for this component despite widespread owner reports of the same failure. I am requesting NHTSA investigate this issue and compel BMW to issue a formal recall and provide reimbursement to affected owners.Read more

I am filing a safety complaint regarding my 2025 Cadillac Escalade, which experienced a catastrophic engine failure while being driven on a public roadway with approximately 13,000 miles on the vehicle. While traveling from [XXX] to [XXX] , the vehicle suddenly lost power while in motion. A warning message appeared stating “Press to Start Again.” The engine shut off completely while driving,
... causing an immediate loss of propulsion. The vehicle went into neutral, and I was forced to maneuver to the side of the road to avoid a potentially dangerous situation. Once stopped, the vehicle would not restart. Multiple attempts were made using both the push-button ignition and key fob remote start, but the vehicle remained inoperable. The vehicle was towed and later diagnosed as requiring a complete engine replacement despite being less than one year old. I believe this is a serious safety defect because the vehicle lost power without warning while being driven. Had this occurred in heavier traffic, while crossing an intersection, merging, or traveling at higher speeds, it could have resulted in a serious accident, injury, or death. This was not the first major issue with this vehicle. Within weeks of taking delivery, multiple components required repair or replacement, including: • Complete replacement of the camera system due to defective cameras • Replacement of a defective lighting component • Transmission failure concerns • Cylinder #2 misfire issues The repeated failures of major vehicle systems, culminating in a complete engine failure and loss of propulsion while driving, raise serious concerns about the safety and reliability of this vehicle. Additionally, after the engine failure, the vehicle could not be placed into neutral and had to be dragged onto a tow truck, causing damage to the tires and raising concerns about emergency roadside recovery procedures. My vehicle is currently subject to an open Ford transmission safety recall for which no repair remedy is available at this time. The vehicle is experiencing a recurring and dangerous failure while driving. Approximately twice per day, during my normal commute, all vehicle electronics suddenly shut off while the vehicle is in motion. At the same time, the wheels appear to lock up, causing a sudden
... loss of vehicle control and creating a significant crash risk. When the failure occurs, I am forced to immediately shift the transmission into neutral while the vehicle is still moving. After approximately 5 to 10 seconds, I can shift back into drive and continue operating the vehicle. However, vehicle electronics often remain inoperative for the remainder of the trip. This condition is occurring repeatedly and has become more frequent. The issue currently occurs approximately once during my drive to work and once during my drive home. My commute is approximately 20 minutes each way. The vehicle has not yet been involved in a crash, but I believe a crash is likely if this condition continues. The sudden loss of power, loss of electronics, and wheel lockup while driving creates an immediate safety hazard to myself, my passengers, and other motorists. I am submitting this complaint because the vehicle is subject to an active safety recall, the condition appears related to the recall, and no repair remedy is currently available. SEVERE SAFETY HAZARD: CASCADING ELECTRICAL FIRE RISK. My 2019 Ram 1500 suffered a catastrophic electrical failure while driving. The alternator's voltage regulator melted, overcharging the system and causing the battery to boil and vent toxic sulfuric acid fumes into the cabin, creating an immediate respiratory and fire hazard. This is the direct result of Manufacturer Defect XL1/XG1 (Rear Windo

They returned the vehicle claiming it was fixed, but I have geolocated photos showing the window leaking heavily just 17 days later. Because the leak was never resolved, water continuously entered the rear cabin, corroding the RF Hub and creating a parasitic drag that destroyed the electrical grid. CRITICAL NEGLIGENCE: On June 1, 2026, Stellantis Corporate (Case XXX) acknowledged the severity of the fire risk by immediately inquiring if Emergency Services had been contacted, yet subsequently refused to offer a safety tow. When I attempted to escalate, their agents [XXX] , [XXX] , [XXX] , [XXX] ) refused to provide IDs. The vehicle displayed a hybrid system failure
... alert, lost the ability to accelerate normally, and became unable to maintain safe driving speeds. On the most recent occurrence, the sudden and unexpected loss of vehicle power created an immediate risk of collision. The vehicle’s failure while in traffic nearly resulted in a serious crash. I was driving with passengers at the time. The vehicle was taken to an authorized Kia dealership for repair following each incident. The same defect has now recurred after the first repair was completed, demonstrating that the underlying cause has not been identified or corrected. This defect substantially impairs the safety of the vehicle. The sudden loss of hybrid system function and propulsion while driving at road speed creates a direct and serious risk of accident, injury, or death to the vehicle’s occupants and to other drivers on the road. I am a [XXX] resident and am aware that this defect is not isolated to my vehicle. Numerous other 2025 Kia Carnival Hybrid owners have filed similar NHTSA complaints regarding battery failure and sudden loss of acceleration. The vehicle has experienced repeated high‑voltage battery system failures consistent with the symptoms described in the current open safety recall for this model. The vehicle first displayed a “High Voltage System Fault” warning, loss of A/C cooling, and inability to charge. A dealer confirmed an HV isolation fault and documented an isolation reading of only 38 megaohms, which is far below n
... ormal and indicates a dangerous high‑voltage isolation breakdown. They replaced the front inverter (EPIC), the 3‑phase cable, and the high‑voltage cable to the battery. Less than a year later, the same “High Voltage System Fault” warning reappeared and the vehicle again became unable to charge. The dealer stated that “the same repair performed in the front is now needed in the rear,” indicating a second HV isolation failure in another section of the high‑voltage system. This suggests a systemic defect in the high‑voltage battery pack or associated components, consistent with the recall involving thermal overload, internal shorts, and isolation failure. The vehicle is currently subject to an open recall instructing owners to limit charging to 90% and charge only outdoors due to fire risk. The repeated failures create a safety hazard due to potential thermal events, loss of propulsion, and inability to safely charge the vehicle. The problem has been confirmed by the dealer multiple times. Despite this, the dealer is attempting to charge for repairs that appear directly related to the recall defect, and no permanent remedy has been provided.Read more
