I received a Recall notice around January 2026 for the trailer hitch wiring harness on my 2023 Hyundai Tucson Hybrid. For 6 months the local dealer told me there was no fix available from Hyundai. I finally received a notice informing me that parts were now available and to bring it in to dealer for repairs. I took the vehicle in for recall repairs on May 27, 2026. Instead of repairing/replac
... ing the wiring harness, Hyundai removed the entire wiring harness making the trailer hitch which I use for my business inoperable legally. When I noticed the wiring harness missing several days later, the dealer said that was Hyundai's "fix" for the recall issue. I contacted Hyundai USA Customer Service who also told me that removing the wiring harness and replacing it with an inexpensive dust cover was the fix. They told me that sometime in the future there will be another recall to have the wiring harness replaced. In the meantime, they have taken my vehicle VIN off the open recall list, since it has been "fixed". Hyundai sold me a vehicle with a trailer hitch and now they have without my permission or prior knowledge disabled it.Read more

I experienced a safety critical failure in my 2024 Hyundai Ionic 6. While attempting to start the vehicle, the car displayed a red 12 Volt battery warning light, a battery discharge warning, and then refused to enter ready mode. The vehicle would only shift into neutral and was completely undrivable. After sitting for many hours, the vehicle started again, which is consistent with an intermittent
... ICCU/DC-DC converter dropout. This type of failure is known to cause loss of power and has been documented in NHTSA investigation PE23-011 for other Hyundai EV's. The dealership states they cannot replicate the issue and refuses further diagnostics. Hyundai corporate is deferring entirely to the dealership and will not authorize a loaner or confirm reimbursement, leaving me without transportation. They also stated they cannot commit to any repair unless the failure happens again despite the clear safety implications. This failure mode can cause sudden loss of motive power, which poses a serious risk if it occurs at highway speeds. I am reporting this because the vehicle is unsafe to operate, the failure was severe, and Hyundai is refusing to investigate the root cause unless it reoccurs. I am requesting NHTSA review this incident as part of the ongoing concerns regarding ICCU/DC-DC failures in Hyundai E-GMP vehicles.Read more

Issue 1: Cooling Fan Remained On After Engine Shutdown Component or system involved: Engine cooling system and fan control system, including the radiator cooling fan, fan relay, coolant temperature sensor, wiring, or engine control module (ECM). The vehicle is available for inspection upon request. What happened: After turning off the engine, the radiator cooling fan remained running continuously
... for approximately five minutes and would not shut off as expected. No warning light, message, or other dashboard indication appeared before or during the event. This was unusual behavior compared with normal operation. Safety risk: The unexpected fan operation created concern that the cooling system was malfunctioning and that the vehicle could overheat, experience an electrical problem, or become unreliable while driving. A cooling system or electrical failure while in traffic could put me and other drivers at risk. Reproduction or confirmation: The issue occurred several time and was confirmed by a Chevrolet dealer, and was told that my warranty did not cover the issue. I took it to outside mechanic to fix the issue as Chevrolet wanted to charge over $1500 for the issue. No warning lamps, messages, overheating indicators, or other symptoms appeared before the fan remained on. The problem occurred suddenly back in 2025. Issue 2: Ignition Key Gets Stuck and Cannot Be Removed Ignition lock cylinder, ignition switch, transmission shift-interlock system, park-position switch, or steering column lock mechanism. The vehicle is available for inspection upon request. The ignition key intermittently becomes stuck and cannot be removed from the ignition. The problem occurs after parking and turning the vehicle off, and I sometimes need to move the shifter or steering wheel repeatedly before the key can be removed. A stuck ignition key can prevent me from properly shutting down and securing the vehicle. It may also indicate a failure in the ignition or shift-interlock system.Read more

