My 2017 Hyundai Sonata SE, equipped with the Theta II 2.4L GDI engine, is experiencing severe powertrain failures including rhythmic metallic engine knocking, catastrophic oil consumption, and un-commanded surges/jumping in speed while driving. These symptoms manifested following an unauthorized engine replacement performed by Hyundai of El Cajon under Repair Order [XXX], where the dealer covertly
... outsourced my vehicle to a third-party shop (Legacy Motor Works) to install an unverified used salvage engine block rather than a certified factory assembly. This vehicle is a Class Vehicle under In re: Hyundai and Kia Engine Litigation (Case No. [XXX]), and the KSDS software update (Campaign 953) was successfully completed on my vehicle in 2019. The engine knock, surging, and oil consumption are consistent with the known connecting rod bearing failure defect described in NHTSA investigation PE17-007. The dealership is currently denying warranty coverage, citing a mileage limit that does not exist under my Lifetime Powertrain Warranty, and they are withholding physical documentation of the repair terms. The vehicle is also experiencing secondary safety failures, including a continuous airbag SRS warning light (two years) and electrical shorts caused by the salvage-yard alternator installation, which have destroyed 4 to 5 car batteries. This engine failure represents a significant safety hazard, as the vehicle periodically loses power and surges without driver input. I am requesting that NHTSA investigate this pattern of warranty denial, parts-sourcing fraud, and the safety risks posed by this substandard engine installation. The center infotainment display flickers then goes fully blank while driving, caused by a defective coaxial cable with undersized shield width (Hyundai TSB MC-11010978-0001). When the screen blanks, the backup camera also stops working, leaving no rear visibility when reversing. Audio and head-up display still function. The vehicle and component are available for inspection. Safety risk: the displ
... ay and backup camera fail without warning during normal driving and while reversing, eliminating rear visibility and risking striking pedestrians, objects, or vehicles. Failures are triggered by heat, potholes, and braking—routine conditions—so the loss is unpredictable. The dealer reproduced and confirmed the problem, identified the TSB cable defect as root cause, and made multiple failed repairs: tightening clips, replacing the wiring harness, replacing the head unit, and performing the TSB repair. The identical symptom returned after the TSB fix. The vehicle has been inspected only by Hyundai's authorized dealer through these repeated visits—not by police, insurance, or a field engineer (field engineer involvement is being requested). No warning lamps or messages precede the failure. The first symptom is the screen flickering, immediately followed by the screen going blank and the backup camera failing. I purchased the above-listed 2016 Chevrolet Volt through Carvana, relying on the contractual promise that the BECM was covered under Special Coverage N232432680 for up to 15yrs or 150k miles. The vehicle has suffered persistent, severe mechanical and electrical issues resulting in a prolonged loss of use: 1. It was then delivered to Vic Koenig Chevrolet (1040 E Main St, Carbondale, IL 62901) via Tow By Freedom Towing the following Friday, May 29, 2026. 2. Delayed Work Start & Shop Stay: Vic Koenig Chevrolet was able to officially begin diagnostic and repair work on Wednesday, June 3, 2026. The facility held the vehicle for several weeks to address the failures. 3. Brief Release and Immediate Re-Breakdown: I picked up the vehicle upon its supposed completion on Tuesday, June 16, 2026. Then less than 24 hours later, on Wednesday, [XXX], the vehicle suffered a recurring breakdown and had to be returned to the dealership immediately. 4. Wrongful Coverage Denials: On Thursday June 19, 2026, Silver Rock claims representatives issued explicit coverage denials to me via text message. That same day, service representatives from Vic Koenig Chevrolet phoned Silver Rock multiple times to provide diagnostic evidence, But SilverRock repeatedly issued oral denials over the phone. They stated the PCM and BECM were not included in the contract, screenshots of the vontract state the opposite. SilverRock sent this refusal via email to me on June 19. I also contacted GM by phone on Wednesday 5/27/26 and so did the dealership and they refused to cover my vehicle under the Special Warranty for the known issue. My vehicle is confirmed affected by three active NHTSA safety recalls, none of which currently have an available remedy: • NHTSA Recall 25V741000 – High Voltage Battery May Fail and Cause Fire. Issued October 2025. This is the third recall issued for this same underlying defect. I have been monitoring this recall since it was issued — now over eight months later — with no resolution. •
... NHTSA Recall 25V710 (Manufacturer Recall A7C) – Loss of Motive Power While Driving. Issued October 2025. An interim backend policy update was implemented but does not constitute a full fix. • NHTSA Recall 26V363000 – Power Steering Wiring May Catch Fire. Issued June 2026. The electric hydraulic power steering pump wiring may overheat and cause a fire even when the vehicle is parked with the ignition off. No remedy currently available. As a result of recalls 25V741000 and 26V363000, Chrysler/FCA has officially instructed me to park my vehicle outside and away from all structures at all times, and to refrain from charging my vehicle until a remedy is available. That charging restriction is a serious problem independent of the safety concerns. I specifically chose and leased the 4xe model for its plug-in hybrid capability and the fuel economy benefits it provides. I am now officially instructed not to charge it — meaning the vehicle cannot be used as intended or as it was represented to me at the time of lease signing. I have been unable to charge my vehicle for eight months, with no remedy available and no timeline provided. I do not feel safe driving this car. It can catch fire not only while parked, but also while driving. I am paying $500 per month for a vehicle that cannot operate as intended, poses active safety risks with no fix available, and has been in this condition for the majority of my lease term. The value I am receiving does not reflect the payments I am making.Read more

