Incident date: Jan 13, 2026
The vehicle exhibits a chronic, systemic electrical failure characterized by the intermittent loss of critical lighting systems, including turn signals and brake lights. This failur5e has persisted over several years and thousands of miles, independent of battery replacements or specific usage patterns. The issue has been confirmed via multiple independent service interventions. Most notably, in
... February 2024, a third-party professional repair - which included the replacement of the Power Distribution Unit (Central Fuse Box) was performed to address these electrical faults. Despite this professional intervention, the defect recurred in May 2026. The failure of the turn signals and brake lights while in traffic resulted in a Law Enforcement stop and the issuance of a citation. Prior to total failure, the vehicle often displays erratic engine performance, including sputtering and hesitation. These symptoms were identified early in the Vehicle's history, but inquiries regarding recall campaigns for similar Makes and Models were denied by Authorized Dealerships, who cited recall registry despite exhibiting identical Mechanical and Electrical failure modes. The failure of the brake lights and turn signals poses a severe and immediate safety risk to myself, my passengers, other motorists and those on the road...as it creates an unpredictable environment were emergency stopping and lane changes cannot be communicated to the surrounding traffic. The electrical architecture of this vehicle is fundamentally flawed, as evidenced by the recurring nature of the defect despite multiple repair attempts, including the replacement of the primary fuse distribution center. The vehicle, all service records. third-party repair receipt, and relevant law enforcement citation are available for inspection upon request to support this report. The recurring loss of safety-critical signaling systems represents a systemic design defect rather than isolated wear & tear.Read more

While driving at normal operational speeds, the vehicle suffered a sudden, un-commanded, and catastrophic loss of motive power while navigating a live traffic intersection. The transmission completely disengaged all gears without warning, rendering the vehicle entirely stationary and immobilized in the path of oncoming traffic. This failure created an immediate, severe safety hazard, exposing the
... occupants to a high risk of a side-impact or rear-end collision due to the inability to clear the intersection under the vehicle's own power. The vehicle had to be flatbedded to an authorized dealer (Audi - Glenwood Springs). The professional diagnostic evaluation confirmed an internal failure of the transmission assembly and Transmission Control Module (TCM) / Mechatronic unit, requiring a total replacement of the powertrain assembly. The failure occurred at 59,000 miles.Read more

The vehicle was taken back to the dealer, where it was determined that the high-pressure fuel pump needed to be replaced; however, after the repair, the vehicle started making abnormal sounds. Additionally, the contact stated that the abnormally strong fuel odor had worsened. The vehicle was taken back to the dealer, who informed the contact that no diagnostic trouble code was retrieved, and the contact was advised to pick up the vehicle. The contact expressed safety concerns that the vehicle was unsafe to drive due to the persistent failure. The manufacturer was notified of the failure. The failure mileage was approximately 40,000.Read more

Failed Component: Powertrain (PCM/TCM/HCP software calibration) & Electrical/Steering (Fire Recalls). Safety Risk & Description: Following recent mandatory manufacturer software updates, this vehicle has developed a severe, dangerous drivetrain defect. Under light acceleration from a complete stop, the vehicle exhibits an extreme throttle lag and hard delay before violently surging forward, creati
... ng a high risk of intersection collisions. Furthermore, at sustained highway speeds (52–63 MPH), the vehicle abruptly drops out of Electric Mode, causing the gas engine to violently spike to 5,000 RPMs before dropping back. This erratic cycling causes the vehicle to lurch, jerk, and pull, actively disrupting lane control. Compounding this, due to consecutive, unresolved fire safety recalls—including Recall 68C (Battery Fires) and the June 2026 Power Steering Pump wiring fire recall—the vehicle cannot be safely parked inside my garage or near structures while turned off. It represents a total loss of normal use and a severe safety liability. Dealer Confirmation & History: The driving defect was formally duplicated and confirmed by a certified technician on 06/09/2026 at Ryan Auto Mall (Repair Order #471599). The technician explicitly documented the electric mode dropouts and acceleration lag in writing. However, because the vehicle’s software masking prevents standard Diagnostic Trouble Codes (DTCs) from triggering, the dealer closed the ticket stating it is "operating as designed." The vehicle's electrical and hybrid systems have been repeatedly altered via mandatory safety recalls: Recall 23A/04B (05/14/2024), Recall 95B (05/27/2025), and Recall 68C (02/11/2026). No dashboard warning lights are present; the dangerous handling is entirely coded into the manufacturer's latest software updates. Vehicle is intact and available for inspection.Read more