Vehicle was inoperable and over 100F in our driveway, with a flashing "critical high voltage battery fault". My wife injured her hand attempting to get in the vehicle. The service center has claimed that moisture inside the HVAC system caused the High Voltage (HV) battery system to fail, throwing a critical high-voltage battery fault. Rivian is attempting to deny warranty coverage based on this mo
... isture. Linkage to Warranted Component: At 85,690 miles, my vehicle is fully covered under Rivian’s 8-year High Voltage Battery Limited Warranty. The critical high-voltage fault and subsequent component failures were directly caused by water entering the electrical/battery system. Proximate Cause via Known Manufacturing Defect: Rivian has an active Technical Service Bulletin (TSB) regarding moisture ingress via the HVAC assembly for 2022–2024 R1S models. The moisture that caused the battery fault is a documented, inherent design defect of the vehicle, not user negligence or external environmental damage. An inherent defect in an out-of-warranty system cannot be used to void coverage for a failure it causes inside a component that is still heavily protected by an active warranty. The photograph provided by the service center does not depict superficial 'moisture in the HVAC' causing external damage. It shows severe fluid ingress and terminal corrosion inside the Woory Industrial High Voltage Cabin Heater assembly. Because this is an internal, high-voltage powertrain component operating directly off the traction battery system, it falls squarely under Rivian's 8-year / 175,000-mile High Voltage System Warranty. The corrosion present on the high-voltage circuit board is a direct result of the factory HVAC water ingress defect acknowledged in Rivian's technical service bulletins for 2022-2024 R1S vehicles. A manufacturing defect that allows water to breach a sealed, high-voltage housing cannot be used to deny warranty coverage on a high-voltage system that is actively wRead more

The plastic upper rail glass guide frame on the rear sliding window assembly of my 2023 GMC Sierra developed structural micro-cracks during normal vehicle operation. This manufacturing defect is a known issue tracked internally by the manufacturer under Technical Service Bulletin (TSB) #18-NA-383.This structural failure allows severe water intrusion into the passenger cabin. The leaking water runs
... straight down the inside back wall of the cab, completely flooding and corroding sensitive, factory-installed electronic safety modules mounted directly beneath the leak path. This continuous water intrusion caused the catastrophic electrical failure and corrosion of the video processing control module located behind the rear seat. The sudden failure of this module completely disabled the rearview digital mirror camera and the 360-degree perimeter driver assistance cameras, creating immediate blind spots and an active driving safety hazard while operating the vehicle. The vehicle was inspected and repaired by an authorized dealership, confirming the rear window leak as the direct cause of the electronic destruction. The out-of-pocket cost to replace the water-damaged safety components was $2,747.00. Because the manufacturer refused to cover the replacement of the defective rear glass assembly, the structural leak remains unresolved. This active water intrusion continues to flood the cabin, creating a health hazard via mold growth and guaranteeing that the newly replaced safety cameras will inevitably short-circuit and fail again.Read more

