I turned my car on and it didn't want to crank up. Once it did crank up every warning light came on. My son ran the code and P00087 came up. I tried to drive my car in my parking lot and it kept stalling like it was going to cut off. Now I drove my car earlier in the day and didn't have any problems. It wasn't until after my car sat throughout the day when this took place. I know it's a recall for

After recall-related software updates were performed on my 2021 Ford Mustang Mach-E, the vehicle began experiencing repeated electrical and climate control failures. The heating and defrost system stopped working, leaving the vehicle without heat and causing the windows to fog, which reduces visibility in cold weather and creates unsafe driving conditions. The vehicle has been taken to multiple a
... uthorized Ford dealerships for inspection and repair; however, the problem returned immediately after the repair. The issue was reproduced and confirmed by a second dealer. A required part is currently on back order until the end of Feburary, leaving the vehicle unrepaired for an extended period. In addition, the vehicle experienced 12-volt battery failure and slow charging after the last recall update. These issues were not present prior to the recall. There were no warning lights before the failures occurred. During severe winter weather conditions in Pennsylvania, the lack of heat and defrost made it extremely difficult and unsafe to operate the vehicle. Because of reduced visibility and freezing interior temperatures, the vehicle could not be safely driven and, due to frigid temperatures, has remained parked. I am still making monthly payments on a vehicle that cannot be reliably or safely used.Read more

While driving in the evening with an estimated 180 miles range remaining a "pop" noise was heard. About a minute later a vague warning message the car had not displayed before appeared. We tried to make it home but concerning (and still vague) messages kept appearing quite rapidly. The car slowed down and came to a halt and died in an intersection. This all happened within five minutes of hearing
... the "pop" noise. Once the vehicle died the head and taillights went off and the car would not allow the emergency flasher lights to be turned on. It would not allow it to be put into neutral to allow it to be moved out of the intersection. It also would not allow any of its doors to be locked (manually or otherwise) except for the driver's door until the vehicle was repaired one week later. We found out following that the 12 volt battery had not charged due to a DC/DC converter input voltage sensor fault related to ICCU failure. The popping noise had been a fuse. As the applicable ICCU parts were replaced by the dealership I doubt they will be available for inspection. The safety of my family was put at risk by: - the car's warnings not clearly explaining what the issue was. - the car not giving clear indication of how quickly it would die. - the car dying in a busy intersection. - the car being unable to be shifted to neutral. - the car's warning flashers not being operational when it died in the middle of the intersection at night. - us being left without warmth in outside temperatures around 6 degrees Fahrenheit. The ICCU problem was confirmed by the dealership we purchased the vehicle from. The vehicle was only inspected by the dealership acting on behalf of Kia.Read more

[XXX] [XXX] , [XXX] [XXX] 2/7/26 To Whom It May Concern, I am writing to formally express my concern regarding ongoing, critical safety issues with my vehicle, which have been increasingly dangerous while driving. These issues appear to be interconnected and involve multiple components, including the valve body, transmission, TCM, ABS module, ECM, and throttle body. These problems have posed s
... ignificant risks to my safety and the safety of others on the road. On several occasions, while driving at highway speeds (approximately 70 mph), the traction control and ABS warning lights have illuminated, and the vehicle has suddenly lost acceleration. This malfunction seems to cause the car to change gears unexpectedly or, in some instances, stop accelerating altogether. As a result, I have been forced to pull over to the side of the road in order to perform a YouTube-suggested reset just to get my vehicle back in motion. This situation has nearly led to accidents, as I have found myself stranded in traffic, unable to accelerate, or worse, pulling into traffic where the car fails to accelerate, almost causing collisions with other vehicles. Additionally, during colder weather, I have experienced issues with the throttle, where it becomes erratic and unresponsive. Despite having multiple repairs made, the problem persists, and I have observed that the throttle may either rev unexpectedly or fail to shift at all. While I understand that some repairs have been made, I am deeply concerned that the vehicle's ongoing malfunctions are indicative of serious safety hazards, which should be addressed by a professional dealership, at no cost rather than being dismissed with the assertion that "it won’t hurt anything." Given the potential for life-threatening incidents and the fact that I have already had several close calls, I strongly urge that these issues be diagnosed and rectified by a certified dealership or technician. , ran out of spac INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(Read more

