Had no warning messages or alerts. The Instrument Cluster Display Screen went black. We have no way of knowing the amount of gas, speed, mileage, tire pressure or any other alerts. I believe this to be a safety issue. Have replaced both the main and auxiliary battery. There are many 2020 Jeep Wranglers that are recalled for this very issue. Wondering why mine is not.

The radiator was replaced at a second Mazda dealership, we were told "that should fix the problem". There was no indication the radiator needed replaced due to no fluids leaking and no overheating of the vehicle. The battery did fail during this visit. On June 26, 2026, when we were on I-90 in traffic again, the vehicle went into limp mode again with the same hybrid engine malfunction warning. The vehicle was towed to a third Mazda dealership to finally have the hybrid battery finally replaced. The only warnings were when we went into limp mode, no dash lights or check engine lights, nor overheating. On 6/11/26, around 2:10 pm, when i was slowly pulling over to park on a road side, the car suddenly rushed forward unexpectedly. It was so powerful, at the time I could only hear the panic reactions from the two passengers and sense the imminent collision with the parked bmw in the front of several car length away. Right before the collision i steered the wheel to the right to avoid the bmw and m
... y car came to violent halt less than a foot next to the bmw. I came out of my car to check the tire position before backing out to park a bit behind. This was on a country road, a Thursday afternoon with not much traffic. There was no distraction. If not were the enough spacing i would have no doubt crashed into the bmw in the front. When the dust settled, I did a "power off" reset to the model S and drove the rest of the day normally without any incidents. I went to Tesla service center near where i live the next morning, 6/12/26, to report it. They asked the precise time the incident occurred. But did not inspect the car. I was told an inner inquiry will be opened and it would take at least a few days. They said i could continue to drive the car normally which i have not because of not knowing what had caused it. I went to the service center today 6/29/26 after more than two weeks of no feedback. I was very disappointed in the answers given. The guy said basically based on the design of the car the incident could not have been caused by the car ,without providing any info as to what had been found from the promised inquiry. There was no written report on this. The person offered to check the brakes but also said he did not think its necessary. Just to made me feel better he said. At last, he provided the tesla web site to request "private data" during time window of the incident. I told him i will seek help from NHTSA now that tesla is not. I was told by AI, tesla has collected data on every car they sold to the details that can reveal how the paddles were pressed.Read more

Traveling had an engine light come on. stopped at a dealer they inspected found multiple errors reset engine light and said continue home. when home bring into my dealer. Brought truck into shop received a call two days later that it is fixed and working fine. Went to the shop asked what the problem was, told there is a ground strap under the truck that is like the main part to make sure everything works regarding electronics. The dealer has had to repair many of these ground straps, they are exposed to the elements and come loose I was told. They had two other GMC trucks in the shop with the same issue that day. the ground strap is exposed to the elements do not understand why this would not be covered just like a battery lead to stop from corroding and have to clean it or replace it. Not to mention there is no where in the service manual I can find about any maintenance that needs to be done with it. Appears to be a very bad design from the car manufacture.Read more

System/Component Malfunction: High-voltage hybrid powertrain system failure. Code P0DAF-00 (Hybrid EV Battery Cell Balancing Circuit) is present and locked in the Hybrid Control Processor (HCP). Vehicle is available for inspection. Safety Risk: Operating a vehicle with an unresolved high-voltage hybrid cell balancing defect poses an immediate safety risk. Cell imbalances can cause sudden loss of
... motive power at highway speeds, propulsion shutdown, or an increased risk of a thermal event in the high-voltage pack. The manufacturer (Stellantis/RAM Care) refuses to provide a written safety determination, yet the dealer instructed me to continue driving a compromised high-voltage system on public roads until the failure worsens. Dealer Confirmation & Inspection: Confirmed and reproduced by authorized dealer (Ron Tonkin RAM) across multiple visits: Feb 25, 2026 (RO 502329): Code P0DAF-00 present. Dealer performed a BPCM software flash, which failed to resolve the underlying fault. June 2026 (RO 504097): Held for 10 days. Technician documented: "Scanned codes and found code P0DAF-00- Hybrid EV Battery Cell Balancing Circuit present but code is stored (not active) and also not stored in PCM, only the HCP." Tech test-drove 97 miles but corporate engineering (STAR) rules prevent parts authorization until the code migrates from HCP to PCM as an active fault. Dealer explicitly instructed me to drive it until it triggers a hard failure. Warning Lamps & Symptoms: First appeared Feb 25, 2026, via a yellow Check Engine Light. The light intermittently cycles on/off as the computer stores the code, but the underlying high-voltage defect is completely un-repaired.Read more