While my wife was operating our 2022 Jeep Wagoneer Series III (5.7L HEMI eTorque, ~44,700 miles) at highway speed, the vehicle suffered a sudden and complete electrical power failure. Electric power steering cut out completely while the vehicle was in motion, creating an immediate safety hazard and nearly causing an accident. The vehicle was disabled and required towing. The dealership (Safford J
... eep) diagnosed fault code P0B32: catastrophic internal hardware and voltage failure of the 48-volt eTorque Hybrid Battery Pack. The 12-volt battery was also destroyed as a consequence of the 48V pack failure — the 12V charges exclusively via a DC-to-DC converter fed by the 48V loop; when the pack failed, the converter lost power and the 12V battery was starved of charge. NOTE: This incident is consistent with the active NHTSA ODI preliminary evaluation of 2022 Jeep Wagoneer and Ram 1500 eTorque electrical failures. FCA/Stellantis has acknowledged these failures are likely caused by an electrical concern. My P0B32 event represents a catastrophic failure of the exact system under investigation, with the additional safety consequence of complete loss of power steering at highway speed. Stellantis Corporate Case #96719164 is open. The vehicle has been repaired (48V battery pack replaced) but Stellantis has declined to address the warranty access barrier created by their diagnostic system architecture. My husband popped the hood and we saw large blue flames coming out from the transmission or the engine and we notice that transmission fluid started to leak under the vehicle. My husband put the fire out with the fire extinguisher. A police report was made. I contacted the dealership immediately. I contacted my auto insurance agent the next morning. This complaint is in regards to Safety Recall 272 Component Failure and Vehicle Disabling / SOS System Failure On [XXX], at approximately [XXX] PM, I was driving my Genesis vehicle during rush hour when I heard a loud popping sound. Immediately following the sound, the dashboard displayed a critical warning light stating: "Electrical Charging System Fault — Check Electrical System." Because I wa
... s actively driving in heavy rush-hour traffic, this sudden loss of charging system functionality posed an immediate safety hazard. I pressed the in-vehicle "SOS" button to contact Genesis emergency services. The operator, who identified himself as "Eddie," refused to dispatch a tow truck to assist me. Instead, he offered to read a phone number aloud for me to write down. I explained that I was actively operating a malfunctioning vehicle in heavy traffic and could not safely write down information, but he still refused to call for emergency towing. I managed to safely limp the vehicle back to my residence and subsequently arranged for the local dealership, Genesis of Las Vegas, to tow the vehicle. On Friday, June 5, 2026, the dealership confirmed that the vehicle had suffered a failure of the Integrated Charging Control Unit (ICCU), which also destroyed the 12V battery. They informed me that this failure was directly tied to NHTSA Safety Recall 272 (Integrated Charging Control Unit). While the dealership performed the ICCU remedy, they are currently refusing to release my vehicle unless I pay $500.00 for the replacement of the 12V battery. Because the 12V battery was directly damaged as a consequential result of the recalled ICCU defect, it should be covered under the safety recall remedy and vehicle warranty. As of this reporting, the dealer is holding my vehicle hostage over this fee, and Genesis Motor America has failed to resolve the issue despite a formal complaint being filed with them. Four days after I took delivery of this used vehicle (April 25, 2026), multiple warning lights appeared at once: BRAKE, ABS, PARK, Check Engine, Blind Spot Monitor Unavailable, and RCTA OFF. An independent Toyota dealership diagnosed the vehicle April 27-28, 2026 (approx. 109,000 miles) and found (1) water leaking into the rear hybrid battery area through a sunroof drain "installed incorrectly," c
... ausing a Brake System Control Module low-voltage code, starting issues, a headlight only partially functioning on one side, possible air in the brake system, and a musty HVAC odor; and (2) blind spot monitor fault code C1AC2 (Slave Module Horizontal Axis Misalignment) - the blind spot and rear cross-traffic monitoring systems do not function reliably. At a second visit on May 4, 2026, recalibration was unsuccessful; per Toyota procedure the only remaining option is replacing the right-rear blind spot sensor. The technician wrote the cause is a mystery, "whether it be body damage related or electrical short." Toyota service records show the same C1AC2 code was documented during the selling dealer's own pre-sale inspection, and that in December 2024 another Toyota dealership pulled the rear bumper and found the RR blind spot sensor bracket bent, causing incorrect readings - a recurring safety defect that prior calibrations and repairs have not resolved. Separately, federal safety recall 23V865 (occupant classification sensors; the passenger airbag may not deploy as designed) was open and incomplete when I purchased the vehicle and remains incomplete, and Toyota service campaign 25TD01 (rear traction motor cable corrosion) is also outstanding. The defect has been reproduced and confirmed by two Toyota dealerships, reported to the selling dealer in writing, and reported to Toyota's customer-care center. The vehicle is available for inspection upon request. No crash, fire, or injury.Read more

2025 Triumph Speed 400 — Moisture-Triggered Limp Mode and Loss of Turn Signals My 2025 Triumph Speed 400 enters limp mode and displays a check engine light and wrench warning whenever ridden in rain or high humidity. Highway Incident: While traveling at highway speeds, a sudden evasive braking maneuver caused the truck to drop power. Upon attempting to re-accelerate, the throttle pedal went to the floor with zero response, and the vehicle continued to decelerate on an active highway. Multiple trailing vehicles had to make emergency maneuvers to swerve around the decelerating truck to avoid a high-speed, rear-e
... nd collision. Subsequent Incidents: The vehicle repeatedly forces an uncommanded speed limit of 11 mph while attempting to accelerate from a complete stop or merge from stop signs. This severe lack of predictable acceleration creates an immediate risk of being T-boned or rear-ended by cross-traffic, making the vehicle entirely unsafe to operate in normal traffic conditions. An authorized Ford dealership has confirmed the underlying failure of the hybrid system. They kept the vehicle for approximately one month to replace the entire hybrid drive unit. Following that repair, the vehicle was returned to the dealer a second time, where technicians confirmed a software communication failure between the electric and gas systems, attempting to resolve it by flashing/reprogramming the ECU. The vehicle’s hybrid system and software have been extensively inspected and worked on by manufacturer-certified technicians at an authorized Ford dealership. It has not been inspected by police or insurance, as no physical collision has occurred yet. No warning lamps or messages.Read more

The rearview backup camera on my 2019 Jeep Compass Latitude has completely failed to function, creating a severe blind-spot safety hazard during reverse maneuvers. Screen goes black, or flickers on and off, or has lines and hotspots. It is not usable at all. This failure directly compromises the safety mandates established under Federal Motor Vehicle Safety Standard (FMVSS) No. 111.

