The Automatic Emergency Braking (AEB) system falsely engages under normal driving conditions when no obstacles, vehicles, or other hazards are present. These false activations occurred at very low speeds, while stopped, while parking, and while the vehicle is in neutral. When this happens, the vehicle brakes abruptly, shifts itself into park, and shuts off entirely. This is not normal system opera
... tion and creates a significant safety concern because the vehicle has stopped abruptly and disabled itself unexpectedly on numerous occasions while moving. While my vehicle was in neutral and moving through an automatic car wash on a track, the Automatic Emergency Braking (AEB) system malfunctioned. My vehicle shifted itself into park and shut down. This sudden engagement caused the vehicle behind me to collide with the rear of my vehicle. As a result, the driver of the following vehicle has filed a collision claim against my insurance policy. My vehicle has been taken to the dealership 6 times for this AEB-related issue, along with other ongoing concerns, on the following dates: 09/11/25, 10/07/25, 11/12/25, 01/19/26, 03/09/26, and as of 6/05/26 is back at the dealership. The vehicle will unexpectedly shift out of gear at low speeds, requiring me to stop and place it back into gear before I can continue driving. The vehicle is currently at the dealership as of 06/05/26. Despite several repair attempts, the problems have not been resolved. I am extremely concerned that the AEB system will continue to malfunction causing a more serious collision and injury. The dealership was able to reproduce the problem in September; however, they have not been able to reproduce it again since that time. I have attempted to document each malfunction, including photos of the failure messages. I can provide additional information (receipts, video from accident, photos etc.).Read more

During routine driving, the vehicle suddenly displayed an electrical system. Error code followed several minutes later by a 12-volt battery error. The vehicle was stopped and turned off. Vehicle was stranded in a secure parking lot after hours with Windows unable to be closed and doors unable to lock. The vehicle is immobilized due to being unable to put the vehicle into neutral to move it to a suitable location for a tow. Vehicle was certified by dealership as a certified pre-owned vehicle approximately 45 days ago. All listed recalls have been checked and no undue stress has been placed on the vehicle since purchase. The vehicle has suffered a catastrophic, intermittent low-voltage network crash caused by a failing Body Control Module (BCM), resulting in multiple dangerous safety-critical failure modes. On a previous occasion, the vehicle woke up with severe network faults and automatically forced itself into Neutral. When the driver stepped out to get the child and shut the door, the system crashed entirely a
... nd the exterior electronic door handles failed to present, trapping an infant child inside the running vehicle. Most recently, while driving in a rainstorm, the vehicle suffered a complete failure of the windshield wipers, a total loss of the high-beam headlights, a total loss of the horn, and a total loss of electronic communication to the electronic parking brake system. Most critically, the vehicle experienced an uncommanded propulsion hazard where it hard-locked itself in the 'Drive' gear and absolutely refused to shift into 'Park' or 'Neutral,' forcing the driver to physically maintain heavy hydraulic brake pressure to keep the vehicle from creeping forward. Because the horn failed and the parking brake was disabled, the driver was trapped in the driver's seat holding the brakes and was forced to call 911 for emergency first responders to come and physically block the wheels so the driver and infant could safely exit. Tesla Service has diagnosed the issue as a failed central Body Control Module (Part #1010906-00-D), but the cascading network failure modes present an extreme, life-threatening hazard regarding vehicle control and child entrapment.Read more

I am adding a major update to my safety recall complaint because this dealership just tried to pull an illegal financial ambush on me.After my 2019 Focus started stalling out on the highway right after they did Recall 18V735, I brought it back. When I got there, they slapped an unauthorized invoice for $988.82 in front of my face for parts and labor. They did nearly $1,000 worth of work after I told them 'no' in writing, clearly trying to quietly fix their botched recall mistake under the table and force me to pay for it.When I showed the advisor the texts proving I denied the charges, the staff panicked and couldn't answer who authorized the job. I refused to sign it or pay it, and I walked out. My car is currently stranded on their lot because they are holding my keys hostage over unauthorized bills on a federal safety recall file. On June 23, 2026, while operating my 2021 Genesis GV80, the vehicle's entire infotainment and system software rebooted without warning as I was leaving a parking lot. As a direct result of this reboot, the driver seat automatically moved out of my saved position. I was no longer able to reach the pedals and could not properly control the vehicle. This incident occurred while the vehicle was in mot