-I had just exited from the highway and was driving into my town. While doing so, I had both hands on the steering wheel and suddenly the car started beeping and flashing warnings/ alerts indicating that my hands were not on the steering wheel, which they were. Within seconds the car began to deaccelerate and pull me off the road to my right hand side. I initially had no control over the vehicle.
... Then again- without explanation it began to register hands on the steering wheel and I was able to resume control of the vehicle. I believe the lane assist feature was on at the time. It all happened very quickly. Had I still been on the highway as I was moments before, there was enough traffic that I would have been pulled into another vehicle or caused an accident if people saw me suddenly rapidly deaccelerating/braking in front of them because they would have no warning and a very tiny amount of time to react. Then, in an other instance that followed shortly thereafter, my boyfriend was driving with one of the lane assist on. While driving, the weather changed and it began snowing and freezing rain that was freezing when it hit the windshield. The lane assist began to have trouble seeing the lines on the road since the snow and ice were obscuring them somewhat. It began to beep and gently correct the car- which was not appropriate in this case (we understand that the road conditions were beyond its abilities to safely discern things accurately). My boyfriend who had two hands on the wheel trying to carefully navigate this sudden burst of weather so at not to slide. He tried turning it off, but it wasn't turning off, and couldn't troubleshoot it without taking his hands off the wheel. He instructed me to turn it off through the settings menu in the infotainment system, which seemed to disable it. Prior to these dangerous problems, I had experienced some seemingly sensor- related malfunctions with the tire pressure monitoring system. I'm out of room to write more.Read more

The vehicle experiences a persistent stutter/hesitation while driving, which is especially pronounced during acceleration from a stop or at low to moderate speeds. This condition occurs consistently during normal operation and is not a one-time or intermittent event. The hesitation creates a safety concern when accelerating from intersections, merging into traffic, or maintaining smooth vehicle s
... peed. The vehicle feels as though power delivery is interrupted or unstable during acceleration. Multiple warning lights are present. Dealer diagnostics confirmed permanent communication-related fault codes, including CAN bus communication errors and camera/radar-related safety system failures. These fault codes return after clearing and coincide with the drivability hesitation. During inspection, a transmission fluid service was discussed; however, no transmission-specific fault codes were identified. The drivability hesitation has continued alongside the documented electronic and communication-related faults. The issue has been inspected and documented by a dealership. No crash, fire, or injury has occurred, but the condition remains unresolved and continues to affect drivability and safety-related systems.Read more

On 11/22/18, Thanksgiving Day, I was driving our 2 year-old Ford Lariat F-150 Supercrew alone on a backroad in Northern Colorado. I took the truck that was still under warranty to the local dealer we bought the truck from the following week as it was a holiday, where I left the truck for a few days, and upon pickup, was told my concerns could "not be replicated" other than an oil leak. I spoke to the 'managers' in a backroom who were 2 men who LITERALLY told me I just needed to learn how to drive my truck better. I asked them if they had actually driven the truck to LISTEN to what a truck still under warranty sounded like after it how downshifted for no logical reason and dead stopped in the road after locking up. They did not. I told them I thought they wiped the codes and were relying on computers to diagnose issues and if Ford knew they had a problem and were hiding it, and people died, Ford would be sued. True story. I reported this to the FBI due to the response because it was disturbing. I know what happened to me when our truck downshifted. If I was on the highway people would have died. Period. The vehicle has been brought to an authorized dealership for numerous service visits related to electrical system issues that ultimately affected the braking system. During one service visit, the vehicle was returned to me with multiple active dashboard warning lights illuminated related to the braking system, including Electronic Stability Control and Automatic Emergency Braking. I questioned th