On 6/16 we experienced a Freightliner Optiview Dash Flat Panel Delamination with resulted in lines going through the gauges on the Dash. The Dash appears to be functioning with the defect. Initially we thought the flat panel display had cracked. We contacted Freightliner 24/7 Technical Support (Case #15355287) to report the issue. The Representative said that he had not heard of this issue. Whe
... n in fact Freightliner had a recall on the flat panel units manufactured by Medallion Inc, that our Motorhome was not included based on the 2020 model year. We asked the Freightliner representative if the Dash flat panel could stop working which would present a safety issues while traveling down the highway. The Representative would not offer any guidance. He said we should check our fluid levels and in the event the Dash fails we should monitor our speed with a GPS. The representative indicated that we should contact a local Freightliner Service Center to determine what the replacement cost would be for a new Dash flat panel since we were out of warranty. We contacted our local Freightliner Service Center in Hattiesburg, MS. Freightliner part number 66-02439-000 They quoted the cost at $5133 + Installation A search on the Medallion Delamination issue is wide spread across Trucks and many different Motorhome manufacturers. When we first purchased our Motorhome in 2020 the Dash would blackout while traveling down the highway. Freightliner indicated that is was a firmware issue. Flashed the unit and issue has not repeated itself. This was 1 model year after the Medallion flat panel recall. Component Affected: Service Brakes / ABS Module / Hydraulic Braking System Description of Safety Defect: There is an active, intermittent, catastrophic failure in the front braking system/ABS module causing the front brakes to completely lock up mid-transit while operating at high speeds (60+ MPH). First Incident (April 24, 2026): During operation on a busy interstate at over 60 MPH, the front br
... aking system experienced a sudden, catastrophic thermal lock-up. This caused violent vehicle shuddering, extreme friction heat, and heavy, thick, acrid smoke. The vehicle was brought to a dealer shop for repair. Second Incident (April 29, 2026): Four days after delivery, the identical catastrophic brake lock-up recurred mid-transit, creating an immediate fire hazard due to friction heat and smoke. An independent repair facility diagnosed the failure and replaced the ABS module. Third Incident (June 4, 2026): The identical component failure recurred while traveling at over 60 MPH on Interstate 694 with a minor child in the vehicle. The front brakes fully locked up mid-transit, creating an immediate risk of a high-speed collision, severe thermal friction, and heavy burning odors, rendering the vehicle entirely un-roadworthy. Current Status: > The vehicle has sat grounded at a repair shop for 30 cumulative days out of 54 days of ownership due to the persistent re-locking of the front hydraulic braking system. The vehicle is unsafe to operate on public roads. No crash or fire occurred, but the failure poses an imminent thermal and collision hazard.Read more

Vehicle has experienced multiple electrical and mechanical failures at unusually low mileage. Purchased at 32,000 miles. At approximately 50,000 miles, the factory radio/infotainment system failed completely. Electrical systems were observed remaining active after the vehicle was powered off, suggesting an underlying electrical fault. At approximately 60,000 miles, the power steering rack and fuel
... pump both required full replacement and were confirmed and replaced by an authorized dealership. No warning lights appeared prior to these failures — components failed without any prior indication. At approximately 100,000 miles, the transmission began failing and exhibits the known torque converter shudder issue widely reported on this vehicle platform. The shudder was present prior to full transmission failure symptoms appearing. The pattern of premature failures across multiple unrelated systems — electrical, steering, fuel delivery, and transmission — suggests a broader manufacturing defect.Read more

My 2023 Chevrolet Silverado 1500 experiences an intermittant, uncommanded mechanical drivetrain lockup and severe binding when accelerating from a complete stop. The vehicle rolls fine in a straight line for several feet, but the exact moment the steering wheel is turned slightly (a couple degrees), the drivetrain violently binds up. This creates resistance in the steering and acceleration, as wel
... l as a severe physical and audible metallic clunking slam and massive shockwave vibrations felt directly through the steering wheel, floorboards, brake/gas pedals, and driver’s seat. During the last event, the vehicle threw a "Service 4 Wheel Drive" dash message and refused to shift out of "Auto" mode back into 2WD. Because this is a pure mechanical interference/binding issue, it does not consistently throw active electronic trouble codes. A standard OBD2 code scan will show nothing. After turning the vehicle off for several hours and testing again, the problem persisted. After towing the vehicle to a dealership (6/10/2026) it sat for at least 5 days before dealership service looked at it. By this time, the active warning lights cleared. My initial servicing dealership refused to perform physical hardware or fluid diagnostics under my factory warranty, stating they would not look at the vehicle unless I paid $860 out-of-pocket or until the drivetrain "completely grenaded" on the road. Because this uncommanded lockup restricts my ability to steer the vehicle safely from a stop, it presents an immediate crash hazard. At the direction of General Motors Corporate, I am escalating this issue and moving the vehicle to a secondary certified facility (Feldman Chevrolet of Novi) on June 23, 2026, for a formal physical hardware and fluid tear-down inspection.Read more