This is a safety defect, not a minor software issue. A seat repositioning without driver input while a vehicle is in motion constitutes a loss of vehicle control. Had this occurred at highway speed the consequences could have been fatal. I am aware that Genesis issued Safety Recall 031G for mid-drive infotainment reboots on other GV80 model years. I am requesting NHTSA investigate whether this defect extends to 2021 model year vehicles and whether a recall expansion is warranted.Read more

RE: SAFETY COMPLAINT FOLLOWING RECALL 68C (NHTSA Recall Number 24V-111 / Mopar 68C) I am the owner of a 2022 Jeep Wrangler Rubicon 4xe. Prior to having safety recall 68C performed, the hybrid system functioned flawlessly. Immediately upon receiving the vehicle back from the dealership after the 68C software flash, a severe drivability and safety hazard emerged. When driving in standard Hybrid/El
... ectric mode, the vehicle experiences a sudden loss of power, hesitation, and a physical 'bogging down' sensation. Following this lag, the vehicle violently surges into gasoline mode, causing a sudden and unpredictable acceleration lurch after the vehicle hesitates to accelerate. This behavior occurs randomly at both low neighborhood speeds and highway speeds, making it impossible to smoothly modulate throttle input. This creates an immediate safety hazard when attempting to pull out into cross-traffic, merge onto highways, or maintain a safe, predictable following distance. The vehicle hesitates when throttle is demanded, creating a risk of being rear-ended, followed by an uncommanded surge. The dealership states the vehicle is 'operating as designed' according to a regional Jeep field engineer, implying this dangerous lag-and-surge behavior was intentionally introduced into the vehicle's management software by the manufacturer to mitigate, the recall notice noted above, battery thermal risks at the expense of occupant driving safety. The manufacturer’s recall remedy has traded a fire risk for an active moving traffic hazard." Several forums and AI searches show this is a common problem the manufacturer has created to avoid a costly recall by addressing the real issue with the battery’s fire risk. Instead they have intentionally reduce the battery’s performance causing a safety issue with the vehicle’s acceleration and operation.Read more

Vehicle: 2023 Ford Explorer ST VIN: [XXX] I am reporting ongoing safety-related powertrain and drivability defects that continue despite numerous repair attempts, including complete engine and transmission replacement. Since approximately June 2025, my vehicle has experienced delayed acceleration, hesitation, erratic RPM behavior, harsh shifting, loss of power, and intermittent warning lights. T
... he vehicle has also stalled, jerked during acceleration, and exhibited transmission shuddering and lurching. The problems have been repeatedly reproduced, documented, inspected, and repaired by Kendall Ford of Anchorage, Alaska, the sole servicing Ford dealership involved. Repairs have included replacement of fuel injectors, cylinder heads, a complete engine assembly, and a complete transmission assembly. Despite these repairs, the safety concerns remain. Current symptoms include: • 1-4 second delay in throttle response. • Vehicle may fail to accelerate below approximately 30 mph. • Engine RPM rises significantly without proportional vehicle acceleration. • Erratic shifting, surging, and harsh jolts. • Intermittent check engine lights, FordPass alerts, and electrical warnings. These conditions create a significant safety risk because the vehicle may fail to accelerate when entering the highway, merging into traffic, crossing traffic, making left turns, or avoiding hazards. The delayed throttle response and sudden acceleration after the delay increase the risk of a crash. Attached service records document stalling, misfires, loss of propulsion concerns, and repeated powertrain failures requiring replacement of both the engine and transmission, yet the drivability concerns continue. I believe this may involve a defect in the powertrain, transmission controls, engine management system, electronic throttle control system, or related vehicle control modules. I respectfully request that NHTSA investigate these recurring safety-related defects. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.CRead more

