On XXX, my 2025 Hyundai Ioniq 9 displayed a battery fault warning (DTC P1AAF00). I was instructed by Hyundai to bring the vehicle to a dealership immediately as other Ioniq 9 owners had experienced sudden and complete loss of motive power during highway operation. The vehicle has been at Pacifico Hyundai in [XXX] since that date. I was initially told the battery control module required replacement
... with only 7 units available nationwide. On May 5 I was informed via text that the entire high voltage battery pack requires replacement with no release date provided. The vehicle has been out of service for 48 days with no repair timeline. This fault presents a serious safety risk as sudden loss of power at highway speeds creates an immediate collision hazard for the driver and surrounding vehicles. My car is a limited version which equipped a Microsoft SYNC system which providing backup camera and vehicle system setting and information such as hybrid status, driven mode, battery charging status, GPS navigation, etc. Just because this 'Upgrade', the clock and date setting is different with regular escape hybrid cars which comes from the satellite only. It was working well until one year ago. Now, my car is showing Oct, 2006 and running day after day. I did some research on internet and found a lot of drivers with same issue. A engineer revealed the root cause that Ford used a ten digits week counting method based on a starting date in 2006 to display the current date which could only working within 1023 weeks (17.5 years). Without changing the hardware, software might not correct this issue. I'm not sure and went to Ford dealer shop last week, they denied this issue and refused to recall even repair this car. I hope Ford could recall the fix this obvious defective product soon.Read more

Parking Brake The Sticker on this U-Haul 15 foot Truck says, "Engage the Parking Brake before turning off the Engine, otherwise UNCONTROLLED OPERATION OF VEHICLE may occur.". I pulled on the Parking Brake as the Sticker warned. With my foot off the Gas Pedal, at about 25 - 30 MPH the U-Haul Steered way to the right by itself, and drove onto the Right Curb. This was quite Frightening and Une
... xpected. Then it happened again later that day at just about 30 MPH, again with my foot off the Gas Pedal and Kind of Coasting into a small Town. Later that Night it happened again at about 25 -30 MPH and pulled to the right and almost into the right side ditch, on a two lane Highway. My Safety was at risk because I could have hit a Telephone Pole or have been steered into the right side ditch. IF it had happened, on a Multi-Lane Highway, I could have Hit a Vehicle in the Right Lane. Others are put at Risk, because anyone on a Sidewalk near the Right side Curb, Could have been Hit, by this Uncontrolled Steering. The U-Haul Regional Manager, Emily Noe said it had not been reported by anyone else. This U-Haul Truck had about 14K miles on it and is fairly new. I assume this U-Haul Truck had been Inspected, before being placed on the Road. There is the Yellow Sticker inside the Truck, with Black Letters that warn about pulling On the Parking Brake before Parking and shutting down the engine....which I did!!! I feel other U-Haul Customers should be warned before an Accident DOES occur. Better Yet this U-Haul Truck should be taken off the Road.Read more

I purchased my 2020 Chrysler Pacifica in January 2020. By April 2021, I began experiencing ongoing issues that have never been fully resolved. In the six years I have owned this vehicle, I have gone through six batteries. The vehicle has stalled in the middle of the road and, at other times, has failed to start altogether. The vehicle also does some jolting randomly which can be very harsh to the
... passengers inside. I specifically purchased this vehicle because of its safety features, as I have young children. Unfortunately, these repeated failures have left me feeling unsafe and concerned about the reliability of the vehicle. Each time I have brought the Pacifica to the dealership, the battery has been replaced or recharged and I have been told the issue was fixed. However, within six months to a year, the same problem returns and the vehicle is back in the shop. The issue has occurred again, and I am at a loss as to what to do next. It is clear that there is an underlying problem that has not been properly diagnosed or repaired. After six years of recurring breakdowns and unsuccessful repair attempts, I have been unable to fully enjoy or rely on this vehicle. I am requesting that this matter be taken seriously and that a thorough investigation be conducted to permanently resolve this ongoing issue. Given the repeated repair history and the safety concerns involved, I believe this situation warrants immediate attention.Read more