The vehicle experienced a charging system failure while driving. The battery warning light illuminated and battery voltage dropped to approximately 11.7 volts while the engine was running. The vehicle continued to discharge the battery while being driven, creating a risk of loss of electrical power and possible vehicle shutdown. Diagnostic trouble codes included P0562 (System Voltage Low), P0BDE
... (Drive Motor Inverter Temperature Sensor E Circuit High), and P0AEE (Drive Motor Inverter Temperature Sensor A Circuit Range/Performance). The charging system was inspected by an independent repair facility. The serpentine belt was verified to be functioning properly and related fuses were checked. The repair facility diagnosed a failure of the 48-volt eTorque Belt Starter Generator (Motor Generator Unit/MGU). The replacement part is reportedly on backorder. The condition is repeatable and occurs during normal driving. The battery warning lamp and low voltage condition were observed and documented. This failure creates a potential safety concern because the vehicle can lose charging capability while operating on public roads.Read more

While driving my 2015 Chrysler 200, the vehicle randomly shifts into neutral without the driver initiating any gear change. Prior to this occurring, all dashboard warning lights illuminate simultaneously and the odometer display starts blinking so many lights — these appear to be warning signs of an electrical failure that then causes the transmission to shift into neutral while driving and some
... times even not able to shift gears. This has occurred multiple times at normal road speeds and causes sudden loss of power and vehicle control, creating a serious safety hazard. The vehicle was taken to an independent repair shop who confirmed it was an electrical issue beyond their capability. It was then taken to a Chrysler dealership who charged $250 per hour for diagnostics, kept the vehicle for a full day, and was unable to identify the cause. Their only suggestion was an aftermarket radio installed by a prior owner — however an aftermarket radio would not cause all dashboard lights to activate and the transmission to shift into neutral while driving. I contacted Chrysler corporate (Case #97099954) who confirmed no open recalls exist and refused to cover the repair stating the warranty has expired. However this is not a wear and tear issue — this is a safety defect. The sequence of all warning lights activating followed by the transmission shifting to neutral points to a failure in the vehicle electrical control system, possibly the TIPM or Transmission Control Module — both known problem areas on this model. This is a serious safety defect and I am requesting NHTSA investigate this issue on the 2015 Chrysler 200.Read more

1. The vehicle has experienced multiple safety-related defects and is available for inspection upon request: The Instrument Cluster Panel was subject to Safety Recall and required service. The radio system suffered an internal malfunction. The Electric Hydraulic Power Steering system was subject to a safety recall issued on June 11, 2026 and no remedy is currently available from the manufacturer.
... 2. The instrument cluster defect could prevent the driver from viewing critical safety information, like speed, odometer, and warning indicators. The Electric Hydraulic Power Steering defect poses an even greater risk, as a loss or reduction of steering assist could impair vehicle control and increase the likelihood of a crash. Additionally, the recalled steering system may overheat and cause a fire, whether the vehicle is being driven or the engine is off, creating an immediate life-threatening hazard to occupants, nearby persons, vehicles, and property. This is especially concerning as a Texas resident driving in the summer months with extreme temperatures. 3. The instrument cluster issue was addressed through recall service performed by an authorized Jeep dealership. The radio/internal failure was reported to the dealership, which acknowledged the malfunction. The power steering issue has also been recognized by the manufacturer through the issuance of a safety recall on 6/11/2026. 4. The vehicle has been inspected and serviced by authorized Jeep dealerships in connection with the instrument cluster recall and the reported radio malfunction. Additionally, the vehicle underwent multiple routine service inspections before the Power Steering recall was issued, and no steering-system defects or safety concerns were identified or disclosed. 5. No warnings or messages appeared before the failures. I presented the vehicle to Billion Auto Kia in Rapid City, SD — an authorized Kia dealership with these exact symptoms. The service department was unable to identify a root cause and provided no repair path, returning the vehicle to me unrepaired. 7 months later, Courtesy Kia of Brandon in Brandon, FL, another authorized Kia dealership diagnosed the root cause as a failed Body Control Module (BCM). During the service visit, a Kia dealership technician personally witnessed the alarm activating spontaneously while operating the vehicle, further confirming the defect. The BCM failure was present and active at the time of the prior authorized dealer visit and was not detected. The failed BCM has also caused the gear shifter to become completely stuck, unable to shift out of Park, leaving the vehicle fully inoperable. This vehicle has had 6 recall actions performed since purchase — 4 completed after my purchase in August 2023, and two completed prior to my purchase. Despite this recall history, the vehicle has experienced an engine failure and a BCM failure within less than 3 years of my ownership. Being trapped inside a vehicle with a malfunctioning lock system represents a serious safety hazard, particularly in the event of an accident or emergency requiring rapid exit. I am requesting NHTSA investigate whether a safety defect exists in the Body Control Module of 2017 Kia Sorento vehicles and whether additional recall action is warranted.Read more