... later.”. On Feb. 22, I was attempting to reverse from my garage and my camera failed to engage. I saw a black screen without picture when the car was first put in reverse, so I put in park and then switch back to reverse. The camera did not engage and the screen remained on the car's menu. Camera then flashed on briefly and then went back to the black screen, then moved multiple time between to the blue screen with the warning message “Incoming Camera Signal Error. Please Try Again Later.” and the green screen with message “No Camera Signal Received. Please try again later.” The issue eventually began occurring both while in reverse and in drive. During this time frame, I took the vehicle to the dealership on multiple occasions, Feb. 10 and Feb. 19, and they were unable to replicate the challenge. I took the vehicle back to the dealer on Feb. 23, and the issue was replicated. The issue was 'repaired' and picked up on Mar. 19, but had to return 3 hours later due to the same issue then in an exacerbated state. I am reporting a safety concern involving my vehicle’s steering system, front-end condition, and a recall I was not informed about. I brought the vehicle in for a routine tire rotation, at which time I was informed that the front camber was out of specification and that there was uneven wear on the front tires. An alignment was recommended. In addition, the vehicle is experiencing abnormal steer
... ing behavior. The steering wheel has a noticeable shake and does not feel normal during operation. This suggests a possible issue with the power steering system or related suspension components. The combination of uneven tire wear, improper camber, and a shaking steering system presents a significant safety risk. These issues may affect vehicle stability, steering control, and tire integrity, increasing the risk of loss of control while driving. I have also recently learned that there is an active recall affecting this vehicle, of which I was not previously notified. The fact that this recall was not communicated raises additional safety concerns, especially given the steering-related symptoms currently present. These conditions were identified during a recent service visit, and the underlying cause has not yet been resolved. I am concerned that this may represent a broader defect involving the steering or suspension system that could impact vehicle safety and that proper recall notification procedures may not have been followed.Read more

I am reporting a severe safety hazard regarding an immediate, drastic increase in fuel consumption and erratic fuel range calculations following a manufacturer recall repair. My 2025 Ford Explorer ST-Line was operating perfectly until a Ford Mobile Service technician performed safety recall 26S21 (IPMA Reset). According to official Ford manufacturer specifications, this SUV has an expected drivi
... ng range of up to 501 miles on a full 17.9-gallon tank of fuel. However, immediately following this software update, the vehicle's total driving range drastically dropped. On a 100% full tank of gasoline, the dashboard's "Distance to Empty" (DTE) display now only reads 316 miles left to empty—nearly 200 miles below the manufacturer's expected capability. Furthermore, during a subsequent continuous, short 1.8-mile drive near my residence with no stop-and-go traffic, the DTE display rapidly plummeted from 15 miles down to 5 miles remaining, dropping visibly every two seconds. The vehicle is physically burning through gasoline at an accelerated, unsafe rate. This creates an immediate safety risk. If the vehicle is suffering from an uncalibrated software glitch in the powertrain/fuel modules due to the recall update, or if the procedure triggered a mechanical fault, the vehicle is at high risk of a sudden highway stall or leaving the driver stranded in live traffic due to inaccurate fuel signaling. I request that NHTSA look into whether the software installation process for recall 26S21 can corrupt engine/fuel management maps or trigger fuel delivery faults on the 2.3L EcoBoost platform.Read more

Since purchasing this vehicle new, the Auto Idle Stop (AIS) system on my 2017 Honda Pilot has intermittently failed during the engine restart sequence after the vehicle comes to a complete stop. When the AIS system shuts the engine off and I release the brake to resume driving, the vehicle intermittently fails to restart properly or does not restart at all, preventing the vehicle from moving as ex
... pected. In some instances, warning messages, including a collision mitigation message, has appeared on the dashboard before the car restarts. This condition has occurred repeatedly over several years and has happened at intersections, while stopped in traffic, and while stopped on highways. The inability of the vehicle to restart and move creates a safety risk because the vehicle may remain disabled in active traffic when acceleration is needed. The issue was reported to the selling and servicing Honda dealership (Cambridge, Honda in Cambridge MA) multiple times over several years. In August 2025, the vehicle received Honda's prescribed repair under Service Bulletins 23-008 and 23-009, including replacement of the starter assembly, starter relay, and battery. Despite these repairs, the same condition returned less than one year later and has occurred at least seven additional times. The vehicle stalls and sometimes displays the collision mitigation/crash warning message before restarting. In June 2026, the vehicle was inspected again by the dealership. Technicians were unable to reproduce the condition during testing and declined to perform additional repairs. The defect remains unresolved. The vehicle has already received Honda's recommended AIS repair, yet the same stalling and crash mitigation message continues to occur. The intermittent nature of the defect makes it difficult to reproduce during short dealership evaluations, but it continues to occur during normal operation and presents an ongoing safety concern.Read more