A major engineering and material design failure occurred involving internal fluid containment seals, resulting in a severe electrical safety hazard. Pressurized engine coolant breached an internal component seal and began migrating internally directly through the vehicle's engine wiring harness via capillary/siphoning action. The fluid used the sealed wiring loom as a literal pipeline, traveling
... inside the wire insulation to bypass all outer weather seals and completely flood the Engine Control Module (ECM) pin connectors. The vehicle's dealership service department documented extensive galvanic corrosion and shorting inside the primary vehicle computer pins due to this blue-green fluid pooling. Because the dealership has not yet completed the full repair, the exact originating component source of the leak (e.g., the engine coolant temperature sensor or the coolant expansion tank housing barrier) remains unconfirmed. However, the physical mechanism of the coolant migration through the sealed harness into the primary electrical architecture is fully verified. This defect presents a critical safety risk. The sudden flooding and short-circuiting of the Engine Control Module can cause an immediate and unexpected loss of engine propulsion, power steering assist, and electrical control while driving at highway speeds, creating an imminent risk of a high-speed collision.Read more

This complaint involves a violation of safety recall obligations for Jeep Recall 68C (NHTSA Campaign 25V741000) concerning a high-voltage hybrid battery fire hazard. On Friday, May 29, 2026, the vehicle was brought to Carman Chrysler Jeep Dodge (New Castle, DE) for mandatory recall testing. The vehicle failed the manufacturer's diagnostic analysis due to a critical internal cell separator defect
... . Because of the immediate thermal and fire risk, the dealership service department immediately grounded the vehicle on-site, stating it is completely unsafe to drive or park near structures. Stellantis/FCA has failed to provide a timely remedy or replacement parts, leaving our vehicle stranded on a commercial lot. Furthermore, the manufacturer has refused to provide direct-billed alternative transportation, offering only an out-of-pocket rental reimbursement model. Because parts are delayed for several months, bankrolling thousands of dollars upfront for a rental vehicle is a massive financial burden that is completely unfeasible for our family. The dealership has also failed to provide physical or digital copies of the failed diagnostic paperwork or Repair Order. We are currently left with a grounded vehicle and no viable, safe transportation.Read more

I was able to slowly pull over to the fire right hand shoulder, even though it was during peak traffic time. As I pulled over, I started seeing smoke come from under the hood. I then seen a big flame which made me quickly get out of the vehicle and then seconds two minutes later the entire vehicle was in golf in flames and had many explosions throughout. I had no prior indications notifying me that there was something wrong with my vehicle. I had no problems with it. There was only 12,500 miles on the vehicle. There was no obvious cause to this happening. It was only me in the car however if there were small children in car seats or less mobile elderly people in the car, I don’t think that they would have made it out safely. The driver's seat wiring harness on my daughter's 2007 Acura MDX has failed under normal operating conditions. The physical adjustment and movement of the driver's seat causes the factory wiring harness to over-tension and fracture internally. This has resulted in an intermittent, escalating failure of the Supplemental Restraint System (SRS), throwing diagnostic trouble code DTC 71-11 ("open circu
... it in the driver's seat position sensor"). This is a known manufacturer engineering defect documented under Acura Technical Service Bulletin TSB 11-023. This defect poses an immediate safety hazard because the vehicle's computer automatically disables the driver's side airbag safety deployment when the circuit loses connection. The manufacturer is currently refusing to cover the repair due to the age of the vehicle, despite it being a critical safety-restraint system failure that leaves the driver unprotected in a collision. (The exact date of first onset is estimated, as this safety defect is an ongoing, intermittent electrical failure that began several months ago but only recently became more frequent. We also were originally unaware that the airbag warning light coming on meant the airbags were disabled, or we would have brought the car in sooner.)Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE97AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is a
... vailable for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset by standard non-dealership methods like unplugging the battery.Read more

