Our 3 yrs old + Toyota RAV4 Hybrid SUV with barely over 30,000 miles on it had 2 safety issues within 2 months of each other! First was the driver's side mirror- it was rattling, feeling like it might fall off! Seems common issue, perhaps due to cheap plastic parts inside. We had it fixed- Toyota Brand Engagement Center gave a 'Goodwill' gesture to cover us replacing it at Dealership, though it sh
... ould never have happened on such a newer vehicle in the first place! Then not even 2 months later, a Warning flashed- "Pre Collision System Malfunction!" Also "Headlights" with same warning- "SEE DEALER NOW!" Toyota Dealer. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). It's infuriating that Toyota, who obviously is making inferior safety products, does not offer to help out in any way. We're both [XXX] and entitled to these safety features as we bought the car both new and paid top dollar! Our service advisor at the Toyota Dealership agrees this is 'not normal', the car is too young to have this happen! He will attest to this & we have paperwork. It stated 'No immediate remedy'. (replacement camera not available) I believe this is happening to hundreds of other RAV4 owners. It's outrageous! Please help! These electronics for safety should not fail! On Saturday, June 20, I was driving my son to soccer. We stopped at a stoplight. When the light turned green, I pressed the accelerator and we went forward. However, when I took my foot off the accelerator, the car continued to accelerate. Luckily it slowed down when I pushed on the break. But every time I took my foot off the break the car continued to accelerate. I did not have a good place to p
... ull over to the side. So at the next stop light, I turned the car completely off. I turned it back on again, but the same thing happened. I arrived at the soccer field and parked the car for the remainder of the day, which was several hours. It did not do that again that evening. The other thing that has happened a couple of times prior to the incident and since is, the Nissan panel display that lights up when you turn the car on turned completely off and on again several times at random while I was driving. Usually the panel stays with eliminated and plays audio or shows a map. It turned itself completely off to Black and then back on again as if the car was starting up again. On Tuesday, I took the car to Nissan because I had called the problem in over the weekend, but their service center was not open. They were unable to replicate the problem. However, on my way to Nissan, the accelerator again pressed forward independently of my actions and the car accelerated without warning. This is very scary and felt extremely dangerous. I told the technicians that Nissan that the problem appeared similar to a recall that had been in place a few years ago. But they said that unless they could replicate the problem, they weren’t able to do anything about it. I was very unsatisfied with that because it seemed like a pretty dangerous situation.Read more

A year ago while driving on the interstate, my Ford Maverick suddenly threw a "Stop Safely Now" alert. I was able to turn the ignition off and back on to safely exit the highway. The alert came back once I made it to a parking lot and stuck around for a while. I was able to get it started and drove to a Ford dealership that told me it was just a software update. The same issue happened 2 more time
... s before I made it to my destination after leaving the dealership, showing that it actually wasn't caused by a missed software update. Returning from my trip it happened 4th time. Afterward, the issue seemed to go away. Now a year later I got in the Maverick to leave work and the alert came back. I was not able to get it started and will attempt to get it over to a Ford dealership tomorrow to be seen for what seems to be the same issue, only worse if it still is not starting in the morning.Read more

I am filing a formal safety defect report regarding a severe, non-functional Advanced Passenger/Pedestrian Emergency Braking (PEB) and Forward Collision Warning (FCW) system that has been completely inoperable ("INOP") since delivery of this brand-new vehicle. The vehicle has been subjected to four (4) independent, consecutive service repair attempts at authorized franchise dealerships with a compl
... ete failure to resolve the active safety hazard.Critical Evidence of Manufacturer Negligence: Corporate Techline Admission: On 12/2/24 (3,327 miles), factory technicians performed module updates but noted on the official invoice: "Issue persist. Contact Techline was told update coming." Corporate engineering knowingly discharged a defective vehicle back into public operation without a functioning collision avoidance system.Star Engineering Admission: On 5/15/26 (34,461 miles), following extensive factory hardware resets via the wiTECH diagnostic network and manual sensor/camera recalibrations, the safety defect persisted. Stellantis Corporate Engineering (Star Agent) explicitly ordered the dealership techs to return the malfunctioning vehicle to the consumer because it lacked an active Diagnostic Trouble Code (DTC), stating the safety hazard was merely "currently under investigation."Current Grounded Status: On 6/8/26 (approx 36,000 miles), Certified technicians determined an instrument cluster needed ordered and legally "grounded" the vehicle on the dealer lot due to federal odometer rules. The vehicle is currently sidelined awaiting a backordered instrument panel cluster (IPC). Public Safety Impact: Stellantis is willfully ignoring a widespread component or software defect in its integrated digital instrument cluster and forward-facing camera arrays. By ordering dealerships to return vehicles with dead pedestrian emergency braking systems to consumers due to a lack of DTC’s, they are creating an immediate, severe hazard to occupants and pedestrians on public roadsRead more