After driving for approximately 30-minutes in a heavy rain i pulled over and the automatic engine shutoff feature activated while i was waiting to go inside for an appointment. When attempting to restart the car, the engine would start but then shut off showing a "low power steering" warning message on my dash. The problem persisted for approximately 15-20 minutes. Eventually after about 10 attemp
... ts to restart the engine, the car started back up and drove fine the rest of the day. I have heard of this happening to many other rav4s on the Autosafety.org website and it causes me concern, especially if this problems causes the car to stall in high speed traffic in the rain. It is my understanding that this maybe a problem with the Mass Airflow Sensor getting wet in heavy rainfall.Read more

After being stranded, getting a new battery, being stranded again and again (with a newborn and toddlers). This is a serious electrical issue causing the battery to not fully be charged even when car is off. No external components plugged in. Many people have this issue with the car and it needs to be addressed as a recall.

VIN # [XXX] Ford Customer Service Current Case Number: [XXX] Integrated Trailer Module Failure after recall completion. NHTSA Campaign ID Recall # 26C10 was completed at Coconut Point Ford in Florida on 3/26/2026. This recall identified that when towing a trailer, the integrated Trailer Module may lose communication with the trailer possibly causing a loss of brake and turn signal lights or a lo
... ss of brake function. On [XXX] in the middle of a seriously steep and dangerous decent, the system reported that the trailer was disconnected from the truck causing, loss of trailer brake control which in turn cause a reliance on the truck. The antilock brake system failed moments later. I had to literally stand on the brakes to slow down to keep from going over the side of the mountain. At the end of the dirt road where it Ts onto the highway we could not stop. We travel with a 23 foot 2019 Vintage Cruiser manufactured by Gulf Stream. That weighs approximately 5200 lbs empty; I have a Cat Scale reading of 4840 pounds from a 12/6/24 weight with my previous truck. We do not travel with any water in our Black, Grey or Fresh Water tanks so even fully loaded we are still well within specs for this trucks towing capacity. The brake control on the truck was set to 5. Faricy Boys Ford in Canon City Colorado, verified that a recall had been completed and worked on the truck for 3 full days, testing many things including tailgate malfunctions and miscellaneous electrical systems trying to replicate the problem, no dealer has been able to replicate the problem. They finally replaced all the brakes and rotors as well as flushing the brake system. I respectfully requested that Ford Motor Company assign the review of this case to a regional Customer Service Manager / Engineer to review the complete repair history of this vehicle, via registered mail. We haven heard anything yet. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The issues I am having are exactly the ones that were subject to an earlier recall. NHTSA: 24V344 and Volkswagen 919A. From the text of that recall: "Due to a software issue, affected vehicles may experience center displays and instrument panel displays that do not boot, or that sporadically reset. This can result in loss of speedometer inforation or loss of rearview camera image. As such, these
... vehicles fails to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 101, "Control and Displays" and 111, "Rear Visibility." Displays that do not show critical information, such as vehicle speed or the rearview camera image, increase the risk of a crash." These symptoms are recurring in my vehicle and I have video documentation. Volkswagen is refusing to deal with me because "their diagnostic tools do not show any error message."Read more

We have leased this car since May of 2025. It has been in the Dodge repair shop over a dozen times for the electrical system warning light coming on. It had to be towed tot he dealer twice for it, other times it was drivable to get it there. This car is a complete Lemon and we are trying to get Dodge/Stellantis to pay us or preferably to buy it back and they are stalling. The car is a danger t