I own a 2022 Nissan Rogue with approx 71,139 miles on. While driving on the freeway at 65mph, the vehicle suddenly lost power and became disabled. Prior to the failure, the vehicle did not dispaly an overheating warning, coolant warning, or other indication the engine was overheating or experiencing coolant loss. The vehicle was being driven normally and was on the way to routine maintenance and f
... uel stop when the failure occurred. An independent repair facility diagnosed coolant intrusion into the combustion chamber and determined the engine had overheated and required replacement. The vehicle's sudden loss of propulsion occurred without warning creating a safety hazard for myself and my daughter as I was forced to coast to a stop in the Gore Area of the freeway just as you entered the [XXX] from [XXX] due to the inability to maintain speed on the freeway. This was the safest area for us to park but it was terrifying as cars zoomed past us from both sides. I am concerned that the engine failed without adequate warning to the driver and that similar complaints have been noted by several others describing the same issue with the same year, make and model as my car. Multiple reports have said it occurred while on the freeway, making this a greater concern for safety. I took my car to Nissan who stated they see the coolant intrusion, but that it was not covered under recall. I asked what caused the internal leak, to which they replied that would require them to take apart the engine to examine further at my cost. I advised the issue with the car not indicating it was even out of coolant to cause it to overheat was concerning because even if I replace the engine, if the electrical failure isn't addressed I could run into the same issue and this time actually get into an accident that could cause death or serious injury. Dear Sir or Madam: I am submitting a safety complaint regarding my 2022 Audi Q3 involving water intrusion from the sunroof. At approximately 41,000 miles, approximately a month ago, water began leaking through the front edge of the sunroof despite the sunroof never being opened prior to the issue. The leak was not caused by clogged drains or lack of maintenance. Water entered through the overhe
... ad console, resulting in failure of the interior lighting system causing lights to flicker on/off and water dripping directly onto me, the driver, while operating the vehicle. These issues created a significant distraction and potential safety hazard, especially during night driving. The dealership diagnosed the issue and confirmed that the repair required modification of the sunroof assembly and track to correct a water intrusion problem. This was not a routine maintenance issue. The total cost of repair was $1,259.18, which I paid out-of-pocket. Additionally, the service advisor at the dealership stated that this is a frequent problem seen on Audi Q3 vehicles and expressed surprise that it has not already been subject to a recall. Given that this issue involves water intrusion affecting electrical components and actively interfering with safe vehicle operation, I believe this may represent a broader design or manufacturing defect impacting vehicle safety. I respectfully request that NHTSA review this issue for potential investigation and possible recall action. Thank you for your attention to this matter.Read more

On May 23, 2026 I was driving family and friends around San Francisco, CA in my 2017 Honda Pilot (48K miles). We were driving up multiple steep hills, up to a 20% grade in some places with a full car of 7 people. The car handled it well. However, as we neared my home 25 miles away, the car started shaking and the check engine light started flashing. We searched online and the advice was to pull
... over immediately to prevent the car from overheating and causing permanent, expensive damage. We eventually made it home, shaken, and didn't touch the car for a couple days to let it rest. I learned that there is a warranty extension "Warranty Extension TSB 20-100" for what seems like this exact problem concerning the fuel injectors. The spark plugs have been replaced in the last year, which eliminates that as a potential issue. The next week, I took the car to Honda San Carlos where it's been maintained most of its life. I informed them of the symptoms and that the car is eligible for this warranty extension. They tested it and found nothing. I told them that testing it in the shop or driving it around a few minutes on flat ground with one person is not going to replicate the driving conditions we encountered but they didn't seem to care and I had to pay $250 anyway. I took this to another dealership and the guy said it sounded exactly like an Warranty Extension TSB 20-100 issue. They tested it and also could not find anything. I called Honda Corporate and they will also do nothing. This is clearly a safety issue as this problem will happen again at some point. I have a family and young children and do not want to be stuck on the freeway as a road hazard to ourselves and other drivers. Honda has clearly identified this as a problem yet they won't authorize the repair. They are waiting for this to happen again before they'll fix it, and who knows if this ticking time bomb will occur in a safe place or a dangerous one.Read more

