While driving my 2018 Hyundai Tucson Limited 1.6L Turbo with the 7-speed EcoShift dual clutch transmission (DCT), the vehicle has repeatedly failed to engage and hold gears, resulting in loss of motive power in traffic. Diagnostic scanning retrieved codes P074B (Unable to Engage Gear 3), P074B71 (Unable to Select 3rd Gear), P074B07 (Gear Dropped Out of 3rd Gear), P092177 (1st Gear System Referenci
... ng Failure), and P0921 (Gear Shift Forward Actuator Circuit Range/Performance). A Hyundai dealer diagnosed the failure as requiring transmission replacement at approximately 93,000 miles. This is the same 7-speed DCT and the same failure mode addressed by NHTSA Campaign 16V628000 (Hyundai Recall 149) and Hyundai Service Campaign T1B, which covered 2016–2017 Tucson vehicles but excluded the 2018 model year despite using the same transmission. Hyundai has acknowledged this defect in 2018 vehicles through TSB 21-AT-014H and TSB 22-AT-007H, which prescribe a TCU software update, but issued these as service bulletins rather than a recall, so owners were never notified. When I requested the TSB software update, the Hyundai dealer refused to perform it. This defect creates a serious safety hazard: the vehicle can drop out of gear or fail to accelerate without warning while in traffic, increasing the risk of a crash — the identical hazard NHTSA cited in granting recall 16V628000. I request that NHTSA review whether the existing recall scope should be expanded to include 2018 Tucson vehicles.Read more

I leased a new 2026 Cadillac VISTIQ on XXX. The vehicle had approximately 960 miles when the incident occurred. After DC fast charging at a Tesla Supercharger in Las Vegas, NV, the charging session did not complete normally. Shortly afterward, the vehicle displayed a “Reduced Acceleration – Drive with Care” warning message and vehicle performance was significantly reduced. The air condition
... ing also stopped cooling properly while outside temperatures exceeded 100°F. Because vehicle acceleration was limited and the warning indicated reduced propulsion performance, I did not feel comfortable continuing to operate the vehicle. I canceled a planned trip from [XXX] to [XXX] because I no longer believed the vehicle was reliable for highway travel. The vehicle was taken to the selling Cadillac dealership for diagnosis and remains available for inspection. At the time of this complaint, I have not yet received the dealership’s diagnosis or repair findings. This incident occurred on a brand-new vehicle with fewer than 1,000 miles. I am submitting this report because a sudden reduction in propulsion immediately following DC fast charging could create a safety concern if it occurs while merging, passing, or traveling at highway speeds. My 2018 Ram 2500 has been experiencing a message about servicing the electronic braking system and it is causing issues with my entire truck. This causes concern for not only my safety but my families safety as multiple people operate my vehicle along with my child. The last thing anyone wants is for someone to get hurt especially when it could be avoided however I feel that the incident is being avoided since it has taken over almost two (2) years since this recall was issued and nothing has been done to repair said recall. I am a Law Enforcement Officer in WV along with a Volunteer Firefighter. I see the outcomes of issues with negligence of persons driving vehicles and rarely do I see anything where it is not the driver's fault. I would not wish to have to investigate into the cause of a crash that took someone's life where a known issue was able to cause the accident and no sort of remedy has been found for almost two years after being known.Read more

On 06/26/26, while traveling at highway speeds, the vehicle suffered a sudden, catastrophic loss of motive power in active traffic due to a blown out/melted Cylinder 4 ignition coil. This unexpected engine failure forced raw fuel into the exhaust system, causing an immediate internal thermal meltdown of the catalytic converter, rendering the vehicle entirely undriveable and creating a severe highw
... ay safety hazard. This vehicle was previously serviced under NHTSA Campaign 21V264000 (Subaru Recall WRE-21), which was intended to permanently remedy an ECM software defect that over-energizes and melts the ignition coils. Subaru of America (Case # 260630-2101598) is denying coverage, claiming that because the recall was executed 90,000 miles/5 years ago, and because the current software Calibration ID (CID) matches their post-recall database, the thermal melting is standard "wear and tear." I am filing this complaint because a safety recall remedy must be permanent and effective. The physical evidence, a melted plastic coil housing with a hole blown through it, is an engineering impossibility under normal wear and tear and proves that the original software patch failed to permanently mitigate the high-voltage overloading defect. Over a 5-year period of gradual thermal degradation, the defect reoccurred, creating the exact highway hazard the recall was mandated to prevent. I request a federal investigation into the long-term efficacy and failure rates of the WRE-21 software reflash.Read more