The rear power liftgate on my 2024 Hyundai Santa Fe Hybrid (VIN [XXX]) repeatedly opened and closed on its own, without any command from the key fob, interior button, or any occupant. This occurred over a period of several months. It happened while the vehicle was parked and, more dangerously, while the vehicle was in motion at normal driving speeds. An un-commanded liftgate opening while driving
... creates a serious safety hazard: loss of rear visibility, the risk of unsecured cargo being ejected onto the roadway, and the gate striking a person or object when it deploys unexpectedly. The overnight, un-commanded cycling while parked is extensively documented on my home security cameras, which recorded the gate opening and closing repeatedly throughout the night. The dealer (Williams Hyundai, Lansing, MI) diagnosed the cause as a faulty resistance fault in the rear hatch wiring harness assembly, which sent false signal inputs to the vehicle and triggered the motorized latch and actuators. The repair was completed under the New Vehicle Limited Warranty. A secondary effect of the continuous overnight cycling was repeated deep-discharge of the 12V battery, leaving the vehicle dead on multiple mornings. I am reporting this because the un-commanded operation of a motorized liftgate — particularly while the vehicle is moving — is a safety defect that may not be isolated to my vehicle. I have retained service records, the dealer's diagnostic notes, and the home security footage, and can provide them on request. This vehicle has experienced two separate failures of the high-voltage (HV) battery pack, each resulting in a different drive unit being damaged and total loss of electrical systems while driving. First incident (July 28, 2025): While driving, all warning lights illuminated and the vehicle indicated a possible electrical hazard. The vehicle lost power completely. It was taken to a Rivian service c
... enter on July 29, 2025, and was not returned until August 15, 2025 — a total of 17 days out of service. Rivian’s technicians found multiple faults in the HV battery pack and replaced it. Rivian’s own repair notes state that its engineering team requested the front drive unit also be replaced “due to the nature of the failure with the HV battery pack” — i.e., Rivian attributed the drive unit damage to the battery pack failure, not to an independent defect. Both the HV battery pack and front drive unit were replaced under warranty. Second incident (June 29, 2026, approx. 16,256 miles): After charging at a Tesla Supercharger, the vehicle displayed a “Rivian thermal system fault” warning, which indicated driving could continue. While subsequently driving on the highway, the vehicle displayed a “reduced performance” warning, followed by an “all-wheel drive system fault” warning. A loud popping sound was then heard from the rear drive unit, accompanied by visible and smell of smoke in the vehicle. The driver safely pulled over. All electrical systems malfunctioned in the same manner as the first incident. The vehicle is currently undergoing diagnosis; based on the symptoms (identical to the first incident) and Rivian’s own causal link between battery failure and drive unit damage in the first incident, it is possible the HV battery pack is again involved, this time in connection with the rear drive unit.Read more