Upon attempting to charge the vehicle with factory supplied Level 1 charger, a dashboard message appeared stating "Service Charging System" and the vehicle would not charge. Took the vehicle to a service station and scanned the vehicle. Listed error code is p0d27-00. The code could not be cleared and the vehicle was unable to be charged. Vehicle had to be taken to a Chrysler dealer for the code
... to be cleared in order to charging capability to be restored. This was considered a "software" issue and not covered under the vehicles warranty - despite it directly affecting the fuel/hybrid propulsion system. This defect and inability to clear the code means the vehicle cannot be charged, reducing its range and fuel economy, and increasing emissions. This is a critical issue and should not incur customer expense to have the code cleared.Read more

Component Involved: Engine Lubrication / Cylinder Head / Variable Valve Timing (VVT) Spool Valve (Solenoid Assembly) and Electrical Charging System (Alternator). Vehicle Affected Example: Honda vehicles equipped with the 3.5L V6 (J35 Engine Series) with Variable Cylinder Management (VCM), including the Honda Pilot. Description of the Safety Defect & Failure Mechanism: The rear (bank 1) variable va
... lve timing spool valve assembly is mounted directly above the vehicle's alternator. Due to thermal cycling and material degradation, the molded rubber gasket on the upper portion of this spool valve assembly experiences premature compression set and fails, resulting in a pressurized engine oil leak. Because of the physical packaging and architecture of the J35 engine, the oil dripping from this failed gasket falls directly into the open housing of the alternator located immediately below it. The engine oil contaminates the alternator's internal brushes, slip rings, and diodes, leading to a catastrophic electrical short circuit and total failure of the charging system. Description of the Safety Risk: When the alternator fails due to oil contamination, the vehicle experiences an immediate drop in system voltage, resulting in a sudden and un-warned loss of electrical power while the vehicle is in motion. This triggers a cascade of safety hazards: Loss of Power Steering Assist: The sudden loss of electrical generation drops the vehicle into battery-reserve power, frequently causing the Electronic Power Steering (EPS) to shut down immediately, drastically increasing steering effort and the risk of a crash. Engine Stalling: Total depletion of the battery occurs rapidly without alternator output, leading to sudden engine stalling in active traffic without the ability to restart. Loss of Safety Systems: Reduced system voltage disables critical safety components, including Advanced Driver Assistance Systems (ADAS), supplemental restraint systems (airbags).Read more

1. The vehicle caught fire during normal operation. It is a 2021 Jeep Wrangler (VIN [XXX]) subject to FCA US Recall 21D / NHTSA Campaign 26V363000, which identifies the electric hydraulic power steering (EHPS) pump and its wiring as a fire source that can overheat and ignite. The exact failed component has not yet been confirmed by diagnostic, but the fire is consistent with the defect described i
... n that recall. The vehicle is intact and available for inspection — it was towed to Waxahatchie Jeep, an authorized Stellantis dealer in Waxahachie, TX, on June 19, 2026 and remains in their custody under repair order [XXX]. 2. The vehicle caught fire while being driven on the highway between Waxahachie and Ennis, Texas, with two occupants returning from a Boy Scout trip, my husband, the driver, and our [XXX] son. The driver was forced to take evasive action, steering off the roadway into a service station to get the occupants clear and stop the fire from spreading further. This created a serious risk of burns, of a collision during the evasive maneuver, and of harm to the occupants and other motorists. 3. Has the problem been reproduced…? No. No diagnostic has been performed. The vehicle has been in the authorized dealer’s custody since June 19, 2026 ([XXX]), but the manufacturer has not completed an inspection or diagnostic. The underlying defect is the subject of an active recall (21D / 26V363000) affecting more than one million 2021–2025 Jeep Wrangler and Gladiator vehicles, for which the manufacturer has stated no remedy is yet available. 4. The vehicle was towed by AAA to Waxahatchie Jeep on June 19, 2026 and has remained there since. It has not yet been formally inspected or diagnosed by the manufacturer. 5. There were no warning lamps or messages prior to the fire. The first indication of a problem was smoke from the engine area at which point the driver immediately pulled off the road and saw a plume of smoke coming from the exhaust.