I was driving my 2024 Jeep Gladiator Willys Texas Edition on the FM 3180 and I 10 when the vehicle experienced a complete electrical failure without warning. The instrument panel began flashing, the vehicle lost power and became completely inoperative while I was traveling. I was able to get the vehicle off the roadway, but it would not restart. At first it would start and flash, windshield wiper
... s were on, even though control was off. It would go about 10 feet and die again for several minutes. I eventually got it off the road and into a parking lot where it beeped and clicked, but would not turn on. The vehicle had no electrical power. A roadside jump box had no effect. The tow operator had to manually override the transmission to place the vehicle in neutral so it could be loaded onto the tow truck. The vehicle was towed to the dealership and remains inoperable. This vehicle previously received a software repair/recall/update in 2025. I do not yet know whether this failure is related to that recall. This was a serious safety event because the vehicle completely shut down while traveling on a high traffic area. I believe this defect could create a significant risk of a crash if it occurs at a high rate of speed. I had no control of the vehicle at times.Read more

My Nissan Leaf was affected in the NHTSA Recall 24V-700. We took it into the Valley Nissan of Longmont, where we initially purchased the car, on 6/19/2026. The dealership checked it out and told us it was fine, so we took the car home that day. We drove it around on 6/20/2026 and received an urgent EV service warning. The car went into turtle mode while we were driving on the highway. We called th
... e dealer, and they said it was the code that indicated our battery was actually degraded. So, they gave us back a car that was unsafe to drive. They weren't able to look at it again til 6/22/2026, but they refused to provide us with a rental or loaner car for the weekend, despite this being our only car, and the recall specifically having a stop-drive order on affected vehicles. On 6/22/2026, we took the car in again, and they confirmed the battery was actually degraded due to the recall. Again, they refused to get us a rental or loaner car. Since Monday, the car has been sitting at the dealer, and as of 6/25/2026, we still have no information about when we will get the car back, if we can get a battery replacement, or what our options are. They have provided us with no information and no alternative vehicle, despite our car being undriveable. They said we could rent a car but refused to confirm if this would be reimbursed as part of the recall. We called Nissan Customer Affairs, and they have also been unhelpful and not provided any further information on a battery replacement or loaner vehicle. We have been calling multiple times this week to no avail. We want to have the battery fixed, but we have heard from other Leaf owners that this can take many months, and this was our only vehicle. We would not be able to have the battery replaced at no cost to us without a provided loaner vehicle. Since this comes as a result of a stop drive order recall, I think that should be provided from Nissan through direct payment rather than reimbursement.Read more

I am submitting this complaint regarding my Volkswagen vehicle because it has experienced two recurring defects during the warranty period that remain unresolved despite repeated repair attempts by the dealership. Issue 1 – Excessive Oil Consumption The vehicle has had a persistent excessive oil consumption problem since the warranty period. The dealership has attempted to repair this defect si
... x times, including replacing an engine valve, but the issue continues. The vehicle still consumes excessive oil, requiring frequent monitoring and topping off. This creates a serious safety concern because low engine oil can result in engine damage, sudden loss of engine power, engine seizure, or an increased risk of fire while driving. Issue 2 – Electrical System Failure The vehicle has also experienced ongoing electrical problems involving the keyless entry system, repeated 12V Battery Low warnings, inoperative interior lights, and loss of connectivity with the Volkswagen mobile app. These problems have persisted despite nine repair attempts by the dealership and remain unresolved. The vehicle has spent a significant amount of time at the dealership throughout 2025 and 2026 for these recurring defects. The repeated electrical failures raise significant safety concerns. The keyless entry system can fail to unlock or recognize the key, the low 12V battery warnings indicate an unresolved electrical issue, and the loss of interior lighting reduces visibility inside the vehicle. The repeated failures also raise concerns about the reliability of other critical electrical systems and the possibility of the vehicle failing to start or operating unpredictably. Despite six repair attempts for excessive oil consumption and nine for electrical defects, Volkswagen has been unable to permanently fix these issues. I request NHTSA investigate whether these recurring failures indicate a broader safety defect requiring corrective action. Currently car is back to dealership for repair againRead more

The infotainment system is experiencing the "ghost touch" problem already identified by Mazda and there has already been a class action lawsuit won regarding this problem. My infotainment system is completely unusable and I have been told it is over $500 to repair/replace. I am the original o
... wner of this vehicle. The system continuously beeps and changes all settings while I am driving. If I am using navigation, the screen switches to something else so that I can't see where I'm supposed to be going, causing undue slowing and or braking in city traffic. This is a safety problem. My infotainment system continuously goes into the settings and changes the navigation guidance volume and the car yells at me that, "NAVIGATION WILL BE AT THIS VOLUME." This has startled me and caused me to either swerve or brake abruptly due to being yelled at with out warning while alone in the vehicle. This is a safety issue. As a female who drives by alone 99% of the time, I have my doors set to lock and unlock in a specific manner to keep me safe. The system randomly resets all of these settings while I'm driving so I have no idea if the doors will be locked or unlocked and whether I will be safe when stopping in city traffic. This is a personal safety issue and causes undue stress. The system calls and hangs up on my family repeatedly, making them think that I am in danger or hurt. They try to call by and get hung up on. This is a safety issue because it causes distracted driving because I am focused on that problem instead of the road. All in all, the whole system is a safety problem and needs attention, especially since Mazda has already admitted to the problem.Read more