The engine remained on, but the vehicle did not respond properly. The vehicle
... struck a fixed object/rock. A police report was filed/requested, and medical evaluation was sought afterward. The suspected failed or malfunctioning systems are the engine/powertrain response, electrical system, and related control systems. The vehicle should be available for inspection upon request and should be preserved before any diagnostic codes, freeze-frame data, event data, modules, or electronic records are cleared, reset, repaired, overwritten, or destroyed. Before this incident, repeated drivability and safety-related concerns were reported to Infiniti/dealer service departments, including vibration during acceleration, abnormal gas pedal vibration, lack of acceleration, poor power response, jerking/jolting, rough ride, abnormal noises, front-end noise, and safety/reliability concerns. Some concerns were documented in service records. Prior repairs included front passenger-side wheel bearing replacement and cooling fan/IPDM-related warranty work, but the drivability/power-response concerns continued. UNKNOWN whether the manufacturer completed a full post-incident inspection. The vehicle has been reviewed by police and insurance-related parties to the extent related to the collision. This complaint is being filed because the condition appears to involve a safety defect causing loss of power response and system fault warnings while driving. I drive the car with the engine-idle-at-stop function disengaged. Several times when slowing down, but still moving, the car shifts into park; the AC and radio remain on. He said the New battery is fine, the alternator is fine, and they do not know what to troubleshoot. **Vehicle:** 2025 Kia Carnival **Current Mileage:** Approximately 18,000 miles I am reporting a serious and ongoing safety defect involving the center LCD display in my 2025 Kia Carnival. The problem began immediately after the vehicle's first scheduled service at an authorized Kia dealership and has continued ever since. The center LCD screen intermittently flickers and then goes completely bla
... nk while I am driving. When this happens, the rearview camera image is also lost while the vehicle is in Reverse, leaving the display completely black and preventing me from seeing behind the vehicle during backing. This creates a significant safety hazard and increases the risk of striking a pedestrian, child, vehicle, or other object. The display failure while driving is also extremely distracting because critical vehicle information and functions become unavailable. I have taken the vehicle to authorized Kia dealerships multiple times for warranty repairs, but the defect remains unresolved. The LCD display has been replaced, yet the problem continues to occur. The dealerships have kept my vehicle for extended periods during several repair attempts without permanently correcting the issue. Service dates include: * September 9, 2025 * March 10, 2026 * March 18, 2026 * June 1, 2026 The issue is intermittent but repeatedly occurs without warning. I transport my family, including infants, in this vehicle, and I am extremely concerned about their safety and the safety of others. Losing the backup camera and display while reversing or driving creates an unacceptable safety risk. I believe this may be a safety-related defect affecting the vehicle's display system. I respectfully request that NHTSA investigate this issue because multiple warranty repair attempts, including replacement of the display, have failed to resolve the defect.Read more

I am reporting recurring failures involving the vehicle's electronic control systems. These issues have been documented with the dealership on multiple occasions and a Nissan District Technical Service Manager (DTSM) has also inspected the vehicle. Nissan has not been able to replicate the issues as they are intermittent. Braking System: When sitting at a stop light, where I had my foot on the br
... ake, I heard a pump/whooshing sound from the front of the vehicle while the brake pedal suddenly traveled to the floor. On two separate occasions, the pedal has also become rock-hard. The Nissan DTSM was not able to replicate and suggested that the brake pedal becoming hard or going to the floor is "normal", and that my car showed no codes, and there was no recommended action required--that as long as the vehicle stopped, it was not a problem. After the DTSM inspected the vehicle, the brake pedal went fully to the floor five consecutive times while backing out of a parking space. I heard the same pump/whooshing sound from the front of the vehicle. Because I was backing up slowly, and the brake pedal was not responding to me given it was going fully to the floor, it was unclear how much braking power I truly had. The pump continued to make noise as the first few times I pressed the brakes driving away. Engine/Transmission: When the gas pedal is released, the engine intermittently continues to rev, and not respond to throttle input as designed, requiring the use of the brake to force a shift. But, Nissan stated they are aware that there is "general dissatisfaction" with the transmission overall, and suggested I wait for a future software update in the hopes that it may address some of the concerns. Electrical/Integrated Components: Failures include side mirrors failing to open or lock into position when driving. Audio/Nav system: Once when driving in an unfamiliar area, the audio/sound went out, with no turn by turn (or radio) sound at all, forcing me to watch the screen.Read more