The engine cooling fan activates at high speed and runs continuously for an extended period — in one instance, overnight — after the vehicle has been turned off, locked, and left unattended. This has occurred on multiple occasions. No warning lamps or messages appear before or during the event. The vehicle gives no indication anything is wrong; the fan simply runs at full speed on its own, wit
... hout the ignition on, for hours at a time. The component involved is the engine cooling fan and/or its control module. Based on owner community research, the likely cause is a faulty signal being sent to the Engine Control Module, which triggers the fan to run even when the vehicle is not overheating and is fully shut off. The component is available for inspection. Safety risk: The vehicle is parked in an attached garage. An electrical component running uncontrolled and unattended for hours inside an enclosed structure attached to a residence poses a credible fire hazard. A bedroom is located directly above the garage. This is not an inconvenience — it is a fire risk to occupants of the home. The problem has been reported to an authorized Kia dealer on multiple occasions. The dealer was unable to replicate the condition during the service visit and did not perform a repair. The defect is intermittent in nature, which makes dealer replication difficult, but video documentation of the fan running with the vehicle off and unattended exists for multiple incidents. The vehicle has not been inspected by police or insurance representatives. No warning lamps illuminate during or prior to the event. The fan simply activates after shutoff with no prior symptoms. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The primary failure involved the engine. The vehicle experienced a major engine failure that resulted in an engine replacement under a manufacturer recall. After the replacement engine was installed, the vehicle later developed additional engine-related problems. An independent mechanic advised th
... at the vehicle would require another engine replacement or extensive engine repairs. Additional defects included a malfunctioning moonroof that would no longer close properly and other mechanical issues affecting the vehicle's operation. The vehicle may be available for inspection depending on its current status and location. 2. How was your safety or the safety of others put at risk? The repeated engine problems created concerns about the reliability and safe operation of the vehicle. Sudden engine failure while driving resulted in loss of power, inability to maintain speed in traffic, and being stranded in unsafe conditions. The ongoing mechanical failures caused significant concern regarding the vehicle's dependability and safety. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The original engine issue was confirmed by authorized service personnel and resulted in an engine replacement under a manufacturer recall. The subsequent engine-related problem was evaluated by an independent mechanic, who advised that the vehicle required another engine replacement or significant repairs. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected in connection with the original engine recall and replacement. It was also inspected by an independent mechanic regarding the subsequent engine issues. At this time, I am not aware of any inspection by police or insurance representatives related to these mechanical failures. 5. yes, right awayRead more

The hybrid cooling system on my 2019 Porsche Cayenne E-Hybrid repeatedly displayed a red dashboard warning while the vehicle was in motion: "Hybrid coolant level too low / Park vehicle safely / Let vehicle cool down." This warning appeared on multiple occasions at speeds of approximately 22 mph, 38–40 mph, and 47 mph, as documented in timestamped photos and videos taken in November 2025. A separ
... ate warning in July 2025 read: "Engine cannot be started / Remove charger plug." The warnings indicate a potential thermal management or hybrid cooling system failure. A moving vehicle being instructed to park and cool down presents a safety risk to the driver and others, particularly if the condition causes sudden power loss or thermal event. The vehicle was serviced at an authorized Porsche dealer in 2025. Despite that service activity, the hybrid coolant warning recurred while driving in November 2025, suggesting the underlying condition was not fully resolved. The issue was reported to the manufacturer (PCNA), who acknowledged it under a case number and offered a service voucher. The selling dealer attributed the concerns to a manufacturer charging-cable campaign. Neither response addressed the recurring in-motion warning or confirmed the hybrid cooling system was repaired. Timestamped photo and video evidence is available showing the dashboard warnings and vehicle speed at time of occurrence. Complete repair orders have been requested but not yet provided. The component may be available for inspection upon request.Read more