2021 Subaru Forester The vehicle has experienced recurring electrical and battery-related issues for several years. The original battery was replaced by the Subaru dealer with a larger-capacity battery because of repeated starting concerns. The replacement provided only temporary improvement. The vehicle continues to experience rough starting on a regular basis, even after installation of a newe
... r aftermarket battery with a higher cold-cranking-amp rating. The current battery is approximately five months old. The most significant safety concern occurs during Auto Start-Stop operation. On multiple occasions, the engine has shut off at a stop as intended, but then failed to restart when attempting to move again. When this occurs, multiple warning lights illuminate on the dashboard and the vehicle is unable to proceed until it is shifted into Park and restarted manually. Because of this issue, I now disable the Auto Start-Stop feature on every drive to avoid a potential failure to restart while in traffic. The issue has been ongoing for years and has occurred with multiple batteries, including the dealer-installed replacement battery and a newer aftermarket battery. The dealer suggested that an aftermarket dash camera could be contributing to battery drain. The dash camera was disconnected, but the problem continued. I am concerned that failure of the vehicle to restart after Auto Start-Stop operation could leave the vehicle disabled in traffic and create a safety hazard. The condition appears to involve the vehicle’s electrical system, battery management system, charging system, Auto Start-Stop system, or a combination of those components. The vehicle is available for inspection upon request.Read more

I was driving in the left lane of this road at 40-45 mph. My view of the road was immediately obstructed, but I was able to crouch sufficiently to s

I was away from home when this occurred. When I got in the care to drive home, there was a warning that came on, for the first time, that read as follows: FRUNK FAULT DETECTED; SERVICE REQUIRED. There was no advice to avoid driving the car. I checked the hood/frunk to be sure that it was firmly closed, then started for home, avoiding my usual freeway drive at 65mph for a slower trip on local roads. It is a good thing I did, because the risk of a major accident with injury and serious property damage would have been far greater at higher speed. I arrived home without the frunk opening again, called for Ford Roadside Assistance, and arranged to have the car towed to Germain Ford of Columbus, where it still is, awaiting the delivery of a new frunk latch system. The dealer has determined that there is likely a defective sensor in the frunk latch. This is a known issue with the Ford Mach-e, affecting not only the frunk but also sometimes the hatch lid in the rear. This is a serious safety hazard, and a review of reports over several models years from Mach-e owners indicates that I am not alone in having experienced it. , I had MY Honda dealership replace my touchscreen and head unit due to issues with ghost touches and apps switching back and forth. Once I got my car back, which took two weeks, the problems seemed to be resolved. However, after a few weeks, the ghost touches started happening again, about once a week. I was planning to live with that, given the amount of time they had my car. Now, fast forward
... to the present time: the ghost touches are back, and the touchscreen refuses to function 95% of the time. I had contacted my service advisor at the dealership on 5/28/26 and replied that she would get with the Service manager and shop foreman. This is ashame that a company like Honda could not stand by their product better to fix this issue that has been known since 2024.Read more

I was driving down the road and came to a stop at a stop light. After accelerating again my car lost the ability to increase speed above 20 miles/hour and would not shift gears. I was heading to the turn lane to get on the interstate and quickly had to change lanes to find an area to pull over as I could not reach highway speeds. I turned the car off. I turned the car back on and the problem resol

The DQ250 6-speed DSG (dual clutch) automatic transmission in my 2017 Volkswagen CC Executive has catastrophically failed at 37,768 miles. COMPONENT FAILURE: The mechatronic unit, the electro-hydraulic controller that manages all gear selection and clutch engagement inside the DSG transmission has failed. The transmission is available for inspection upon request. WHAT HAPPENED: Symptoms began with
... intermittent failure to engage gear after stopping at intersections and stop signs. The transmission would slip out of gear or refuse to engage, leaving the vehicle momentarily unable to accelerate from a dead stop in traffic, a dangerous condition with cross-traffic approaching. The condition progressed to include an audible whining and slipping noise during downshifts, consistent with inadequate hydraulic clamping pressure on the clutch pack. The transmission then lost the ability to engage reverse gear entirely. The vehicle was driven home in forward gears only and is now inoperable and parked. SAFETY RISK: On multiple occasions, the vehicle was unable to accelerate from a complete stop at intersections with active cross-traffic. Loss of reverse gear eliminates the ability to maneuver out of dangerous situations in parking lots, driveways, and streets. This vehicle is unsafe to operate. WARNING SIGNS: No warning lamps or dashboard messages appeared prior to or during the failure. The first symptom was intermittent hesitation and slipping when attempting to engage from a stop. The condition worsened rapidly over a short period until reverse was lost completely. DEALER/MANUFACTURER CONTACT: The manufacturer was contacted. A callback was promised within two business days and was never received. The vehicle has not yet been inspected by a dealer because the servicing dealer, a Volkswagen-authorized dealership, has permanently closed. The vehicle is awaiting tow to a specialist for diagnostic confirmation. MAINTENANCE HISTORY: At SM VW until it closed. Once at Indy ShopRead more