After driving the vehicle for a short period, I became uncomfortable continuing to operate it with brake-related warning lights still active and returned it to the dealer. Upon further diagnosis by a service technician, it was discovered that the vehicle has significant brake system defects requiring replacement of major components, including the brake control module and master cylinder. During diagnosis, the brakes locked and the parking brake would not disengage, rendering the vehicle undrivable. No collision or injury occurred. However, the vehicle was released to me with active brake system warnings and later found to have serious brake defects. Due to the safety-critical nature of the braking system and the repeated electrical and brake-related service history, I am reporting this for documentation and review.Read more

To Whom It May Concern, I am submitting this complaint to report ongoing engine and emissions system problems with my 2016 Honda Pilot that raise serious safety concerns and may be related to a broader defect currently under investigation. I purchased this vehicle from a dealership in April of 2024 and the issues began shortly afterward at approximately 67,000 miles. The vehicle currently has ap
... proximately 74,600 miles and has received regular oil changes and routine maintenance. The vehicle has repeatedly displayed emissions system warning messages and engine fault codes. It has experienced persistent engine misfiring even after a mechanic performed repairs on cylinder 1. In addition, the vehicle exhibits rough engine shaking during driving and idling, engine stalling, and rough or delayed gear changes continuously since 2024. Despite attempted repairs, the problems continue to occur to present. The stalling, misfiring, and unstable engine operation create a potential safety risk, particularly while driving at speed or in traffic. The rough shifting and loss of smooth power delivery further impact safe vehicle operation. Given the relatively low mileage, proper maintenance, and the recurrence of these symptoms, this does not appear to be normal wear and tear. I am aware that similar engine-related complaints have been reported by other owners of 2016 Honda Pilot vehicles and that this model year is included in an ongoing federal investigation into potential engine defects. I am requesting that this complaint be reviewed as part of that investigation and considered for further action, including possible recall or service remedy, to address the safety risks posed by these engine and emissions system failures. While driving, my 2017 Ford F‑150 experienced a sudden loss of the Output Shaft Speed (OSS) sensor signal, causing the transmission to stay stuck in a higher gear and triggering the wrench light. The truck lost power and would not shift properly. A diagnostic scan showed two transmission codes: one for the OSS sensor and a second code indicating an intermittent electrical signal drop. I contac
... ted a Ford dealer before the repair, described the symptoms, and asked for an estimate. The service advisor did not disclose any known issues and never called me back after saying he would check parts availability. These symptoms match the known failure pattern of the molded lead frame in the 6R80 transmission. My truck exhibited intermittent behavior, harsh shifting, and unsafe drivability. I had to pay $1,250 out of pocket to replace the lead frame to restore normal operation. I later learned that NHTSA has opened an Engineering Analysis into 2015–2017 F‑150 trucks for this exact issue (OSS signal loss due to internal electrical degradation of the lead frame). My vehicle’s symptoms, diagnostic codes, and required repair are consistent with the defect under investigation. This failure creates a safety hazard because the truck can suddenly lose the ability to accelerate or shift correctly, especially in traffic or at highway speeds. I am requesting that NHTSA include my vehicle in the investigation and that Ford be required to address this defect.Read more