own a 2015 Ford Focus that has experienced repeated transmission-related problems over several years. The symptoms have included harsh shifting, hesitation, failure to shift properly, and at one point the vehicle would not shift into reverse. These transmission issues have caused ongoing concerns about the vehicle's safety and reliability. I have brought the vehicle to Ford dealerships on multiple
... occasions seeking diagnosis and repair. Ford replaced the Transmission Control Module (TCM) on November 16, 2024 under Ford Field Service Action 14M02 after diagnosing transmission-related problems, including Diagnostic Trouble Code P087A. Despite that repair, the vehicle continued to experience transmission-related concerns. Approximately one year later, another Ford repair order documented multiple TCM-related diagnostic codes and directed that the vehicle be returned to a Ford dealership for further evaluation. The transmission concerns persisted even after the previous TCM replacement. Most recently, I again presented the vehicle to a Ford dealership because of continuing transmission issues. I was provided an estimate of approximately $2,000 for additional transmission repairs. At the time of the estimate, I was not provided the complete diagnostic trouble codes or technician findings explaining why another major transmission repair was necessary. Over the course of ownership, I have made multiple appointments, diagnostic visits, and repair visits related to the transmission and have incurred significant out-of-pocket expenses, including diagnostic charges, towing costs, and repairs. The repeated failures and recurring transmission concerns have required multiple visits to Ford dealerships and have not resulted in a lasting repair. My concern is not only the financial cost but also the safety of operating this vehicle. Unexpected transmission malfunction can affect acceleration, shifting, and the vehicle's ability to move normally in traffic, creating a potential safeRead more

1. Car's computer abruptly shutdown all controls/systems when lightly applying brake to stop at yellow light. Lost Total control of car, steering locked, 4 wheel drive locked all wheels, park brake inoperable, doors locked, windows would not operate, key fob was made inoperable, HVAC was inoperable, no lights worked except for Emerg flashers, wipers activated and went erratic, multiple alarms went
... off saying what Not to do but gave No advice on "What to do", MB SOS made inoperable. see MB Inv 379224 they determined it was EZS control module. BUT MB gave no diagnostics why the car left driver with control of nothing. 2. My safety was put at high risk. Why? I manually opened driver door, it closed itself car on a slight inclined Rd, manually opened trunk to warn others, returned to car and all doors are locked, plus all retractable handles flush with door body manual key useless. Due to perfect weather day, passenger window down 3in allowing to insert arm to unlatch door manually. Car was in lane 2 of 3 lane traffic light intersection. Very busy major road accessing a 6 lane US Hwy. Luckily Police arrived on their way to work, stressed very dangerous location, stayed until car towed away. 3. MB dealer returned car. Will not confirm they will do a root-cause analysis of the total loss of control when one item (EZS) fails and leaves driver totally unsafe. There must be a software malfunction.....too many alarms. MB cannot assure this E350 or (any other one) will not have this type of shutdown again leaving the driver Unsafe. Reading MB car safety policy this dealer and MB USA Georgia do not comply with the policy. Concerns over Driver safety remain very high despite the replaced EZS, software was not diagnosed 4. Dealer stated on Jun 26, the failed EZS is yet to be analyzed and gave no timeline to do so. I gave 26 days for dealer to comply with MB policy. It is not forthcoming 5. On [XXX] after spending the day at Henson Creek Golf Course in Fort Washington, MD, I returned to my vehicle and discovered that it had completely lost all electrical power and would not start. The vehicle exhibited no warning signs prior to the failure. Believing the issue to be related to the battery, I contacted AAA for roadside assistance. AAA attempted to jump-start the vehicle, but the jump-
... start was unsuccessful. The vehicle was subsequently towed on May 18, 2026, to Pohanka Hyundai in Capitol Heights, MD for diagnosis and repair. After approximately nine days, the dealership advised that the battery terminals had been secured and that no additional problem could be identified. On [XXX], the exact same issue occurred again; however, this incident was significantly more dangerous because it occurred while the vehicle was being driven. While traveling during morning rush hour through a busy intersection in Northeast Washington, DC, the vehicle suddenly and completely lost power without warning while in traffic. The vehicle stalled in the middle of the intersection, creating an extremely hazardous situation. Fortunately, motorists traveling behind me were able to stop in time, and no collision occurred. However, this incident could easily have resulted in a serious crash involving injuries or fatalities. The vehicle was towed to Ourisman Hyundai in Bowie, MD, where it is currently undergoing additional diagnosis. I am extremely concerned that this vehicle has experienced two complete electrical failures within approximately one month, including one incident that occurred while driving in heavy traffic. Because the dealership was unable to identify the root cause after the first incident, I am concerned that a serious defect may exist involving the vehicle's electrical system, battery system, wiring, power distribution system, or another component affecting vehicle operation and safety.