Complaint Summary: Mandated federal safety recall remedies performed on my vehicle have directly induced a cascading, critical safety failure of the vehicle's high-voltage propulsion management system. My 2022 Hyundai IONIQ 5 (VIN: [XXX]) was brought to an authorized dealer on XXX at 200,224 miles to complete safety recall Campaign 272 / NHTSA Recall 24V-204 (ICCU Software Update and Fuse Inspecti
... on). After receiving this software flash, the vehicle's electrical charging system failed (DTC P0D5C00), requiring a full hardware replacement of the Integrated Charging Control Unit (ICCU) on XXX. 142 miles after the physical replacement of the high-voltage ICCU, the vehicle displayed an emergency battery malfunction message on the dashboard. Diagnostic scans reveal active fault code P1BC700 in the Battery Management System. The manufacturer's dealership is now charging $1,909.77 out-of-pocket to replace the Power Relay Assembly (PRA).The current recall modifications for the E-GMP platform are failing to safely mitigate high-voltage current spikes. The electrical strain or servicing procedure required by the recall has directly compromised the adjacent power relays. This creates a severe safety hazard, leaving drivers exposed to subsequent high-voltage component failure immediately after completing mandated federal safety recalls. NHTSA should investigate cascading high-voltage component failures occurring in vehicles directly following the completion of Recall 24V-204. (Transmission and motor) I had taken my car to the dealership on XXX for recalls and all the signal were fine before going there. They took my car back to do the recalls and my son and I waited . Once they were done we left closer to closing time is noticed there was a burn mark on my dash board that wasn’t there before the recalls. Going forward on the way home. We were waved down by a man stat
... ing that my signal and brake light weren’t working . So that next day we took my car back to the dealership to see what happen, why all the sudden different alerts were coming up. They had the same mechanic relook at the car and a when finish 31/2 hour latter was told I needed a new transmission, costing $7384.94 which I don’t have . They charge me $179.00 just to confirm what they said about needing a new transmission. I was devastated and confused why I wasn’t told that the day before. So a few days later I took it to another machanic to see if the could see why the lights weren’t working and he ended up putting new bulbs in and got them working but also he put it in a diagnosis machine 2 of them and couldn’t get a reading at all and said I should take it back to find out why he couldn’t get a reading. I was so frustrated at that point I just was upset! I then had a friend of the family mechanic look at it and he said it wasn’t the transmission it was a knock sensor that I needed so I ordered it. When I called him to install the knock sensor he was too busy to do it and it had already been a whole month of waiting so I reached out to a place in Cumberland RI to see if he could install the knock sensor had it towed there and he kept it for three days and said it was a transmission computer and he tried two of them and neither of them worked and charged me $150 for that I was humiliated and crying , I went there to have my car towed home seen him while waiting for the tow truck to come he said I needed a new transmission and now a motor, crying again. I own a 2018 Honda Odyssey EX-L (VIN [XXX]). The vehicle experienced a sudden loss of power and entered limp mode, with multiple safety and driver-assistance systems shutting down at once (adaptive cruise, braking assist, lane/road departure mitigation, EPS, etc.). A Honda dealer (Allen Honda, College Station, TX) retrieved DTC P0369 and documented that the front camshaft is worn and has thrust fo
... rward, contacting the camshaft position (CMP/TDC) sensor and damaging the thrust plate. The dealer is recommending an internal engine repair costing several thousand dollars. I am very concerned because Honda has already acknowledged serious engine defects in this same J35 V6 family under Recall 23V751 (connecting rod bearing wear leading to engine damage and stall) for 2018–2019 Odyssey, yet my dealer states my VIN is “not part of the recall” and that this is a “separate issue.” In practice, I am seeing a major internal engine failure that causes sudden derate and disables multiple safety systems, but I am being told there is no recall coverage or assistance simply because my individual VIN falls outside Honda’s current recall production range. I am requesting that NHTSA review this incident as a potential recall exclusion problem: owners with the same engine family and serious internal failures are being denied recall coverage on a technical VIN cutoff, even though the safety consequence—loss of power and shutdown of critical safety systems while driving—is similar to the failures already recognized under Recall 23V751 and the ongoing engine defect investigation. I believe this situation may warrant expansion of existing recall coverage or a new recall campaign for currently excluded VINs like mine, and reimbursement for owners who have already paid for these repairs. After the vehicle is off, and after the smart key fob is removed from the vehicle, the dashboard will flicker, the alarm sometimes goes off and the headlights turn on draining the battery and leading to me having to jump the car to get it started. This makes the car undependable as it could happen after driving somewhere and leave myself or my wife stranded. I have taken this in to the Honda deal
... ership but they noted that it did not occur when they were servicing the vehicle and they weren't sure what the issue could be. Many times the check engine light, FCW and LDW lights will come on and then reset after the car has been driven.Read more