ICCU failure at 3889 miles not charging and 12v battery failure
Incident date: Nov 10, 2025
Description of the Defect: While driving with Full Self-Driving (FSD) engaged, the vehicle's primary computer shuts down and reboots, causing a sudden loss of all safety features, including Blind Spot Monitoring, Lane Departure Warning, Forward Collision Warning, and the driving visualization. The cameras go black while the vehicle is in motion. The Trigger: The failure is triggered by high elect
... rical load. If the Cabin Heater (HVAC) and Seat Heaters are active while FSD is fully computing, the 12V power supply to the computer drops below critical voltage, causing a "Turbo A" processor lockup and system crash. Evidence of Defect: Service Center diagnostics confirmed the specific error code APS_w169_TurboA_SCS_LKUP (Computer Crash) occurred at the exact same timestamp as VCLEFT_a302_blowerGeneralFault (HVAC Blower Failure). This proves a common-mode voltage drop affecting multiple critical systems simultaneously. Manufacturer Response: The manufacturer (Tesla) acknowledged the logs but refused to inspect the wiring harness or ground connections, instead recommending a replacement of the computer unit. However, the failure can be mitigated by unplugging external cameras to reduce load, proving the root cause is an insufficient power delivery design (Wiring/Grounding) rather than a component failure. The computer is running at full load with the newest FSD software and it can no longer keep up with additional loads. Safety Risk: This defect causes a sudden, uncommanded loss of driver assistance and situational awareness tools (cameras/visualization) while the vehicle is maneuvering in traffic. The vehicle’s power distribution system appears insufficient to support the electrical load of the software (FSD) and the hardware (HVAC) simultaneously.Read more

The Integrated Charging Control Unit (ICCU) has intermittent failures. During the last failure, my wife was on a busy freeway (Interstate 5 near Hwy 120 northbound) in heavy but fast moving traffic. She was driving in the #2 lane (of 4 lanes) when she experienced sudden loss of propulsion. The vehicle slowed to 5 MPH in traffic that was moving 70 to 80 MPH. She had to traverse through the #3 a
... nd #4 lanes to the narrow emergency shoulder. Cars were honking and flipping her off. In the emergency lane, the vehicle completely died after 30 to 60 seconds. She was at risk of being rear ended during the dangerous 5 MPH lane changes and in the time parked in the emergency lane. She could not start the car for 15 minutes, after which the system came back to life. She was then able to start it and continue home. The first time the ICCU failed was in a parking lot. This is a known defect with Hyundai which they have attempted to rectify with Recall Campaign 272. However, they refuse to replace the ICCU. After the first event, they replaced the battery after 22 months/32429 miles. Now they want to replace the battery again after another 20 months/34k miles. What they don't get, is that the batteries have not prematurely failed. Instead the ICCU has intermittently and temporarily drawn the batteries down causing the vehicle to lose propulsion and die. They refuse to replace the ICCU because an error code they were looking for was not in the memory. They have performed three software updates on three separate dealer visits, but the issue has not been resolved. During the last visit they kept my vehicle for six weeks while I appealed through Hyundai Consumer Affairs and the Service manager appealed through his Dealer rep. But the factory engineers held firm and would not approve the ICCU warranty replacement, and instead wanted me to pay for 12V battery replacement. We don't know if there were any warning lights since my wife was busy navigating the danger.Read more

The front passenger door will not lock or unlock using the key fob or the interior power door lock button. The door must now be locked manually. This defeats the vehicle’s automatic locking system and creates a safety and security concern. This condition prevents all doors from be
... ing secured properly when driving or when parked. A passenger may believe the door is locked when it is not, increasing the risk of unauthorized entry, theft, or passenger safety issues. The failure occurred during normal use and without warning. The vehicle has been properly maintained. I have learned that multiple Toyota RAV4 owners have reported similar door lock actuator failures, indicating a potential widespread defect. This issue occurred shortly after the factory warranty expired. Due to the safety implications, I believe this issue warrants investigation and possible recall or manufacturer repair program. “This failure occurred without warning and with no dashboard alerts.”Read more

