Monday, June 30th, 2025, while driving on highway trip out of state, the vehicle made severe explosion noise, and sounded like I was dragging something metal under the vehicle for approx 30 seconds. After slowing the vehicle, I confirmed there was nothing under the car and the car seemed to be driving fine. There was zero warning light displaying any type of part failure or warning light addressin
... g anything needed attention. In hindsight, I realize that this is when the transmission transfer case malfunctioned. Entire time since I've had the brand new, premium vehicle the car has had routine, healthy maintence from the dealership and also Christian Brothers Automotive when the dealership service center was under construction. At no point, did the dealership bring this issue to my attention, despite a few multipoint inspections. Once my vehicle was out of warranty, the dealership brought the internal malfunction of the transmission transfer case to my knowledge on June 1, 2026. I am a high mileage driver, and this part malfunctioned (unbeknownst to me) while I was still under warranty, however without the warning light, I was ignorant to what actually occurred. Local acura dealership has briefly commented on this, suggesting we file a good will claim. I am reporting this, because I believe this part needs to be RECALLED. No one needs to deal with an internal part malfunction on a brand new vehicle 50-60k miles in. I have had the dealership, and Christian Brothers automotive both, look at the issue once it was brought to my attention, to confirm it is indeed no longer an AWD vehicle without the 13k repair. I am grateful that I was in a place that was flat, and not in need of the AWD when it went out, it was a very distrubing and distracting situation when it occurred, and with my living conditions I don't used AWD but a few times a year. Glad no one was injured when the part failed, it could have seriously ended differently if there was traffic around.Read more

My 2022 Kia Seltos is equipped with Idle Stop & Go (ISG), which shuts the engine off when stopped and restarts it when the driver releases the brake/presses the accelerator. ISG stopped functioning about a year ago (the engine stayed running at stops). I didn't think anything of it because the ISG had worked sporadically from the day I bought the car. (I purchased it new.) It began working again,
... sporadically, about a week ago. Since then, on 3 occasions, when stopped at a red light with the engine auto-shut-off via ISG, pressing the accelerator to proceed causes the engine to fail to restart as I accelerate, and the vehicle stalls. The dashboard displays a "Press Ignition Button" message. I lose power to the engine, and the vehicle stops accelerating, coasting briefly on momentum until I manually restart it via the ignition button. I have not confirmed whether warning lights were illuminated, as my focus was on regaining control. This appears consistent with documented ISG restart failures reported by other Kia owners online, suggesting a possible pattern rather than an isolated defect. I checked my VIN on NHTSA.gov; zero open recalls. Kia customer care was closed when I called (today is a weekend day). I plan to call on Monday for a diagnostic appointment. This is a safety concern because the stall occurs unpredictably at the exact moment of accelerating from a stop, particularly hazardous at intersections in traffic.Read more

I own a 2020 Porsche Taycan 4S (in service Nov 2, 2020) with a performance+ HV battery. I am currently facing a third round of module replacements. The service chronology and detail is set forth in the attached e-mail I sent to Porsche and the attached service reports. The safety concern I have is that when the defective modules fail, the vehicle suddenly becomes inoperable. The last failure i
... ncident in April 2026 resulted in the vehicle blocking in two other vehicles until it could be removed on a flatbed. I simply do not feel safe driving a vehicle that could shut down whenever the next module failure happens. There are 33 modules in the HV battery. 15 have been replaced already and it is clearly an ongoing issue as the vehicle is currently in service for a third round of module replacement after the vehicle shut down only 4 months and less than 2,000 driving miles after the previous module replacement. As a result of my increasing safety concerns, I requested Porsche replace the entire obviously defective HV battery. As set forth in the attached response, it refused to do so, stating that the "path forward remains continued diagnosis and repair in accordance with Porsche’s technical guidelines." My belief, and I know I am not the only one based on my review of Taycan message boards, is that Porsche is slow playing how it addresses what is a clear defect in many HV batteries to get to the end of the 8-year battery warranty by doing periodic piecemeal module replacements and then taking the position that post-warranty module issues are an "aging" problem for which Porsche is not responsible. One week ago (6/19) while driving the main screen went black. I pulled over and restarted engine 3x with no luck so drove to dealership. Another startup there it was still black. I was told by service manager “I cannot have you drive it like that” and I was offered a rental which I accepted. Just prior to claiming the rental I again started my Murano and the screen was normal again. I took
... my vehicle but made an appt to have things checked on Monday. At that time I was told that it just needed a software update. That was concerning to me because how would I know in the future it needed an update prior to another incident of the screen going black. Response was that I wouldn’t. Yikes! Additionally, the google assist has repeatedly been of concern with audible requests being met by incorrect responses (different radio station than requested, or static); temperature setting set to 3 degrees higher than requested. Audio will cut out randomly that will only reset after the car is shut off for period of time. Talk to text malfunction and looping. During one errand shortly after taking ownership the backup camera froze and stayed like that for a 25 minute drive. The dealership did a factory reset yesterday (6/26) in an attempt to resolve several issues but with no luck. I also got an oil change. My receipt did not reflect the reset so I requested that be added to my service record.Read more