With no warning my truck lost all anti-lock braking, traction control, stability control, trailer brakes, and 4x4 capability (computer will not allow 4x4 to engage). This is due to an "abs module" failure. Ram/Dodge has no new units available to repair this faulty part and no ETA on if/when they will be available. There was thankfully no accident with this failure but obviously, it could lead to o
... ne. Especially since the part cannot be fixed. Owners are left with scavenging used parts that are still operational. As in my case, my vehicle will have to be "fixed" with an abs module from a wrecked truck! Also, the electronic steering can fail with no warnings. Leading to complete loss of power steering, traction control, and stability control. This failed on my truck as I was driving as well. Thankfully again, no accident with this failure. There is no new factory part to replace this unit either. I had this repaired with a remanufactured unit that failed in one month, then it was replaced again and seems to be fixed for now. These are keystone components to an automobile and it is unacceptable for them to just stop working and have no available parts to the responsible consumer. This leads to owners having a useless worthless vehicle rotting in the driveway or a highly unsafe vehicle on the road to those irresponsible. I want to also shed light to the fact that these issues encompass model years from 2009 to brand new 2026 Ram trucks. There are people with 2025-2026 Ram trucks dealing with this same issue, my vehicles age isn't an iota to molten issues under the surface to Ram owners! There is no communication between the vehicle and VW as a result, and the infotainment system is unable to update. The service tech advised that there are a number or potential issues that will arise because this is a vehicle communication failure. I was forced to prepay for the module and labor, which was ordered on 2/18 (in excess of $500). On 5/26/26 the VW dealership service manager confirmed that the module still has not arrived from VW and that there may be a recall issued because this is such a widespread problem, but that they have no estimated delivery time for the part. The vehicle has had no problems prior to this. If the car is in an accident, there will be no SOS function. The infotainment system, which controls CarNet and the ability to communicate using CarPlay will also be compromised, which may result in the inability to call for assistance in event of an incident. VW Customer Service states they have no ability to track service orders or provide information on the ongoing issue, and stated that the dealership has the only ability to track the part. This was only discovered during a recent inspection; I had no prior warning. Inspection by an independent service center is scheduled and I can provide the vehicle for examination
... upon request. This creates a severe safety hazard, for years I've driven with no passenger airbag protection and false reassurance from a working dashboard. The issue was not disclosed by the selling dealer. I have photographic evidence and can provide all records upon request. Problem has not been addressed by the manufacturer, dealer, or police/insurance representatives yet. Safety of all occupants was unknowingly put at serious risk. 2019 Subaru Crosstrek; Battery is drained even when car is fully turned "off". If the car is not driven every day, battery may drain completely and car won't start. If not driven for several days, battery will discharge so low that it is damaged, in which case it may not have sufficient charge to start the car after sitting overnight. After battery is discharged to the point of damage, it must
... be trickle charged anytime the vehicle is not driven, and even short periods and distances of driving will leave the battery at 25% (or less) of fully charged. Battery replacement is the only way to restore normal operation, but the replacement battery is gradually degraded and ruined just like the original. SOA has not recognized this parasitic battery drain problem in this vehicle. Jump starting the car with auxiliary battery is possible, but this does not reduce the risk of battery insufficiency. Subaru instructs me to have dealer diagnose the problem (at a cost of about $200) and then it might consider a gratis contribution to full cost of diagnosis and repair (estimated at $1,662 including cost of diagnosis). No warning, visual or aural, alerts the driver to this problem.Read more