Driving with my wife and [XXX] child, there was a pop sound from under the car, and the car suddenly showed multiple faults, including "Check electric vehicle system”, which cleared on their own. I parked and turned the car off and on again and the faults reappeared. I started driving home, and during the drive it showed "Stop vehicle and check power supply" and audibly alarmed. As I was only h
... alf a mile from my house on local quiet roads, I made it. A few minutes later, I tried to turn on the car and some displays were off, there was no drive power, the steering wheel did not turn, the door locks were flakey, and I wasn't able to tell if I had shifted into Park or if the parking brake was engaged. When the tow truck arrived, the car was completely unresponsive and there was no way to release the parking brake or shift the car in neutral, so the car was dragged across the pavement and onto the tow truck. The failure was quick and comprehensive and could have easily caused injury. There are many highways near me with fast, busy traffic and either no emergency shoulder or a very narrow one. If my car tells me that I need to stop the vehicle, that can create a very dangerous choice between stopping somewhere less safe quickly or risking the car being completely disabled while in the travel lane while I look for a safer spot. Loss of cabin heat can easily become dangerous very quickly in the winter. Many drivers may not even understand the urgency of the warning until the car starts failing to drive. The car is at a Kia dealership for repair who confirmed an ICCU failure. See related NHTSA recall no 24V-867 and others. ICCU failures are endemic to Kia and Hyundai EVs. ICCU failures are widely discussed on many internet forums and seem much more common than the 1% failure rate claimed by Hyundai/Kia. FYI, an interesting root cause analysis was shared on a German EV forum: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Catastrophic Loss of Motive Power and Power Steering at Road Speed Following Recall 68C Update. Description: On February 6, 2026, my 2024 Jeep Wrangler 4xe (Mileage: 24,947) suffered a catastrophic safety failure while in motion on a public roadway. This occurred about 25 hours after the vehicle was "cleared" by a dealership for Recall 68C (NHTSA Recall 25V-741). While driving at approximately 25
... /30 MPH in Electric Mode, the vehicle suddenly displayed a "Service Transmission" warning and a red high-voltage "Service Hybrid Electric Vehicle System" error. The vehicle immediately lost all motive power and entered a "Limp Mode" (Turtle icon) that restricted speed to under 5 MPH. Simultaneously, the vehicle lost power steering and displayed errors for "Service Shifter," "Auto Emergency Braking (AEB) Unavailable," and "Adaptive Cruise Control (ACC) Unavailable." I was forced to drift the unpowered vehicle across lanes of traffic into a shoulder without power steering where another vehicle almost hit me. The vehicle revved excessively but would not provide propulsion. After a hard restart, I was able to move the vehicle at 20 MPH to a safe location, where it was subsequently towed to the dealership. This failure occurred after the manufacturer assured me the vehicle was safe following the Recall 68C software update. This is a life-threatening defect that caused a total loss of control on a live roadway. I am requesting a formal investigation into the effectiveness of the 68C software remedy. I have video and photos of the dashboardRead more

Electric car stopped working with home, (level 2 or AC charging), only level three or DC fast chargers. Took car to local dealership who confirmed the cars ICCU had gone bad, which is a known issue with this vehicle, and needed to be replaced. Check engine turned on and at the same time the vehicle turned into safety mode abruptly, it slowed down to 10 miles an hour while on the high way and receiving a message stating that the "engine power reduced", the issue was presented to the dealership and they believe to be fixed with a sensor issue, the light came back on a few days after the initial repair and once again the engine lost power
... while driving the vehicle, the vehicle was then taken back to the dealership for repair once again, both times I mentioned the service bulletin, which they did not consider in their repair, stating that the issue was specific for a code (which all codes present are in the service bulletin) the 2nd repair was done on a actuator, which they stated should fix the issue, and did not, a 3rd time the vehicle lost power while hauling a trailer, this time I brought up the concern of them not reviewing the service bulletin, this time they confirmed that the service bulletin was the issue causing the check engine light (GM service bulletin no: 21-NA-149) now they are looking to slowly start repairing the issue and wait for the check engine light to turn on again to then go back and forth and review step by step the instructions in the bulletin.Read more

