Ford Motor Company has failed to remedy NHTSA Safety Recall 25V863 (Ford recall number 25C69, Integrated Park Module / Park Pawl Engagement) on my vehicle within the timely manner required, and the recall remains in "Recall Incomplete" status more than four months past Ford's own stated remedy availability deadline. That deadline (March 31, 2026) elapsed without a remedy being authorized. As of NHTSA's recall-data refresh on April 7, 2026, the recall remained "Recall Incomplete" on my vehicle with no remedy available. As of NHTSA's recall-data refresh on June 2, 2026 — 137 days after the original announcement and 63 days after Ford's own Q1 deadline — the recall still shows "Recall Incomplete" on my vehicle, although Ford has now (very late) authorized dealers to perform the SOBDMC (Secondary Onboard Diagnostic Module C) software update as the remedy. I am filing this complaint under the criterion explicitly published on NHTSA's VIN-lookup recall-result page: "If the manufacturer has failed or is unable to remedy this safety recall for your vehicle in a timely manner, please contact the NHTSA Vehicle Safety Hotline." A battery failure can leave drivers stranded unexpectedly, potentially in unsafe locations such as highways, intersections, remote areas, or during adverse weather conditions. The inability to reliably operate the vehicle creates a safety risk for both occupants and other motorists. The fault code BMS_079 was diagnosed and confirmed through Tesla's service system. Tesla service personnel indicated that the HV battery requires replacement and that the vehicle cannot be driven safely until the repair is completed. Prior to the vehicle becoming inoperable, warning messages appeared on the vehicle display indicating a problem with the battery system. Tesla was contacted after the incident, and the representative advised that the vehicle should be taken to a collision center. After shutdown, the hazard lights would not work. One documented incident occurred on the interstate at about 70 mph. This puts occupants and others at risk because the vehicle can suddenly lose motive power and critical electrical/control functions at the same time, including lighting, signals, hazards, instrument display, and steering/control assistance. The vehicle has been inspected multiple times by a Chrysler dealership. Dealer records documented multiple modules losing communication on the CAN C bus. The dealer replaced the shifter module as a possible contributor, but the problem continued. The dealer later corrected loose pin tension at a communication/star connector, but the vehicle continued to shut down and later scan reports still showed widespread active/stored CAN and lost-communication faults across powertrain, hybrid control, body control, braking/ABS, steering, and shifter-related systems. The vehicle is available for inspection upon request. The problem has been confirmed by dealer scan reports and repair records, but the root cause remains unresolved. The manufacturer has been contacted through customer care and repair/scan documentation was submitted. ... a sudden, catastrophic loss of motive power while driving on a public roadway due to a total ICCU failure, which created an immediate hazard for myself and my young child who was inside the vehicle. When the primary ICCU component failed, it starved the low-voltage system, causing severe deep-discharge damage to the 12V battery. Following the warranty repair of the ICCU in February 2026, the vehicle was returned to me. However, Kia's official corporate diagnostic and repair procedures for this defect are fundamentally flawed and incomplete; they do not mandate a comprehensive load-test or automatic replacement of the 12V battery following an ICCU collapse. Because of this failure in the official repair procedures, the vehicle was returned to service with a critically compromised low-voltage system. This directly resulted in recurring battery warning messages over the subsequent months, culminating on May 29, 2026, in a secondary, total 12V battery failure that completely bricked the vehicle. The failure of the manufacturer's official safety recall remedy and the subsequent inadequacy of their post-failure service protocol left this vehicle in a chronically un-roadworthy and unsafe condition.Read more

When I turned on the car to back it out of the garage it got halfway out and the entire system shut down making everything inoperable including flashing emergency lights, windows etc. there was very little time between the warning light going on and the car becoming inoperable. The dealer diagnosed a failure of the ICCU system which has been reported for several years. A quick Google search reveals that many Ionic owners have experienced this failure. I was incredibly lucky that this occurred within a block of my house. The previous night I was driving home on the LA freeways. Had this failure occurred at night on the freeway it could have been life threatening as there is very little time to get to safety and the hazard lights and all electronics become useless. There was also a distinct burning odor. I attempted to restart the vehicle multiple times without success. The dashboard displayed the message: “conditions not suitable to shift.” Due to the vehicle becoming disabled on a curve and posing a safety hazard, I contacted the Sheriff’s Office for assistance to prevent a potential collision. Upon their arrival, it was noted that the engine compartment and hood area were exceptionally hot, particularly where the smoke was originating. The dealership has since confirmed they have also been unable to restart the vehicle. The vehicle has been inspected by the GMC dealer and is currently pending further evaluation by a GM-certified mechanic to determine the root cause. The horn on my vehicle activates (blows) when I turn the steering wheel to certain positions. I believe this to be a road hazard. Especially since I had to remove the horn relay to prevent the horn from blowing in traffic until I can get it repaired. Several other Ford vehicles have been recalled for this exact problem. These symptoms are identical to NHTSA Recall 24V-267 (Ford 24S24) — loss of motive power from the BCM/PCM failing to detect 12V battery degradation — and the follow-on battery recall 25V-158 (Ford 25S26). My vehicle is a 2024 Maverick and is NOT included in 24V-267, which Ford limited to 2022-2023 Mavericks built Feb 2021–Oct 2022. I am reporting because my 2024 shows the identical failure mode, including the total absence of warning before power loss, matching the owner reports that prompted Recall Query RQ24014 into the adequacy of the 24V-267 remedy. I request NHTSA consider whether the affected population should include model year 2024 Mavericks. The instrument cluster displayed a red emergency warning: "Vehicle shutting down — PULL OVER SAFELY." The vehicle required towing. The failure was confirmed by the service center's own follow-up invoice, which documented "trapped air pockets in the coolant system following the most recent service" as the cause. The corrective repair was performed at $0 as a Goodwill service — the service center's internal acknowledgment of liability. The service advisor made a written text admission of causation the following day, then retracted it in a recorded phone call. Both are preserved. No post-repair Isolation Resistance test results were provided. The HV isolation fault may indicate latent thermal damage to HV insulation that could cause a safety failure at higher mileage outside any remaining warranty.
