Vehicle: 2026 Yamaha MT-10 I am reporting a safety-related defect involving my 2026 Yamaha MT-10 motorcycle. The motorcycle has experienced an intermittent no-crank/no-start condition. When the problem occurs, the instrument panel and electrical system power up normally, but pressing the starter button produces no engine cranking. The defect occurs unpredictably. At times the motorcycle starts n
... ormally, while at other times it will not start despite having battery power and the ignition system turned on. Because the condition is intermittent, it can be difficult to reproduce consistently. This defect creates a safety concern because the motorcycle may fail to start when needed, potentially leaving the rider stranded in an unsafe location such as a roadway, parking lot, fuel station, or remote area. Riders must be able to rely on the motorcycle starting when required. The motorcycle has been presented for diagnosis, and possible causes discussed have included components within the starter control circuit, such as the starter switch assembly, safety interlock system, wiring, relays, or related electrical components. However, the intermittent nature of the failure raises concerns about a potential defect affecting reliability and rider safety. I am submitting this complaint so the issue can be documented and investigated for possible defects affecting Yamaha MT-10 motorcycles.Read more

The charging system for the battery failed when I was on an out of state road trip. The voltage of the bike would drop between 11 and 12 volts when slowing down. I slowed down to make a tight turn on a canyon highway when the bike lost power and stalled. The motorcycle continued to stall several more times while in motion on the highway and I had a difficult time restarting the motorcycle on the s

It worked for another few hundred miles until the new battery was also depleted and I had the same issue on the side of the highway outside Sedona. I had to rent a Uhaul for $551 to be able to return home safely. I took the motorcycle to the Harley Davidson dealership in Salt Lake City and paid approx. $1,248 to have the Stator and charging system replaced. the vehicle was driven for approximately 30 minutes, operating with zero dashboard warnings or performance indicators. The vehicle was then parked on my property and turned off completely. Approximately two hours and thirty minutes later, the vehicle spontaneously ignited while stationary and unoccupied. The local fire department responded and successfully extinguished the blaze. Both physical obs

This exact location houses the factory electric water pump and its electrical plug connector, directly matching the failure mechanism detailed in NHTSA Campaign 24V608000, wherein fluid intrusion causes a severe, persistent electrical short circuit capable of igniting a fire hours after the vehicle is parked. The car is completely totaled because of this.Read more

The driver's seat inboard airbag assembly and internal structural wiring harness failed due to a direct short circuit, completely deactivating the vehicle's supplemental restraint system. General Motors has a known engineering bulletin for this issue (#23-NA-085), noting that sharp internal seat frame components slice the airbag wiring harness. Rather than permanently fixing this structural design
... flaw while under warranty, dealership technicians repeatedly executed temporary, non-compliant field patches, including plastic zip-ties and Velcro tape inside the cushion. These temporary patches continuously failed, leaving the airbags disabled. Furthermore, the vehicle was sold under "Certified Pre-Owned" designation with an active, uncompleted 2020 manufacturer Customer Satisfaction Program Campaign (#N202310850-02) for this exact airbag circuit defect. Component failed & available for inspection: The driver's seat inboard airbag assembly and internal structural wiring harness failed due to a direct short circuit. The components are available for inspection inside the vehicle at the servicing dealership. Safety risk: Safety was put at extreme risk because active Diagnostic Trouble Code B14DC completely deactivates the airbags. This means the airbags may fail to deploy in a collision, leaving the driver completely unprotected in an accident. Confirmed by dealer: The problem has been explicitly reproduced and confirmed by an authorized franchise dealership across seven separate service visits spanning from August 2025 to June 2026. Inspected by manufacturer/others: The components have been physically inspected multiple times by authorized franchise dealership technicians acting on behalf of the manufacturer. Warning lamps & symptoms: The vehicle displays "Service Safety Restraint System" warning message on the dashboard. The warning lamp first appeared on August 26, 2025, at 78,472 miles while the vehicle was strictly within its CPO warrantyRead more

I own a 2020 Land Rover Defender and have experienced a failure of the LED halo ring/daytime running light within the headlight assembly. In January I experienced a “backlite leak” that allowed water to enter the cab of my vehicle. As a direct result of this water intrusion, my RFH (Radio Frequency Hub) module failed due to electrical shorting caused by moisture exposure. This failure was sudden and completely outside of my control and was not the result of misuse, neglect, or normal wear and tear. Not to mention the risk it put