My independent repair shop has just diagnosed a coolant leak weeping from the vacuum line connection at the water pump; they referred to this problem as “coolant migration”. As documented extensively by Audi in Technical Service Bulletin 19-23-45 (2070349/2), this indicates an internal seal failure of the water pump, causing pressurized engine coolant to migrate directly into the vehicle’s v
... acuum system. While I understand that my vehicle is technically past its 4-year factory warranty timeline, a mechanical failure of this nature at just 43,000 miles is highly premature and points to a known manufacturer design defect rather than standard wear and tear. More importantly, this defect poses an immediate, severe safety risk. Because the vacuum system directly services the power brake booster, the migration of coolant into these lines risks catastrophic contamination of the braking system. This results in a stiff brake pedal and compromised vehicle stopping ability. I am deeply concerned about operating this vehicle with an active, manufacturer-acknowledged safety hazard affecting my brakes. At this time, I have not had this problem confirmed by the dealer, but plan to in the next week. The vehicle it available for inspection upon request.Read more

To the National Highway Traffic Safety Administration (NHTSA), I am writing to formally submit a safety complaint regarding my 2023 Kia Seltos, which exhibits a recurring, dangerous defect involving the engine stalling without warning. This defect results in a total loss of power while the vehicle is in operation or attempting to accelerate, posing a critical safety risk. The failure manifests in
... two primary scenarios: * **Remote Start Failure:** The vehicle frequently stalls immediately after utilizing the remote start feature when shifting into drive. * **Auto Start/Stop Instability:** The Auto Start/Stop system contributes to the engine shutting down unexpectedly when lifting off the brake or attempting to accelerate from a stop. When this occurs, the dashboard illuminates multiple warning lights (e.g., battery, oil, "key not in vehicle"), and the vehicle loses all forward propulsion. On inclines, this defect causes the vehicle to roll backward, forcing the driver to rely solely on emergency braking to prevent a collision. ### Evidence of Widespread Concern My research indicates this is not an isolated incident; numerous Kia owners report identical, dangerous symptoms: * Drivers across various models report the engine shutting down and the vehicle rolling backward on hills, requiring a full power cycle to resume operation. * Some owners report losing all ability to accelerate at traffic lights, accompanied by critical dashboard warnings. * Others report the vehicle shutting off on its own while driving or refusing to start despite the dashboard indicating it is running. I have attached evidence documenting these experiences: **image.png**, **image_2.png**, **image_3.png**, **image_4.png**, and **image_5.png**. Given the frequency of these reports, I urge the NHTSA to launch a formal investigation into this apparent software or engineering failure. The Multi-Information Display (MID) on the instrument cluster (combination meter) has failed completely. I am aware that Toyota issued Safety Recall 25V595 in September 2025 covering the identical combination meter failure on 2023-2025 model year Tacomas and other Toyota vehicles. The root cause identified in that recall — accelerated deterioration of the memory device inside the combination meter due to improper programming — appears to be the same failure mechanism present in my 2021 model year vehicle. I am filing this complaint to formally document that this known defect extends to 2021 model year Tacoma vehicles and to request that NHTSA investigate expanding recall 25V595 to include earlier model years. Toyota should be required to repair or replace the combination meter on affected 2021 vehicles at no cost to owners, consistent with the remedy already established for 2023-2025 vehicles. The referenced vehicle is a 2026 Toyota CH-R electric vehicl, purchased new on April 30th, 2026 from an authorized Toyota dealership. On June 24th, 2026. I tried to start the car at around 5:30am that was parked at my home, the car failed to start, after several attempts, the car became totally unresponsive to any button or my remote. It appears the battery was extremely low or dead. I ended up