Door lock actuators have failed completely on 2 doors, and intermittently on 1 door of a 2021 Nissan Altima. They do not respond to either the key fob or door button. There are no warnings or indicators associated with this component so it is unknown when the failures first occurred, but it was first noticed in summer of 2025. The components have not been inspected but could be made available for
... inspection if requested. Not being able to unlock doors could present an entrapment hazard in the event of a collision. The doors remaining unlocked is a safety concern as there is nothing preventing the door from potentially being opened while driving, and a security concern as the vehicle may inadvertently be left unlocked even if the lock buttons are pressed on the key fob and/or door. Numerous official and anecdotal consumer reports exist but at the time of this submittal the manufacturer has not issued any notices or recalls regarding this problem.Read more

I made an initial post on 06/18/2026 so please reference that as well as this. I have not been able to drive the car since it died on me on the 18th of June. The car has been looked at a few times (actually stalling a third time while idling but was able to turn back on.) It has also displayed symptoms associated with the Ford Recall #26S40 / NHTSA recall #26V369 but it is listed at NOT included i
... n the recall as of 07/06/2026. MIL is pulling Code P0420 (catalytic system efficiency below threshold) while only having 40k miles. It is also putting out an inaccurate fuel level for miles driven. The fuel tank itself is misshapen (concave bottom of the fuel tank). Again, reiterating here that it stopped twice while driving within 2 days on major PA highways/interstates and stalled for a third time while in idle mode as described earlier in this post. This has all happened within 28 miles of the TCM being replaced.Read more

On 2/2/26 I took my vehicle to Thomson Motor Company to have the 68c recall repair completed. On 2/5/26 my car wouldn’t start and I was stranded. My vehicle was towed back to the same repair facility. On 2/6/26 they told me that my regular battery died and had to be replaced. After researching when the hybrid battery is re-coded/reprogrammed from the recall, it causes the regular battery to die
... due to and system overload to the regular battery. The dealer wouldn’t take responsibility or Jeep wouldn’t take responsibility. The dealer wanted to charge me $500 so I had the local Auto Zone replace the regular battery with a new battery for $238.00. I sent in a copy of the slip from the dealership to show what they had recommended and submitted a copy of the credit card receipt to Jeep and they are refusing to cover the replacement. I would not have had to replace the regular battery if the 68c recall didn’t cause my regular battery to die due to the overload. From my understanding they should have been replaced at the same time to keep this from happening. I want to be reimbursed for replacing the battery, for the cost of the tow and for the diagnostic at Thomson Motor Center.Read more

My 2014 Jeep Grand Cherokee suddenly developed a no-start condition after previously operating normally. The vehicle would crank but would not start. The battery and fuel injectors were checked and replaced as needed, but the problem persisted. Based on the symptoms and known issues affecting 2011-2014 Jeep Grand Cherokees, the internal fuel pump relay/TIPM is suspected. This defect has been the s
... ubject of previous safety recalls because it can cause engine stalling or a no-start condition. Although my vehicle is not currently included in an open recall, it is experiencing the same type of failure. A failure of the fuel pump relay can leave a vehicle stranded or cause the engine to stall unexpectedly, creating a serious safety hazard if it occurs while driving. I believe this issue should be investigated to determine whether additional vehicles should be covered by the recall.Read more