Steering wheel sticks when driving. Seems like I am always having to correct to stay in lane. It is available for an inspection. I am always afraid it won’t go back to center of the lane to drive and will Continue into the passing lane. I called a dealership and they said that my VIN # is not one that was recalled. No one there has inspected it, and there are no warning lamps. The car was driven

Persistent Battery Drain on a 2021 Subaru Forester Limited. Problems began in year 4 of ownership (~17,000 mileage), after Subaru Starlink Subscription services expired. Red (Error) Starlink SOS indicator light was evident, where a normal light would show green. Battery would fully drain after about 3-4 days of inactivity while parked in a commercial garage. After repeated dead batteries, a jump-s
... tarter battery pack was used regularly, as well as a wall-outlet battery charger. The problem has not resolved with a new and appropriately specified AGM battery, replaced in December 2025. The only solution is to pull the engine-bay fuse #5, which disables the Data Communication Module (DCM), as well as other features (hands-free microphones and audio system). By pulling the fuse #5 consistently, the problem has ceased, even after a two-week period of no use. This provides irrefutable evidence that the DCM powered by fuse #5 is the source of the battery drain. The only resolution Subaru would offer was to insist on a dedicated diagnostic visit to a dealership, with the associated costs of hundreds of dollars to be borne by me, the customer, without a clear understanding of the costs or remedies to resolve this problem. I feel unsafe driving my vehicle for more than short trips given the risk of stranding or further electrical malfunctions that would impact critical vehicle functions or safety systems.Read more

The vehicle is a 2015 Toyota Sequoia with the 5.7L V8 engine and approximately 192,632 miles. The vehicle developed an abnormal internal engine/timing noise, especially when warm. An authorized Toyota dealer inspected the vehicle and diagnosed an internal engine/timing-related condition involving timing chain tensioners, timing chains, chain guides, camshaft gears, lifters, gaskets/resealing, and
... related labor. The estimated repair cost is approximately $19,716. The concern is safety-related because if the timing chain, tensioners, or related internal engine components fail while the vehicle is being driven, especially at highway speeds, the engine could stall, lose motive power, or become unable to safely continue operating. A sudden loss of motive power in traffic could increase the risk of a crash and put the occupants and other motorists at risk. This is a family vehicle used to transport young children, so the risk is not theoretical or acceptable. The problem has been confirmed by an authorized Toyota dealer. The vehicle and affected components are available for inspection upon request. The vehicle has been consistently maintained per the manufacture's schedule. Separate wear items were already repaired as needed. The dealer’s diagnosis concerns internal engine/timing components consistent with manufacturer defects, not ordinary maintenance items. To my knowledge, the vehicle has not been inspected by the manufacturer, police, insurance representatives, or any agency other than the Toyota dealer. Warning lamps or messages are UNKNOWN. The primary symptom before repair was abnormal internal engine/timing noise. This complaint is being submitted to document a potentially serious internal engine/timing component condition that could affect the vehicle’s ability to safely propel itself if the failure progresses. Thank you for your time and attention to this matter.Read more

2024 Ford F150 tremor 14,000 miles The gearshift modual locks into position and locks the transmission (light blinking and transmission light shows on dashboard) You cannot shift to park or start the car, it will do this in drive or reverse and locks the transmission. 6/11/2026 merged onto busy highway vehicle was going approximately 45 mph sudden red warning light popped up to pull the car over safely. Vehicle will shut off. There was nowhere for me to pull over and the vehicle immediately shut off attempted to restart several times over the next 15 minutes, unable to restart nothing no power whatsoever called for a tow truck 40 minutes later due to the child
... ren in the car tow truck was not able to tow asked me to try and start again which I did and it started right up. He followed me to the Chrysler service department where my vehicle has remained since they tell me they can’t duplicate it nor can they find a code that I will have to wait for it to happen again and hopefully a code will pop up that they can fix. Spoke to Chrysler corporatio customer service and then called the dealership. They told her the same thing they couldn’t duplicate nothing they can do. She had no options for me other than to ask them to keep having them drive it until they come up with something , customer service did look it up and states that there is a probable transmission electrical wire issue that caused that to happen. I have requested a full transmission scan as recommended bycomputer forum , but so far no response. I have 2 twelve month old babies and a disabled son that are with me in the vehicle. The DC/DC converter failed and caused the vehicle to immediately lose all power at a main intersection. My wife was driving the vehicle when this happened with out two children in the vehicle as well (ages [XXX] and [XXX]. Luckily, she was able to somewhat get out of intersection of a very busy road. The vehicle had to be towed to the Land Rover dealership. The part was covered by a factory extend