... called towing services. The tow truck driver came, used a portable car battery jumper and able to start the car, put the car into neutral gear and tow to the Toyota dealership for investigation. The next day, Toyota dealership called me back and informed me they have completed all the checks and the vehicle is not performing within specification, no issues or fixes were performed, and they said the 12V battery was fine after 2.5 hours of charging. The dealership was unable to reproduce the issue. There was no previous warning of any kind prior to this issue happen. I believe this vehicle may be too frequently transmitting data when the vehicle is OFF, and causing its 12V battery to drain quickly in some cases, which in term causing the vehicle unable to start. Especially if and when the car is parked in poor cellular reception area, the car may constantly try and try to connect and transmit data, and causing rapid discharge of its 12V battery when the car is in OFF position. This can be a safety and reliability issue if not properly addressed. It could caused someone to be stuck somewhere, if in extreme weather conditions, it could be life threatening to someone if their vehicle is unable to start due to this issue. In less than two months of ownership, this has already happened. Based on person research on the internet, here are other owner of the same Toyota CH-R and another similar vehicle of Toyota BZ, also reported similar issue. I have also contacted Toyota brand engagement center and filed a case for this issue. Requesting NHTSA's attention on this matter.Read more

Safety Complaint I am submitting this complaint regarding a recurring fuel gauge malfunction on my 2024 Kia Telluride. On XXX, the vehicle unexpectedly displayed a warning indicating that the fuel tank was empty and that refueling was immediately required. At the time of the warning, there was still a substantial amount of fuel remaining in the tank, and the vehicle had not been driven long enough
... to reasonably consume the displayed amount of fuel. After adding approximately one gallon of gasoline, the fuel gauge immediately returned to a normal reading, indicating that the vehicle had not actually been out of fuel. This strongly suggests that the fuel level indication system or fuel level sensor is intermittently malfunctioning. Since the initial occurrence, this problem has happened three separate times under similar driving conditions. The issue appears intermittently without warning and cannot be predicted by the driver. On XXX, while the vehicle was still covered under the manufacturer's warranty, I brought the vehicle to Cerritos Kia in [XXX] for diagnosis. I provided the dealership with video recordings clearly showing the instrument cluster displaying the false empty fuel warning. Despite reviewing the video evidence, the dealership refused to perform any repair because no warning lights or diagnostic trouble codes (DTCs) were present at the time of inspection. I was told to return only if the condition occurred again. No corrective action was taken while the vehicle remained under warranty. The malfunction has continued to occur after that visit. This defect creates a significant safety concern because drivers rely on the fuel gauge to accurately determine the amount of fuel remaining. A false empty fuel warning can cause drivers to unnecessarily stop in unsafe locations, alter their driving behavior, or lose confidence in the accuracy of the fuel gauge. More importantly, if the underlying defect causes INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On May 29, 2026, my 2025 MINI Countryman SE (battery-electric) became completely immobilized when the charging connector locked into the vehicle's charge port and could not be released. With the connector locked in, the vehicle could not be moved, shifted to neutral, or driven. The defect is in the factory charge-port locking mechanism. The connector could not be freed by me, by a MINI master tec
... hnician working remotely for about 45 minutes, by the tow operator, or by the dealership's own service manager, who tried for over two minutes using both the manual release and the key fob and could not release it (recorded on video). The vehicle had to be towed. Because the wheels were locked, the master technician issued a written instruction that it be transported on dollies. A dealership advisor said the cause was the charge-port locking mechanism and that the lock "wears out over time with use." If accurate, that indicates a wear-related design condition that could immobilize other vehicles of this make and model, and a manual release that did not function when the primary lock failed. An EV that locks its own charge connector and then cannot be released or moved presents a stranding and safety risk, especially away from home or where it obstructs traffic. As of June 26, 2026, nearly four weeks later, the vehicle still has not been successfully repaired. Its own charging data shows no successful charging session since June 3, 2026, and repeated charging attempts at a dealership on June 16, 2026 each delivered zero energy, consistent with the charge-port/connector-lock defect remaining unresolved. The failed component is available for inspection, and all supporting evidence (video, photos, charging data, communications) has been preserved.Read more

