The vehicle has experienced repeated failures of the infotainment and display system, including the screen going blank and displaying “no content available” while functions such as Apple CarPlay are actively in use. The issue affects the ability to control media and view system information from both the main display and steering wheel controls. The vehicle has undergone multiple repair attem
... pts, including replacement of the human machine interface (HMI) module and radio module, as well as repeated programming. Despite these repairs, the issue continues to reoccur, including immediately after the most recent repair. The condition is intermittent and difficult to duplicate, and a field service engineer has been unable to consistently reproduce the issue. The dealership has indicated there may not be a current fix and that resolution could depend on a future software update. This ongoing failure of critical display and control systems raises safety concerns and has not been resolved despite multiple repair attempts.Read more

I was driving my Ram 1500 5.7L eTorque this morning (2022, ~36k miles). About 10 minutes in, I was stopped at a light and the truck shut down completely (engine, electric, etc.) There was no prior notice, nor was it having problems. When I tried to go as the light turned green, obviously the truck did not go. I noticed several panel lights come on, including the battery light. There was also
... a message that said something to the extent that the emergency brake had automatically engaged and needed to be released. After a minute, I was able to re-start the car and drive home. Cars behind me honked, and eventually went around me. I have an appointment to take the truck in, but no diagnosis as of now. I did research and read that this a known problem with the 5.7L eTorque, and that some trucks have been recalled (NHTSA Recall 23V-235) for similar issues, and there is also on ongoing investigation for others that were not part of the recall. My experience is similar to everything I've seen/read reported by other drivers (e.g., seemingly random stall with no prior warning, certain dash lights coming on, auto-engagement of the parking break).Read more

2025 Hyundai Ioniq 5 XRT, VIN [XXX] . Four 12V battery failures requiring jump-starts in under 800 miles of ownership. No fault codes stored at any failure event. Visit 1 (01/12/26, ~100 mi, RO HYCS79193): Factory OEM battery confirmed failed by GDS. Replaced under warranty. Visit 2 (01/30/26, ~300 mi): Second OEM battery confirmed failed by GDS. Replaced. Visit 3 (03/31/26, ~800 mi): Aftermarket
... AGM tested healthy but required jump-starts. GDS recorded 12V SOC dropping from 99% to 21% then recovering to 79% — consistent with intermittent LDC output interruption, not parasitic drain. Hyundai Techline contacted. No repair performed. Visit 4 (04/08/26, ~800 mi): AGM measured 5.5V, required jump-start. Maryland Lemon Law notice submitted to Hyundai Motor America. The Ioniq 5 has no alternator — the 12V system depends entirely on the ICCU’s DC-DC converter. Hyundai has acknowledged this ICCU defect under NHTSA Recalls 24V-204 and 24V-868, covering 2022–2024 Ioniq 5 models. The 2025 model uses the same E-GMP platform and ICCU architecture but is excluded from both recalls. My symptoms — repeated battery failure with no DTCs, confirmed SOC anomaly on GDS, failure across two battery chemistries — match the subclinical LDC failure mode documented in those recalls. A Hyundai North America case is open and engineering has been engaged. No repair has resolved the defect. I request NHTSA investigate whether the 2025 Ioniq 5 is affected by this defect and consider expanding Recall 24V-868 to the 2025 model year. Please add this complaint to Investigation PE23-011. A motorcycle fire occurred involving a Royal Enfield Himalayan 450 while the vehicle was parked and not in operation. The specific component that failed is UNKNOWN. However, based on the observed damage pattern, the fire appears to have originated in the rear-mid section of the motorcycle, in the area near the battery, wiring harness, or fuel-related components. The incident presents a significa
... nt safety concern because the fire occurred without active operation of the vehicle. This raises the possibility of an electrical short, battery-related issue, or fuel system malfunction that could lead to spontaneous ignition. If such a condition were to occur in a confined space (such as a garage) or near structures, it could pose a serious risk of property damage, injury, or loss of life. There were no known warning lights, error messages, or prior symptoms observed before the incident. The failure occurred suddenly. The vehicle has not been inspected by the manufacturer. No technical inspection has been conducted to determine the root cause of the fire. A local fire department investigated the incident and determined that the cause of the fire is “undetermined.” No external ignition source or confirmed arson was identified. The damaged motorcycle remains available for inspection upon request. Given the severity of the incident and the lack of a determined cause, further investigation into potential electrical or fuel system defects is warranted to ensure there is no broader safety risk affecting similar vehicles.Read more