While I was trying to move to the shoulder there were tractor trailers behind me and to my right that began flashing their lights because my speed drastically dropped and was unable to safely stay with the flow of traffic in the heavy rain. While still in Drive car started changing through notices on my digital screen "Check Blind-Spot Safety system" "Check Forward Safety system" with the car crash icon, and "Check Driver Attention Warning (DAW) system." Then I began trying to press the accelerator pedal again and the car would not move at all but the tachometer was going to 2x1000rpm. When I turned the car off and turned it back on again the engine light was still on, but the accelerator pedal was responsive. About an hour later when the tow truck driver from AAA attempted to drive the car on to the flat bed the accelerator pedal was unresponsive again. This was at least the fifth time my vehicle had lost propulsion without warning. Every prior time Bowser Hyundai had different reasons for the failure, made a repair, and said it was safe to drive. Another incident occurred 4/7/2026 using cruise control at 84 mph when the car unexpectedly lost propulsion. The accelerator was again unresponsive, I was on the turnpike with two children in the vehicle. I steered towards the shoulder and the car slowed to a stop. I own a 2025 Volvo XC60 plug-in hybrid (PHEV). The vehicle has recurring software defects that create a serious safety risk. First, the infotainment and central control system repeatedly reboots while I am driving. On a single trip from Washington, DC to Burlington, Vermont, the system rebooted approximately 8 to 10 times. It has also rebooted within minutes of starting the vehicle during local dr
... iving. Each reboot blanks the center display, which disables the backup/parking camera and interrupts the speedometer and driver-information display, climate controls, and other functions. Losing these displays and controls while the vehicle is in motion is a significant safety hazard. Second, after installing a Volvo over-the-air (OTA) software update, the vehicle was rendered completely inoperable. It would not unlock or start and could not be accessed through the key fob or the Volvo Cars app, and it had to be towed to the dealer. Based on numerous owner reports, failed OTA updates that disable the vehicle this way are a known, recurring problem on these models. Third, the front and rear parking cameras have both cracked during normal use, with no impact damage, and the vehicle displays a "parking cameras need cleaning" message at nearly every stop. No crash or injury has occurred. I have reported these issues to the selling dealer and to Volvo customer care. The reboot and lockout problems are recurring and have not been durably resolved.Read more

The car was immovable and unresponsive. We tried to restart the car with the assistance of our Porsche Dealer but nothing worked. We had no choice but to pray for our safety. We have completely documented our experience: photos, videos and phone calls with 911. NOTE: your system is not allowing us to upload any of our photos or videos but we would like to provide these to you directly so please reach out to me to let me know how we can share these with you. When the car malfunctioned, it seemed that every alert in the car's computer system flashed in serial fashion - Engine Control Error, P Button Defective, E Sound Error, Lane Assistance Malfunction, Electrical Parking Brake Error. It was towed to my local Hyundai dealership, but instead of expressing any genuine concern for the significant risk I endured while being placed In harm's way, they refused to fully diagnose the vehicle, and instead accused me of "jumping in reverse," even though I did no such thing (and don't even know what that means). The dealership has no evidence to suggest any such event took place, and simply wants me to pay over $1,800 for replacement of the battery connection terminals, despite being in-warranty. Obviously, the vehicle was already running – there is no way that the dealership's false allegations could possibly be correct. I contacted Hyundai Motor America and was assigned a caseworker, but they were not interested in finding a resolution, and unable to compel the servicing dealership to perform a full diagnostic or provide warranty service. Feeling that I was running out of options for the vehicle to be properly diagnosed, I contacted the dealership where I purchased the car, and provided the photographic evidence & service documentation required. They agreed that there's no way that I could have created this issue, and that further investigation is warranted, but HMA only cares about money – not my or my family's safety.Read more