**Component or System:** Subaru EyeSight Driver Assist System (specifically the Pre-Collision Throttle Management system). **Incident Description & Warning Messages:** There were no warning lamps, messages, or symptoms of any problem prior to this failure. The incident occurred while I was stopped at a red light at a busy intersection. When the light turned green, I removed my foot from the brake

At that exact moment, the car began beeping repeatedly and the EyeSight logo flashed on the dashboard, incorrectly indicating an obstacle or imminent collision, even though the path in front of my vehicle was completely clear. **Safety Risk:** My safety and the safety of others were put at significant risk because the vehicle was unexpectedly immobilized in an active intersection during a green light. This sudden, unwarranted loss of acceleration created a high risk of being rear-ended by the drivers behind me who were expecting the flow of traffic to resume. **Reproduced, Confirmed, or Inspected:** Because this incident just occurred, the problem has not yet been reproduced or confirmed by a dealer or independent service center. The rear view camera has a recall. I took the vehicle into a Ford dealership on 6/23/26 for this recall. This is a risk to myself and others because backup cameras are a federally required safety feature. This is my 2nd attempt for Ford to fix this recall at two separate dealerships. Ford does not care about their recalls and the safety of Ford customers! I almost hit another car that was co

Ford said that they could not find the problem with this issue and further diagnosing must be done on my part for $198. This is not fair to customers as this was a recall issue. If Ford is not properly training their technicians to diagnose this issue, it should not be the responsibility of the customer to pay for their error in producing this car. The problem has been reproduced and confirmed by a Ford dealership. There were no warning lights or issues that were displayed. Ford only sent out letters stating that a recall was issued on the problem. In May of 2026 my backup camera and trunk was not working. Ford inspected and replaced the parts on the vehicle that they assumed was part of the recall. For a 2016 Mustang, a common cause is not just the camera itself but also wiring/connectors in the trunk area or the APIM/radio module that displays the image. If the dealer simply performed the recall software update or replaced one component and the screen is still blank, the underlying problem may not have been fully diagnosed. My PHEV car randomly and suddenly has loss of propulsion creating a safety hazard in traffic. It seems to occur at low speeds, sometimes from waiting in line at an intersection. This fault produces a solid red battery warning light, but no additional safety warnings, nor an error code for diagnostics. I have experienced this fault randomly in April, May, and June. The worst experience was with
... in intersection traffic when I was unable to accelerate; just coasted with cars whizzing past. My usual dealership servicing staff were unable to reproduce the error. I was told they would/could not assess this fault without an error code. I had my car towed to a different service company. They advised that the 12-V battery showed 66% power; I asked them to replace that battery. I understand that this MAY NOT solve the problem. Although I used a rental car for a week while this problem was being attended, I am now driving my car with the new 12-V battery. Yes, I think my car is unsafe--at least, no one can assure me it has been fixed. I do not know how to assess whether it is fixed, short of being its test driver. My car has an open recall on the HV battery for which a fix is not yet available (not for months so far, anyway), so I drive my car as a hybrid, without charging the HV battery.Read more