While driving my 2024 Toyota Grand Highlander Hybrid AWD at highway speed with only 16,755 miles on the vehicle, multiple warning messages suddenly appeared, including warnings related to the engine, hybrid system, AWD system, and other vehicle functions. The vehicle instructed me to pull over immediately and significantly reduced power output, making it difficult to maintain highway speed and saf
... ely maneuver through traffic. I was able to reach the shoulder and had the vehicle towed to a Toyota dealership. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. I request that NHTSA investigate whether the front grille and radiator protection design of the Grand Highlander creates an unreasonable safety riskRead more

The lane-keeping assist function (In-Lane control, part of the driver-assistance suite) does not turn off when commanded. The driver selects the control to disengage, and the system remains active and continues to apply steering input. This is a recurring condition and it is getting more frequent. A driver-assistance steering function that cannot be switched off removes driver authority over the
... steering at the moment the driver is trying to take control, which is a safety risk in normal driving and especially during evasive maneuvers, lane changes, and construction or debris avoidance. The condition was reported to the selling dealer in writing on June 17, 2026 as part of a recurring driver-assistance concern. During the subsequent B service visit the dealer inspected the radar sensors and applied software updates to the steering column module, steering wheel, electric power steering, and driver-assistance control units. The dealer could not duplicate the concern and did not perform a system calibration. The condition has continued after that service. The vehicle has approximately 17,428 miles. There has been no crash, no injury, and no fire. A separate recurring concern on the same vehicle: it reports its doors as unlocked when they are in fact locked, and issues a vehicle-unlocked notification while the vehicle is locked. I am submitting a safety complaint regarding my 2019 Honda Accord LX. While driving, a yellow “Brake System” warning light appeared on the dashboard. In addition, I experienced intermittent episodes where pressing the accelerator pedal d

This occurred while the vehicle was in motion and could create a potentially dangerous situation in traffic. After turning the vehicle off and restarting it, the warning light disappeared. The issue appears intermittent. The brake fluid level is normal, and no diagnostic trouble codes were detected with a basic OBD-II scanner when the warning was present. After researching the issue online, I found multiple owners reporting similar symptoms involving brake system warnings and temporary loss of accelerator response on Honda Accord vehicles. I am concerned that this may indicate an electrical, braking system, sensor, or software-related defect that could affect vehicle safety. I request that NHTSA investigate this issue and determine whether a broader safety concern exists.Read more

On June 24, 2026, my spouse was operating our 2025 Hyundai Santa Fe when the vehicle suffered a sudden, catastrophic, and total electrical failure while actively on the roadway. The vehicle was stopped at a traffic light when all propulsion power was instantly lost, the engine shut down, and all electronic systems—including the center infotainment screens and instrument cluster—went completely
... dark. Safety Hazard & Entrapment Incident: Simultaneous with the total electrical failure, the vehicle's electronic door lock system malfunctioned. The rear passenger doors and rear liftgate locked completely and could not be disengaged or opened from the inside by the passengers. Due to this total power loss and lock failure, my young daughters were trapped in the rear seats. My spouse was forced to climb over the center console from the front seat into the rear cabin to manually extract our children through the front doors. The family had to abandon the disabled vehicle in live traffic and walk down the roadway to reach a safe location. This constitutes a severe safety hazard involving both sudden loss of motive power on a public road and occupant entrapment due to safety lock/latch failure during an electrical short. The vehicle has been towed to Pohanka Hyundai of Capitol Heights for inspection.Read more

While driving my Porsche Taycan through a high-speed intersection, the vehicle suddenly displayed battery and chassis system error messages instructing me to pull over immediately. Before I could safely move the vehicle to the side of the road, it completely lost propulsion, speed control, and steering capability, causing the vehicle to abruptly slow to 0 mph in active traffic and leaving me stran
... ded in the middle of the roadway. This created an immediate safety hazard, as approaching vehicles nearly collided with my car because it effectively performed an unexpected emergency stop without warning. Approximately one month earlier, the vehicle experienced the exact same failure while parked, displaying battery and chassis errors and losing power and steering functionality. The vehicle was towed to an authorized Porsche dealership, where it remained for approximately one month; however, the dealer was unable to diagnose the problem and returned the vehicle to me, stating that no fault could be found. Just two days after the vehicle was returned, the identical failure occurred again while I was driving. After restarting the vehicle multiple times at the roadside, I was eventually able to move it to a safer location, where it failed again shortly thereafter. The vehicle is currently at the Porsche dealership and is available for inspection. The dealership has indicated that the issue may be related to the high-voltage battery but has not yet identified the root cause or completed repairs. The only prior warning of this defect was the nearly identical battery and chassis system failure that occurred approximately one month before this incident.Read more