Three power window regulators have failed on my 2016 Hyundai Tucson within a six-month period. The front driver and front passenger windows both failed first and required replacement. The rear driver-side window has now also failed and is currently stuck in the open position. When the failures occur, the window will move up slightly but then drop back down or will not stay closed, indicating a fa
... ilure of the regulator mechanism rather than normal wear. The fact that multiple window regulators have failed in a short period suggests a defect or premature failure of this component. This creates a safety concern because one window is stuck open, exposing the vehicle interior to weather, reducing security, and potentially affecting driver visibility and safe operation. This appears to be a known issue with this model year, but my vehicle is not included in any recall.Read more

Vehicle was parked, engine off, and unattended. An internal electrical fire originated in the overhead/sunroof wiring area. Damage includes melted wiring harness near the sunroof, burned center console, and soot throughout the interior. The fire originated in a constant-power electrical circuit and spread downward into the cabin. There were no aftermarket modifications, no external heat sources, a
... nd no objects in the vehicle that could have caused ignition. Ambient temperature conditions were normal and insufficient to cause this damage. This represents a serious safety defect because the fire originated spontaneously within the vehicle’s electrical system and could have resulted in a full vehicle fire or injury if occupants were present. The failure appears to involve factory-installed wiring/components and is not consistent with normal wear or external factors.Read more

While driving on a sunny day on a 35mph road, my dashboard reported “Check Electrical Vehicle System”. A red battery symbol appeared and my cruise control disengaged with a message “requirements not met”. When I got home I read my 12 volt battery monitor that reported the battery was 11.99v and only had a 60% state of charge. Just prior to this short drive from home, my battery was 12.77v
... and had a 100% state of charge. I put in my ODB reader and it provided two error codes: P1A9096 and P1A961C. The first error code is the result of an ICCU failure in its DC to DC circuit (the one that charges the 12V battery from traction battery). Using a battery charger, I was able to fully recharge the 12volt battery. My monitor says that it’s healthy with 100% state of charge. Turning the car back on resulted in a rapid drain of the battery from 100% to 93% in just five minutes while parked. The electrical system failure warning lights are still lit. I’m having my car towed for repair. If this failure had occurred more than 10 minutes from my home, I would’ve been stranded.Read more

I am filing a safety complaint regarding an open manufacturer recall on my vehicle that has not been completed despite being scheduled for repair. On or about 2/16/2026, I scheduled the recall repair with an authorized dealership/service center. The repair was accepted and scheduled; however, the repair has not been completed to date. It has now been approximately 50 days (or more) since the sche
... duled appointment, and the recall remedy remains unresolved. Despite multiple attempts to follow up, the repair has not been performed, and no reasonable timeline has been provided for completion. This ongoing delay leaves the vehicle in an unrepaired condition subject to a known safety-related recall issue. Safety Concern: The unresolved recall defect presents a potential safety risk, as the manufacturer has identified the issue as significant enough to warrant a recall. The continued delay in remedy increases the risk of operating a vehicle with a known safety defect. Dealer / Manufacturer Response: •Recall repair was scheduled and acknowledged by the dealer - suburban ford of Ferndale. •Repair has not been completed after an extended period. •No adequate resolution timeline has been provided. •Communication regarding the delay has been insufficient. Impact: •Vehicle remains in an unsafe or potentially unsafe condition. •Loss of use / inability to rely on manufacturer recall remedy. •Unreasonable delay in addressing a safety recall. Requested Action: I am requesting that NHTSA: •Review this matter as a potential failure to timely remedy a safety recall •Ensure the manufacturer and/or dealer fulfills its obligations to complete the recall repair promptly •Investigate whether systemic delays or failures are occurring in the recall remedy process Additional Notes: This issue is not merely about the existence of a recall, but rather the failure to complete a scheduled recall repair within a reasonable timeframe after acceptance by the dealership.Read more