WHAT FAILED: Ford's recall-tracking and consumer-information system. Ford has internally inconsistent records re: Safety Recall 25V315 / 25S49 (Rear Camera Screen Software Update) on my VIN [XXX] . Vehicle and Ford-account portal available for inspection. STATUS SUBSYSTEM SAYS CLOSED: (1) Mailed "Successful OTA Update" notice received 6/1/2026 (verbatim): "the software to remedy this concern has
... been installed on your vehicle through a free Over-The-Air (OTA) software update, meaning the recall is now closed" (2) NHTSA database refreshed 6/2/2026: 25V315 no longer in unrepaired list (3) Ford VIN-specific recall dashboard 6/3/2026: 25S49 removed; only 25C69 remains DELIVERY SUBSYSTEM SAYS NOT DELIVERED: (4) ford.com/support/software-updates 6/2/2026: 9 of 10 expanded OTA detail cards captured, NONE references 25S49. Portal DOES show Compliance Recall 26C10 (ITRM-25.B04) with explicit completion language ("This completes Compliance Recall 26C10. No further action is required.") - proving the portal surfaces recall OTAs when they actually deploy. (5) In-vehicle SYNC Software Updates 6/2/2026: silent on 25S49. SYNC version 25221_PRODUCT Rev 2025 unchanged from 3/26/2026 despite 25S49's remedy being an APIM software update. (6) Ford MODEL-level recall page 6/3/2026 (verbatim): "Recall service not available right now. We have a remedy but parts are not available right now." SAFETY RISK: Ford simultaneously tells me the recall is closed via OTA delivered to my VIN AND publishes that the remedy parts are not yet available. Both cannot be true. Consumers cannot verify whether safety-critical recalls have been delivered. REPRODUCED/CONFIRMED: Contradiction is Ford-generated, reproducible by any reviewer. INSPECTED: No dealer or independent inspection of this discrepancy has occurred. WARNING LAMPS / FIRST APPEARANCE: Not lamp-driven. Confirmed [XXX]. I request ODI review of Ford's recall-tracking accuracy for OTA-remediated recalls. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552Read more

WHAT FAILED: Ford-shipped OTA software (update ID OBCC-AS.AU, "Plug & Charge Improvements," delivered May 10, 2026) generates an admitted false-positive instrument-cluster wrench-light and Ford App service-needed notification. The OTA is installed; vehicle available for inspection on request. FORD'S OWN ADMISSION (verbatim, from Ford's published OTA release note): "In some instances after this upd
... ate, a 'Service vehicle soon' wrench icon may appear in your instrument cluster... You may also receive a Ford App notification indicating service is needed on the vehicle, which you can disregard. Please be assured this is a visual notification only..." SAFETY RISK: The same wrench-light is the documented symptom of NHTSA Safety Recall 25V863 / Ford 25C69 (Park Pawl engagement failure) - active and unrepaired on this VIN per NHTSA's June 2, 2026 refresh (see complaint 11741722). Ford's 25V863 NHTSA filing states a Park Pawl failure displays "A WRENCH LIGHT AND SHIFT SYSTEM FAULT MESSAGE... IN THE INSTRUMENT PANEL CLUSTER" with automatic EPB application and roll-in-park crash risk. Ford has instructed me in writing to disregard the same notification surface that would warn of a genuine 25C69 failure. The two are visually indistinguishable without dealer diagnosis. REPRODUCED/CONFIRMED: Ford itself confirms the defect in its written release note. INSPECTED: No dealer or independent inspection of this OTA-induced false-positive has occurred; Ford preemptively instructed disregard. WARNING LAMPS / FIRST APPEARANCE: The wrench-light icon and Ford App service-needed notification ARE the symptoms. They began with the May 10, 2026 OTA install per Ford's published note. Both are also the warning channels for the unrepaired 25C69 safety condition. I request ODI review of whether OBCC-AS.AU constitutes a defect under 49 U.S.C. 30102(a)(11) when it masks an active federal-recall warning channel on the same VIN.Read more