While idling at a standstill (in a drive-thru queue), my 2018 Hyundai Tucson suddenly experienced a severe engine overheat, causing the vehicle's air conditioning system to immediately blow hot air and shut down. The vehicle was brought directly to an authorized Hyundai dealership (Chapman Hyundai on [XXX]) for an emergency inspection. The dealership's certified technicians confirmed that the elect
... ric Engine Cooling Fan Assembly completely failed and subsequently caused the main Front Wiring Harness and Fuse Box to physically melt. The dealership documented this catastrophic electrical and thermal meltdown under Repair Order #XXX.This is a critical safety defect. A melted engine wiring harness poses an immediate and severe risk of an electrical short circuit, sudden vehicle stalling in live traffic, or a catastrophic under-hood engine fire. Because this component controls the primary cooling of the powertrain, its failure creates an extreme hazard to the vehicle occupants and surrounding traffic. Hyundai must investigate this component failure before it results in a thermal fire incident or an accident due to sudden power loss. My 2021 Ford F-150 intermittently displays the warning message “Reverse Brake Assist Not Available – See Manual” when the vehicle is placed in reverse. When this message appears, the reverse brake assist / parking collision warning feature is not available, which may reduce safety while backing up. The vehicle was inspected by a Ford dealership. The dealership confirmed DTC U3000:49 in the
... IPMA module and identified Ford TSB 23-2040 as applicable to the concern. The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. This appears to be a software/programming-related condition affecting a driver assistance and brake-related safety feature, not normal wear and tear or a failed mechanical component. The warning is intermittent, but it has appeared multiple times. The vehicle is available for inspection upon request. No crash, injury, or fire has occurred. Ford Customer Support was contacted and case number XXX was opened. Ford advised that no active warranty, recall, or customer satisfaction program currently provides repair cost coverage, and that because this involves Reverse Brake Assist, I may choose to file a safety concern report with NHTSA. A 2023 Tesla Model Y experienced a sudden electrical failure while parked/being prepared for use. Multiple warnings appeared on May 26, 2026, including high-voltage system service required, low-voltage electrical system power reduced/vehicle shutting down, cabin climate control limited, and steering/braking/parking brake functions degraded. The vehicle could not be driven normally and required fla
... tbed recovery. The failure created a safety concern because the vehicle showed high-voltage and low-voltage faults together with steering/braking/parking brake warnings and could have left the vehicle disabled without reliable restart or parking brake function. The manufacturer service center inspected and repaired the vehicle after several days. The component should be available through manufacturer service records, and screenshots of the warnings, tow/recovery, and service records are available.Read more

Issue: Garage caught fire. All wood (trusses, joists, walls) were in tact. Estimated in tact amount=99%. The vehicle was completely consumed. The hood melted, plastic, seats, tires consumed. Car was not driven day of incident. Origin was attributed to garage. Cause was undetermined by fire department. Report does not today address the vehicle in any way shape or form. There is no documentation as
... to what was seen upon removing garage doors. Had the can already been consumed? Did vehicle initially encapsulate the fire prior to moving into garage structure? It is speculated that NHTSA part 573 recall report 22V-875 may be applicable when viewed from design engineering point of view and not strictly to only vehicles addressed by Nissan in that report. There were no prior warnings of any kind that foretold the ensuing events. I am trying to get the fire department to amend report to record stated belief of fire origin (vehicle) which is not today evidenced in the report and as per the stated opinion by one of the fire fighters. I am further insisting that the insurance company assign a forensic expert in evaluating the origin and cause of the fire. Nissan has not been notified to date. I am imploring NHTSA to take this apparent prior documented safety issue further beyond what has been concluded in order to prevent any future safety issues should it so be judged by NHTSA as the authority in these matters.Read more