I was making a left hand turn at an intersection with no cars in immediate proximity to my car and the emergency braking system activated violently causing the car to shut off. The pre sense light came on the dashboard. This happened 3 different times. I went to Freehold Audi and they stated the pre sense was doing its job. I reported this to Audi Headquarters last week and a report was written. O
... ther electrical issues started happening, the lane change sensor was going off randomly, the windshield wipers were going off randomly. I traded the car in yesterday to Lexus of Eatontown. They said they were going to sell the Audi retail, I’m thinking to the auction. The car has less than 10,000 miles on it. I informed them about the issues I was having with the car but I’m worried they will sell it to someone without repairing the pre sense issue. I do not want anyone to get hurt in this vehicle. I hurt my neck and chest when the car violently stopped, so did my son. Someone could have easily rear ended me.Read more

On June 18, 2026, I was driving my 2024 Grand Wagoneer on a busy city road. As I approached a busy intersection, I stopped at the red light. When I attempted to move forward when the light turned green, it moved a few feet (putting us in the middle of the intersection), the vehicle seized and became immobile. Almost every light on the dashboard lit up. Several messages flashed back and forth o
... n the screen including "Service transmission" and then "service electronic stability control". I immediately tried to use the SOS function, and when they answered, they were unable to hear me. A person in a truck behind me told me to put my vehicle in Neutral so that the vehicle could move out of the road as it was blocking traffic. When I tried to turn the gear dial, it turned, but the P light stayed on and the vehicle would not go into Neutral. I checked the parking brake and it was not on, nor would it engage. The vehicle being stalled, with zero ability to move put my children and I in danger as there was not a safe place to even exit the vehicle as traffic was completely surrounding us. I tried to turn off the vehicle, but the vehicle stayed in the "run" function even after being pressed. Emergency services was my only next choice. One more push of the off button and the vehicle finally turned off. I waited for 2 minutes and tried to turn the vehicle on again. It did turn on, showing the check engine light. I then was able to put the car into gear, and it moved. I moved the vehicle out of traffic to the next available parking lot. I immediately contacted the dealership and told them I would be having the vehicle towed there for inspection and I do not feel comfortable driving this vehicle again. Unfortunately, this is the 3rd time my car has put me in a dangerous situation in the middle of an intersection, the other 2 being false crash alerts (turning left in turning lane and car thought I was going into oncoming traffic).Read more

Dear NHTSA Vehicle Safety Hotline Team, I am writing to formally report a serious safety concern regarding my 2021 BMW X7. My vehicle is exhibiting a dangerous condition where it fails to start on the initial attempt, displays a "Repeat Start Attempt" warning message on the dashboard, and then heavily strains to turn over on the second attempt. While I am aware that BMW issued a safety recall (NHT
... SA Campaign Number 24V-576) regarding starter motor overheating and engine fire risks for certain 2019–2021 models, I believe the current remediation or scope may not fully protect consumers from the immediate hazards of this failure. When the vehicle demands a second start attempt, it forces a high electrical draw that risks severely overheating the starter mechanism. This creates an immediate risk of electrical thermal overload and potential engine compartment fires while trying to get the vehicle moving.Below are the specific details of my vehicle and the safety incident:Vehicle Make and Model: 2021 BMW X7 Vehicle Identification Number: [XXX] Current Mileage: 57226 Incident Description: The vehicle consistently fails its initial ignition sequence. It prompts a "Repeat Start Attempt" notification, forcing a high-stress second startup sequence that poses an immediate electrical fire hazard.I request that the NHTSA review this ongoing issue, ensure that BMW's remedy effectively eliminates the fire risk during these secondary start cycles, and hold the manufacturer accountable for immediate component replacement.Thank you for your prompt attention to this safety matter. I look forward to your investigation. Sincerely, INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Power steering and StabiliTrack warning pops-up randomly and cuts off power steering assistance while I'm driving on the highway or on country roads. The power steering is very hard to maneuver. However, when it's safe to do so, I have to pull over on a safe place and have to restart my truck twice, in order to regain power steering. I have taken it to an independent mechanic and to a Chevy dealer
... ship on Merced. The dealership service tech informed me that in order to service the vehicle and find the code that is making this issue, that I would have to continue driving the truck and bring it in to the dealership with the power steering and StabiliTrack warning. According to the technician, they can't do much due to that they don't know what is causing the power steering to malfunction. I haven't been able to take it to the dealership because the warnings have occurred while I'm far away from a dealership or diagnosis area. Also, I will not risk my safety maneuvering a vehicle that has lost electric power steering around a city where lots of left or right turns are necessary to drive to a locality. This is the only vehicle I have to go to work or travel and fear for my safety when this happens. PLEASE HELP.Read more