My 2025 Porsche Cayenne S was involved in a low-speed crash and experienced multiple safety failures related to the Driver Assistance Control Unit, including phantom braking and camera failure. This crash, I believe, is a direct result of the safety defect identified in Recall ASB2 (NHTSA 25V896), which causes the Driver Assistance Control Unit to fail to process image and sensor data correctly. M
... y vehicle has exhibited a dangerous pattern of electronic and sensor instability that aligns perfectly with the technical failures described in the recall: The Collision: While reversing in a parking garage on November 25, 2025, I struck a wall because the rearview camera and parking sensors failed to provide any notification or visual feed. I now understand this was caused by the "transient signal noise" cited in the recall, which prevents the system from detecting obstacles in specific lighting environments. Phantom Braking: On two separate occasions, the vehicle abruptly "slammed" on the brakes for no apparent reason, nearly causing high-speed rear-end collisions. When reported to The Collection Porsche Service on January 23, 2026, the Service Manager admitted the system is hypersensitive to shadows. This confirms the control unit is failing to correctly interpret image data—the exact root cause of the current recall. Total Electrical Failure: On December 2, 2025, the vehicle suffered a defective alternator, causing a total shutdown in a live intersection. This resulted in a seven-week repair (completed Jan 23, 2026). Unstable voltage from a failing alternator is a primary source of the "signal noise" that causes the driver assistance systems to malfunction. I initially believed the bumper contact was a personal error. However, given the documented failure of the alternator and the subsequent recall regarding camera/sensor data processing, it is clear that the vehicle’s safety systems were compromised at the time of the incident. Porsche NA has been notified.Read more

I am reporting a safety-related failure involving my 2021 Volkswagen ID.4. The vehicle has experienced two separate incidents where it suddenly lost power while driving and became inoperable without warning. The first occurrence happened while I was in the middle of an intersection. The vehicle lost power and could not continue moving, creating a dangerous situation with surrounding traffic. The s

In both cases, after the failure occurred, the vehicle would power on and could be shifted into drive or reverse; however, the vehicle would not move or provide any propulsion in either gear. The vehicle had to be manually pushed out of the roadway. The vehicle has been brought to the dealership multiple times over the course of ownership for issues involving the electrical and high-voltage systems. Service records show repeated diagnostic procedures, recall and campaign work, and repairs involving the battery system and related control functions. Diagnostic fault codes associated with this issue have included P0BEE00 and P0BE600 (drive motor current sensor circuit faults), along with communication-related faults. Despite these multiple service visits and repairs, the condition has not been reliably resolved, indicating an ongoing and intermittent system failure. Most recently, the dealer diagnosed a failure related to the drive motor system. The rear drive motor was replaced; however, after replacement, the vehicle still would not properly communicate with the motor. The dealer has since identified the issue as being related to the control module responsible for communication with the drive motor (gateway), which also requires replacement. The vehicle has been out of service for extended periods, including a current repair period exceeding one month. Given that the vehicle has lost propulsion without wRead more