I was driving down the road with my family in the vehicle when without any warning the truck completely shut down and didn’t have any power. I brought it to the dealership I bought it from and they stated they fixed the leak and replaced the RFH. Fast forward to this past week, there was another leak in the rear window. The dealership then stated it was a faulty back window unit that needs to be completely replaced. Stellantis (formerly FCA) previously acknowledged this defect through extended warranty coverage. However, my vehicle’s model year was later excluded despite being affected by the same known issue. This exclusion is particularly concerning given that the defect ultimately caused collateral electrical damage beyond the window itself. Given the documented nature of this issue, its prior recognition under extended warranty, and the risk of safety it put my family in I wanted to document this issueRead more

My 2019 Nissan Leaf SL Plus is subject to NHTSA Recall 24V-700 for a battery defect that poses a fire risk during DC fast charging. The recall was issued in September 2024 with a fix promised by November 2024. Nissan repeatedly delayed the remedy, first to Spring 2025, then Q3 2025, then March 2026, then an indefinite date in 2026. During this time I was instructed not to use Level 3 DC fast charg
... ing. I was without this capability for approximately 18 months. I purchased the SL Plus specifically for the extended range provided by the 62 kWh battery, which depends on fast charging for longer trips. Without fast charging the vehicle was reduced to short-range local use only. The remedy Nissan eventually provided was a software update only. It does not repair the underlying battery defect; excessive lithium deposits within battery cells causing increased electrical resistance. The software monitors for conditions that could lead to a thermal event and shuts down charging if detected. The physical defect remains in the battery. Nissan included a letter with the software update stating that if the vehicle triggers a specific error code within one year of the update, they would replace the battery or buy back the vehicle. This suggests Nissan acknowledges the software update does not adequately address the defect. I am filing this complaint because the recall remedy does not fix the root cause of the safety defect, and because Nissan took 18 months to provide even this inadequate remedy. The component is the lithium-ion battery pack and it is available for inspection.Read more

The vehicle ended in an irregular/distressed parking position after trying to locate a concrete block to force stop the car due to ABSOLUTE BRAKE FAILURE, E BRAKE SEEMED TO NOT WORK EITHER. The concern is not simply that a warning light appeared. The event involved steering, braking, airbag/SRS, stability, and electrical warnings at the same time, making the vehicle unsafe to operate AND SOMEHOW THE BRAKES WEREN’T OPERATING DUE TO THE LACK OF FUNCITONING. Photos were taken of the warning messages and vehicle position. Prior related history:
This vehicle has prior safety-system history involving SRS Airbag Service Urgent warnings and braking/vehicle malfunction and movement concerns. Earlier review and discussion involved Volvo engineering, VDDM replacement, CAN/network concerns, calibration requirements, voltage/12V battery considerations, software/module updates, and post-repair validation. The VDDM is understood to participate in vehicle dynamics, stability control, brake coordination, and PHEV regenerative/hydraulic brake interaction. Prior documentation also raised concern that intermittent brake, SRS, VDDM, CAN, and calibration issues may not reproduce during a short dealer test drive. Safety risk:

After a software update to the infotainment system (Android Automotive OS version 5.1.17, installed on 06/03/2026), my 2022 Volvo XC60 B5 intermittently loses all audio output. When the failure occurs, the vehicle produces no sound of any kind: no audible rear parking/proximity sensor warning while in reverse, no turn-signal indicator tones, and no media audio. The rear-view camera image still di
... splays normally, so the loss of the audible backup warning is not obvious to the driver — the parking-assist system appears functional while its audible warnings are in fact disabled. On multiple occasions the failure was already present as I backed out of my garage and began driving, leaving me with no audible obstacle warning while reversing and no turn-signal tones while maneuvering. The display, navigation, and camera continue to function while audio is dead, which indicates the audio service stops responding rather than a hardware fault. The only way to restore audio is to fully restart (reboot) the infotainment head unit; the failure then recurs intermittently. The fault began immediately after the 5.1.17 update and was sporadic at first. After noticing a pattern, I documented occurrences on 06/15/2026, 06/17/2026, and 06/20/2026 with timestamped photographs, which are available on request. No crash or injury has occurred, but the loss of audible backup-warning tones while reversing is a safety hazard, particularly for pedestrians or obstacles behind the vehicle. Hello, I recently brought my 2014 Ford F-150 (VIN: [XXX]) to the dealership regarding Recall 24S37. However, the vehicle continues to exhibit transmission-related symptoms, and I am concerned that the underlying condition may be related to or aggravated by the conditions addressed in Recall 24S37. I am requesting that Ford review this case and determine whether additional diagnostic evaluation or repairs should be considered. However, they refuse to accept that the codes have anything to do with the recall, even though it is the same part and the affected sensor that is triggering the error codes on the scanner. I am writing to bring to your immediate attention a serious safety incident involving our brand‑new 2026 Tesla Model Y (Juniper), equipped with FSD version 14.2.2.5 and Software Version 2025.45.10. We purchased this vehicle as our dream car, and for nearly six months it delivered exactly what we hoped for—innovation, safety, and an exceptional driving experience. That trust was the foundation
... of our decision to choose Tesla. On [XXX], at approximately [XXX] in Frisco, TX, that trust was shaken by a sudden and alarming event. After coming to a stop and while making a routine right turn at under 10 mph, the vehicle unexpectedly accelerated on its own, causing an immediate loss of control. The entire incident unfolded in seconds, leaving no opportunity to react or correct the vehicle’s behavior. The incident happened at the intersection of [XXX]. The "Autopilot" feature in Tesla was on at the time of the incident. The impact was severe. The airbags deployed, the vehicle was declared a total loss by insurance, and first responders—including the Frisco Police Department—were required to clear the wreckage. My wife sustained injuries in the crash and continues to recover. Witnesses at the scene confirmed that the acceleration was abrupt and not driver‑initiated. The emotional and physical toll has been significant. More importantly, the nature of this incident raises serious concerns about a potential safety issue that could affect other Tesla owners. We feel compelled to ensure this incident is thoroughly examined. If there is a systemic issue—software‑related or otherwise—it is critical that it be identified and addressed to protect other drivers and uphold the integrity of the Tesla name. We truly miss our vehicle and the confidence we once had in it. We hope Tesla will treat this matter with the seriousness it deserves and provide a transparent path forward. Thank you for your time, attention, and commitment to safety. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On June 22, 2026, my 2025 Ford Bronco caught fire while parked outside a store. The vehicle had been parked for approximately five minutes when the fire began. There were no warning lights, alerts, unusual odors, or other indications of a problem before the incident. The fire and heavy smoke appeared to originate from the center console area near the gear shifter. The flames quickly spread throug
... hout the driver’s side, center console, front passenger area, and the rest of the vehicle’s interior. The entire interior sustained severe fire and heat damage and was substantially melted. The responding fire department advised that temperatures inside the vehicle were well over 300 degrees. Firefighters disconnected the vehicle’s battery, after which the vehicle began to cool. Based on their initial observations, they indicated that the fire appeared to be electrical in nature. The vehicle had recently been filled with gasoline, which made the situation even more dangerous. Fortunately, no one was inside the Bronco, no other vehicles were parked nearby, and the fire department responded quickly. The vehicle has been transported to a repair facility but has not yet been inspected to determine the precise cause of the fire. I am extremely concerned that a nearly new vehicle could suddenly catch fire while turned off and parked, without any warning. This could have occurred while I was driving, with passengers inside, or while the Bronco was parked in my garage. Under those circumstances, the fire could have caused serious injury or death, destroyed my home, or endangered other people and property. I am submitting this complaint because I believe this incident presents a serious public-safety concern. I respectfully request that NHTSA investigate whether other 2025 Ford Bronco owners have experienced similar electrical fires and whether a defect exists that could place drivers, passengers, families, first responders, and the public at risk.Read more

While driving on the highway, my 2024 Santa Fe Hybrid suddenly and without warning lost propulsion power. The loss of power was immediately accompanied by multiple simultaneous warning lights across the instrument cluster, glitching and abnormal behavior of the cluster display, and then a complete electrical shutdown of the vehicle. After the shutdown, the vehicle would not restart. The failure o
... ccurred in connection with a software update on the vehicle. At the time of the incident the vehicle had approximately 12,828 miles on it. Losing motive power while traveling at highway speed created a serious crash risk, as the vehicle was unable to maintain or recover speed in moving traffic. No crash, injury, or fire resulted. The vehicle was towed to an authorized Hyundai dealer for diagnosis. I have photographs of the instrument cluster taken at the time of failure showing numerous warning indicators illuminated at once. The symptoms I experienced — progressive or sudden loss of drive power, 12-volt/charging-related faults, and failure to restart — appear consistent with charging-control and DC-to-DC converter failures that have been the subject of prior recalls and complaints on Hyundai electrified vehicles. I am filing this report to document the incident as a safety concern.Read more

I’ve had issues with the blind spot sensors. After driving a mile or two on open road, the warning pops up saying “Blind Spot sensors unavailable” then the lights on the mirror stay on. This was happening a few months ago so the right sensor was replaced. Now it’s happening again. In a vehicle that is two years old this shouldn’t be an issue.