While stopped in traffic with the vehicle’s automatic engine start-stop feature engaged, the vehicle failed to respond when the accelerator was pressed. The engine had shut off normally due to the start-stop system, but when attempting to move again, the vehicle did not accelerate and appeared temporarily disabled. A “Gearbox fault detected” warning message was displayed. In other occurrence
... s, a “Charging system fault detected” warning appeared immediately after restart. The vehicle required an ignition cycle (turning the vehicle off and back on) to regain normal operation. This condition has occurred on multiple occasions over the past year, including recently, and is intermittent and difficult to reproduce on demand. This condition creates a safety risk because the vehicle can become temporarily immobilized in active traffic, including at stoplights and intersections, leaving the driver unable to proceed as expected. The vehicle has been inspected multiple times by authorized Land Rover dealers for related electrical and charging system issues. Components such as the battery have previously been replaced, but the condition has continued to occur. At this time, the issue has not been consistently reproduced by the dealer. Warning messages associated with the incidents include “Gearbox fault detected” and “Charging system fault detected.” These warnings typically appear during or immediately after an automatic engine restart event. The vehicle remains available for inspection.Read more

Recall issue: The vehicle failed and became disabled due to the shift cable bushing while in a parking lot on private property, with driver in the vehicle. The gear would not shift or go into park and continued to roll forward until either I hit the curb or applied the brake. Ford will not pay for the tow or diagnostic fee to have the problem diagnosed at another body shop (before I knew of the r
... ecall) Ford will not replace shift cable, only install protective cap. Recall issue: rearview camera displays distorted images, and does not always show rear view when backing up. It shows blank sometimes, and forward when reversing. Visibility unclear when backing. Ford will not replace as they say they do not have the parts to replace. This leaves the vehicle at high risk for safety issues.Read more

ICCU failed. Level 2 charges didn’t work.
Incident date: Feb 3, 2026
The failure occurred in the rear sliding window defroster electrical system, specifically at the left-side electrical connection of the power sliding rear window. The component overheated and ignited, causing fire damage and shattering of the rear left portion of the window. The vehicle and damaged components are currently available for inspection upon request. The defect resulted in an active ele
... ctrical fire during normal vehicle operation (remote start while warming up). This posed a serious risk of vehicle fire, injury to occupants, and potential spread to nearby vehicles or structures. As a firefighter, I recognized the danger and immediately shut off the vehicle and ventilated it; without trained action, the incident could have resulted in a total vehicle loss or more severe consequences. Yes. Copeland Chevrolet in Hudson, Massachusetts inspected the vehicle and confirmed that the damage was consistent with the known defect in the rear sliding window defroster system, consistent with GM Customer Satisfaction Program XXX. The vehicle has been inspected by an authorized GM dealership (Copeland Chevrolet). No police or insurance inspection has occurred to date. No warning lamps, messages, or prior symptoms were present. The failure occurred suddenly and without warning during normal operation while the vehicle was warming up using the factory-installed remote start. This is a known defect acknowledged by GM under Customer Satisfaction Program XXX (which expired in 2022), which described overheating and fire risk in the rear sliding window defroster electrical connection. That program was limited by state and excluded Massachusetts, so I was never notified despite identical risk. This same defect is currently the subject of an active safety recall in Canada, where it is treated as a fire hazard requiring corrective action. While driving to work on [XXX] in Huntsville AL at approximately 55MPH. I was using Chevrolet SuperCruise. I tried turning off super cruise with the super cruise button. It did not turn off, tried pressing cruise control button, nothing, tried the cancel button, nothing, pressed brakes and would not turn off. Truck started slowing in very heavy traffic, it turned hazards on. I was finally able to
... get it turned off. I have reported to dealer, no surprise they didn't answer, left message at 9 am this morning, no call back. I have made a report to GM. I no longer feel safe putting my family in this truck. This truck has been in shop several times for "Service driver assist", no on-star, no cellular data, extremely slow infotainment system and no supercruise available. The truck at last visit for the service driver assist took more than 2 week. I was told a camera and a software update were performed. The issues have persisted. The super cruise event yesterday was the first occurrence of this type. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