I am reporting a systemic safety failure involving a 2023 Jeep Grand Cherokee sold as a "Certified Pre-Owned" (CPO) vehicle. Despite the mandatory 125-point CPO safety inspection, the vehicle was delivered with multiple safety-critical defects that should have prevented certification. Primary Safety Risks: Suspension/Stability: The vehicle exhibits vibration and instability at all speeds, result
... ing in a loss of steering stability and difficulty maintaining lane position at highway speed. Tire Integrity: The vehicle was sold with dry-rotted tires, indicating structural degradation that poses a high risk of catastrophic failure/blowout. Electrical/Powertrain: The Auto Start/Stop system is faulty. Additionally, the paddle shifters are non-functional, which interferes with manual gear selection and driver control over the powertrain. The selling dealership has refused to acknowledge these defects or bring the vehicle into compliance with manufacturer safety standards. Furthermore, Stellantis Corporate has failed to provide a safety compliance review. These mechanical and electrical defects existed at the time of sale and represent a failure of the CPO safety process.Read more

The DCM (Data Communication Module), also referred to as the Starlink telematics module, has failed. This failure is causing a continuous parasitic battery drain, which rapidly depletes the vehicle’s battery even after charging or replacement. The component remains installed in the vehicle and is available for inspection upon request. The failure has resulted in repeated battery drain and loss
... of vehicle operability. I have experienced a dead battery approximately 4–6 times between September 2025 and present, and the condition has worsened to the point where the vehicle cannot remain running or operable without the battery dying within approximately 5 minutes. This creates a safety risk because the vehicle can become inoperable without warning, increasing the likelihood of being stranded in unsafe conditions or unable to use the vehicle in an emergency. Yes, the issue has been diagnosed and confirmed by a Subaru dealership, which identified the DCM module as failed and requiring replacement. The failure is consistent with a parasitic battery drain caused by the DCM. The vehicle has been inspected by an authorized Subaru dealership.I have also contacted Subaru of America and opened a case requesting assistance with the repair. The primary symptom was repeated battery drain and failure to start beginning around September 2025. The issue progressed over time, with multiple instances of a dead battery, and has now worsened to the point where the battery drains within minutes, preventing normal vehicle operation. There were no consistent warning lights directly identifying the DCM as the cause prior to diagnosis.Read more

... L AGAIN AND TOOK IT TO A BMW REPAIR SHOP. THEY CONFIRMED THAT THE PROBLEM WAS WITH THE EPS (ELECTRIC POWER STEERING CONTROL). UPON INVESTIGATION, A SPECIFIC RECALL EXISTS FOR 2012 BMW Z4 REGARDING THE ELECTRIC POWER STEERING (EPS) SYSTEM. (NHTSA RECALL CAMPAIGN #12V302000: ELECTRIC POWER STEERING CONTROL MODULE.) THE RECALL CAMPAIGN NOTES THAT, AFTER THE ENGINE IS SHUT OFF AND THEN RESTARTED, THE EPS MODULE MAY FUNCTION PROPERLY. IN THAT CASE, THE WARNING SYMBOL IN THE INSTRUMENT CLUSTER WILL NO LONGER APPEAR, AND THE AUDIBLE WARNING WILL NOT ACTIVATE. HOWEVER THE CONDITION MAY OCCUR AGAIN. THIS IS EXACTLY WHAT IS OCCURRING WITH MY CAR AND YOU NEVER KNOW WHEN IT WILL OCCUR. THE CAMPAIGN COVERS 2012 Z4'S AND COVERED SOME CARS MANUFACTURED IN LATE 2011. HOWEVER, THE VIN FOR MY 2011 BMW Z4 SDRIVE 30I E89 N52K (MANUAL TRANSMISSION) IS WBALM5C58BE162421 AND IT WAS MANUFACTURED IN 04/2010. While driving under normal conditions and driving into my driveway on Thursday, March 19, I experienced a sudden and temporary loss of braking ability. There were no warning lights or prior indications of a braking issue before the incident. The loss of br

The vehicle has been regularly maintained and had no known prior brake issues although there are times when the brakes skip when traction is involved. There are also sudden issues with the acceleration. Given the safety risk, I believe this incident may be related to a potential defect in the braking system or electronic control systems.Read more
